A systematic review of therapeutic interventions to reduce acute and chronic post-surgical pain after amputation, thoracotomy or mastectomy

BACKGROUND: Perioperative neuropathic pain is under-recognized and often undertreated. Chronic pain may develop after any routine surgery, but it can have a far greater incidence after amputation, thoracotomy or mastectomy. The peak noxious barrage due to the neural trauma associated with these operations may be reduced in the perioperative period with the potential to reduce the risk of chronic pain. DATABASES AND DATA TREATMENT: A systematic review of the evidence for perioperative interventions reducing acute and chronic pain associated with amputation, mastectomy or thoracotomy. RESULTS: Thirty-two randomized controlled trials met the inclusion criteria. Gabapentinoids reduced pain after mastectomy, but a single dose was ineffective for thoracotomy patients who had an epidural. Gabapentinoids were ineffective for vascular amputees with pre-existing chronic pain. Venlafaxine was associated with less chronic pain after mastectomy. Intravenous and topical lidocaine and perioperative EMLA (eutectic mixture of local anaesthetic) cream reduced the incidence of chronic pain after mastectomy, whereas local anaesthetic infiltration appeared ineffective. The majority of the trials investigating regional analgesia found it to be beneficial for chronic symptoms. Ketamine and intercostal cryoanalgesia offered no reduction in chronic pain. Total intravenous anaesthesia (TIVA) reduced the incidence of post-thoracotomy pain in one study, whereas high-dose remifentanil exacerbated chronic pain in another. CONCLUSIONS: Appropriate dose regimes of gabapentinoids, antidepressants, local anaesthetics and regional anaesthesia may potentially reduce the severity of both acute and chronic pain for patients. Ketamine was not effective at reducing chronic pain. Intercostal cryoanalgesia was not effective and has the potential to increase the risk of chronic pain. TIVA may be beneficial but the effects of opioids are unclear

Vector Bin Packing with Multiple-Choice

We consider a variant of bin packing called multiple-choice vector bin packing. In this problem we are given a set of items, where each item can be selected in one of several $D$-dimensional incarnations. We are also given $T$ bin types, each with its own cost and $D$-dimensional size. Our goal is to pack the items in a set of bins of minimum overall cost. The problem is motivated by scheduling in networks with guaranteed quality of service (QoS), but due to its general formulation it has many other applications as well. We present an approximation algorithm that is guaranteed to produce a solution whose cost is about $\ln D$ times the optimum. For the running time to be polynomial we require $D=O(1)$ and $T=O(\log n)$. This extends previous results for vector bin packing, in which each item has a single incarnation and there is only one bin type. To obtain our result we also present a PTAS for the multiple-choice version of multidimensional knapsack, where we are given only one bin and the goal is to pack a maximum weight set of (incarnations of) items in that bin

Engineering Branch-and-Cut Algorithms for the Equicut Problem

A minimum equicut of an edge-weighted graph is a partition of the nodes of the graph into two sets of equal size such hat the sum of the weights of edges joining nodes in different partitions is minimum. We compare basic linear and semidefnite relaxations for the equicut problem, and and that linear bounds are competitive with the corresponding semidefnite ones but can be computed much faster. Motivated by an application of equicut in theoretical physics, we revisit an approach by Brunetta et al. and present an enhanced branch-and-cut algorithm. Our computational results suggest that the proposed branch-andcut algorithm has a better performance than the algorithm of Brunetta et al.. Further, it is able to solve to optimality in reasonable time several instances with more than 200 nodes from the physics application

Lonsdalea quercina (ex Brenneria quercina) subsp. populi es una bacteria recientemente identificada en Hungría y China como una nueva subespecie causante del chancro de la corteza del chopo. Los árboles afectados presentan chancros longitudinales en el tronco, de los que fluyen abundantes exudados espumosos de color blanco y exudados acuosos oscuros que tiñen la corteza. Los chopos afectados muestran un debilitamiento generalizado. En España se han localizado choperas con síntomas similares y se ha identificado L. quercina subsp. populi como el agente causal de los mismos. En este trabajo se presenta la primera detección de esta bacteria en España y se describen los métodos de diagnóstico e identificación utilizados. Esta nueva bacteriosis podría tener un impacto económico importante, siendo necesario evaluar las pérdidas causadas y realizar nuevas prospecciones para determinar su distribución en el país

Re-storying and visualizing the changing entrepreneurial identities of Bill Gates and Richard Branson.

The storytelling in textual and visual re-constructions of Bill Gates and Richard Branson by their organizations produces entrepreneurial identities bound into particular social power-knowledge relations. Our purpose is to examine how these organizations, and their critics, mobilize storytelling in acts of re-storying (enlivening) or re-narrating (branding a monologic) practices using Internet technologies to invite viewers to frame the world of entrepreneurship. We use visual discourse and storytelling methods to analyze how Microsoft and Virgin Group use various kinds of entrepreneurial images and textual narratives to re-narrate and produce particular brands of capitalism. These organizations' scoptic regimes of representation are contested in counter-visualizing and counterstory practices of external stakeholders. We suggest that the image and textual practices of storytelling have changed as both entrepreneurs court philanthropic and social entrepreneur identity markers. Our contribution to entrepreneurial identity is to apply double and multiple narrations, the appropriation of another's narrative words (or images) into another's narrative, and relate such storytelling moves to visuality

Collider aspects of flavour physics at high Q

This review presents flavour related issues in the production and decays of heavy states at LHC, both from the experimental side and from the theoretical side. We review top quark physics and discuss flavour aspects of several extensions of the Standard Model, such as supersymmetry, little Higgs model or models with extra dimensions. This includes discovery aspects as well as measurement of several properties of these heavy states.
