    Los principales problemas de salud según la opinión de los usuarios

    ObjetivoConocer los problemas de salud o enfermedades que los usuarios de 2 áreas básicas de salud (ABS) consideran más importantes para la población española y para sí mismos; averiguar si existe relación entre estos problemas y los que afectan a miembros de su entorno familiar y social.DiseñoEstudio observacional, transversal, descriptivo.EmplazamientoCuatro y 2 consultas de las ABS Sant Josep (L’Hospitalet de Llobregat) y San Martí (Barcelonés), respectivamente.PacientesSe incluyeron 360 pacientes mayores de 26 años que acudieron a las consultas por algún problema de salud. Los participantes fueron elegidos por muestreo aleatorio sistemático entre los meses de mayo y octubre de 2000.Mediciones y resultados principalesLos datos se obtuvieron a partir de una encuesta con 10 preguntas. En opinión de los participantes, los principales problemas de salud en la población española y para sí mismos fueron: cáncer, enfermedades cardiovasculares y sida. El cáncer (58,61%; IC del 95%, 53,53–63,69) y el sida (15,27%; IC del 95%, 11,56–18,98) fueron los problemas considerados como prioritarios para la investigación. Los problemas del aparato locomotor (22,10%; IC del 95%, 17,82–26,38), la hipertensión (14,74%; IC del 95%, 11,08–18,40) y la diabetes (13,14%, IC del 95%, 9,66–16,62) fueron los principales problemas que afirmaron tener los encuestados.ConclusionesEl cáncer y las enfermedades cardiovasculares se revelan como las que más preocupan a los encuestados y que afectan a más miembros de su entorno familiar y social. En cambio, su preocupación por el sida no refleja la realidad de dicho entorno. Con frecuencia no reconocen el problema de salud que ha motivado su visita como una verdadera enfermedad.ObjectiveTo know the health problems or diseases that patients of 2 basic health areas (BHA) assess as the most important for Spanish population and for themselves; to know if any relation exists between these problems and their existence in the family or social patients’ environment.DesignAn observational cross-sectional and descriptive studySettingFour clinics of the BHA Sant Josep (L´Hospitalet de Llobregat) and 2 clinics of the BHA Sant Martí (Barcelonés).PatientsThe sample consists of 360 patients aged above 26 years who attended clinics for some health problem. Participants were chosen by a randomised systematic sampling, from May to October 2000.Measurements and main resultsData were gathered from a questionnaire of ten items. Acording with the participants, the main problems for Spanish population and for themselves were: cancer, cardiovascular diseases and AIDS. Cancer (58,61%; 95% CI, 53,53–63,69) and AIDS (15,27%; 95% CI, 11,56–18,98) are the problems pointed out as research priorities. The aparato locomotor (22,10%; 95% CI, 17,82–26,38), hypertension (14,74%; 95% CI, 11,08–18,40) and diabetes (13,14%, 95% CI, 9,66–16,62) are the main problems suffered by the surveyed. Cancer is the disease that more participants’ relatives suffered.ConclusionsCancer and cardiovascular diseases are the pathologies that cause more concern among the surveyed and these are the diseases which mostly affect their relatives and relationships. Nevertheless their worry for the AIDS don´t show their immediate reality. Frequently, patients don´t recognize the health problem that motivated their visit as a real disease

    Key Rotation for Authenticated Encryption

    A common requirement in practice is to periodically rotate the keys used to encrypt stored data. Systems used by Amazon and Google do so using a hybrid encryption technique which is eminently practical but has questionable security in the face of key compromises and does not provide full key rotation. Meanwhile, symmetric updatable encryption schemes (introduced by Boneh et al. CRYPTO 2013) support full key rotation without performing decryption: ciphertexts created under one key can be rotated to ciphertexts created under a different key with the help of a re-encryption token. By design, the tokens do not leak information about keys or plaintexts and so can be given to storage providers without compromising security. But the prior work of Boneh et al. addresses relatively weak confidentiality goals and does not consider integrity at all. Moreover, as we show, a subtle issue with their concrete scheme obviates a security proof even for confidentiality against passive attacks. This paper presents a systematic study of updatable Authenticated Encryption (AE). We provide a set of security notions that strengthen those in prior work. These notions enable us to tease out real-world security requirements of different strengths and build schemes that satisfy them efficiently. We show that the hybrid approach currently used in industry achieves relatively weak forms of confidentiality and integrity, but can be modified at low cost to meet our stronger confidentiality and integrity goals. This leads to a practical scheme that has negligible overhead beyond conventional AE. We then introduce re-encryption indistinguishability, a security notion that formally captures the idea of fully refreshing keys upon rotation. We show how to repair the scheme of Boneh et al., attaining our stronger confidentiality notion. We also show how to extend the scheme to provide integrity, and we prove that it meets our re- encryption indistinguishability notion. Finally, we discuss how to instantiate our scheme efficiently using off-the-shelf cryptographic components (AE, hashing, elliptic curves). We report on the performance of a prototype implementation, showing that fully secure key rotations can be performed at a throughput of approximately 116 kB/s

    Promoting workplace-based training to fight youth unemployment in three EU countries: different strategies, different results?

    During the economic crisis, youth unemployment grew exponentially in many European countries. It was argued that countries with a high level of firm involvement in the provision of initial vocational training were better equipped to address this problem. Boosting workplace‐based training was therefore seen as the right strategy to tackle unemployment. Using Denmark, Spain and the UK as case studies, this article analyses how countries with different skill formation systems have improved this type of training. While the UK reinforced the voluntaristic character of its training regime, Denmark improved the quality of its vocational education, and Spain made reforms to the training and apprenticeship contract. Interestingly, the countries achieved different results. To explain this divergence, it is argued that while the reforms made in the UK and Denmark were compatible with the national institutions and coordination mechanisms, this was not the case in Spain, where reforms were implemented in a non‐complementary way.info:eu-repo/semantics/acceptedVersio

    Making Password Authenticated Key Exchange Suitable For Resource-Constrained Industrial Control Devices

    Connectivity becomes increasingly important also for small embedded systems such as typically found in industrial control installations. More and more use-cases require secure remote user access increasingly incorporating handheld based human machine interfaces, using wireless links such as Bluetooth. Correspondingly secure operator authentication becomes of utmost importance. Unfortunately, often passwords with all their well-known pitfalls remain the only practical mechanism. We present an assessment of the security requirements for the industrial setting, illustrating that offline attacks on passwords-based authentication protocols should be considered a significant threat. Correspondingly use of a Password Authenticated Key Exchange protocol becomes desirable. We review the signif-icant challenges faced for implementations on resource-constrained devices. We explore the design space and shown how we succeeded in tailoring a partic-ular variant of the Password Authenticated Connection Establishment (PACE) protocol, such that acceptable user interface responsiveness was reached even for the constrained setting of an ARM Cortex-M0+ based Bluetooth low-energy transceiver running from a power budget of 1.5 mW without notable energy buffers for covering power peak transients

    Deterministic Encoding and Hashing to Odd Hyperelliptic Curves

    The original publication is available at www.springerlink.comInternational audienceIn this paper we propose a very simple and efficient encoding function from Fq to points of a hyperelliptic curve over Fq of the form H : y2 = f(x) where f is an odd polynomial. Hyperelliptic curves of this type have been frequently considered in the literature to obtain Jacobians of good order and pairing-friendly curves. Our new encoding is nearly a bijection to the set of Fq -rational points on H . This makes it easy to construct well-behaved hash functions to the Jacobian J of H , as well as injective maps to J (Fq ) which can be used to encode scalars for such applications as ElGamal encryption. The new encoding is already interesting in the genus 1 case, where it provides a well-behaved encoding to Joux?s supersingular elliptic curves

    Spatial variation of trace metals within intertidal beds of native mussels (Mytilus edulis) and non-native Pacific oysters (Crassostrea gigas): implications for the food web?

    Abstract Pollution is of increasing concern within coastal regions and the prevalence of invasive species is also rising. Yet the impact of invasive species on the distribution and potential trophic transfer of metals has rarely been examined. Within European intertidal areas, the non-native Pacific oyster (Crassostrea gigas) is becoming established, forming reefs and displacing beds of the native blue mussel (Mytilus edulis). The main hypothesis tested is that the spatial pattern of metal accumulation within intertidal habitats will change should the abundance and distribution of C. gigas continue to increase. A comparative analysis of trace metal content (cadmium, lead, copper and zinc) in both species was carried out at four shores in south-east England. Metal concentrations in bivalve and sediment samples were determined after acid digestion by inductively coupled plasma-optical emission spectrometry. Although results showed variation in the quantities of zinc, copper and lead (mg m-2) in the two bivalve species, differences in shell thickness are also likely to influence the feeding behaviour of predators and intake of metals.     Antiplatelet therapy with aspirin, clopidogrel, and dipyridamole versus clopidogrel alone or aspirin and dipyridamole in patients with acute cerebral ischaemia (TARDIS): a randomised, open-label, phase 3 superiority trial

    Background: Intensive antiplatelet therapy with three agents might be more effective than guideline treatment for preventing recurrent events in patients with acute cerebral ischaemia. We aimed to compare the safety and efficacy of intensive antiplatelet therapy (combined aspirin, clopidogrel, and dipyridamole) with that of guideline-based antiplatelet therapy. Methods: We did an international, prospective, randomised, open-label, blinded-endpoint trial in adult participants with ischaemic stroke or transient ischaemic attack (TIA) within 48 h of onset. Participants were assigned in a 1:1 ratio using computer randomisation to receive loading doses and then 30 days of intensive antiplatelet therapy (combined aspirin 75 mg, clopidogrel 75 mg, and dipyridamole 200 mg twice daily) or guideline-based therapy (comprising either clopidogrel alone or combined aspirin and dipyridamole). Randomisation was stratified by country and index event, and minimised with prognostic baseline factors, medication use, time to randomisation, stroke-related factors, and thrombolysis. The ordinal primary outcome was the combined incidence and severity of any recurrent stroke (ischaemic or haemorrhagic; assessed using the modified Rankin Scale) or TIA within 90 days, as assessed by central telephone follow-up with masking to treatment assignment, and analysed by intention to treat. This trial is registered with the ISRCTN registry, number ISRCTN47823388. Findings: 3096 participants (1556 in the intensive antiplatelet therapy group, 1540 in the guideline antiplatelet therapy group) were recruited from 106 hospitals in four countries between April 7, 2009, and March 18, 2016. The trial was stopped early on the recommendation of the data monitoring committee. The incidence and severity of recurrent stroke or TIA did not differ between intensive and guideline therapy (93 [6%] participants vs 105 [7%]; adjusted common odds ratio [cOR] 0·90, 95% CI 0·67–1·20, p=0·47). By contrast, intensive antiplatelet therapy was associated with more, and more severe, bleeding (adjusted cOR 2·54, 95% CI 2·05–3·16, p<0·0001). Interpretation: Among patients with recent cerebral ischaemia, intensive antiplatelet therapy did not reduce the incidence and severity of recurrent stroke or TIA, but did significantly increase the risk of major bleeding. Triple antiplatelet therapy should not be used in routine clinical practice
