Factors Influencing Performance of Cholangioscopy-Guided Lithotripsy Including Available Different Technologies: A Prospective Multicenter Study with 94 Patients
Background: Peroral cholangioscopy (POC)-guided lithotripsy is an effective treatment for difficult biliary stones. A clear definition of factors associated with the efficacy of POC-guided lithotripsy in one session and the performance of electrohydraulic lithotripsy (EHL) and laser lithotripsy (LL) have not clearly emerged.
Methods: This was a non-randomized prospective multicenter study of all consecutive patients who underwent POC lithotripsy (using EHL and/or LL) for difficult biliary stones. The primary endpoint of the study was the number of sessions needed to achieve complete ductal clearance and the factors associated with this outcome. Secondary endpoints included the evaluated efficacies of LL and EHL.
Results: Ninety-four patients underwent 113 procedures of EHL or LL. Complete ductal clearance was obtained in 93/94 patients (98.94%). In total, 80/94 patients (85.11%) achieved stone clearance in a single session. In the multivariate analysis, stone size was independently associated with the need for multiple sessions to achieve complete ductal clearance (odds ratio = 1.146, 95% confidence interval: 1.055-1.244; p = 0.001). Using ROC curves and the Youden index, 22 mm was found to be the optimal cutoff for stone size (95% confidence interval: 15.71-28.28; p < 0.001). The majority of the patients (62.8%) underwent LL in the first session. Six patients failed the first session with EHL after using two probes and therefore were crossed over to LL, obtaining ductal clearance in a single additional session with a single LL fiber. EHL was significantly associated with a larger number of probes (2.0 vs. 1.02) to achieve ductal clearance (p < 0.01). The mean procedural time was significantly longer for EHL than for LL [72.1 (SD 16.3 min) versus 51.1 (SD 10.5 min)] (p < 0.01).
Conclusions: POC is highly effective for difficult biliary stones. Most patients achieved complete ductal clearance in one session, which was significantly more likely for stones < 22 mm. Fisher's perceptions about a marine protected area over time
The perceptions of fishers towards the Arrábida Marine Park, a marine protected area (MPA) in the west coast of
Portugal, were studied through face-to-face interviews in two different moments of the MPA life cycle. Fishers'
perceptions about the MPA and the impact it had on the fishing activity over time were identified just before the
full implementation of the zoning and regulations of the management plan and 10 years later. This study aimed
to investigate fishers' knowledge, acceptance and perceptions about the MPA changed with time, if support for
the MPA was linked to the impact of the MPA on the fishing activity, and if fishers' perceptions about the impact
of the MPA on the fishing activity match with local landings trends. Results show that although knowledge about
the marine park significantly improved over time, fishers' acceptance did not. A decrease on fishers' support was
not substantial but occurred. Issues such as the disagreement with regulations reinforced concerns raised during
the implementation of the marine park, particularly in relation to the top-down decision-making, which commonly
confers minor participation, recognition and legitimacy to fishers. Apparently, fisheries benefits were still
not perceived by local fishers, though they are central for fishers' support. Further, the perceived negative
impacts of the park seemed to be more related to social aspects and individual interests than to impacts on
catches. Addressing adequate management, enforcement and participation of local fishers is still possible and are
advocated here as to contribute to the expected socioecological outcomes and respective support, leading to the
future successful performance of the Arrábida Marine Park. Assessing fishers’ perceptions towards an MPA over

Co-design of a marine protected area zoning and the lessons learned from it
Marine Protected Areas (MPAs) are a tool to safeguard marine natural systems,
yet their effectiveness depends on how well they are integrated into the existing
socioeconomic context. Stakeholder engagement in MPA design can
contribute to increasing integration. This study focuses on the co-design of
an MPA between researchers, public administration, the private sector, and
non-governmental organizations. The proposed MPA is in Portugal and
includes an area that is a hotspot for biodiversity and economic activities.
This is the first MPA proposal in mainland Portugal co-designed using a
participatory approach. This study highlights the steps of the zoning process
and synthesizes the eight main lessons learned, useful for other cases,
particularly for relatively small coastal MPAs with multiple socioeconomic
activities. Three zoning proposals were developed and discussed within the
participatory process. The proposals considered the best scientific and local
knowledge available and were defined using ecological, socioeconomic, and
shape-area guiding principles. In an iterative manner and following a
participatory approach, compromises with stakeholders were achieved, and a
final proposal, scientifically sound and socially accepted by most stakeholders,
was delivered to the government. The final zoning plan will achieve ambitious
conservation goals, including the largest fully protected area to be declared in
mainland Portugal, while minimizing the impacts on the existing economic activities and promoting its sustainability. This process resulted in valuable
lessons that may be applied elsewhere and guide future MPA implementation
or rezoning of existing ones. These transdisciplinary and participatory
processes can be time and resource-consuming but are vital for ensuring

Setting performance indicators for coastal marine protected areas: An expert-based methodology
Marine Protected Areas (MPAs) require effective indicators to assess their performance, in
compliance with the goals of relevant national and international commitments. Achieving
and prioritizing shortlists of multidisciplinary indicators demands a significant effort from
specialists to depict the multiple conservation and socioeconomic interests, and the large
complexity of natural systems. The present paper describes a structured expert-based
methodology (process and outputs) to co-define a list of multidisciplinary MPA
performance indicators. This work was promoted by the management authority of
coastal MPAs in mainland Portugal to gather a consensual and feasible list of indicators
that would guide the design of a future national monitoring program. Hence, Portuguese
coastal MPAs served as a case study to develop such a process between 2019 and 2020.
In the end, participants (1) agreed on a shortlist of prioritized indicators (i.e., environmental,
governance, and socioeconomic indicators) and (2) defined minimum monitoring
frequencies for the indicators in this list, compatible with the potential replicability of the
associated survey methods. The present approach recommends that management plans
incorporate monitoring procedures and survey methods, with a validated list of indicators
and associated monitoring periodicity, agreed among researchers, MPA managers and
governance experts. The proposed methodology, and the lessons learned from it, can

Brazil in the Era of Fascism: The “New State” of Getúlio Vargas
The New State established in Brazil by Getúlio Vargas (1937–1945) is the most important case of the institutionalisation of a dictatorship of the fascism era in Latin America. During this time, an impressive spectrum of authoritarian regimes was established, some of which were very instable and poorly institutionalised, while others were more consolidated. Roger Griffin coined the concept of para-fascism for some of them, and the “New State” of Getúlio Vargas in Brazil is a paradigmatic case. Seagrass and submerged aquatic vegetation (VAS) habitats off the Coast of Brazil: state of knowledge, conservation and main threats
Seagrass meadows are among the most threatened ecosystems on earth, raising concerns about the equilibrium of coastal ecosystems and the sustainability of local fisheries. The present review evaluated the current status of the research on seagrasses and submerged aquatic vegetation (SAV) habitats off the coast of Brazil in terms of plant responses to environmental conditions, changes in distribution and abundance, and the possible role of climate change and variability. Despite an increase in the number of studies, the communication of the results is still relatively limited and is mainly addressed to a national or regional public; thus, South American seagrasses are rarely included or cited in global reviews and models. The scarcity of large-scale and long-term studies allowing the detection of changes in the structure, abundance and composition of seagrass habitats and associated species still hinders the investigation of such communities with respect to the potential effects of climate change. Seagrass meadows and SAV occur all along the Brazilian coast, with species distribution and abundance being strongly influenced by regional oceanography, coastal water masses, river runoff and coastal geomorphology. Based on these geomorphological, hydrological and ecological features, we characterised the distribution of seagrass habitats and abundances within the major coastal compartments. The current conservation status of Brazilian seagrasses and SAV is critical. The unsustainable exploitation and occupation of coastal areas and the multifold anthropogenic footprints left during the last 100 years led to the loss and degradation of shoreline habitats potentially suitable for seagrass occupation. Knowledge of the prevailing patterns and processes governing seagrass structure and functioning along the Brazilian coast is necessary for the global discussion on climate change. Permanent education in health: a review
OBJECTIVE : To undertake a meta-synthesis of the literature on the main concepts and practices related to permanent education in health. METHODS : A bibliographical search was conducted for original articles in the PubMed, Web of Science, LILACS, IBECS and SciELO databases, using the following search terms: “public health professional education”, “permanent education”, “continuing education”, “permanent education health”. Of the 590 articles identified, after applying inclusion and exclusion criteria, 48 were selected for further analysis, grouped according to the criteria of key elements, and then underwent meta-synthesis. RESULTS : The 48 original publications were classified according to four thematic units of key elements: 1) concepts, 2) strategies and difficulties, 3) public policies and 4) educational institutions. Three main conceptions of permanent education in health were found: problem-focused and team work, directly related to continuing education and education that takes place throughout life. The main strategies for executing permanent education in health are discussion, maintaining an open space for permanent education, and permanent education clusters. The most limiting factor is mainly related to directly or indirect management. Another highlight is the requirement for implementation and maintenance of public policies, and the availability of financial and human resources. The educational institutions need to combine education and service aiming to form critical-reflexive graduates. CONCLUSIONS : The coordination between health and education is based as much on the actions of health services as on management and educational institutions. Thus, it becomes a challenge to implement the teaching-learning processes that are supported by critical-reflexive actions. It is necessary to carry out proposals for permanent education in health involving the participation of health professionals, teachers and educational institutions.OBJETIVO : Realizar metasíntesis de la literatura sobre los principales conceptos y prácticas relacionados con la educación permanente en salud. MÉTODOS : Se realizó búsqueda bibliográfica de artículos originales en las bases de datos PubMed, Web of Science, Lilacs, IBECS y SciELO, utilizando los siguientes descriptores: “ public health professional education”, “permanent education”, “continuing education”, “permanent education health ”. De un total de 590 artículos identificados, posterior a los criterios de inclusión y exclusión, fueron seleccionados 48 para análisis, los cuales fueron sometidos al análisis individual, análisis comparativo, análisis con criterios de agrupamiento de elementos-clave y sometidos a metasíntesis. RESULTADOS : Los 48 artículos originales fueron clasificados como elementos-clave en cuatro unidades temáticas: 1) Concepciones; 2) Estrategias y dificultades; 3) Políticas públicas e 4) Instituciones formadoras. Se encontraron tres concepciones principales de educación permanente en salud: ubicación del problema y enfocarlo en el trabajo en equipo, directamente relacionado con la educación continua y educación que se da a lo largo de la vida. Las principales estrategias para efectivar la educación permanente fueron la ubicación del problema, mantenimiento de espacios para la educación permanente y polos de educación permanente. El mayor factor limitante estuvo relacionado con la gerencia directa o indirecta. Fueron mencionadas la necesidad de implementación y mantenimiento de políticas públicas, así como la disponibilidad de recursos financieros y humanos. Las instituciones formadoras tendrían la necesidad de articular educación y servicio para la formación de egresados críticos-reflexivos. CONCLUSIONES : La articulación educación y salud se encuentra pautada tanto en las acciones de los servicios de salud, cuanto en la gestión y de instituciones formadoras. Así, se torna un desafío implementar procesos de educación-aprendizaje que sean respaldados por acciones crítico-reflexivas. As instituições formadoras teriam necessidade de articular ensino e serviço para a formação de egressos críticos-reflexivos. CONCLUSÕES : A articulação educação e saúde encontra-se pautada tanto nas ações dos serviços de saúde, quanto de gestão e de instituições formadoras. Assim, torna-se um desafio implementar processos de ensino-aprendizagem que sejam respaldados por ações crítico-reflexivas.
