    Dynamics of a self--gravitating magnetized source

    We consider a magnetized degenerate gas of fermions as the matter source of a homogeneous but anisotropic Bianchi I spacetime with a Kasner--like metric. We examine the dynamics of this system by means of a qualitative and numerical study of Einstein-Maxwell field equations which reduce to a non--linear autonomous system. For all initial conditions and combinations of free parameters the gas evolves from an initial anisotropic singularity into an asymptotic state that is completely determined by a stable attractor. Depending on the initial conditions the anisotropic singularity is of the ``cigar'' or ``plate'' types.Comment: 7 pages, 1 figur

    Introduction: An aphysiological pattern in computerized dynamic posturography (CDP) may be produced by numerous causes. We analyze the results obtained in this test by a group of malingering patients. Patients and method: This study analyzes a particular group of 7 malingerers, ie patients simulating a false and unreal equilibrium disorder, without any a priori awareness of the gain expected from a pathological report. The condition can only be identified with the very complex postural disorder known as “continuous imbalance.” We analyze the results of the dynamic posturography test following the diagnostic criteria described by other authors. Results: Of the criteria analyzed, we found Cevette’s to be the most frequently positive for our study group. Conclusions: The wide diversity in the criteria used makes it necessary to apply them together, assuming a high level of suspicion and great care in the diagnostic process

    Assessing the performance of a renewable district heating system to achieve nearly zero-energy status in renovated university campuses : a case study for Spain

    This paper presents the implementation of a biomass-fuelled District Heating System (DHS), as a part of a deep energy renovation exercise to achieve a climate-resilient campus with minimum carbon dioxide emissions. The case study is carried out for the University of Valladolid, an average-sized university in Spain, with a continental weather climate. Prior to renovation, the different building blocks had a wide-ranging level of fossil fuel consumption for space heating and domestic hot water ranging between 60 and 430 kWh/m2 ⋅year. The application of this centralised heating system allows to achieve the minimum threshold for near zero-energy buildings (nZEB) of 100–120 kWh/m2 , in accordance with the Spanish Standards. These values correspond to the maximum European indicators for offices in continental weather conditions. Results of this comprehensive study show that 15 out of the 19 buildings reached the nZEB target, due to the proposed strategy. The overall carbon dioxide emissions have dropped by 92.69% as compared to the original fossil-fuel powered boiler, thus bringing carbon dioxide emissions down to 1.57 kgCO2/m2 ⋅y. Therefore, it is shown that deep energy renovation strategies through renewable energy DHS have the potential of achieving nZEB for universities in continental weather conditions.peer-reviewe

    Effectiveness of steroids versus placebo in preventing upper airway obstruction after extubation in critically ill children: rationale and design of a multicentric, double-blind, randomized study

    BACKGROUND: Post-extubation upper airway obstruction (UAO) is a frequent complication causing stridor and respiratory distress, which occasionally require reintubation, thereby increasing morbidity and mortality rates. Contradictory results have been obtained in studies assessing the effectiveness of steroids in preventing post-extubation UAO, and the available evidence is limited. We designed a multicentric randomized, placebo-controlled study to explore the effectiveness of dexamethasone in preventing post-extubation UAO in children. METHODS: A multicentric, prospective, double-blind, randomized, placebo-controlled, phase IV clinical trial has been designed. The sample will include pediatric patients who are between 1 month and 16 years of age and who have been intubated for more than 48 h. Patients who have airway disorders or who have received steroids within the previous seven days will be excluded. Patients will be randomly assigned to receive either placebo or a therapy with dexamethasone 0.25 mg/kg every 6 h to be started 6 to 12 h prior to extubation (to a total of four doses). Randomization will be performed at a 1:1 ratio. Follow-up of patients will be carried out for 48 h after extubation. The main objective of this study is to access the reduction in the incidence of moderate to severe UAO symptoms following extubation. Secondary objectives include assessing the decrease in the incidence of reintubation, evaluating the use of additional therapies for UAO, and monitoring potential side effects of dexamethasone. DISCUSSION: The results of this study will contribute to the existing evidence on prophylaxis for post-extubation airway obstruction. TRIAL REGISTRATION: EudraCT identifier: 2009-016596-30.     Rationale of the association between Mediterranean diet and the risk of frailty in older adults and systematic review and meta-analysis

    Frailty is a geriatric syndrome whose frequency is increasing in parallel with population aging and is of great interest due to its dire consequences: increased disability, hospitalizations, falls and fractures, institutionalization, and mortality. Frailty is multifactorial but nutritional factors, which are modifiable, play a crucial role in its pathogenesis. Epidemiologic evidence supports that high-quality dietary patterns can prevent, delay or even reverse the occurrence of frailty. In order to add new knowledge bridging the gap as the main purpose of the present article we performed a comprehensive review of the rationale behind the association of MedDiet with frailty and a systematic review and meta-analysis updating the latest ones published in 2018 specifically examining the relationship of Mediterranean diet (MedDiet) and incident frailty. Adding the updated information, our results confirmed a robust association of a higher adherence to MedDiet with reduced incident frailty. Key components of the MedDiet, i.e., abundant consumption of vegetables and fruit as well as the use of olive oil as the main source of fat, all of which have been associated with a lower incidence of frailty, may help explain the observed benefit. Future well-designed and sufficiently large intervention studies are needed to confirm the encouraging findings of the current observational evidence. Meanwhile, based on the existing evidence, the promotion of MedDiet, a high-quality dietary pattern, adapted to the conditions and traditions of each region, and considering lifelong and person-tailored strategies, is an open opportunity to reduced incident frailty. This could also help counteract the worrying trend towards the spread of unhealthy eating and lifestyle models such as those of Western diets that greatly contribute to the genesis of chronic non-communicable diseases and disability

    Topology Change in Canonical Quantum Cosmology

    We develop the canonical quantization of a midisuperspace model which contains, as a subspace, a minisuperspace constituted of a Friedman-Lema\^{\i}tre-Robertson-Walker Universe filled with homogeneous scalar and dust fields, where the sign of the intrinsic curvature of the spacelike hypersurfaces of homogeneity is not specified, allowing the study of topology change in these hypersurfaces. We solve the Wheeler-DeWitt equation of the midisuperspace model restricted to this minisuperspace subspace in the semi-classical approximation. Adopting the conditional probability interpretation, we find that some of the solutions present change of topology of the homogeneous hypersurfaces. However, this result depends crucially on the interpretation we adopt: using the usual probabilistic interpretation, we find selection rules which forbid some of these topology changes.Comment: 23 pages, LaTex file.     A Complete Spectroscopic Survey of the Milky Way Satellite Segue 1: The Darkest Galaxy

    We present the results of a comprehensive Keck/DEIMOS spectroscopic survey of the ultra-faint Milky Way satellite galaxy Segue 1. We have obtained velocity measurements for 98.2% of the stars within 67 pc (10 arcmin, or 2.3 half-light radii) of the center of Segue 1 that have colors and magnitudes consistent with membership, down to a magnitude limit of r=21.7. Based on photometric, kinematic, and metallicity information, we identify 71 stars as probable Segue 1 members, including some as far out as 87 pc. After correcting for the influence of binary stars using repeated velocity measurements, we determine a velocity dispersion of 3.7^{+1.4}_{-1.1} km/s, with a corresponding mass within the half-light radius of 5.8^{+8.2}_{-3.1} x 10^5 Msun. The stellar kinematics of Segue 1 require very high mass-to-light ratios unless the system is far from dynamical equilibrium, even if the period distribution of unresolved binary stars is skewed toward implausibly short periods. With a total luminosity less than that of a single bright red giant and a V-band mass-to-light ratio of 3400 Msun/Lsun, Segue 1 is the darkest galaxy currently known. We critically re-examine recent claims that Segue 1 is a tidally disrupting star cluster and that kinematic samples are contaminated by the Sagittarius stream. The extremely low metallicities ([Fe/H] < -3) of two Segue 1 stars and the large metallicity spread among the members demonstrate conclusively that Segue 1 is a dwarf galaxy, and we find no evidence in favor of tidal effects. We also show that contamination by the Sagittarius stream has been overestimated. Segue 1 has the highest measured dark matter density of any known galaxy and will therefore be a prime testing ground for dark matter physics and galaxy formation on small scales.Comment: 24 pages, 4 tables, 11 figures (10 in color).     Tuberculosis transmission patterns among Spanish-born and foreign-born populations in the city of Barcelona

    AbstractDuring a 2-year period (2003–2004), tuberculosis (TB) transmission in Barcelona and the factors related to transmission among the Spanish- and foreign-born populations were studied by molecular epidemiology. Data were obtained from TB cases and Conventional Contact Tracing registries and genotyping was performed using restriction fragment length polymorphism (RFLP)-IS6110 and MIRU12 as a secondary typing method. Of the 892 TB cases reported, 583 (65.3%) corresponded to Spanish-born and 309 (34.6%) to foreign-born. Six hundred and eighty-seven cases (77%) were confirmed by culture. RFLP typing of 463/687 (67.4%) isolates was performed, revealing 280 (60.5%) unique and 183 (39.5%) shared patterns, which were grouped into 65 clusters. Spanish-born individuals were significantly more clustered than foreign-born individuals (44.6% vs. 28.8%; p 0.016). Clustering in foreign-born individuals was associated with HIV (p 0.051, odds ratio = 3.1, 95% confidence interval 1–10.9) and alcohol abuse (p 0.022), whereas, in the Spanish-born individuals, clustering was associated with age in the range 21–50 years, (p 0.024). Of the total clusters, 36/65 (55.3%) included only Spanish-born patients, whereas 22/65 (33.8%) included individuals from both populations. In mixed clusters, the index case was Spanish-born in 53% and foreign-born in 47%. Among the foreign-born, 2.8% were ill on arrival, 30% developed TB within the first year and 50.3% developed TB within the first 2 years; 58.3% were from South America. In conclusion, half of the foreign-born TB patients developed the disease during the first 2 years after arrival, which, in most cases, was the result of endogenous reactivation. Recent TB transmission among Spanish-born and foreign-born populations, as well as bidirectional transmission between communities, contributed significantly to the burden of TB in Barcelona, suggesting the need to improve Public Health interventions in both populations
