    Gastrointestinal Tract Multiple Lymphomatous Polyposis Presented As Mantle Cell Lymphoma

    DergiPark: 640245tmsjAims: Mantle cell lymphoma is a mature B cell non-Hodgkin lymphoma which may be presented with the involvement of the gastrointestinal tract as multiple lymphomatous polyposis. The aim of this case report is to increase the awareness of including lymphomatoid polyposis as an entity in the differential diagnosis of multiple polyposis of the gastrointestinal tract. Case Report: A 69-year-old male patient was admitted to the Trakya University Emergency Department with acute abdominal pain. His cli-nical findings were anorexia that started 3-4 months ago together with constipation and nausea causing him to lose 10-15 kg in 7-8 months, with denial of other parameters of B-symptoms (fevers and night sweats). Endoscopic biopsies that were taken from bulbus and duodenum were investigated and he was diagnosed with mantle cell lymphoma. The patient went through an ileoce-cal resection due to his intussusception that caused abdominal pain in the first place. Conclusion: Although being an infrequent disease, gastrointestinal lymphomatoid polyposis should be an entity comprised in differential diagnosis for multiple polyposis of the gastrointestinal tract. On the other hand, there is still not a therapeutic protocol with a definitive cure for gastrointestinal tract mantle cell lymphoma. Elderly patients in high risk group such as our patient should be given treatment by taking their conditions into consideration. Keywords: Mantle cell lymphoma, polyp, non-Hodgkin lymphom

    Formant frequencies of vowels in 13 accents of the British Isles

    International audienceThis study is a formant-based investigation of the vowels of male speakers in 13 accents of the British Isles. It provides F1/F2 graphs (obtained with a semi-automatic method) which could be used as starting points for more thorough analyses. The article focuses on both phonetic realization and systemic phenomena, and it also provides detailed information on automatic formant measurements. The aim is to obtain an up-to-date picture of within-and between-accent vowel variation in the British Isles. F1/F2 graphs plot z-scored Bark-transformed formant frequencies, and values in Hertz are also provided. Along with the findings, a number of methodological issues are addressed

    Estonia between the two world wars was among the few societies in Europe to have a strong eugenics movement (eugenics society was established in 1924) and to accept eugenic legislation (Sterilisation Law from the year 1936). The roots of the eugenics ideology reach into the pre-independence era, when, in the end of the 19th and the beginning of the 20th century the Estonian national discourse was notoriously biologized owing to the so-called small nation’s self perception, highlighting, and both the quality and quantity issues of the "national body". The birth of the eugenics ideology in Estonia can be linked to the developments in the local abstinence movement. The latter, which emerged in the 1880s, became by the early 20th century medically oriented and dominated by the leaders of the national movement who tried to use the initiative for political goals. The latter aspect gave the movement a paternalist accent, alienating the national elites from the masses involved in the movement. There was also a gap within the ideological framework of the movement itself, taking place between the proponents of the so-called direct and indirect anti-alcohol work. The first, representing mainly the modest and liberal section of the national elites, stressed the people’s free will and personal choice. The second wing, deriving from radical intellectuals, many of which later turned to solidarism, favoured the imposing of limitations and regulations by the society to keep people away from alcohol. This approach was criticized by the liberals for the paternalist approach it was containing, rendering human beings to plainly biological entities. This also played an important role in the increasing significance of the anti-alcohol movement in supporting the emergence of a eugenic society. The Estonian eugenics movement and the ideology it was carrying reached its peak in 1934 when the state followed an autocratic/totalitarian path of development. The concept supporting antidemocratic developments was “national entirety”, the latter containing a notorious degree of solidarist (biologized) approach.     Das Recht auf Selbstbestimmung und somit ebenfalls auf selbstbestimmtes Wohnen gehört zum Leben eines jeden Menschen. Dies gilt selbstverständlich auch für Menschen mit intellektueller Beeinträchtigung. Die Gewährleistung dieses Rechtes ist jedoch nicht immer gegeben. Aus diesem Grund wurde in der vorliegenden Arbeit den Fragen nachgegangen, was rechtliche Rahmenbedingungen für selbstbestimmtes Wohnen für Menschen mit intellektueller Beeinträchtigung sind, auf welchen Überlegungen entsprechende Gesetzestexte basieren, wie diese in ihrer Handhabung in den verschiedenen österreichischen Bundesländern aussehen und ob dadurch tatsächlich eine selbstbestimmte Lebensführung für Menschen mit intellektueller Beeinträchtigung im Lebensbereich Wohnen ermöglicht wird. Am Beispiel des Landes Oberösterreich sollte versucht werden festzustellen, wie wirksam nationale Gesetze auf Landesebene sind. Dies sollte mittels leitfadengestützter Interviews mit Menschen mit intellektueller Beeinträchtigung geklärt werden. Analysiert wurden die Interviews anhand der qualitativen Inhaltsanalyse nach Mayring. Anhand dieser Gespräche konnte herausgefunden werden, dass die interviewten Personen selbst kein Wissen über rechtliche Maßnahmen und Möglichkeiten haben. D.h. die intellektuell beeinträchtigten Menschen wussten selbst nicht über die ihnen zustehenden Möglichkeiten bescheid. Sie mussten sich dabei auf ihre Betreuer, Sachwalter und Angehörigen verlassen. Ob somit in diesem Zusammenhang überhaupt von Wirksamkeit der Gesetze gesprochen werden kann, ist fraglich.     Myeloid Sarcoma of the Skin in a Patient with Myelodysplastic Syndrome

    ABSTRACT We report the case of a 76-year-old woman who presented with asymptomatic extensive erythematous. Firm plaques were noted over the right cheek. Complete blood count was normal, as was a peripheral smear. An excision biopsy taken from the cheek showed infiltration of the dermis and hypodermis with atypical cells which were strongly positive for human leukocyte antigen (HLA-DR) and lysozyme and were moderately myeloperoxidase (MPO) enzyme. The results of immunohistochemical staining for CD34, CD117, CD3, CD4, CD8, CD20, CD23, CD56, and ALK-1 were negative. Bone marrow analysis indicated myelodysplastic syndrome RAEB 1 while cytogenetic finding showed tetrasomy 8. It was recommended that the patient undergo local radiotherapy of skin lesions, but she refused and was lost to follow-up. KEY WORDS: skin; myeloid sarcoma; myelodysplastic syndrome </p
