Effective detection of human leukocyte antigen risk alleles in celiac disease using tag single nucleotide polymorphisms.
Background: The HLA genes, located in the MHC region on chromosome 6p21.3, play an important role in many
autoimmune disorders, such as celiac disease (CD), type 1 diabetes (T1D), rheumatoid arthritis, multiple sclerosis, psoriasis
and others. Known HLA variants that confer risk to CD, for example, include DQA1*05/DQB1*02 (DQ2.5) and DQA1*03/
DQB1*0302 (DQ8). To diagnose the majority of CD patients and to study disease susceptibility and progression, typing these
strongly associated HLA risk factors is of utmost importance. However, current genotyping methods for HLA risk factors
involve many reactions, and are complicated and expensive. We sought a simple experimental approach using tagging
SNPs that predict the CD-associated HLA risk factors.
Methodology: Our tagging approach exploits linkage disequilibrium between single nucleotide polymorphism (SNPs) and
the CD-associated HLA risk factors DQ2.5 and DQ8 that indicate direct risk, and DQA1*0201/DQB1*0202 (DQ2.2) and
DQA1*0505/DQB1*0301 (DQ7) that attribute to the risk of DQ2.5 to CD. To evaluate the predictive power of this approach,
we performed an empirical comparison of the predicted DQ types, based on these six tag SNPs, with those executed with
current validated laboratory typing methods of the HLA-DQA1 and -DQB1 genes in three large cohorts. The results were
validated in three European celiac populations.
Conclusion: Using this method, only six SNPs were needed to predict the risk types carried by .95% of CD patients. We
determined that for this tagging approach the sensitivity was .0.991, specificity .0.996 and the predictive value .0.948.

The construction of a consistent theory of quantum gravity is a problem in
theoretical physics that has so far defied all attempts at resolution. One
ansatz to try to obtain a non-trivial quantum theory proceeds via a
discretization of space-time and the Einstein action. I review here three major
areas of research: gauge-theoretic approaches, both in a path-integral and a
Hamiltonian formulation, quantum Regge calculus, and the method of dynamical
triangulations, confining attention to work that is strictly four-dimensional,

Asthma control cost-utility randomized trial evaluation (ACCURATE): the goals of asthma treatment

remains a source of significant morbidity and use of health care resources. The
central research question of the ACCURATE trial is whether maximal doses of
(combination) therapy should be used for long periods in an attempt to achieve
complete control of all features of asthma. An additional question is whether
patients and society value the potential incremental benefit, if any,
sufficiently to concur with such a treatment approach. We assessed patient
preferences and cost-effectiveness of three treatment strategies aimed at
achieving different levels of clinical control: 1. sufficiently controlled asthma
2. strictly controlled asthma 3. strictly controlled asthma based on exhaled
nitric oxide as an additional disease marker DESIGN: 720 Patients with mild to
moderate persistent asthma from general practices with a practice nurse, age
18-50 yr, daily treatment with inhaled corticosteroids (more then 3 months usage
of inhaled corticosteroids in the previous year), will be identified via patient
registries of general practices in the Leiden, Nijmegen, and Amsterdam areas in
The Netherlands. The design is a 12-month cluster-randomised parallel trial with
40 general practices in each of the three arms. The patients will visit the
general practice at baseline, 3, 6, 9, and 12 months. At each planned and
unplanned visit to the general practice treatment will be adjusted with support
of an internet-based asthma monitoring system supervised by a central
coordinating specialist nurse. Patient preferences and utilities will be assessed
by questionnaire and interview. Data on asthma control, treatment step, adherence
to treatment, utilities and costs will be obtained every 3 months and at each
unplanned visit. Differences in societal costs (medication, other (health) care
and productivity) will be compared to differences in the number of limited
activity days and in quality adjusted life years (Dutch EQ5D, SF6D, e-TTO, VAS).
This is the first study to assess patient preferences and cost-effectiveness of
asthma treatment strategies driven by different target levels of asthma control.

Dissolution dominating calcification process in polar pteropods close to the point of aragonite undersaturation
Thecosome pteropods are abundant upper-ocean zooplankton that build aragonite shells. Ocean acidification results in the lowering of aragonite saturation levels in the surface layers, and several incubation studies have shown that rates of calcification in these organisms decrease as a result. This study provides a weight-specific net calcification rate function for thecosome pteropods that includes both rates of dissolution and calcification over a range of plausible future aragonite saturation states (Omega_Ar). We measured gross dissolution in the pteropod Limacina helicina antarctica in the Scotia Sea (Southern Ocean) by incubating living specimens across a range of aragonite saturation states for a maximum of 14 days. Specimens started dissolving almost immediately upon exposure to undersaturated conditions (Omega_Ar,0.8), losing 1.4% of shell mass per day. The observed rate of gross dissolution was different from that predicted by rate law kinetics of aragonite dissolution, in being higher at Var levels slightly above 1 and lower at Omega_Ar levels of between 1 and 0.8. This indicates that shell mass is affected by even transitional levels of saturation, but there is, nevertheless, some partial means of protection for shells when in undersaturated conditions. A function for gross dissolution against Var derived from the present observations was compared to a function for gross calcification derived by a different study, and showed that dissolution became the dominating process even at Omega_Ar levels close to 1, with net shell growth ceasing at an Omega_Ar of 1.03. Gross dissolution increasingly dominated net change in shell mass as saturation levels decreased below 1. As well as influencing their viability, such dissolution of pteropod shells in the surface layers will result in slower sinking velocities and decreased carbon and carbonate fluxes to the deep ocean

Consistent phenological shifts in the making of a biodiversity hotspot: the Cape flora
Background
The best documented survival responses of organisms to past climate change on short (glacial-interglacial) timescales are distributional shifts. Despite ample evidence on such timescales for local adaptations of populations at specific sites, the long-term impacts of such changes on evolutionary significant units in response to past climatic change have been little documented. Here we use phylogenies to reconstruct changes in distribution and flowering ecology of the Cape flora - South Africa's biodiversity hotspot - through a period of past (Neogene and Quaternary) changes in the seasonality of rainfall over a timescale of several million years.
Results
Forty-three distributional and phenological shifts consistent with past climatic change occur across the flora, and a comparable number of clades underwent adaptive changes in their flowering phenology (9 clades; half of the clades investigated) as underwent distributional shifts (12 clades; two thirds of the clades investigated). Of extant Cape angiosperm species, 14-41% have been contributed by lineages that show distributional shifts consistent with past climate change, yet a similar proportion (14-55%) arose from lineages that shifted flowering phenology.
Conclusions
Adaptive changes in ecology at the scale we uncover in the Cape and consistent with past climatic change have not been documented for other floras. Shifts in climate tolerance appear to have been more important in this flora than is currently appreciated, and lineages that underwent such shifts went on to contribute a high proportion of the flora's extant species diversity. That shifts in phenology, on an evolutionary timescale and on such a scale, have not yet been detected for other floras is likely a result of the method used; shifts in flowering phenology cannot be detected in the fossil record

MHC-based detection of antigen-specific CD8+ T cell responses
The hallmark of adaptive immunity is its ability to recognise a wide range of antigens and technologies that capture this diversity are therefore of substantial interest. New methods have recently been developed that allow the parallel analysis of T cell reactivity against vast numbers of different epitopes in limited biological material. These technologies are based on the joint binding of differentially labelled MHC multimers on the T cell surface, thereby providing each antigen-specific T cell population with a unique multicolour code. This strategy of ‘combinatorial encoding’ enables detection of many (at least 25) different T cell populations per sample and should be of broad value for both T cell epitope identification and immunomonitoring
