    La responsabilitĂ© sociale des enseignants de l’enseignement supĂ©rieur questionne les modĂšles et modalitĂ©s de transmission des diffĂ©rentes formes de savoirs et d’accompagnement des Ă©tudiants. Elle interroge les dispositifs qui permettent la transmission des mĂ©ta compĂ©tences que sont l’agir collectif, l’organiser responsable, le penser RSE. Elle inscrit la rĂ©flexion dans une valorisation des actions d’analyse et de remise en cause des pratiques en environnement complexe. Donner du sens devient un objectif de l’aujourd’hui et du demain. La recherche s’appuie sur un dispositif de premiĂšre annĂ©e universitaire intitulĂ© « Dynamique de l’entreprise ». Elle propose, dans une approche exploratoire, d’identifier l’apport d’une Ă©quipe pluridisciplinaire Ă  l’apprentissage de l’interdisciplinaritĂ© auprĂšs de publics hĂ©tĂ©rogĂšnes.     Les caractéristiques des auto-entrepreneurs bretons : comparaison avec les autres entrepreneurs

    While mutations accelerate and instability seems to be permanent, entrepreneurship is valued by public and private actors. In France, the auto-entrepreneur, created by Law No. 2008-776 of 4 August 2008, promotes access to entrepreneurship to greater numbers. Since 2009, the auto-entrepreneurs account for more than half of self-employed entrepreneurs. Also, the existence of a specific entrepreneurial approach to business creation projects under this status arises. This research is part of a constructivist approach to explain the phenomenon by identifying the characteristics of auto-entrepreneurs in comparison to other entrepreneurs. It is rooted in the examination of 700 projects evaluated in upstream phase of the creation / buyout in the southern FinistĂšre, from late 2008 to early 2012, almost 1% of the creation / buyout firms on the Brittany area. The support of entrepreneurial paradigms leads to closer generic theoretical standard to findings from the field. A counterfactual approach identifies the characteristics of auto-entrepreneurs in comparison to other entrepreneurs. Thematic qualitative analysis of biographical interviews of auto-entrepreneurs triangulates and confirms the results. The research questions the paradigm of creating / obtaining new or existing value. She questions the Schumpeterian process of creative versus destructive innovation, and its possible mobilization in a reading of the success of the auto-entrepreneur. It offers effectual approach creation depending on the status of auto-entrepreneur. It confirms the role of the accompanying upstream phase of the creation / buyout business.Alors que les mutations s’accĂ©lĂšrent et l’instabilitĂ© semble permanente, l’esprit d’entreprise est valorisĂ© par les acteurs publics et privĂ©s. Depuis 2009, les crĂ©ations d’entreprises comptent pour plus de moitiĂ© d’auto-entrepreneurs. Aussi, l’existence d’une dynamique entrepreneuriale spĂ©cifique Ă  ces crĂ©ations se pose. La recherche s’inscrit dans une approche constructiviste d’essai d’explication du phĂ©nomĂšne par l’identification des caractĂ©ristiques des auto-entrepreneurs en comparaison aux autres entrepreneurs. Elle prend racine dans l’examen de 700 dossiers d’évaluation de projets en phase amont de la crĂ©ation/ reprise dans le sud FinistĂšre, soit, de fin 2008 Ă  dĂ©but 2012 prĂšs d’1% des crĂ©ations / reprises en Bretagne. L’appui sur les paradigmes entrepreneuriaux amĂšne Ă  rapprocher des standards thĂ©oriques gĂ©nĂ©riques Ă  des constatations issues du terrain. Une approche hypothĂ©tico-dĂ©ductive permet d’identifier les caractĂ©ristiques des auto-entrepreneurs en comparaison aux autres entrepreneurs. La recherche questionne le paradigme de la crĂ©ation/ obtention de valeur nouvelle ou existante. Elle interroge le processus schumpĂ©tĂ©rien d’innovation crĂ©atrice versus destructrice, et sa possible mobilisation dans une lecture du succĂšs de l’auto-entrepreneur. L’objet est de rĂ©pondre Ă  la question "En quoi une telle formation initiale prĂ©pare au dĂ©veloppement de compĂ©tences RSE?".L’aspect exploratoire de cette recherche a permis de mettre en Ă©vidence une apprĂ©hension de la RSE par des Ă©tudiants de premiĂšre annĂ©e de licence.     Volumetric Enhancing Tumor Burden at CT to Predict Survival Outcomes in Patients with Neuroendocrine Liver Metastases after Intra-arterial Treatment

    Purpose: To investigate whether liver enhancing tumor burden (LETB) assessed at contrast-enhanced CT indicates early response and helps predict survival outcomes in patients with multifocal neuroendocrine liver metastases (NELM) after intra-arterial treatment.Materials and Methods: This retrospective study included patients with NELM who underwent intra-arterial treatment with transarterial embolization (TAE) or chemoembolization (TACE) between April 2006 and December 2018. Tumor response in treated NELM was evaluated by using the Response Evaluation Criteria in Solid Tumors (RECIST) and modified RECIST (mRECIST). LETB was mea-sured as attenuation 2 SDs greater than that of a region of interest in the nontumoral liver parenchyma. Overall survival (OS); time to unTA(C)Eable progression, defined as the time from the initial treatment until the time when intra-arterial treatments were considered technically unfeasible, either not recommended by the multidisciplinary tumor board or until death; and hepatic and whole-body progression-free survival (PFS) were evaluated using multivariable Cox proportional hazards analyses, the Kaplan-Meier method, and log-rank test.Results: The study included 119 patients (mean age, 60 years +/- 11 [SD]; 61 men) who underwent 161 treatments. A median LETB change of -25.8% best discriminated OS (83 months in responders vs 51 months in nonresponders; P = .02) and whole-body PFS (18 vs 8 months, respectively; P < .001). A -10% LETB change best discriminated time to unTA(C)Eable progression (32 months in re-sponders vs 12 months in nonresponders; P < .001) and hepatic PFS (18 vs 8 months, respectively; P < .001). LETB change remained independently associated with improved OS (hazard ratio [HR], 0.56), time to unTA(C)Eable progression (HR, 0.44), hepatic PFS (HR, 0.42), and whole-body PFS (HR, 0.47) on multivariable analysis. Neither RECIST nor mRECIST helped predict patient outcome. Conclusion: Response according to LETB change helped predict survival outcomes in patients with NELM after intra-arterial treat-ments, with better discrimination than RECIST and mRECIST

    Prolonged Survival in a Patient with Neuroendocrine Tumor of the Cecum and Diffuse Peritoneal Carcinomatosis

    Peritoneal carcinomatosis is a well-known factor of poor prognosis in patients with digestive adenocarcinomas. Peritoneal dissemination may also occur in midgut well-differentiated neuroendocrine tumors, but its influence on survival is ill-defined. We report here the history of a 64-year-old woman who had a neuroendocrine tumor of the cecum with multiple synchronous metastases in the liver and diffuse peritoneal carcinomatosis. She underwent surgical resection of the primary tumor and cytoreduction of liver metastases, and received subsequently chemotherapy and somatostatin analogs. In spite of the widespread extension of the disease, she survived for 13 years and died from a carcinoid heart disease. We discuss the natural history and prognostic factors in patients with midgut well-differentiated neuroendocrine tumors, with a focus on the impact of the peritoneal carcinomatosis

    Chemotherapy for Well-Differentiated Pancreatic Neuroendocrine Tumours with a Ki-67 Index ≄10%: Is There a More Effective Antitumour Regimen? A Retrospective Multicentre Study of the French Group of Endocrine Tumours (GTE)

    BACKGROUND: The best chemotherapy regimen for well- differentiated pancreatic neuroendocrine tumours (pNETs) with a Ki-67 index ≄10% is still debated. We evaluated the antitumour efficacy of various first-line chemotherapy regimens (streptozocin based, platinum based, or dacarbazine/temozolomide based) in this situation. METHODS: In this retrospective multicentre study of the French Group of Endocrine Tumours (GTE), we recruited consecutive patients with advanced well-differentiated pNETs and a Ki-67 index ≄10% receiving chemotherapy between 2000 and 2012. The primary endpoint was progression-free survival (PFS) according to RECIST. RESULTS: Seventy-four patients (42 men, median age 55.5 years) were enrolled from 10 centres. Fifty-one patients (69%) had grade 2 NET and 61 (82%) were stage IV. Median overall survival was 36.3 months. Forty-four patients (59%) received streptozocin-based, 18 (24%) platinum-based, and 12 (16%) dacarbazine/temozolomide-based chemotherapy regimens. These 3 groups were similar regarding age, functioning tumours, grade, the number of metastatic sites, and surgery for primary tumours, but not regarding surgery for metastases and time since diagnosis. Grade 3 NET (HR 2.15, 95% CI: 1.18-3.92, p = 0.012) and age above 55 years (HR 1.84, 95% CI: 1.06-3.18, p = 0.030) were associated with shorter median PFS in the multivariate analyses. Compared to streptozocin-based chemotherapy, no difference was found in terms of PFS for the platinum-based or for the dacarbazine/temozolomide-based chemotherapy regimen: median PFS was 7.2, 7.5, and 7.2 months, respectively (p = 0.51). CONCLUSIONS: Patients with intermediate or highly proliferative well-differentiated pNETs may benefit from 1 of the 3 chemotherapy regimens. Increased age and grade 3 were associated with shorter median PFS. Randomised studies searching for response predictors and the best efficacy-tolerance ratio are required to personalise the strategy

    Introduction: The optimal treatment for small, asymptomatic, nonfunctioning pancreatic neuroendocrine neoplasms (NF-PanNEN) is still controversial. European Neuroendocrine Tumor Society (ENETS) guidelines recommend a watchful strategy for asymptomatic NF-PanNEN 18 years, the presence of asymptomatic sporadic NF-PanNEN ≀2 cm proven by a positive fine-needle aspiration (FNA) or by the presence of a measurable nodule on high-quality imaging techniques that is positive at 68Gallium DOTATOC-PET scan.
