    The impact of face coverings on audio-visual contributions to communication with conversational speech

    The use of face coverings can make communication more difficult by removing access to visual cues as well as affecting the physical transmission of speech sounds. This study aimed to assess the independent and combined contributions of visual and auditory cues to impaired communication when using face coverings. In an online task,150 participants rated videos of natural conversation along three dimensions: (1) how much they could follow, (2) how much effort was required, and (3) the clarity of the speech. Visual and audio variables were independently manipulated in each video, so that the same video could be presented with or without a superimposed surgical-style mask, accompanied by one of four audio conditions (either unfiltered audio, or audio-filtered to simulate the attenuation associated with a surgical mask, an FFP3 mask, or a visor). Hypotheses and analyses were pre-registered. Both the audio and visual variables had a statistically significant negative impact across all three dimensions. Whether or not talkers’ faces were visible made the largest contribution to participants’ ratings. The study identifies a degree of attenuation whose negative effects can be overcome by the restoration of visual cues. The significant effects observed in this nominally low-demand task (speech in quiet) highlight the importance of the visual and audio cues in everyday life and that their consideration should be included in future face mask designs

    The impact of resources on decision making

    Decision making is a significant activity within industry and although much attention has been paid to the manner in which goals impact on how decision making is executed, there has been less focus on the impact decision making resources can have. This article describes an experiment that sought to provide greater insight into the impact that resources can have on how decision making is executed. Investigated variables included the experience levels of decision makers and the quality and availability of information resources. The experiment provided insights into the variety of impacts that resources can have upon decision making, manifested through the evolution of the approaches, methods, and processes used within it. The findings illustrated that there could be an impact on the decision-making process but not on the method or approach, the method and process but not the approach, or the approach, method, and process. In addition, resources were observed to have multiple impacts, which can emerge in different timescales. Given these findings, research is suggested into the development of resource-impact models that would describe the relationships existing between the decision-making activity and resources, together with the development of techniques for reasoning using these models. This would enhance the development of systems that could offer improved levels of decision support through managing the impact of resources on decision making

    Pharmacists' Perspectives on the Use of My Health Record

    Background: My Health Record (MHR) is a relatively new nationwide Australian digitalhealth record system accessible by patients and a range of healthcare professionals. Pharmacists willbe key contributors and users of the MHR system, yet little is known about the perceived barriers andbenefits of use. (2) Objective: To explore pharmacists’ perspectives related to potential benefits andbarriers associated with use of MHR. (3) Methods: An online survey was developed and face-validated.The survey was advertised to Australian pharmacists on pharmacy professional bodies’ websites. Thiswas a cross-sectional study using an anonymous questionnaire. Descriptive statistics were used todescribe the distribution of the data. Chi-square, Kendall’s tau coefficient (tau-c) and Kruskal–Wallistests were used to examine the relationships where appropriate. (4) Results: A total of 63 pharmacistscompleted the survey. The majority of respondents worked in a metropolitan area (74%), and themost common workplace setting was community pharmacy (65%). Perceived benefits identifiedby responders include that the use of MHR would help with continuity of care (90%), and that itwould improve the safety (71%) and quality (75%) of care they provided. Importantly, more thanhalf of pharmacists surveyed agreed that MHR could reduce medication errors during dispensing(57%) and could improve professional relationships with patients (57%) and general practitioners(59%). Potential barriers identified by pharmacists included patients’ concerns about privacy (81%),pharmacists’ own concern about privacy (46%), lack of training, access to and confidence in using thesystem. Sixty six percent of respondents had concerns about the accuracy of information containedwithin MHR, particularly among hospital and general practice pharmacists (p = 0.016) and almosthalf (44%) had concerns about the security of information in the system, mainly pharmacists workingat general practice and providing medication review services (p= 0.007). Overall satisfaction withMHR varied, with 48% satisfied, 33% neither satisfied nor dissatisfied, and 19% dissatisfied, witha higher satisfaction rate among younger pharmacists (p = 0.032). (5) Conclusions: Pharmacistsconsidered that the MHR offered key potential benefits, notably improving the safety and quality ofcare provided. To optimize the use of MHR, there is a need to improve privacy and data securitymeasures, and to ensure adequate provision of user support and education surrounding the ability tointegrate use of MHR with existing workflows and software

    Structure of the regulatory hyaluronan binding domain in the inflammatory leukocyte homing receptor CD44

    Adhesive interactions involving CD44, the cell surface receptor for hyaluronan, underlie fundamental processes such as inflammatory leukocyte homing and tumor metastasis. Regulation of such events is critical and appears to be effected by changes in CD44 N-glycosylation that switch the receptor "on" or "off" under appropriate circumstances. How altered glycosylation influences binding of hyaluronan to the lectin-like Link module in CD44 is unclear, although evidence suggests additional flanking sequences peculiar to CD44 may be involved. Here we show using X-ray crystallography and NMR spectroscopy that these sequences form a lobular extension to the Link module, creating an enlarged HA binding domain and a formerly unidentified protein fold. Moreover, the disposition of key N-glycosylation sites reveals how specific sugar chains could alter both the affinity and avidity of CD44 HA binding. Our results provide the necessary structural framework for understanding the diverse functions of CD44 and developing novel therapeutic strategies

    Using saturated models for data synthesis

    The use of synthetic data sets are becoming ever more prevalent, as regulations such as the General Data Protection Regulation (GDPR), which place greater demands on the protection of individuals’ personal data, are coupled with the conflicting demand to make more data available to researchers. This paper discusses the approach of synthesizing categorical data at the aggregated (contingency table) level using a saturated count model, which adds noise - and hence protection - to cell counts. The paper also discusses how distributional properties of synthesis models are intrinsic to generating synthetic data with suitable risk and utility profiles

    On integrating the number of synthetic data sets m into the a priori synthesis approach

    The synthesis mechanism given in Jackson et al. (2022) uses saturated models, along with overdispersed count distributions, to generate synthetic categorical data. The mechanism is controlled by tuning parameters, which can be tuned according to a specific risk or utility metric. Thus expected properties of synthetic data sets can be determined analytically a priori, that is, before they are generated. While Jackson et al. (2022) considered the case of generating m = 1 data set, this paper considers generating m > 1 data sets. In effect, m becomes a tuning parameter and the role of m in relation to the risk-utility trade-off can be shown analytically. The paper introduces a pair of risk metrics, τ3(k,d) and τ4(k,d) that are suited to m > 1 data sets; and also considers the more general issue of how best to analyse categorical data sets: average the data sets pre-analysis or average results post-analysis. Finally, the methods are demonstrated empirically with the synthesis of a constructed data set which is used to represent the English School Census
