    Understanding workflow in telehealth video visits: Observations from the IDEATel project

    AbstractHome telemedicine is an emerging healthcare paradigm that has the potential to transform the treatment of chronic illness. The purpose of this paper is to: (1) develop a theoretical and methodological framework for studying workflow in telemediated clinician–patient encounters drawing on a distributed cognition approach and (2) employ the framework in an in-depth analysis of workflow in the IDEATel project, a telemedicine program for older adults with diabetes. The methods employed in this research included (a) videotaped observations of 27 nurse–patient encounters and (b) semi-structured interviews with participants. The analyses were used to provide a descriptive analysis of video visits, understand the mediating role of different technologies and to characterize the ways in which artifacts and representations are used to understand the state of the patient. The study revealed barriers to productive use of telehealth technology as well as adaptations that circumvented such limitations. This research has design implications for: (a) improving the coordination of communication and (b) developing tools that better integrate and display information. Although home telemedicine programs will differ in important respects, there are invariant properties across such systems. Explicating these properties can serve as a needs requirement analysis to develop more effective systems and implementation plans

    Hiring expert consultants in e-healthcare: an analytics-based two sided matching approach

    Very often in some censorious healthcare scenario, there may be a need to have some expert consultancies (especially by doctors) that are not available in-house to the hospitals. Earlier, this interesting healthcare scenario of hiring the expert consultants (mainly doctors) from outside of the hospitals had been studied with the robust concepts of mechanism design with money and mechanism design without money. In this paper, we explore the more realistic two sided matching market in our healthcare set-up. In this, the members of the two participating communities, namely the patients and the doctors are revealing the strict preference ordering over the members of the opposite community for a stipulated amount of time. We assume that the patients and doctors are strategic in nature. With the theoretical analysis, we demonstrate that the TOMHECs, that results in the stable allocation of doctors to the patients, satisfies the several economic properties such as strategy-proof-ness (or truthfulness) and optimality. Further, the analytically based analysis of our proposed mechanisms i.e. RAMHECs and TOMHECs are carried out on the ground of the expected distance of the allocation done by the mechanisms from the top most preference.     Protocol of the Healthy Brain Study: An accessible resource for understanding the human brain and how it dynamically and individually operates in its bio-social context

    The endeavor to understand the human brain has seen more progress in the last few decades than in the previous two millennia. Still, our understanding of how the human brain relates to behavior in the real world and how this link is modulated by biological, social, and environmental factors is limited. To address this, we designed the Healthy Brain Study (HBS), an interdisciplinary, longitudinal, cohort study based on multidimensional, dynamic assessments in both the laboratory and the real world. Here, we describe the rationale and design of the currently ongoing HBS. The HBS is examining a population-based sample of 1,000 healthy participants (age 30-39) who are thoroughly studied across an entire year. Data are collected through cognitive, affective, behavioral, and physiological testing, neuroimaging, bio-sampling, questionnaires, ecological momentary assessment, and real-world assessments using wearable devices. These data will become an accessible resource for the scientific community enabling the next step in understanding the human brain and how it dynamically and individually operates in its bio-social context.     Grammatical aspect and L2 learners’ on-line processing of temporarily ambiguous sentences in English: A self-paced reading study with German, Dutch and French L2 learners

    The results of a self-paced reading study with advanced German, Dutch and French second language (L2) learners of English showed that their online comprehension of early closure (EC) sentences which are initially misanalysed by native English speakers (e.g. While John hunted the frightened rabbit escaped) was affected by whether or not, like English, their first language (L1) encodes aspect grammatically (French) or only via lexical means (German, Dutch). The English and the higher proficiency French participants showed a processing asymmetry in their online reading of the temporarily ambiguous sentences, assumed to be caused by the difference in the aspectual perspective a comprehender takes when initial verbs appear in the past simple vs. the past progressive. In contrast, the German and Dutch learners, irrespective of proficiency, treated both progressive and simple sentences in the same way, despite the fact that all the L2 learners were matched according to their metalinguistic knowledge of English aspectual distinctions. Furthermore, despite patterning with the German learners online, the Dutch L2 learners’ offline judgments were more akin to those of the English native speakers and the French L2 learners, showing an effect of aspect, which could be argued to lend support to the idea that progressive aspect may be becoming grammaticalized in Dutch. Taken together, the results of this study add to our growing understanding of cross-linguistic influences during online L2 sentence processing, and differences between L2 parsing and learners’ metalinguistic L2 performance

    An iconic language for the graphical representation of medical concepts

    <p>Abstract</p> <p>Background</p> <p>Many medication errors are encountered in drug prescriptions, which would not occur if practitioners could remember the drug properties. They can refer to drug monographs to find these properties, however drug monographs are long and tedious to read during consultation. We propose a two-step approach for facilitating access to drug monographs. The first step, presented here, is the design of a graphical language, called VCM.</p> <p>Methods</p> <p>The VCM graphical language was designed using a small number of graphical primitives and combinatory rules. VCM was evaluated over 11 volunteer general practitioners to assess if the language is easy to learn, to understand and to use. Evaluators were asked to register their VCM training time, to indicate the meaning of VCM icons and sentences, and to answer clinical questions related to randomly generated drug monograph-like documents, supplied in text or VCM format.</p> <p>Results</p> <p>VCM can represent the various signs, diseases, physiological states, life habits, drugs and tests described in drug monographs. Grammatical rules make it possible to generate many icons by combining a small number of primitives and reusing simple icons to build more complex ones. Icons can be organized into simple sentences to express drug recommendations. Evaluation showed that VCM was learnt in 2 to 7 hours, that physicians understood 89% of the tested VCM icons, and that they answered correctly to 94% of questions using VCM (versus 88% using text, <it>p </it>= 0.003) and 1.8 times faster (<it>p </it>< 0.001).</p> <p>Conclusion</p> <p>VCM can be learnt in a few hours and appears to be easy to read. It can now be used in a second step: the design of graphical interfaces facilitating access to drug monographs. It could also be used for broader applications, including the design of interfaces for consulting other types of medical document or medical data, or, very simply, to enrich medical texts.</p

    English in product advertisements in non-english speaking countries in western europe: Product image and comprehension of the text

    Although English has been shown to be the most frequently used foreign language in product advertisements in countries where it is not the native language, little is known about its effects. This article examines the response to advertisements in English compared to the response to the same ad in the local language in Western Europe on members of the target group for which the ad was intended: 715 young, highly educated female consumers. The use of English in a product ad does not appear to have any impact on image and price of the product, but it does affect text comprehension: the meaning of almost 40% of the English phrases was not understood. These results were the same for all countries involved in the study, irrespective of whether the respondents\u27 (self-) reported proficiency in English is high or low. © Taylor & Francis Group, LLC
