    3-(1-Ethyl-4-hydroxy-2-oxo-2(1H)-quinolin-3-yl)-3-oxopropanoic acid (2) has been synthesized. The chemical behaviour of β-ketoacid 2 was studied towards condensation reactions with salicylaldelyde, 2-hydroxy-1-naphthaldehyde, 1-phenyl-4-hydroxy-2-oxo-quinoline-3-carboxaldehyde, 2-hydroxy-4-oxo-4H-pyrido[1,2-a]pyrimidine-3-carboxalde-hyde, 2-amino-3-formylchromone and its 8-allyl analog, 3-cyanochromone and its 8-allyl analog.     Flow Cytometry of Breast Cancer Resistant Protein and microRNA in Breast Cancer Patients Post Metformin Effect

    The goal of the present study is to investigate the role of metformin (MF) as a target of miRNAs in breast cancer resistant protein (BCRP) inhibition in an attempt to develop treatment strategies that may improve the response of breast cancer (BC) patients to chemotherapy (CT). In order to fulfill this target, non-diabetic female subjects were categorized into three groups: control group (group 1) (n=5), CT group of BC patients (group 2) (n=25) and CT plus MF group of BC patients (group 3) (n=25). All patients were subjected to full history taking, laboratory studies including mammogram, chest X-ray, pelvic-abdominal ultrasound and isotopic bone scan, in addition to ER and PR states. CT group was treated with neoadjuvent CT in the form of 5-FU (500 mg/m2), Adriamycin (50 mg/m2) and cyclophosphamide (500 mg/m2). Flow cytometry (FC) of BCRP and MiRNA was carried out on blood samples at every cycle of treatment for all partners. The results showed the presence of miRNA was higher than the presence of BCRP in the normal healthy control group. In most cases of CT and CT plus MF groups (group 2, 3) it was well noticed that the amount of BCRP in the blood samples exceeded that of miRNA illustrated the dysregulation of miRNA in BC patients and also to prove the basic role of BCRP as a multidrug resistance (MDR) for chemotherapeutic agents in patients with BC. It is concluded that the role of MF was well proved in targeting of miRNA to reinforce BC medication, so oncologists can be advised to use MF equivalent to CT in the recommended doses

    Reliable Machine Learning Model for IIoT Botnet Detection

    Due to the growing number of Internet of Things (IoT) devices, network attacks like denial of service (DoS) and floods are rising for security and reliability issues. As a result of these attacks, IoT devices suffer from denial of service and network disruption. Researchers have implemented different techniques to identify attacks aimed at vulnerable Internet of Things (IoT) devices. In this study, we propose a novel features selection algorithm FGOA-kNN based on a hybrid filter and wrapper selection approaches to select the most relevant features. The novel approach integrated with clustering rank the features and then applies the Grasshopper algorithm (GOA) to minimize the top-ranked features. Moreover, a proposed algorithm, IHHO, selects and adapts the neural network’s hyper parameters to detect botnets efficiently. The proposed Harris Hawks algorithm is enhanced with three improvements to improve the global search process for optimal solutions. To tackle the problem of population diversity, a chaotic map function is utilized for initialization. The escape energy of hawks is updated with a new nonlinear formula to avoid the local minima and better balance between exploration and exploitation. Furthermore, the exploitation phase of HHO is enhanced using a new elite operator ROBL. The proposed model combines unsupervised, clustering, and supervised approaches to detect intrusion behaviors. The N-BaIoT dataset is utilized to validate the proposed model. Many recent techniques were used to assess and compare the proposed model’s performance. The result demonstrates that the proposed model is better than other variations at detecting multiclass botnet attacks

    Effectiveness of tranexamic acid in preventing postpartum

    Background: Caesarean section delivery is associated with severe maternal morbidity, including obstetric haemorrhage, hysterectomy, anaemia, blood transfusion, and infection. Among these operative morbidities associated with CS, obstetric haemorrhage is the leading cause of maternal mortality worldwide.Objective: The aim of this work was to achieve the minimal blood loss during elective cesarean section (CS) in order to decrease patients' morbidity by using tranexamic acid (TXA) injection before operation time.Patients and Methods: The current study was randomized-controlled clinical trial that was conducted at Department of Obstetrics and Gynecology, Zagazig University Hospitals through the period from April 2021 to September 2022.Results: The mean of blood loss during CS in tranexamic acid intervention group was 484.87 cc and mean of blood loss during CS in control group was 705 cc. The difference was highly statistically significant p=0.0001. Per cent of blood loss was 37% more among control group.Conclusions. Tranexamic acid is a good option to reduce the amount of blood loss during CS on high risk pregnancy

    Une action concertée du microARN miR-134 et de la protéine LIM Kinase 1 dans la modulation de la douleur neuropathique dans la moelle épinière du rat

    Les douleurs neuropathiques ayant une origine à la suite de blessures traumatiques du SNC ou du SNP sont particulièrement difficiles à traiter en utilisant les moyens thérapeutiques actuellement disponibles. Il est donc nécessaire d'identifier de nouvelles stratégies thérapeutiques. Notre objectif était donc de définir les mécanismes impliqués dans ces douleurs neuropathiques. LIMK1 est l'un des acteurs possibles de la réorganisation épinière qui caractérise les lésions nerveuses. Une fonction très caractérisé de cette protéine, est la phosphorylation d'une famille de protéines appelées cofilines . Sa phosphorylation, ce qui induit la réorganisation du cytosquelette d'actine. Récemment, il a été montré qu un microARN (miARNs) nomé miR-134 régule l'expression de LIMK1 en se liant au messager de LIMK1 (ARNm), inhibant sa traduction en protéine physiologiquement active. Notre hypothèse était que la régulation de LIMK1 par miR-134 pourrait jouer un rôle essentiel dans la sensibilisation à la douleur. Cette régulation pourrait ainsi être liée non seulement à la modulation neurochimique neuronale mais aussi à la plasticité fonctionnelle associée. Au cours de cette thèse, l HIS a montré une diminution de miR-134 chez des rats SNL (neuropathique), cette sous-expression était concomitante à une augmentation de LIMK1 illustrée par l IHC. Il est important de noter ici que l'ISH est une méthode de détection connue récemment et qui a été identifiée pour visualiser les miARNs. Des différents protocoles de l HIS ont également été discutés dans le cadre de cette thèse. Ce résultat a été confirmé par Le qRT-PCR . Par la suite, afin de vérifier les changements comportementaux douloureux induits par miR-134 et LIMK1. Nous avons effectués des injections intrathécales de siRNA anti-LIMK1 pour inhiber l'expression endogène de LIMK1 chez les SNL. C était intéressant de ne pas avoir trouvé aucun changement comportemenal chez les SNL après ce type d injection. Une surexpression artificielle de miR-134 en utilisant un précurseur de miR-134 (premiR-134) chez les SNL a montré le même effet. Ensuite, nous avons essayé d'effectuer les mêmes injections chez les Sham (control), et c était plus intéressant de trouver que ces injections (siRNA LIMK1 et premiR-134) ont provoqué une hypersensibilité douleureuse chez les sham. Cela a été illustré au moyen de deux tests de comportement; le Von Frey (VF) et la distribution pondérale dynamique (DWB). Pour etudier l'effet inverse, nous avons inhibé miR-134 en utilisant une sonde spécifique KD (Knock-Down); une diminution significative inattendue dans le seuil de retrait a été observée avec VF et DWB. qRT-PCR dans la plupart de ces cas, a confirmé la corrélation in vivo entre miR-134 et LIMK1. Enfin, nous avons cherché un mécanisme d'action possible qui pourrait réguler cette modulation. Des données récentes publiées ont montré une implication de l'ADF/cofiline sur le trafic des récepteurs AMPA (AMPAR). En accord avec les résultats mentionnés ci-dessus, la transfection du KD de miR-134 a montré une diminution dans AMPAR adressés à la membrane plasmique. Tout ensemble ces données suggèrent que l'effet antinociceptif de KD de miR-134 et la surexpression de LIMK1 sont indirectement régulé par l'insertion des AMPAR à la membrane plasmique.Il semble que miR-134 exerce un effet différent sur la douleur neuropathique que miR-103, discuté aussi dans le cadre de cette thèse. Il était demontré comme un régulateur de plusieurs cibles, les trois sous-unités formant les canaux calciques de type-L Cav1.2 LTC . MiR-103 a été trouvé également réprimés chez les SNL. La surexpression de miR-103 soulage la douleur neuropathique. Contrairement au miR-134, miR-103 exerce un rôle pronociceptive pendant la douleur neuropathique.Pains having a neuropathic origin following CNS or PNS traumatic injury are particularly difficult to treat using the actually available therapeutic means. It is thus necessary to identify new therapeutic strategies. Hence, our aim was to define the mechanisms implicated in these neuropathic pains. Nervous lesions are characterised by an anatomical reorganization of the neuronal network of the dorsal horn. Neurochemical alterations are also involved. Some of the molecular mechanisms underlying the neuronal plasticity (a main feature of neuropathic pain) have been emphasized here by a variety of complementary technical approaches. LIMK1 is one of the possible actors of this reorganization. Among this protein s known functions, and the most characterized is the phosphorylation of a family of proteins known as cofilins. Their phosphorylation induces the reorganization of actin cytoskeleton. Recently, it has been shown that a miR-134 miRNA regulates LIMK1 expression by binding to the LIMK1 messenger, inhibiting its translation into physiologically active protein. Our hypothesis is that LIMK1 regulation by miR-134 might play an essential role in pain sensitization by modulating neuron neurochemical reorganization and the associated functional neuronal plasticity. Firstly, by means of IHC and ISH, we studied miR-134/LIMK1 distribution within the dorsal horn of the spinal cord in sham animal (control group) and in neuropathic pain model (SNL model). Important to note here that ISH is a known detection method recently identified to visualize miRNA. Different protocols of ISH were discussed in a part of this thesis. ISH showed a decrease in miR-134 expression in SNL rats concomitantly with an increase in LIMK1 illustrated by IHC. This finding has been confirmed by qRT-PCR techniques. Afterward, in order to check for the possible behavioural-induced changes of miR-134 and LIMK1. We intrathecally injected an anti-LIMK1 siRNA to inhibit endogenous LIMK1 expression in SNL rats. Interestingly no significant changes in pain behaviour have been observed. Artificial overexpression of miR-134 using a PremiR-134, showed the same effect. Then we tried to perform the same injections on sham rats, and more interestingly, siRNA LIMK1 and premiR-134 evoked pain hypersensitivity in shams rats. This was illustrated by means of two behaviour tests; Von Frey (VF) and the Dynamic Weight bearing (DWB). To explore the reverse effect, we inhibited miR-134 using a specific KD probe in SNL rats; unexpectedly a significant decrease in pain withdrawal threshold was observed with VF and DWB. qRT-PCR in most cases confirmed the in vivo correlation between miR-134 and LIMK1. Finally, we searched for the possible mechanism of action that could regulate this modulation. Recent published data showed an involvement of ADF/cofilin on AMPAR trafficking. In line with the above mentioned findings, miR-134 KD transfection showed a decrease in AMPAR addressed to the plasma membrane. Altogether suggest that the antinociceptive effect of miR-134 KD and LIMK1 overexpression are mediated by AMPAR insertion at the plasma membrane. It seems that miR-134 exerts a different effect on neuropathic pain than miR-103 another miRNA discussed within the frame of this thesis. MiR-103 has been proved to regulate multiple targets, the three subunits forming Cav1.2 LTC. Pain sensitization involves Cav1.2 activation which consequently alters gene expression during this form of plasticity. MiR-103 was found downregulated also in the SNL model. Conversely to miR-134, overexpression of miR-103 partially alleviates pain. It decreases pain withdrawal threshold of the Von Frey test.     A model study of corona emission from hydrometeors

    The maximum measured electric fields in thunderclouds are an order of magnitude less than the fields required for electric breakdown of the air. One explanation for lightning initiation in these low fields is that electric breakdown first occurs at the surfaces of raindrops where the ambient field is enhanced very locally due to the drop geometry. Laboratory experiments [Crabb & Latham, 1974] indicate that colliding raindrops which coalesce to form elongated water filaments can produce positive corona in ambient fields close to those measured in thunderclouds. We calculate the E-field distribution around a simulated coalesced drop pair and use a numerical model to study the positive corona mechanisms in detail. Our results give good agreement with the laboratory observations.     Endogenous bone marrow stem cell mobilization in rats: Its potential role in homing and repair of damaged inner ear

    AbstractThe stem cells are widely used in the last few years in different fields of medicine, either by external transplantation or endogenous mobilization, most of these studies are still experimental on animals; few were tried on humans as in the spinal cord injury or myocardial infarction. As regards its use in the inner ear, stem cell transplantation was examined in many previous studies, while the mobilization idea is a new method to be experimented in inner ear hair cell regeneration.The aim of this work was to assess the use of GCSF to induce bone marrow SC mobilization to home and repair the damaged inner ear hair cells in rats after the damage had been inflicted by Amikacin injection.First we had to develop an animal model for damaged inner ear, so we used the well known ototoxic effect of Amikacin intra-tympanic injection till we reached complete loss of hearing function. And this can be assessed by DPOAEs. Now we have an animal model of damaged inner ear hair cells and mobilization of bone marrow SCs was induced by subcutaneous injection of G-CSF for 5days.Clinical assessment was done using DPOAEs before and after G-CSF injection also histological assessment of the inner ear was done before and after G-CSF by two methods H&E staining and Scanning Electron Microscopy.The results of histological examination of the specimens showed some promising changes that may be responsible for the improvement of hearing function in some rats that was detected by OAEs

    Correlation of polycystic ovary syndrome and recurrent miscarriage

    Background: Numerous studies have investigated the association between polycystic ovary syndrome (PCOS) and recurrent miscarriage (RM). Increased rates of PCOS have been reported for women with RM. However, the actual prevalence is controversially discussed by the scientific community and remains unclear, since a wide range of rates from 8–82% can be found in the literature. Objective: To assess the relation between polycystic ovary syndrome and recurrent miscarriage and to evaluate the prevalence of polycystic ovarian syndrome within the recurrent miscarriage population. Patients and method: It is a cross sectional conducted at the infertility clinic in the outpatient department of University Hospital from March 2019 to March 2020. Patient sample was 47 participant. Hormonal profile FSH, LH, TSH, TSH, assessment of glycemic status and insulin resistance and ultrasonography for diagnosis of polycystic ovaries were done to every patients. Result: There was high statistically significant relation between the PCO and total number of follicles, number of follicles ≥ 18 mm and endometrial thickness. There was high statistically significant relation between the PCO and testosterone hormones and statistically significant relation between the PCO and FSH and LH. There was no statistically significant relation between the PCO and insulin resistance. Conclusion: In this study we concluded that the prevalence of PCOS seems slightly increased in women with recurrent miscarriage, PCOS on the other hand showed a rather high prevalence compared to the general population
