Adaptive Segmentation of Knee Radiographs for Selecting the Optimal ROI in Texture Analysis
The purposes of this study were to investigate: 1) the effect of placement of
region-of-interest (ROI) for texture analysis of subchondral bone in knee
radiographs, and 2) the ability of several texture descriptors to distinguish
between the knees with and without radiographic osteoarthritis (OA). Bilateral
posterior-anterior knee radiographs were analyzed from the baseline of OAI and
MOST datasets. A fully automatic method to locate the most informative region
from subchondral bone using adaptive segmentation was developed. We used an
oversegmentation strategy for partitioning knee images into the compact regions
that follow natural texture boundaries. LBP, Fractal Dimension (FD), Haralick
features, Shannon entropy, and HOG methods were computed within the standard
ROI and within the proposed adaptive ROIs. Subsequently, we built logistic
regression models to identify and compare the performances of each texture
descriptor and each ROI placement method using 5-fold cross validation setting.
Importantly, we also investigated the generalizability of our approach by
training the models on OAI and testing them on MOST dataset.We used area under
the receiver operating characteristic (ROC) curve (AUC) and average precision
(AP) obtained from the precision-recall (PR) curve to compare the results. We
found that the adaptive ROI improves the classification performance (OA vs.
non-OA) over the commonly used standard ROI (up to 9% percent increase in AUC).
We also observed that, from all texture parameters, LBP yielded the best
performance in all settings with the best AUC of 0.840 [0.825, 0.852] and
associated AP of 0.804 [0.786, 0.820]. Compared to the current state-of-the-art
approaches, our results suggest that the proposed adaptive ROI approach in
texture analysis of subchondral bone can increase the diagnostic performance
for detecting the presence of radiographic OA
Early detection of knee osteoarthritis using deep learning on knee magnetic resonance images
The aim of this study was to investigate the influence of MRI and patient
data on the prediction of knee osteoarthritis (OA) incidence using different
deep learning architectures. Knee OA incidence within 24 months was predicted
using the intermediate-weighted turbo spin-echo (IW-TSE) sequence of 593
patients from the Osteoarthritis Initiative. To extract a region of interest
containing the knee joint from the IW-TSE sequence, a U-Net model was trained
and used to segment bone on a dual echo steady state (DESS) sequence.
Subsequently, IW-TSE and DESS sequences were registered and the DESS
segmentations were transformed to the corresponding IW-TSE scans. The
performance of MRI-based features in the prediction of knee OA incidence was
tested using three different deep learning architectures: a residual network
(ResNet), a densely connected convolutional network (DenseNet), and a
convolutional variational autoencoder (CVAE). To evaluate the predictive
performance of MRI-based features alone, the outputs of ResNet, DenseNet, and
CVAE were coupled with patient data (i.e., age, gender, BMI) and used as input
to a Logistic Regression (LR) Classifier. Knee OA was defined based on visual
MRI and X-ray-based OA features. The ResNet and DenseNet showed similar
results, with both methods having the area under the receiver operating
characteristic curve (AUC) values up to 0.6269. The best performing OA
detection model was CVAE with an AUC of 0.6699 when combined with patient data
and an AUC of 0.6689 when used alone as input to the LR classifier. The results
showed that three deep learning algorithms have similar metrics when using
IW-TSE MRIs and their performance increased with the inclusion of patient data,
which shows the strong influence of variables such as age, gender, and BMI on
the detection of knee OA

Oulun Omahoitopalvelu on Oulun kaupungin tarjoama ja kansallisesti palkittu vuorovaikutteinen terveydenhuollon internet-palvelu, joka tukee kuntalaisten omahoitoa ajasta ja paikasta riippumatta. Omahoitopalvelu antaa uudenlaisia mahdollisuuksia hyvinvoinnin ylläpitoon, terveyden edistämiseen ja sairauksien hoitoon. Omahoitopalvelu tukee kuntalaisten voimaantumista ja aktivoi heitä toimimaan hoitoprosessissa yhteistyössä ammattilaisen kanssa. Omahoitopalvelun kehittämisen myötä ammattilaiset ovat saaneet uuden sähköisen työvälineen päivittäiseen työhön. Hoitohenkilöstö käyttää Omahoitopalvelua asiakasohjauksen tukena, erilaisissa ryhmätoiminnoissa, asiakkaiden kotimittausten seurannassa sekä kiireettömien kysymysten vastaamisessa. Omahoitohankkeessa on kehitetty myös uudenlaisia toimintatapoja tuotetestaukseen Kaakkurin kaupunginosassa sijaitsevaan Teknologiaterveyskeskus Ouluun. Uudessa teknologiapainotteisessa terveyskeskuksessa hyvinvointiteknologiayritykset voivat testata tuotteitaan aidossa perusterveydenhuollon ympäristössä. Omahoitohankekokonaisuuden kehittämistyöstä saatuja hyviä käytäntöjä tullaan hyödyntämään Oulun kaupungissa vuoden 2009 aikana käynnistyneissä Pisara- ja TERO-hankkeissa
Itsemyötätunnon yhteys tunteiden säätelyyn ja mielenterveyden oireisiin
Tiivistelmä. Itsemyötätunto tarkoittaa kärsimyksen huomaamista itsessä sekä halua ja pyrkimystä lievittää kärsimystä. Vaikka itsemyötätunnon hyödyistä mielenterveydelle on jo paljon näyttöä, on taustalla oleva mekanismi edelleen tuntematon. Itsemyötätunnon vaikutusta tunteiden säätelyyn on ehdotettu yhdeksi mahdolliseksi mekanismiksi itsemyötätunnon ja mielenterveyden välillä.
Tavoitteena tässä kandidaatintutkielmassa oli tarkastella, miten itsemyötätunto on yhteydessä tunteiden säätelyyn. Lisäksi tavoitteena oli tarkastella, onko itsemyötätunto yhteydessä mielenterveyden oireisiin ja mielenterveyden häiriöihin, erityisesti depressioon ja ahdistuneisuushäiriöihin, tunteiden säätelyn kautta. Menetelmänä käytettiin kuvailevaa kirjallisuuskatsausta. Kirjallisuuskatsaukseen koottiin tuoreita, vertaisarvioituja tutkimuksia, joissa oli tutkittu itsemyötätunnon ja tunteiden säätelyn yhteyttä ja/tai tunteiden säätelyä välittävänä tekijänä itsemyötätunnon ja mielenterveyden oireiden välillä.
Kirjallisuuskatsauksen perusteella itsemyötätunto on negatiivisessa yhteydessä useiden epäadaptiivisten tunteiden säätelystrategioiden käytön kanssa ja positiivisessa yhteydessä joidenkin adaptiivisten tunteiden säätelystrategioiden käytön kanssa. Itsemyötätunto on myös negatiivisessa yhteydessä erilaisiin vaikeuksiin tunteiden säätelyssä, kuten vaikeuksiin tunteiden tiedostamisessa, erottamisessa, hyväksymisessä ja tehokkaiden tunteiden säätelystrategioiden saavuttamisessa. Lisäksi itsemyötätunto on yhteydessä korkeampaan sykevälivaihteluun, minkä on esitetty olevan tunteiden säätelykykyä heijastava fysiologinen indeksi. Kirjallisuuskatsauksessa saatiin myös viitteitä siitä, että itsemyötätunto olisi yhteydessä mielenterveyden oireisiin joidenkin tunteiden säätelystrategioiden kautta. Suurin osa kirjallisuuskatsaukseen kootuista tutkimuksista on kuitenkin poikkileikkaustutkimuksia, mikä rajoittaa kausaalisuuspäätelmiä. Lisäksi suurimmassa osassa tutkimuksista koehenkilöinä on ollut opiskelijoita, mikä rajoittaa tulosten yleistämistä laajemmin väestöön tai mielenterveyden häiriöistä kärsiviin
Happamat sulfaattimaat Perämerenkaaren alueella ja niiden haittojen ehkäiseminen maa- ja metsätaloudessa
Hankkeessa koottujen kartoitustulosten perusteella happamia sulfaattimaita esiintyy melko yleisesti, pääasiassa jokiuomien varrella aina 90 metrin korkeustasoon saakka. Paikoin happamat sulfaattimaat levittäytyvät laajoiksi yhtenäisiksi alueiksi topografialtaan alaville maille. Esiintymisessä voi olla kuitenkin suurta vaihtelua pienelläkin alueella. Kartoitetusta alueesta Tornionjoella n. 40 % on happamia sulfaattimaita, Kemijoella 35 %, Kaakamojoella 40 %, Simojoella 20 % ja Perämeren rannikkoalueella 10 %. Suurin osa (87 %) sulfidikerroksista alkaa 1–2 metrin syvyydellä. Simojoella ja Perämeren rannikkoalueella sulfidikerrostumia esiintyy lähempänä maanpintaa kuin muilla valuma-alueilla. %. Perinteisten hienorakeisten happamien sulfaattimaiden lisäksi esiintyy myös karkeampia happamia hietoja ja hiekkoja, lähinnä Simojokilaaksossa, Kemijoen suistoalueella sekä mahdollisesti mustaliuskeisiin liittyen Tornion ja Ylitornion rajalla Martimossa.
Laadittaessa kunnostusojitussuunnitelmaa Litorina-alueella on tärkeää selvittää happamien sul-faattimaiden esiintymisen todennäköisyys esim. Geologian tutkimuskeskuksen kartta-aineiston avulla. Karttaselvityksen ohella riskialueilla on aina syytä tarkistaa maastossa sulfidikerrosten esiintyminen ja esiintymissyvyys. Esiintymien syvyyden perusteella päätetään, voidaanko kunnostusojitus toteuttaa normaalisti vai vaaditaanko kaivutyössä ja vesiensuojelussa lisätoimia. Jos muuttumattomat sulfidikerrokset ovat alle 1 metrin syvyydessä, tulee ojien kunnostus tehdä eritysen huolella ja jättää ojat selvästi tavoitesyvyyttä matalemmiksi (0,5 – 0,8 m). Vesiensuojelu toteutetaan pintavalutuskentillä. Mikäli sulfidikerrostumat ovat yli 1,5 metrin syvyydessä, voidaan ojien kunnostus toteuttaa muutoin normaalisti, mutta lasketusaltaita ei tule kaivaa. Kaivun aikana voidaan kuormitusta vähentää jättämällä kaivukatkoja ja säätämällä tarvittaessa kaivusyvyyttä matalammaksi. Myös metsänuudistamisen yhteydessä tehtävän maanmuokkauksen suunnittelussa tulee selvittää, onko muokkauskohde riskialueella. Soistuneiden kankaiden mätästyksessä tai säätöaurauksessa pintaan nostettu kivennäismaa kuivuu ja hapettuu. Tällaisten uudistusalojen osalta on syytä arvioida kuormitusriskit samalla tavalla kuin kunnostusojituksissa.201
Rapid CT-based Estimation of Articular Cartilage Biomechanics in the Knee Joint Without Cartilage Segmentation
Knee osteoarthritis (OA) is a painful joint disease,
causing disabilities in daily activities. However, there is no
known cure for OA, and the best treatment strategy might be
prevention. Finite element (FE) modeling has demonstrated
potential for evaluating personalized risks for the progression
of OA. Current FE modeling approaches use primarily
magnetic resonance imaging (MRI) to construct personalized
knee joint models. However, MRI is expensive and has lower
resolution than computed tomography (CT). In this study,
we extend a previously presented atlas-based FE modeling
framework for automatic model generation and simulation
of knee joint tissue responses using contrast agent-free CT. In
this method, based on certain anatomical dimensions measured from bone surfaces, an optimal template is selected and
scaled to generate a personalized FE model. We compared
the simulated tissue responses of the CT-based models with
those of the MRI-based models. We show that the CT-based
models are capable of producing similar tensile stresses, fibril
strains, and fluid pressures of knee joint cartilage compared
to those of the MRI-based models. This study provides a new
methodology for the analysis of knee joint and cartilage
mechanics

The KNee OsteoArthritis Prediction (KNOAP2020) challenge:An image analysis challenge to predict incident symptomatic radiographic knee osteoarthritis from MRI and X-ray images
Objectives: The KNee OsteoArthritis Prediction (KNOAP2020) challenge was organized to objectively compare methods for the prediction of incident symptomatic radiographic knee osteoarthritis within 78 months on a test set with blinded ground truth. Design: The challenge participants were free to use any available data sources to train their models. A test set of 423 knees from the Prevention of Knee Osteoarthritis in Overweight Females (PROOF) study consisting of magnetic resonance imaging (MRI) and X-ray image data along with clinical risk factors at baseline was made available to all challenge participants. The ground truth outcomes, i.e., which knees developed incident symptomatic radiographic knee osteoarthritis (according to the combined ACR criteria) within 78 months, were not provided to the participants. To assess the performance of the submitted models, we used the area under the receiver operating characteristic curve (ROCAUC) and balanced accuracy (BACC). Results: Seven teams submitted 23 entries in total. A majority of the algorithms were trained on data from the Osteoarthritis Initiative. The model with the highest ROCAUC (0.64 (95% confidence interval (CI): 0.57–0.70)) used deep learning to extract information from X-ray images combined with clinical variables. The model with the highest BACC (0.59 (95% CI: 0.52–0.65)) ensembled three different models that used automatically extracted X-ray and MRI features along with clinical variables. Conclusion: The KNOAP2020 challenge established a benchmark for predicting incident symptomatic radiographic knee osteoarthritis. Accurate prediction of incident symptomatic radiographic knee osteoarthritis is a complex and still unsolved problem requiring additional investigation.</p
