Estrategias narrativas en la película Siete psicópatas de Martin McDonagh (2012)

Este Trabajo de Fin de Grado consiste en un análisis narratológico de la película Siete Psicópatas (2012), una comedia negra del director y guionista británico-irlandés Martin McDonagh. Este proyecto cuenta con dos objetivos planteados: conocer los juegos de guion que el autor propone y relacionarlos con las reacciones que genera en el espectador. Para ello, se ha realizado un análisis minucioso de la estructura narrativa de la película. Se expone la trama de la película, donde podemos encontrar varios ejemplos de juegos de guion. Mediante estos juegos, McDonagh esconde información relevante de la fábula en la trama. Se han deducido las fábulas de los personajes principales, estas irán seguidas de una representación visual de las líneas narrativas. A través de los juegos de guion, el director ha conseguido generar incertidumbre en un caso, y sorpresa en otro. También hace uso del metacine para confundir al espectador.<br /

Paraciclisme: estudi sobre els processos d’integració en l’àmbit internacional

El paraciclisme o ciclisme adaptat és una de les primeres modalitats esportives adaptades integrades en la federació internacional de l’esport corresponent. Aquest esport és un bon exemple a l’hora d’estudiar els processos d’integració en les federacions uniesportives. L’objectiu de l’estudi és analitzar el grau d’implantació d’aquests processos en el ciclisme en l’àmbit internacional i conèixer els indicadors concrets que el caracteritzen. L’estudi es va dur a terme en el Campionat del Món de Paraciclisme en ruta, disputat a Baie-Comeau (Canadà, 2010). Van participar-hi 13 països. Es va administrar un qüestionari ad hoc de 20 preguntes a un representant qualificat de cada expedició, que avaluava els àmbits de gestió, necessitats, aspectes clau i opinió sobre els processos d’integració del ciclisme al seu país. En el 61,5 % dels països estudiats era la federació nacional de ciclisme qui gestionava el ciclisme adaptat, encara que només el 30 % tenen totes les competències transferides a les federacions uniesportives. Les discapacitats menys integrades van ser l’auditiva i la intel·lectual. Els aspectes més valorats en els processos van ser la determinació de normatives específiques, la formació de tècnics esportius, l’existència d’una institució garant dels processos i el suport econòmic específic. El procés d’integració va ser avaluat de forma molt positiva pel 87,5 % dels enquestats: fins i tot sembla derivar en un augment del nombre de llicències i del rendiment esportiu. Aquests indicadors poden ser extrapolables a altres esports i útils per afavorir els processos d’integració al nostre país

Misconceptions of Spanish general practitioners' attitudes toward the management of urinary tract infections and asymptomatic bacteriuria: an internet-based questionnaire study.

OBJECTIVE: The diagnosis and management of urinary tract infections (UTI) vary widely across countries and practices. The objective of this study was to gain insight into general practitioners' (GP) perceptions on the current management of UTIs and asymptomatic bacteriuria in Spain. METHODS: Cross-sectional, internet-based questionnaire study answered from July to September 2013. GPs affiliated with the largest Spanish scientific society in primary care (Sociedad Española de Medicina Familiar y Comunitaria) were invited to participate in the study. They were asked about the tests ordered in both uncomplicated and complicated UTIs and about the management in three clinical scenarios, depicting a 50-year woman with: 1. An uncomplicated UTI, 2. A complicated UTI, and 3. An asymptomatic bacteriuria. RESULTS: The questionnaire was completed by 1,239 GPs (6.7%). Urine cultures were reportedly requested by 26.3% of the GPs in uncomplicated UTIs and by 71.8% of the cases corresponding to the complicated UTIs whereas it was declared that dipsticks were the preferred tests in only uncomplicated UTIs (38.2%). A total of 22% and 13.2% of the GPs stated that they would withhold antibiotic therapy in patients with low-count and high-count asymptomatic bacteriuria, respectively. CONCLUSIONS: GPs have important misconceptions as to the indications for ordering urine cultures and in interpreting the definitions of common UTIs and treating UTIs and asymptomatic bacteriuria. Evaluation of the silver species nature in Ag-ITQ2 zeolites by the CO oxidation reaction

Red de investigación en docencia de la traducción económica

Esta memoria recoge el trabajo realizado por la red 3131 en el marco del Proyecto Redes 2014-2015, consistente en la revisión del estado de la docencia de las asignaturas de traducción económica, financiera y comercial (alemán, francés e inglés) impartidas en el Grado de Traducción e Interpretación de la Universidad de Alicante durante el curso 2014-2015. Su objetivo primordial es contribuir a la mejora de los métodos de enseñanza-aprendizaje de esta materia, de cara a maximizar el aprovechamiento por parte del alumnado en su preparación para el mercado laboral. Siguiendo un programa de trabajo, los profesores que imparten estas asignaturas y un alumno de último curso de grado han llevado a cabo reuniones, debates e informes para poner en común experiencias, identificar las carencias y realizar propuestas de mejora. Como resultado de este proyecto, se pone de manifiesto la necesidad de revisar la metodología docente, bien que respetando siempre la libertad de cátedra, y adaptarla a la cada vez más exigente actividad profesional

Autonomy, power dynamics and antibiotic use in primary healthcare: A qualitative study

Antibiotic resistance is a global health concern. Although numerous strategies have tried to reduce inadequate antibiotic prescribing, antibiotics are still prescribed in 60% of acute lower respiratory tract infections (ALRTIs) cases in Catalonia (Spain). This study aims to explore service users' experiences of ALRTIs, the quality and access to healthcare services, and health education. Selective purposive sampling was carried out, based on a prior definition of participant characteristics. These were sex, age, ethnicity, date of the last ALRTI, number of ALRTIs in the last year, and treatments received. Participants with a previous diagnosis of ALRTIs were recruited from three primary health care centres in Barcelona and one in Tarragona. Twenty-nine interviews were conducted between April and June 2019. A content thematic analysis was performed. Three themes were identified: 1) risk perceptions and help-seeking; 2) treatment preferences and antibiotic use; and 3) relationship dynamics and communication with healthcare professionals. Accounts of service users' sense of autonomy towards their health and power dynamics within the healthcare system were apparent. Supporting service users to become reliable, subjective and agentic experts of their health and bodies could help them to voice their healthcare agendas. Power structures embedded within healthcare, political and economic institutions should be challenged so that healthcare services can be co-developed (with service users) and based on service users' autonomy and horizontal relationships. Special consideration should be paid to the intersection of social vulnerabilities. A concordance approach to prescribing could be key to improve the responsible use of antibiotics and to contribute to the prevention of AMR in primary healthcare. The marketisation of health, and the increased demands of private healthcare in Spain due to the financial pressures on public healthcare as a consequence of the financial crisis of 2008 and the COVID-19 pandemic, are a risk for promoting adequate antibiotic prescribing and use. Assessing the risk of cardiovascular events in patients receiving immune checkpoint inhibitors

However, despite their excellent therapeutic effect, these medications typically result in a broad spectrum of toxicity reactions. Immune-related cardiotoxicity is uncommon but can be potentially fatal, and its true incidence is underestimated in clinical trials. The aim of this study is to assess the incidence and identify risk factors for developing a cardiac event in patients treated with ICIs. Methods: We conducted a single-institution retrospective study, including patients treated with ICIs in our center. The main outcomes were cardiac events (CE) and cardiovascular death. Results: A total of 378 patients were analyzed. The incidence of CE was 16.7%, during a median follow-up of 50.5 months. The multivariable analysis showed that age, a history of arrhythmia or ischemic heart disease, and prior immune-related adverse events were significantly associated with CE. Conclusion: CE during ICI treatment are more common than currently appreciated. A complete initial cardiovascular evaluation is recommended, especially in high-risk patients, being necessary a multidisciplinary approach of a specialized cardio-oncology team.publishersversionpublishe

Antimicrobial consumption at the hospital level in latin america. Similarities and differences according to each country

One of the main problems for health service around the world is the antimicrobial resistance (AMR). Objective: to describe the antimicrobial consumption (AMC) at hospital level in Latin American countries and compare the amount and type of antibiotics usage among them in order to guide local public health actions towards AMR prevention.Design: A descriptive study of antimicrobial consumption at hospital level among six health institution in Latin America with an analytical comparative stage. Antimicrobials included corresponded to the WHO Anatomical Therapeutic Chemical (ATC) classification system subgroups: J01, A07A and P01AB. Methods: WHO GLASS methodology was applied for surveillance of AMC, using the ATC classification based on Defined Daily Dose (DDD) and DDD/100 hospital discharges as standard unit of measurement. Antimicrobials consumed were also classified according to the WHO AccessWatch-Reserve (AWaRe) classification. Results: The quantitative data, measured in DDD/100 hospital discharges, showed a wide range of consumption (182.48 - 2260.95). Qualitative analysis according to the AWaRe classification also showed a wide range in terms of consumption of Access (38.14% - 73.64%), Watch (24.93% - 60 .53%) and Reserve (0.31% - 3.55%) groups expressed as a percentage of the total consumption.Conclusion: Great heterogeneity and arbitrariness exist in the selection of antimicrobials for hospital use. Although this situation might be explained on local antimicrobial resistance, the history of prescription, local pharmaceutical promotion, and pharmacological education of health professional in each country, particular habits and distinctive culture may justify the differential consumption patterns observed in each institution in this study.Fil: Marin, Gustavo Horacio.
