    Peripheral neuropathy in patients with long-chain 3-hydroxyacyl-CoA dehydrogenase deficiency - A follow-up EMG study of 12 patients

The acute symptoms of LCHADD are hypoketotic hypoglycemia, failure to thrive, hepatopathy and rhabdomyolysis. Long term complications are retinopathy and neuropathy. Speculated etiology of these long term complications are the accumulation and toxicity of hydroxylacylcarnitines and long-chain fatty acid metabolites or deficiency of essential fatty acids. Aims: To study the possible development of polyneuropathy in LCHADD patients with current dietary regimen. Methods: Development of polyneuropathy in 12 LCHADD patients with the homozygous common mutation c.G1528C was evaluated with electroneurography (ENG) studies. The ENG was done 1-12 times to each patient, between the ages of 3 and 40 years. Clinical data of the patients were collected from the patient records. Results: The first sign of polyneuropathy was detected between the ages of 6-12 years, the first abnormality being reduction of the sensory amplitudes of the sural nerves. With time, progression was detected by abnormalities in sensory responses extending to upper limbs, as well as abnormalities in motor responses in lower limbs. Altogether, eight of the patients had polyneuropathy, despite good compliancy of the diet. Conclusions: This study is the first to report the evolution of polyneuropathy with clinical neurophysiological methods in a relative large LCHADD patient group. Despite early start, and good compliance of the therapy, 6/10 of the younger patients developed neuropathy. However, in most patients the polyneuropathy was less severe than previously described.     Interdisciplinary team education promotes innovations in the home care of older people

    This article describes a new type of team training that involves undergraduate students of medicine, students from the Aalto University (industrial engineering and management, architecture, information networks, collaborative and industrial design and bioinformation technology) and specialized home care nurses. During the course, the students learned interdisciplinary teamwork and created innovations in the care of older people. The 18 participants formed six microteams (three persons in each team: one specialized nurse, one medical student and one from Aalto University). The course consisted of two seminars and 3 full days of home visits to older people's homes. Participants were encouraged to make one innovation in each home visit that would improve the older person's well-being or streamline the processes of home care. During the course, the participants promptly formed tight teams. They valued the know-how of the other team members and learned openly from each other. They also created a number of practical innovations in home care which they presented to executives of older people's care in a final seminar. The course received very good feedback from the students. This course is an encouraging example of how gerontological interdisciplinary team training may be successfully applied. The article describes both the learning outcomes and the innovations the students produced during their home visits. It also discusses the learning theories behind effective interdisciplinary team learning.Peer reviewe

    Teaching Archaeological Heritage Management : Towards a Change in Paradigms

    The concept of archaeological heritage management (AHM) has been key to wider archaeological research and preservation agendas for some decades. Many universities and other education providers now offer what is best termed heritage management education (HME) in various forms. The emphasis is commonly on archaeological aspects of heritage in a broad sense and different terms are often interchangeable in practice. In an innovative working-conference held in Tampere, Finland, we initiated a debate on what the components of AHM as a course or curriculum should include. We brought together international specialists and discussed connected questions around policy, practice, research and teaching/training, at local, national, transnational and World Heritage levels. In this article we take the Tampere discussions further, focusing especially on the meaning, necessity, implications and prerequisites of interdisciplinary HME. We offer our thoughts on developing HME that reflects the contemporary aspects and needs of heritage and its management.Peer reviewe

    Literary tourism in Finland

    This thesis arose from the author’s own interest in the literature, and tourism surrounding it. The objective of this study was to examine the phenomenon of literary tourism in Finland. The phenomenon was studied through different theories related to the subject and by theme interviews. In the theoretical part theories on special interest tourism, cultural tourism, heritage tourism and screen tourism are introduced, as all of them have an important role in explaining literary tourism. Data for this study were collected by theme interviews with professionals working in the literary places in Finland. The empirical part introduces literary places in Finland, of which six were included also in the data collection. The results of this study explain what kind of literary tourism exists in Finland, who the literary tourists are and what kind of literary places can be found in Finland. The results can be used in further research, for developing literary tourism or the literary destinations in Finland
