    Four layer bandage compared with short stretch bandage for venous leg ulcers: systematic review and meta-analysis of randomised controlled trials with data from individual patients

    <p><b>Objective:</b> To compare the effectiveness of two types of compression treatment (four layer bandage and short stretch bandage) in people with venous leg ulceration.</p> <p><b>Design:</b> Systematic review and meta-analysis of patient level data.</p> <p><b>Data:</b> sources Electronic databases (the Cochrane Central Register of Controlled Trials, the Cochrane Wounds Group Specialised Register, Medline, Embase, CINAHL, and National Research Register) and reference lists of retrieved articles searched to identify relevant trials and primary investigators. Primary investigators of eligible trials were invited to contribute raw data for re-analysis.</p> <p><b>Review:</b> methods Randomised controlled trials of four layer bandage compared with short stretch bandage in people with venous leg ulceration were eligible for inclusion. The primary outcome for the meta-analysis was time to healing. Cox proportional hazards models were run to compare the methods in terms of time to healing with adjustment for independent predictors of healing. Secondary outcomes included incidence and number of adverse events per patient.</p> <p><b>Results:</b> Seven eligible trials were identified (887 patients), and patient level data were retrieved for five (797 patients, 90% of known randomised patients). The four layer bandage was associated with significantly shorter time to healing: hazard ratio (95% confidence interval) from multifactorial model based on five trials was 1.31 (1.09 to 1.58), P=0.005. Larger ulcer area at baseline, more chronic ulceration, and previous ulceration were all independent predictors of delayed healing. Data from two trials showed no evidence of a difference in adverse event profiles between the two bandage types.</p> <p><b>Conclusions:</b> Venous leg ulcers in patients treated with four layer bandages heal faster, on average, than those of people treated with the short stretch bandage. Benefits were consistent across patients with differing prognostic profiles.</p&gt

    Bunburra Rockhole: Exploring the geology of a new differentiated asteroid

    Bunburra Rockhole is the first recovered meteorite of the Desert Fireball Network. We expanded a bulk chemical study of the Bunburra Rockhole meteorite to include major, minor and trace element analyses, as well as oxygen and chromium isotopes, in several different pieces of the meteorite. This was to determine the extent of chemical heterogeneity and constrain the origin of the meteorite. Minor and trace element analyses in all pieces are exactly on the basaltic eucrite trend. Major element analyses show a slight deviation from basaltic eucrite compositions, but not in any systematic pattern. New oxygen isotope analyses on 23 pieces of Bunburra Rockhole shows large variation in both δ17O and δ18O, and both are well outside the HED parent body fractionation line. We present the first Cr isotope results of this rock, which are also distinct from HEDs. Detailed computed tomographic scanning and back-scattered electron mapping do not indicate the presence of any other meteoritic contaminant (contamination is also unlikely based on trace element chemistry). We therefore conclude that Bunburra Rockhole represents a sample of a new differentiated asteroid, one that may have more variable oxygen isotopic compositions than 4 Vesta. The fact that Bunburra Rockhole chemistry falls on the eucrite trend perhaps suggests that multiple objects with basaltic crusts accreted in a similar region of the Solar System

    Bladder neck mobility in continent nulliparous women

    To evaluate the mobility of the vesical neck during coughing and valsalva in healthy nulliparous volunteers and to test the reliability of the technique applied. Design Clinical observational study. Setting Department of Obstetrics and Gynaecology, Cantonal Hospital Lucerne, Switzerland. Population Thirty-nine nulliparous volunteers. Methods Vesical neck motion was assessed with perineal ultrasound. Intra–abdominal pressure was controlled for with an intrarectal probe. Intra-rater reliability was evaluated. Results Vesical neck mobility was significantly lower during coughing (8 mm, SD 4 mm) than during valsalva (15 mm, SD 10 mm) ( P < 0.005 ). Between individuals mobility varied from 4 mm to 32 mm during coughing and from 2 mm to 31 mm during valsalva. Test-retest-studies showed a maximum difference between to tests during coughing of 4 mm and during valsalva of 5 mm.     Development of a fully automatic shape model matching (FASMM) system to derive statistical shape models from radiographs: application to the accurate capture and global representation of proximal femur shape

    SummaryObjectiveTo evaluate the accuracy and sensitivity of a fully automatic shape model matching (FASMM) system to derive statistical shape models (SSMs) of the proximal femur from non-standardised anteroposterior (AP) pelvic radiographs.DesignAP pelvic radiographs obtained with informed consent and appropriate ethical approval were available for 1105 subjects with unilateral hip osteoarthritis (OA) who had been recruited previously for The arcOGEN Study. The FASMM system was applied to capture the shape of the unaffected (i.e., without signs of radiographic OA) proximal femur from these radiographs. The accuracy and sensitivity of the FASMM system in calculating geometric measurements of the proximal femur and in shape representation were evaluated relative to validated manual methods.ResultsDe novo application of the FASMM system had a mean point-to-curve error of less than 0.9 mm in 99% of images (n = 266). Geometric measurements generated by the FASMM system were as accurate as those obtained manually. The analysis of the SSMs generated by the FASMM system for male and female subject groups identified more significant differences (in five of 17 SSM modes after Bonferroni adjustment) in their global proximal femur shape than those obtained from the analysis of conventional geometric measurements. Multivariate gender-classification accuracy was higher when using SSM mode values (76.3%) than when using conventional hip geometric measurements (71.8%).ConclusionsThe FASMM system rapidly and accurately generates a global SSM of the proximal femur from radiographs of varying quality and resolution. This system will facilitate complex morphometric analysis of global shape variation across large datasets. The FASMM system could be adapted to generate SSMs from the radiographs of other skeletal structures such as the hand, knee or pelvis

    A mutli-technique search for the most primitive CO chondrites

    As part of a study to identify the most primitive COs and to look for weakly altered CMs amongst the COs, we have conducted a multi-technique study of 16 Antarctic meteorites that had been classified as primitive COs. For this study, we have determined: (1) the bulk H, C and N abundances and isotopes, (2) bulk O isotopic compositions, (3) bulk modal mineralogies, and (4) for some selected samples the abundances and compositions of their insoluble organic matter (IOM). Two of the 16 meteorites do appear to be CMs – BUC 10943 seems to be a fairly typical CM, while MIL 090073 has probably been heated. Of the COs, DOM 08006 appears to be the most primitive CO identified to date and is quite distinct from the other members of its pairing group. The other COs fall into two groups that are less primitive than DOM 08006 and ALH 77307, the previously most primitive CO. The first group is composed of members of the DOM 08004 pairing group, except DOM 08006. The second group is composed of meteorites belonging to the MIL 03377 and MIL 07099 pairing groups. These two pairing groups should probably be combined. There is a dichotomy in the bulk O isotopes between the primitive (all Antarctic finds) and the more metamorphosed COs (mostly falls). This dichotomy can only partly be explained by the terrestrial weathering experienced by the primitive Antarctic samples. It seems that the more equilibrated samples interacted to a greater extent with 16O-poor material, probably water, than the more primitive meteorites

    A Single-Lumen Central Venous Catheter for Continuous and Direct Intra-abdominal Pressure Measurement

    Background: Abdominal compartment syndrome (ACS) is associated with high morbidity and mortality rates. Therefore, the need for a good diagnostic tool to predict intra-abdominal hypertension (IAH) and progression to ACS is paramount. Bladder pressure (BP) has been used for several years for intra-abdominal pressure (IAP) measurement but has the disadvantage that it is not a continuous measurement. In this study, a single-lumen central venous catheter (CVC) is placed through the abdominal wall into the abdominal cavity to continuously and directly monitor the intra-abdominal pressure (CDIAP). The aim of this study was to evaluate the use of CDIAP to measure BP as a representative of the true IAP. Methods: Both BP and CDIAP were prospectively recorded on a variety of surgical patients admitted to the intensive care unit (ICU) from March 2003 up to December 2004. At the end of the surgical procedure, the CVC was placed through the abdominal wall and connected to a pressure transducer. In addition, the BP was measured through the urine drainage port after clamping the catheter and filling the bladder with 50 ml of 0.9% saline. At least three paired measurements (BP and CDIAP) were performed for at least one day on the ICU in a standardized manner at preset time intervals on each patient. The paired measurements were compared using the Bland-Altman (B-A) method. Data are presented as mean ± standard deviation. Results: Over a period of 22 months (March 2003 until December 2004), 125 paired measurements of both BP and CDIAP were recorded on 25 patients. The mean age was 72.4 ± 6.6 years. Eighteen patients underwent central vascular surgery, and seven patients with peritonitis received laparotomy. The mean CDIAP was 11.4 ± 4.8 (range 2-30) mmHg, and the BP was 12.9 ± 5.3 (range 3-37) mmHg. The mean difference between CDIAP and BP was 1.6 ± 2.7 mmHg. There was an acceptable level of agreement (intraclass correlation 0.82) between IAP measured by BP and IAP measured via CDIAP. Conclusion: Continuous direct intra-abdominal pressure measurement proved that the BP measurement approach of Kron is representative of the IAP. CDIAP measurement is accurate and makes it easier for the nursing staff to be informed of the IAP

    Quasiparticle dynamics in ferromagnetic compounds of the Co-Fe and Ni-Fe systems

    We report a theoretical study of the quasiparticle lifetime and the quasiparticle mean free path caused by inelastic electron-electron scattering in ferromagnetic compounds of the Co-Fe and Ni-Fe systems. The study is based on spin-polarized calculations, which are performed within the GWGW approximation for equiatomic and Co- and Ni-rich compounds, as well as for their constituents. We mainly focus on the spin asymmetry of the quasiparticle properties, which leads to the spin-filtering effect experimentally observed in spin-dependent transport of hot electrons and holes in the systems under study. By comparing with available experimental data on the attenuation length, we estimate the contribution of the inelastic mean free path to the latter.Comment: 10 pages, 10 figure

    Galaxy and mass assembly: Luminosity and stellar mass functions in GAMA groups

    How do galaxy properties (such as stellar mass, luminosity, star formation rate, and morphology) and their evolution depend on the mass of their host dark matter halo? Using the Galaxy and Mass Assembly group catalogue, we address this question by exploring the dependence on host halo mass of the luminosity function (LF) and stellar mass function (SMF) for grouped galaxies subdivided by colour, morphology, and central/satellite. We find that spheroidal galaxies in particular dominate the bright and massive ends of the LF and SMF, respectively. More massive haloes host more massive and more luminous central galaxies. The satellites LF and SMF, respectively, show a systematic brightening of characteristic magnitude, and increase in characteristic mass, with increasing halo mass. In contrast to some previous results, the faint-end and low-mass slopes show little systematic dependence on halo mass. Semi-analytic models and simulations show similar or enhanced dependence of central mass and luminosity on halo mass. Faint and low-mass simulated satellite galaxies are remarkably independent of halo mass, but the most massive satellites are more common in more massive groups. In the first investigation of low-redshift LF and SMF evolution in group environments, we find that the red/blue ratio of galaxies in groups has increased since redshift z ~ 0.3 relative to the field population. This observation strongly suggests that quenching of star formation in galaxies as they are accreted into galaxy groups is a significant and ongoing process

    Advances in multispectral and hyperspectral imaging for archaeology and art conservation

    Multispectral imaging has been applied to the field of art conservation and art history since the early 1990s. It is attractive as a noninvasive imaging technique because it is fast and hence capable of imaging large areas of an object giving both spatial and spectral information. This paper gives an overview of the different instrumental designs, image processing techniques and various applications of multispectral and hyperspectral imaging to art conservation, art history and archaeology. Recent advances in the development of remote and versatile multispectral and hyperspectral imaging as well as techniques in pigment identification will be presented. Future prospects including combination of spectral imaging with other noninvasive imaging and analytical techniques will be discussed
