    Designing a web-application to support home-based care of childhood CKD stages 3-5: Qualitative study of family and professional preferences

    Background: There is a lack of online, evidence-based information and resources to support home-based care of childhood CKD stages 3-5. Methods. Qualitative interviews were undertaken with parents, patients and professionals to explore their views on content of the proposed online parent information and support (OPIS) web-application. Data were analysed using Framework Analysis, guided by the concept of Self-efficacy. Results: 32 parents, 26 patients and 12 professionals were interviewed. All groups wanted an application that explains, demonstrates, and enables parental clinical care-giving, with condition-specific, continously available, reliable, accessible material and a closed communication system to enable contact between families living with CKD. Professionals advocated a regularly updated application to empower parents to make informed health-care decisions. To address these requirements, key web-application components were defined as: (i) Clinical care-giving support (information on treatment regimens, video-learning tools, condition-specific cartoons/puzzles, and a question and answer area) and (ii) Psychosocial support for care-giving (social-networking, case studies, managing stress, and enhancing families' health-care experiences). Conclusions: Developing a web-application that meets parents' information and support needs will maximise its utility, thereby augmenting parents' self-efficacy for CKD caregiving, and optimising outcomes. Self-efficacy theory provides a schema for how parents' self-efficacy beliefs about management of their child's CKD could potentially be promoted by OPIS. © 2014 Swallow et al.; licensee BioMed Central Ltd

    Addressing Water Resource Issues In Barbados Through An Isotopic and Atmospheric Characterization of Precipitation Variability

    Numerous studies have analyzed isotopic variation of meteoric and dripwater in karst environments for paleoclimate reconstructions or aquifer recharge capacity. What is poorly understood is how the isotopic signal of ή18O and ή2H is transferred through the hydrologic cycle based upon storm type, frequency, intensity, and teleconnection activity in the tropical karst areas. At Harrison’s Cave, Barbados, a Hobo Onset event data logger was attached to a tipping bucket rain gauge to count the tips and record the total rainfall every 10 minutes. In the cave a Hobo data logger was used to record relative humidity and temperature at 10-minute intervals. Rainwater, dripwater, and stream water samples were collected at a weekly resolution and refrigerated before sample analysis. The study period was from July, 2012 to October, 2013, with data from the data loggers only until June, 2013 due to inability to reach the study site. The samples were analyzed using the Picarro Cavity Ring Down Spectroscopy Unit-Water L1102-I through laboratories at the University of Kentucky and the University of Utah. The samples were reported in per mil and calibrated. The teleconnection (NAO, AMO, and ENSO) and other atmospheric data were obtained from the Climate Prediction Center or the NOAA Earth System Research Laboratory-Physical Sciences Division. The weekly isotope signatures were linearly regressed against total rainfall for Harrison’s Cave and surface temperature with no statistically significant correlation, indicating the amount effect was not present at a weekly resolution. The amountweighted precipitation ή18O values were calculated on a monthly basis and compared to TRMM monthly rainfall and island-wide monthly rainfall, and a statistically significant negative correlation was found between both datasets. This confirmed that the amount effect dominates the island’s rainfall isotopic signature at a monthly resolution, and that specific atmospheric influences represented in weekly rainfall were less influential on a weekly basis. It is hypothesized that the variation in weekly rainfall is due to quick initiating, rain-out, and dissipation of convective storm systems over the island. In terms of evaporative influences, the samples do not deviate much from the Global Meteoric Water Line (GMWL), indicating minimal evaporation, which is typical for tropical locations. When the d-excess parameters were calculated, there were distinct variations with minimal evaporation occurring in the 2013 calendar year.     #MeToo & the Courts: The Impact of Social Movements on Federal Judicial Decisionmaking

    In late 2017, the #MeToo movement swept through the United States as individuals from all backgrounds and walks of life revealed their experiences with sexual abuse and sexual harassment. After the #MeToo movement, many scholars, advocates, and policymakers posited that the watershed moment would prompt changes in the ways in which sexual harassment cases were handled. This Article examines the impact the #MeToo movement has had on judicial decisionmaking. Our hypothesis is that the #MeToo movement’s increase in public awareness and political attention to experiences of sexual misconduct should lead to more pro-claimant voting in federal courts at the district and courts of appeals levels. For district courts, we find that the probability of a pro-employee ruling in a district court increased drastically after November 1, 2017. However, while pro-employee rulings increased in district courts during the #MeToo era, pro-employee rulings decreased in circuit courts during this time period. Our findings suggest that the #MeToo movement—an extralegal social movement—impacted legal rulings that occurred in its wake before district courts but courts of appeals were more restrained in their reaction to the movement. Importantly, the law and legal standards in place during the time period of our study did not meaningfully change. In short, the #MeToo movement had a statistically significant impact on rulings from district court judges

    Development and Validation of the Robust - Pandemic Coping Scale (R-PCS)

    The psychological consequences of epidemics/pandemics, such as the COVID-19 pandemic, include an increase in psychopathological symptoms, such as depression, anxiety, and stress, and negative emotions, such as fear. However, relatively little attention has been paid to how people cope with the pandemic. Coping is a multi-component process, helping to diminish the traumatic impact of stressful events in a variety of ways. We studied how university students coped with the first wave of the COVID-19 pandemic, by developing the Robust - Pandemic Coping Scale (R-PCS), a new scale for measuring coping strategies related to epidemics/pandemics. The scale is based on a classification of coping strategies referred to the needs of competence, relatedness, and autonomy. To create a robust scale, such that the item values would be independent of the sample used for developing it, we employed Rasch modeling. We used a sample of 2,987 Italian university students who participated in an online survey including the R-PCS and the Power to Live with Disasters Questionnaire (PLDQ), during March 2020. First, we applied a dual approach combining exploratory and confirmatory factor analyses, which supported the goodness of a 4-factor model (i.e., Despair, Adjustment, Proactivity, and Aversion) for the R-PCS, invariant across gender and age of respondents (younger or as old as 23 years, older than 23 years). We then transformed the raw scores of the R-PCS into interval logit scale scores applying the Rasch model. Second, our findings supported the discriminant validity and the criterion validity of the R-PCS, examining the correlations with the PLDQ. They also confirmed its predictive validity: the R-PCS scores were related to 2-month-later enjoyment and anger, indicating that Adjustment and Proactivity were adaptive while Despair and Aversion were maladaptive. Third, our study revealed gender and age differences: the scores were higher for Despair, Adjustment, and Proactivity for females; for Aversion for males; and for Proactivity for students older than 23 years.     Introduction In Europe alone, over 70 million people experience tinnitus. Despite its considerable socioeconomic relevance, progress in developing successful treatments has been limited. Clinical effectiveness is judged according to change in primary outcome measures, but because tinnitus is a subjective condition, the definition of outcomes is challenging and it remains unclear which distinct aspects of tinnitus (ie, ‘domains’) are most relevant for assessment. The development of a minimum outcome reporting standard would go a long way towards addressing these problems. In 2006, a consensus meeting recommended using 1 of 4 questionnaires for tinnitus severity as an outcome in clinical trials, in part because of availability in different language translations. Our initiative takes an approach motivated by clinimetrics, first by determining what to measure before seeking to determine how to measure it. Agreeing on the domains that contribute to tinnitus severity (ie, ‘what’) is the first step towards achieving a minimum outcome reporting standard for tinnitus that has been reached via a methodologically rigorous and transparent process. Methods and analysis Deciding what should be the core set of outcomes requires a great deal of discussion and so lends itself well to international effort. This protocol lays out the first-step methodology in defining a Core Domain Set for clinical trials of tinnitus by establishing existing knowledge and practice with respect to which outcome domains have been measured and which instruments used in recent registered and published clinical trials. Ethics and dissemination No ethical issues are foreseen. Findings will be reported at national and international ear, nose and throat (ENT) and audiology conferences and in a peer-reviewed journal, using PRISMA (Preferred Reporting Items for Systematic reviews and Meta-analysis) guidelines.     Emotion in Motion: Investigating the Relationship between Interpersonal Motor Coordination and Emotional States

    The present study investigated the relationship between a person’s emotional state and interpersonal coordination. Researchers primed fifty-one participants to either be angry or happy through a writing prompt. Participants did an arm curl activity while watching a video-taped confederate doing arm curls at a set pace with a congruent or incongruent emotion. The participants were told that this video was a projected live stream. Finally, participants completed measures of post-activity emotions and rapport with and likeability toward the confederate. Our experimental design included an independent samples t-tests to examine the effect of congruency on interpersonal coordination, post-activities, and rapport with the confederate, and one-way ANOVAs to understand the effect of the conditions on interpersonal coordination, post-activity emotions, confederate likeability, and confederate rapport. There was no significant main effect difference of conditions on level of coordination and no significant difference of congruency on coordination. There was a significant effect for condition on sadness and a trending effect on anger and disgust, such that having an angry primed emotion and seeing an angry confederate resulted in higher negative emotions. There was a significant effect of congruency on anger, meaning that those that had incongruent primed emotions to the confederate had increased anger compared to those with congruent emotions. There was no significant difference between conditions or congruency for likeability measures; however, there was a significant difference of condition on rapport ratings, with participants who were primed happy and saw the happy confederate or were primed angry and saw the happy confederate had higher rapport ratings with the confederate compared to any other conditions. Therefore, the results suggest that there is a complex relationship between emotion and interpersonal coordination

    mTORC2 sustains thermogenesis via Akt-induced glucose uptake and glycolysis in brown adipose tissue

    Activation of non-shivering thermogenesis (NST) in brown adipose tissue (BAT) has been proposed as an anti-obesity treatment. Moreover, cold-induced glucose uptake could normalize blood glucose levels in insulin-resistant patients. It is therefore important to identify novel regulators of NST and cold-induced glucose uptake. Mammalian target of rapamycin complex 2 (mTORC2) mediates insulin-stimulated glucose uptake in metabolic tissues, but its role in NST is unknown. We show that mTORC2 is activated in brown adipocytes upon ÎČ-adrenergic stimulation. Furthermore, mice lacking mTORC2 specifically in adipose tissue (AdRiKO mice) are hypothermic, display increased sensitivity to cold, and show impaired cold-induced glucose uptake and glycolysis. Restoration of glucose uptake in BAT by overexpression of hexokinase II or activated Akt2 was sufficient to increase body temperature and improve cold tolerance in AdRiKO mice. Thus, mTORC2 in BAT mediates temperature homeostasis via regulation of cold-induced glucose uptake. Our findings demonstrate the importance of glucose metabolism in temperature regulation

    In‐situ extraction and impregnation of black walnut husk into polyethylene film using supercritical carbon dioxide with an ethanol modifier

    Walnuts are commonly cultivated for their kernel, which is a rich source of antioxidant phenolic compounds. The husk likewise contains antioxidant and antimicrobial compounds, but is typically discarded without further processing. Antioxidant compounds are useful in creating active packaging films, but typically decompose at melt extrusion temperatures in polymer processing. Due to carbon dioxide\u27s low critical point and ability to swell polymer films, supercritical carbon dioxide may be used to impregnate phenolic compounds into polymers. For this study, a novel technique is used to simultaneously produce walnut husk extracts and impregnate the extract into polymer films in the same batch extractor using supercritical carbon dioxide with a 15 wt-% ethanol modifier at 60°C at 19.4 MPa. The effect of varying the loading of walnut husk in the extractor upon impregnation mass was evaluated with the impregnation mass of the film increasing with walnut husk loading. It was determined by FTIR, as well as the eduction of the protein cytochrome c, that antioxidant compounds may be extracted from walnut husks and impregnated into low-density polyethylene film (LDPE) by this technique
