    RISK FACTORS ASSOCIATED TO SUICIDE ATTEMPT IN ADOLESCENCE. YARA 2007¬2009.

    El suicidio, como una de las causas mas frecuentes de defunción a escala mundial, plantea un problema de gran magnitud, que justifica los esfuerzos a desarrollar por las organizaciones de salud en diferentes latitudes para prevenir y controlar los intentos suicidas en la población.  El presente trabajo aborda el estudio observacional del tipo, casos y controles, con los pacientes reportados como intento suicida entre enero del 2007 y diciembre del 2009 pertenecientes al municipio de Yara, con el objetivo de comparar la diferencia entre ellos y personas supuestamente sanas con relación a factores psicológicos, familiares, biológicas y  psicosociales esenciales de riesgo de la conducta suicida. Se estudiaron 92 pacientes y 184 personas controles. Se les aplico un cuestionario de recogida de datos diseñado al efecto, los resultados se sometieron a análisis porcentual y al estadígrafo Odd Ratio. Se concluyo que excite diferencias significativas entre las personas supuestamente sanas y con intento suicida con relaciona los factores estudiados y se encontró que los factores: manejo inadecuado del adolescente ( el mas fuertemente asociado ), depresión, violencia familiar, falta de afecto, sentimientos de desesperanza, tienen relación significativa de casualidad con la conducta suicida, mientras que los factores biológicos y psicológicos no guardan relación significativa de casualidad con esta conducta, en nuestro estudio.Suicide as one of the most frequent causes of death in the world, states a huge problem that justifies the efforts of several health organizations in different latitudes in order to prevent and control the suicide attempts in the population. This work refers to the observational research of the type, cases and controls with the patients reported as suicide attempts since January 2007 to December 2009, in Yara, with the objective to compare the difference among the patients and the apparently health people in regard to the main psychological, familiar, biologic, psychosocial risk factors of the suicide behaviour. There were analyzed 92 patients and 184 controls. There were applied a questionnaire of data developed for that purpose, the results were analyzed in percentage and in ODD ratio estadigraph.  As a conclusion it can be said that there are significant differences among apparently health people and people with suicide attempt in relation to the studied factors, and it was noticed that unappropiated adolescent handling, depression, familiar violence, lack of affection, feelings of hopeless, have a significative relation to suicide behaviour while the biological and psychological factors do not have meaningful relation to this behaviour in our research

    Differences in coping strategies among young adults and the elderly with cancer

    Background: Coping with cancer and the oncologist-patient relationship can vary depending on the patient's age. Our aim is to examine and compare young and elderly adults with non‐metastatic, resected cancer. Methods: Two groups of patients were selected, young ( 70) with a diagnosis of non‐metastatic, resected cancer requiring adjuvant chemotherapy from a pre‐exiting, national database (NEOCOPING Study). Epidemiological variables were collected and subjects' emotional responses, perceptions of the physician-patient relationship, support network, fears, and regret about the decision to receive chemotherapy were assessed with questionnaires validated in previous studies: Mini‐Mental Adjustment to Cancer, Brief Summary Inventory (18 items), European Organization for the Research and Treatment of Cancer Quality of Life Questionnaire‐C30, Shared Decision‐Making Questionnaire‐Physician's version, Shared Decision‐Making Questionnaire‐Patient's version, and Informed Risk (physician and patient versions). Results: Data from 46 young and 46 elderly participants were collected. The most common neoplasms in both groups were breast (50%) and colorectal (22%). The younger adults had a higher level of education and were actively employed (72% vs. 7%). The leading coping strategy in the younger cohort was hope, and resignation among the elderly. Young adults sought more social support and the impact of diagnosis was more negative for them than for older individuals. No significant differences were detected in quality of life; both age groups demanded more time at their first visit with the doctor, while the older group exhibited greater satisfaction with shared decision‐making. At the end of adjuvant chemotherapy, neither age group regretted their decision to receive said treatment. Conclusion: Higher levels of education, greater demands of the labour market, and the advent of the age of information have entailed drastic changes in the physician-patient relationship paradigm. This is especially true in the younger cancer patient population, who require more information and active participation in decision‐making, can display more anxiety about their diagnosis, but also greater capacity to fight

    A Functional Pipeline of Genome-Wide Association Data Leads to Midostaurin as a Repurposed Drug for Alzheimer’s Disease

    Genome-wide association studies (GWAS) constitute a powerful tool to identify the different biochemical pathways associated with disease. This knowledge can be used to prioritize drugs targeting these routes, paving the road to clinical application. Here, we describe DAGGER (Drug Repositioning by Analysis of GWAS and Gene Expression in R), a straightforward pipeline to find currently approved drugs with repurposing potential. As a proof of concept, we analyzed a meta-GWAS of 1.6 × 107 single-nucleotide polymorphisms performed on Alzheimer’s disease (AD). Our pipeline uses the Genotype-Tissue Expression (GTEx) and Drug Gene Interaction (DGI) databases for a rational prioritization of 22 druggable targets. Next, we performed a two-stage in vivo functional assay. We used a C. elegans humanized model over-expressing the Aβ1-42 peptide. We assayed the five top-scoring candidate drugs, finding midostaurin, a multitarget protein kinase inhibitor, to be a protective drug. Next, 3xTg AD transgenic mice were used for a final evaluation of midostaurin’s effect. Behavioral testing after three weeks of 20 mg/kg intraperitoneal treatment revealed a significant improvement in behavior, including locomotion, anxiety-like behavior, and new-place recognition. Altogether, we consider that our pipeline might be a useful tool for drug repurposing in complex diseases.This work was mainly financed by Programa Operativo FEDER funds from the European Union through grant UMA20-FEDERJA-133.     Association Between Preexisting Versus Newly Identified Atrial Fibrillation and Outcomes of Patients With Acute Pulmonary Embolism

    Background Atrial fibrillation (AF) may exist before or occur early in the course of pulmonary embolism (PE). We determined the PE outcomes based on the presence and timing of AF. Methods and Results Using the data from a multicenter PE registry, we identified 3 groups: (1) those with preexisting AF, (2) patients with new AF within 2 days from acute PE (incident AF), and (3) patients without AF. We assessed the 90-day and 1-year risk of mortality and stroke in patients with AF, compared with those without AF (reference group). Among 16 497 patients with PE, 792 had preexisting AF. These patients had increased odds of 90-day all-cause (odds ratio [OR], 2.81; 95% CI, 2.33-3.38) and PE-related mortality (OR, 2.38; 95% CI, 1.37-4.14) and increased 1-year hazard for ischemic stroke (hazard ratio, 5.48; 95% CI, 3.10-9.69) compared with those without AF. After multivariable adjustment, preexisting AF was associated with significantly increased odds of all-cause mortality (OR, 1.91; 95% CI, 1.57-2.32) but not PE-related mortality (OR, 1.50; 95% CI, 0.85-2.66). Among 16 497 patients with PE, 445 developed new incident AF within 2 days of acute PE. Incident AF was associated with increased odds of 90-day all-cause (OR, 2.28; 95% CI, 1.75-2.97) and PE-related (OR, 3.64; 95% CI, 2.01-6.59) mortality but not stroke. Findings were similar in multivariable analyses. Conclusions In patients with acute symptomatic PE, both preexisting AF and incident AF predict adverse clinical outcomes.     Relación de los centros educativos de enseñanza secundaria con el entorno en Iberoamérica

    La presente aportación recoge la visión de 46 especialistas de trece países iberoamericanos sobre las formas de entender y promover la relación de los centros educativos con el entorno. Situados en la enseñanza secundaria (y en la franja de edad de 15 a 18 años), se trata de delimitar la manera cómo se conecta la vida interna y la vida externa del centro educativo, presentando los modelos que se utilizan, las estrategias de intervención y las experiencias más significativas en cada uno de los países. Las aportaciones no buscan tanto revisar la vinculación de los centros educativos entre sí como de analizar fundamentalmente las vinculaciones con la comunidad y las organizaciones que acoge (asociaciones, empresas, organizaciones gubernamentales o no gubernamentales significativas,…). Al respecto, una parte de los escritos recogen experiencias y estrategias que concretan la relación que estudiamos. Esperamos sea así un apoyo para los estudiosos de la temática, pero también aporte contenidos que ayuden a los directivos a mejorar su gestión de las relaciones externas. La orientación de los escritos al análisis organizativo y a la función de los directivos como promotores de las relación con la comunidad tiene que ver con las finalidades y objetivos de la RedAGE; también con el convencimiento por parte de los que escriben que la ordenación que se haga del contexto de intervención y la actuación de los directivos es fundamental para obtener y mantener las respuestas más idóneas a las exigencias del medio socio-cultural-económico. Su realización se vincula al encuentro de especialistas de la RedAGE realizado en el mes de marzo de 2013 en La Paz. Allí, los representantes de las organizaciones miembro presentaron y debatieron, durante el mismo, documentos sobre la temática de la vinculación escuela y entorno, en sus respectivos países, que constituyen la base sobre la que se han realizado las aportaciones definitivas que recoge el presente texto. Se cubre así y de nuevo un propósito fundamental de la Red AGE, como es el de fomentar el intercambio de experiencias, la promoción del conocimiento sobre administración y gestión educativa y la reflexión sobre la práctica de la gestión. La finalidad última es la de mejorar el funcionamiento de los centros educativos (y, a través de ellos, de los sistemas educativos), procurando sean de calidad y un instrumento para el cambio profesional y social
