    Acute adrenal insufficiency secondary to bilateral adrenal B-cell lymphoma: a case report and review of the literature

    Primary adrenal lymphoma is an extremely rare entity which constitutes less than 1% of extranodal lymphomas. Most cases present with bilateral adrenal masses and without extraadrenal involvement, which can lead to symptoms of adrenal insufficiency. The prognosis is usually poor and chemotherapy is the first-line treatment option. We report here on a 78-year-old man admitted to our Internal Medicine Department because of constitutional symptoms and high fever spikes. He was diagnosed with adrenal insufficiency and a CT-scan revealed bilateral adrenal masses of about 6 cm in diameter. A percutaneous biopsy was performed and the histological exam was consistent with diffuse large B cell lymphoma. A review of the literature of this unusual entity was also carried out

    Todas las heridas quirúrgicas tienen riesgo de contaminación por organismos patógenos que pueden entrar por la incisión. El baño prequirúrgico 24 horas antes de la cirugía con un antiséptico cutáneo, el tiempo para la administración de antibióticos profilácticos, la selección, la duración postquirúrgica de los mismo, el número de individuos dentro del quirófano y el movimiento en la sala, al preferir gluconato de clorhexidina por su larga duración contra organismos Gram-positivos y negativos y el uso de un removedor de vello o una rasudadora eléctrica. Todos estos conceptos, algunos nuevos y otros no tanto, llevaron a la realización de una extensa revisión bibliográfica, ya que podría iniciarse un proceso de estandarización que cambiaria la forma de proceder en muchas instituciones que se dedican al reemplazo articular primario, para lograr disminuir la tasa anual de infecciones y mejorar la calidad de vida de los pacientes

    Esta nueva obra de ISF ApD y del grupo de Sistemas Agrarios de la Universidad Politécnica de Madrid es una guía metodológica con indicadores para establecer en qué grado se está alcanzando el objetivo de la sostenibilidad en el sector agropecuario. El desarrollo agropecuario siempre ha estado dentro de las carteras de los programas de desarrollo. Aun así, en 2009 nos encontramos con 963 millones de personas malnutridas, prácticamente todas en países en desarrollo y la gran mayoría en las zonas rurales. Paradójicamente, las mejoras tecnológicas introducidas en las zonas de producción de alimentos durante decenios, a la vez que han mejorado la productividad de grandes extensiones de tierra, han provocado no pocos impactos ambientales y sociales negativos. Cómo conocer o medir si con un programa estamos contribuyendo a alcanzar o no el objetivo de un sistema agrícola sostenible es una pregunta común en los agentes de desarrollo para la que no es fácil encontrar respuesta. Ingeniería Sin Fronteras-Asociación para el Desarrollo (ISF ApD) lleva más de 10 años trabajando en programas de desarrollo agropecuario en Nicaragua. En su plan estratégico para Nicaragua definido en 2005, ISF ApD entiende que una vía para la reducción de la pobreza en el medio rural es el desarrollo económico sostenible a través de actividades productivas que permitan a la población más desfavorecida la generación de ingresos. El grupo de Sistemas Agrarios (AgSystems), perteneciente a la Universidad Politécnica de Madrid (UPM), está formado por investigadores de los departamentos de Producción Vegetal: Fitotecnia y Biología Vegetal. Sus investigaciones se centran en el estudio de sistemas agrarios y su relación con el medio ambiente, con el objetivo final de diseñar estrategias que permitan mantener la sostenibilidad de los sistemas de cultivo. El presente texto pretende ser una guía metodológica para la evaluación de la sostenibilidad. En él se nos presenta y desarrolla una metodología para la evaluación, así como una serie de indicadores útiles para establecer en qué grado se está alcanzando el objetivo de la sostenibilidad

    Teaching skills assessment in initial teacher training in Physical Education

    Producción CientíficaThis paper assessed the impact of two teacher training degrees in Physical Education, one focusing on primary school teaching and the other one concerning secondary school teaching. Data from students of both degrees were statistically compared and contrasted taking into account four training scopes from the Questionnaire of Perception of Teaching Skills in Physical Education. A three-stage analysis was carried out: (i) a descriptive stage, (ii) an invariance analysis, and (iii) a comparison of latent means across the degrees. The results show significant statistical differences between the two degrees regarding the training impact on developing skills for physical and motor development basics, contents for learning, and physical activity promoting health; however, no statistically significant differences were found regarding the learning and teaching processes in Physical Education.     Co-evolutionnary network approach to cultural dynamics controlled by intolerance

    Starting from Axelrod's model of cultural dissemination, we introduce a rewiring probability, enabling agents to cut the links with their unfriendly neighbors if their cultural similarity is below a tolerance parameter. For low values of tolerance, rewiring promotes the convergence to a frozen monocultural state. However, intermediate tolerance values prevent rewiring once the network is fragmented, resulting in a multicultural society even for values of initial cultural diversity in which the original Axelrod model reaches globalization

    This study analysed the new Self-Efficacy Motor Scale (seMs), the motor efficacy evaluation (Mee) in adolescents and its relation with variables such as general self-efficacy, speed-agility, dynamic coordination and cardiorespiratory adaptation. To do so, an ex post facto design was devised, using a sample of 1.288 adolescents (14.9 ± 1.7 years) from six spanish regions. The seMs is one-dimensional with a good internal consistency (a = .89). Boys reported higher motor self-efficacy than girls did, regardless of age. Among girls only, the older the age, the lower the expectations of efficacy. The Mee is closely related to the general self-efficacy variable (r= .62). we conclude that the seMs is an original, unpublished instrument that has been shown to have high reliability and sufficient validity in terms of content, construct and criteria for measuring perceived self-efficacy in challenging situations of physical activityEste trabajo analizó la nueva Escala de Autoeficacia Motriz (e-AeM), la valoración de autoeficacia motriz (AeM) en adolescentes y su relación con variables como la autoeficacia general, la velocidad-agilidad, la coordinación dinámica y la adaptación cardiorrespiratoria. Para ello se procedió con un diseño ex post facto, valiéndose de una muestra 1.288 adolescentes (14.9±1,7 años), de seis comunidades autónomas españolas.     Chemical composition of the foliage meal of Tithonia diversifolia

    With the objective of determining the nutritional constituents, amino acids and the content of secondary metabolites of the foliage meal of Tithonia diversifolia. For this, plant material was collected 70 days after regrowth. The percentages of DM, CP, P, Ca, Si, CF, NDF, ADF, ADL, Cel, Hcel, CC were determined; amino acid profile (methionine, cystine, methionine + cystine, lysine, threonine, aspartic acid, glutamic acid, proline, glycine, alanine, valine, isoleucine, leucine, serine, phenylalanine, arginine, histidine), and content of TT, TP, TCT, TBCT, FCT, Flv, Sap, Alk, Trit and TS. For statistical processing, descriptive techniques (mean and standard deviation) were used. The plant material of Tithonia diversifolia analyzed showed percentages of 22.23, 2.62, 0.013, 45.7, and 29.8% and 6.09 MJ/kg (CP, Ca, P, NDF, ADF and ME). For the secondary metabolites, they presented concentrations of 5.35, 12.38, 13.7, 9.58, 4.15, 24.49, 0.86, 1.36, 7.71 and 10.75 g/kg MS for total tannins, total phenols, total condensed tannins, total bound condensed tannins, free condensed tannins, flavonoids, alkaloids, saponins, triterpenes and total steroids). The highest concentrations of amino acids were for lysine, aspartic acid, glutamic acid, proline, glycine, alanine, valine, leucine, serine and phenylalanine with values ​​of 13-30 g/kg of protein. It is concluded that the foliage meal of Tithonia diversifolia presents an adequate relation in its quality

    Pregnancy rate in dairy cows treated with human chorionic gonadotropin five days after insemination

    This study tested whether the administration of hCG five days after insemination increased progesterone concentration and pregnancy rate (PR) in dairy cows. A total of 989 lactating Holstein cows with different parity and number of prior services wereused. Cows were inseminated after overt estrus or at a fixed-time. Five days post-insemination, cows were randomly assigned to two treatment groups: hCG (n=482), which received 3500 IU of hCG by intramuscular injection; and the control group (n=507), which did not receive any treatment. Pregnancy was diagnosed by ultrasound on day 30 post-insemination, and gestation was confirmed on day 60 post-insemination by rectal palpation. In 15 cows from each treatment group, plasma progesterone concentration was determined on days 5, 11, and 15 post-insemination. Pregnancy rate was analysed using logistic regression. Variations in progesterone concentration between treatments were tested by ANOVA for repeated measurements. Progesterone concentration was higher on days 11 and 15 in cows treated with hCG, compared to the control group (P<0.05). Treatment with hCG increased pregnancy rate (47.5 vs. 37.4%. Odds ratio 1.3; P<0.05). No interaction was observed between treatment and body condition, prior services, milk production, parity, or insemination type (overt estrus or fixed time). Treatment with hCG did not reduce pregnancy losses between day 30 and 60 post-insemination (P>0.1). We conclude that injection of hCG five days after insemination increased progesterone concentration and pregnancy rate in dairy cows
