    The impact and process of a community-led intervention on reducing environmental inequalities related to physical activity and healthy eating - a pilot study

    Background: There is growing recognition that a sedentary lifestyle is being driven, at least in part, by environmental factors that affect individuals’ physical activity choices and health behaviours. In other words, the environments in which we live, and with which we interact, have become ones that encourage lifestyle choices that decrease physical activity and encourage over-consumption of foods. However, evidence from community-led interventions to change local neighbourhood environments to support physical activity and healthy eating is lacking. This article summarises the research protocol developed to evaluate a community-led intervention “My Health Matters” aimed at reducing health inequalities relating to increasing physical activity and healthy eating as defined by community members themselves. Methods/Design: This study includes three of the most deprived electoral wards in Stoke-on-Trent. In each of these areas, environmental factors including proximity of physical activity spaces, greenspace and leisure facilities, neighbourhood connectivity and walkability, land-use-mix and population density, traffic, safety and crime, and food outlets will be mapped using Geographical Information Systems (GIS). A community postal survey of randomly selected addresses assessing environmental characteristics relating to physical activity, perceived health status, social capital, fruit and vegetable consumption and levels of physical activity will be undertaken (baseline and at 2 year follow-up). Based on baseline findings an intervention will be designed and implemented over a 2 year period that includes the following; use of community participatory research to build effective community partnerships; use of partnership consensus to identify, prioritise and design intervention(s) related to specific health disparities; recruitment of local residents to help with the delivery and sustainability of target intervention(s); and the development of local systems for ongoing monitoring and evaluation of the intervention(s). Discussion: A community-led and multidisciplinary approach to modifying environmental factors that support and reinforce healthful behaviours may be more successful than focusing on individual behaviour change as this approach does not exclusively rely upon individual will and capacity. Study findings will be collated in 2012 and, if successful in improving levels of physical activity and healthy eating, will help to inform the design of a larger area-based, cluster randomized controlled trial to determine effectiveness

    A qualitative study of gambling, deprivation and monetary motivations

The current study sought to learn more about whether individuals who gamble consciously perceive themselves to be motivated by feelings of deprivation, and how this is contextualized alongside monetary factors more broadly. Thematic analysis was conducted on 25 in-depth qualitative interviews with UK residents who gamble regularly; most of whom resided in areas of high socio-economic deprivation. Monetary themes relating to financial circumstances, the meaning and value of money, and the perception of gambling as a way to make money, all had strong relevance for deprivation, though people did not often endorse the idea that relative deprivation was important to them, per se. We conclude that gambling motivations are complex and heterogeneous, and that it is pertinent for prevention and intervention strategies for problem gambling to consider individuals’ financial circumstances and how they perceive them, along with how this intersects with their gambling motives

    The relationship between surrounding greenness, stress and memory

    Evidence suggests that surrounding greenness is associated with multiple health-related benefits, including better cognitive functioning. Underlying mechanisms of the relationship between exposure to natural environments and cognitive functioning have not been widely researched. This study aimed to analyse the relationship between surrounding greenness and memory performance, and to explore the potential mediating role of stress. A sample of 185 adults was recruited in Stoke-on-Trent and Newcastle-under-Lyme (UK). Data were collected on exposure to and use of natural environments, stress, three measures of memory performance (short-term memory, working memory, overall memory), and participant socio-demographics. Linear univariate regression was conducted to investigate the relationship between surrounding greenness, memory performance and stress. Mediation analysis was conducted to investigate the role of stress as mediator of the relationship between surrounding greenness and memory performance. Surrounding greenness was significantly associated with better memory performance and lower levels of stress, and lower levels of stress were significantly associated with better memory performance. Stress was a significant partial mediator of the relationship between surrounding greenness and short-term memory, and between surrounding greenness and working memory. One explanation for these findings is that stress is a multifaceted reaction to a demand which involves cognitive functioning, so that less stress might lead to improved cognition. These results suggest that cognitive benefits of exposure to surrounding greenness are partially mediated by lower levels of stress. Future research should consider other potential mediators of the relationships between surrounding greenness and cognitive functioning, such as mood, well-being and social relationships

    Does the Health Impact of Exposure to Neighbourhood Green Space Differ between Population Groups? An Explorative Study in Four European Cities

    It has been suggested that certain residents, such as those with a low socioeconomic status, the elderly, and women, may benefit more from the presence of neighbourhood green space than others. We tested this hypothesis for age, gender, educational level, and employment status in four European cities. Data were collected in Barcelona (Spain; n = 1002), Kaunas (Lithuania; n = 989), Doetinchem (The Netherlands; n = 847), and Stoke-on-Trent (UK; n = 933) as part of the EU-funded PHENOTYPE project. Surveys were used to measure mental and general health, individual characteristics, and perceived neighbourhood green space. Additionally, we used audit data about neighbourhood green space. In Barcelona, there were positive associations between neighbourhood green space and general health among low-educated residents. In the other cities and for the other population groups, there was little evidence that the association between health and neighbourhood green space differed between population groups. Overall, our study does not support the assumption that the elderly, women, and residents who are not employed full-time benefit more from neighbourhood green space than others. Only in the highly urbanised city of Barcelona did the low-educated group benefit from neighbourhood green spaces. Perhaps neighbourhood green spaces are more important for the health of low-educated residents in particularly highly urbanised areas

    Chromenopyrazole, a Versatile Cannabinoid Scaffold with in Vivo Activity in a Model of Multiple Sclerosis

    A combination of molecular modeling and structure activity relationship studies has been used to fine-tune CB2 selectivity in the chromenopyrazole ring, a versatile CB1/CB2 cannabinoid scaffold. Thus, a series of 36 new derivatives covering a wide range of structural diversity has been synthesized, and docking studies have been performed for some of them. Biological evaluation of the new compounds includes, among others, cannabinoid binding assays, functional studies, and surface plasmon resonance measurements. The most promising compound [43 (PM226)], a selective and potent CB2 agonist isoxazole derivative, was tested in the acute phase of Theiler's murine encephalomyelitis virus-induced demyelinating disease (TMEV-IDD), a well established animal model of primary progressive multiple sclerosis. Compound 43 dampened neuroinflammation by reducing microglial activation in the TMEV

    The relationship between natural outdoor environments and cognitive functioning and its mediators

    Background Urban residents may experience cognitive fatigue and little opportunity for mental restoration due to a lack of access to nature. Natural outdoor environments (NOE) are thought to be beneficial for cognitive functioning, but underlying mechanisms are not clear. Objectives To investigate the long-term association between NOE and cognitive function, and its potential mediators. Methods This cross-sectional study was based on adult participants of the Positive Health Effects of the Natural Outdoor Environment in Typical Populations in Different Regions in Europe (PHENOTYPE) project. Data were collected in Barcelona, Spain; Doetinchem, the Netherlands; and Stoke-on-Trent, United Kingdom. We assessed residential distance to NOE, residential surrounding greenness, perceived amount of neighborhood NOE, and engagement with NOE. Cognitive function was assessed with the Color Trails Test (CTT). Mediation analysis was undertaken following Baron and Kenny. Results Each 100 m increase in residential distance to NOE was associated with a longer CTT completion time of 1.50% (95% CI 0.13, 2.89). No associations were found for other NOE indicators and cognitive function. Neighborhood social cohesion was (marginally) significantly associated with both residential distance to NOE and CTT completion time, but no evidence for mediation was found. Nor were there indications for mediation by physical activity, social interaction with neighbors, loneliness, mental health, air pollution worries, or noise annoyance. Conclusions Our findings provide some indication that proximity to nature may benefit cognitive function. We could not establish which mechanisms may explain this relationship

    Development of the natural environment scoring tool (NEST)

    Natural environments (green and blue space) are associated with a range of health benefits, but their use is likely to be influenced by the presence of features, facilities and amenities and the condition/maintenance, or the natural environment quality. Most 'quality' assessment tools have focused on green spaces and their support for physical activity. This limits their utility for assessment of other natural environment typologies and uses (e.g., social, relaxation). We aimed to develop a tool for feasible, in situ assessment of diverse natural environments that might support a variety of uses, and to explore associations between natural environment quality and objectively measured amount of natural environment and neighbourhood-level socio-economic status (SES). This work was conducted as part of the PHENOTYPE project. Data were collected in 124 neighbourhoods in four European cities (Barcelona, Doetinchem, Kaunas, Stoke-on-Trent). The Natural Environment Scoring Tool (NEST) was developed using existing tools, expert input and field-testing. The final tool comprised 47-items across eight domains: Accessibility, Recreation facilities, Amenities, Aesthetics- natural, Aesthetics- non-natural, Significant natural features, Incivilities and Usability; typology-specific Overall Scores were derived. In total, 174 natural environments, covering a range of typologies, were audited. Mean time to complete NEST was 16±28 min. There was good inter-rater agreement. Mean domain scores showed some expected patterns by typology (e.g., higher Recreation Facilities scores in urban parks and formal recreation areas; lower Amenities scores in natural/semi-natural areas). Highest mean Overall Scores were observed for areas of blue space and woodland, the types of area that often lack the recreational facilities or amenities that can be dominant in physical activity-focused audit tools. There was a trend towards lower natural environment quality in neighbourhoods of lower SES, with some inter-city variation. Correlations between NEST scores and amount of natural environment indicated higher natural environment in areas with worse access. We recommend further testing of NEST in other locations in relation to use and health outcomes
