    Modifiable Predictors Associated with Having a Gestational Weight Gain Goal

    The goal of this paper was to determine predictors of having a weight gain goal in early pregnancy. In 2008, we administered a 48-item survey to 249 pregnant women attending obstetric visits. We examined predictors of women having a goal concordant or discordant with 1990 Institute of Medicine (IOM) guidelines, vs. no goal, using binary and multinomial logistic regression. Of the 292 respondents, 116 (40%) had no gestational weight gain goal, 112 (39%) had a concordant goal and 61 (21%) had a goal discordant with IOM guidelines. Predictors of a guideline-concordant goal, vs. no goal, included sugar sweetened beverage consumption < vs. ≥ 1 serving per week (OR = 2.4, 95%CI: 1.1, 5.7), physical activity ≥ vs. <2.5 h per week (OR = 3.6, 95%CI: 1.7, 7.5), agreeing that `I tried to keep weight down not to look pregnant' (OR = 14.3, 95%CI: 1.4, 140.5). Other predictors only of having a discordant goal (vs. no goal) included agreeing that `as long as I am eating well, I don't care how much I gain' (OR = 0.3, 95%CI: 0.2, 0.8) and agreeing that `if I gain too much weight one month, I try to keep from gaining the next' (OR = 4.1, 95%CI: 1.6, 10.4). Women whose doctors recommended weight gains consistent with IOM guidelines were more likely to have a concordant goal (vs. no goal) (OR = 5.3, 95%CI: 1.5, 18.6). Engaging in healthy behaviors and having health providers offer IOM weight gain recommendations may increase the likelihood of having a concordant gestational weight gain goal, which, in turn, is predictive of actual weight gains that fall within IOM guidelines

    The Evolutionarily-conserved Polyadenosine RNA Binding Protein, Nab2, Cooperates with Splicing Machinery to Regulate the Fate of pre-mRNA

    Numerous RNA binding proteins are deposited onto an mRNA transcript to modulate post-transcriptional processing events ensuring proper mRNA maturation. Defining the interplay between RNA binding proteins that couple mRNA biogenesis events is crucial for understanding how gene expression is regulated. To explore how RNA binding proteins control mRNA processing, we investigated a role for the evolutionarily conserved polyadenosine RNA binding protein, Nab2, in mRNA maturation within the nucleus. This work reveals that nab2 mutant cells accumulate intron-containing pre-mRNA in vivo. We extend this analysis to identify genetic interactions between mutant alleles of nab2 and genes encoding the splicing factor, MUD2, and the RNA exosome, RRP6, with in vivo consequences of altered pre-mRNA splicing and poly(A) tail length control. As further evidence linking Nab2 proteins to splicing, an unbiased proteomic analysis of vertebrate Nab2, ZC3H14, identifies physical interactions with numerous components of the spliceosome. We validated the interaction between ZC3H14 and U2AF2/U2AF^(65). Taking all the findings into consideration, we present a model where Nab2/ZC3H14 interacts with spliceosome components to allow proper coupling of splicing with subsequent mRNA processing steps contributing to a kinetic proofreading step that allows properly processed mRNA to exit the nucleus and escape Rrp6-dependent degradation

    Relation of severe COVID-19 to polypharmacy and prescribing of psychotropic drugs: the REACT-SCOT case-control study

    Background: The objective of this study was to investigate the relation of severe COVID-19 to prior drug prescribing. Methods: Severe cases were defined by entry to critical care or fatal outcome. For this matched case-control study (REACT-SCOT), all 4251 cases of severe COVID-19 in Scotland since the start of the epidemic were matched for age, sex and primary care practice to 36,738 controls from the population register. Records were linked to hospital discharges since June 2015 and dispensed prescriptions issued in primary care during the last 240 days. Results: Severe COVID-19 was strongly associated with the number of non-cardiovascular drug classes dispensed. This association was strongest in those not resident in a care home, in whom the rate ratio (95% CI) associated with dispensing of 12 or more drug classes versus none was 10.8 (8.8, 13.3), and in those without any of the conditions designated as conferring increased risk of COVID-19. Of 17 drug classes postulated at the start of the epidemic to be “ "medications compromising COVID", all were associated with increased risk of severe COVID-19 and these associations were present in those without any of the designated risk conditions. The fraction of cases in the population attributable to exposure to these drug classes was 38%. The largest effect was for antipsychotic agents: rate ratio 4.18 (3.42, 5.11). Other drug classes with large effects included proton pump inhibitors (rate ratio 2.20 (1.72, 2.83) for = 2 defined daily doses/day), opioids (3.66 (2.68, 5.01) for = 50 mg morphine equivalent/day) and gabapentinoids. These associations persisted after adjusting for covariates and were stronger with recent than with non-recent exposure. Conclusions: Severe COVID-19 is associated with polypharmacy and with drugs that cause sedation, respiratory depression, or dyskinesia; have anticholinergic effects; or affect the gastrointestinal system. These associations are not easily explained by co-morbidity. Measures to reduce the burden of mortality and morbidity from COVID-19 should include reinforcing existing guidance on reducing overprescribing of these drug classes and limiting inappropriate polypharmacy.     Is macroporosity controlled by complexed clay and soil organic carbon?

    Multi-scale evidence of rapid, climate-induced soil structural changes occurring at yearly to decadal timescales is mounting. As a result, it has become increasingly important to identify the properties and mechanisms controlling the development and maintenance of soil structure and associated macroporosity. This is especially relevant since macroporosity has disproportionate effects on saturated hydraulic conductivity ( ) which strongly influences water storage and flux, thus, affecting the water cycle. In this study, we use decision trees and piecewise linear regression to assess the influence of soil and climate properties on effective porosity (EP; a proxy of macroporosity) in both surface and subsurface horizons under varying land-use and management practices. Data from 1,491 pedons (3,679 horizons) spanning five ecoregions representing bioclimate (e.g., potential vegetation) across the conterminous US demonstrate that, at a continental scale, EP in surface (A) and subsurface (B) horizons is strongly dependent on the complexed fraction of the total mass of soil organic carbon (SOC) and clay; a combined fraction that we refer to as complexed organic carbon and clay (COCC). EP showed a slight positive response to COCC in A horizons but increased steeply with increasing COCC in B horizons. This is because the smaller values of COCC in B horizons reflect a larger pool of clay that has a greater potential to accommodate and complex additions of SOC promoting stronger organo-mineral bonds and the concomitant development and maintenance of soil structure in these horizons. In contrast, larger values of COCC in A horizons reflect conditions where all or most of the clay fraction is effectively complexed with SOC resulting in a larger pool of non-complexed soil organic matter with varying contrasting effects on macroporosity that ultimately mute the response of EP to increases in COCC. In surface horizons, indirect factors such as mean annual precipitation and land use were important predictors of EP, whereas COCC was more influential in controlling EP within the subsoil. The EP-COCC relationship also holds within ecoregions but its effect is mitigated by soil and climate interactions suggesting that the effect of climate on this relationship is indirect and complex. Plowed surface horizons and horizons underlying plowed layers showed greater homogenization (due to disturbance effects reducing heterogeneity in the soil) as well as a reduction in the magnitude and rate of change of EP as a function of COCC compared to undisturbed horizons. Our findings suggest that the complexed fraction of clay and SOC is important for controlling macroporosity and at ecoregion scales and that the EP-COCC relationship may be an important framework for understanding and predicting future land use- and climate-induced changes in soil hydraulic properties.publishedVersio

    A two-arm parallel-group individually randomised prison pilot study of a male remand alcohol intervention for self-efficacy enhancement:The APPRAISE study protocol

    Introduction The prevalence of at-risk drinking is far higher among those in contact with the criminal justice system (73%) than the general population (35%). However, there is little evidence on the effectiveness of alcohol brief interventions (ABIs) in reducing risky drinking among those in the criminal justice system, including the prison system and, in particular, those on remand. Building on earlier work, A two-arm parallel group individually randomised Prison Pilot study of a male Remand Alcohol Intervention for Self-efficacy Enhancement (APPRAISE) is a pilot study designed to assess the feasibility and acceptability of an ABI, delivered to male prisoners on remand. The findings of APPRAISE should provide the information required to design a future definitive randomised controlled trial (RCT). Methods and analysis APPRAISE will use mixed methods, with two linked phases, across two prisons in the UK, recruiting 180 adult men on remand: 90 from Scotland and 90 from England. Phase I will involve a two-arm, parallel-group, individually randomised pilot study. The pilot evaluation will provide data on the likely impact of A two-arm parallel group individually randomised Prison Pilot study of a male Remand Alcohol Intervention for Self-efficacy Enhancement (APPRAISE), which will be used to inform a future definitive multicentre RCT. Phase II will be a process evaluation assessing how the ABI has been implemented to explore the change mechanisms underpinning the ABI (figure 1) and to assess the context within which the ABI is delivered. Ethics and dissemination The APPRAISE protocol has been approved by the East of Scotland Research Ethics Committee (19/ES/0068), National Offender Management System (2019-240), Health Board Research and Development (2019/0268), Scottish Prison Service research and ethics committee, and by the University of Edinburgh’s internal ethics department.     Exploring embodiment through martial arts and combat sports: a review of empirical research

    Since the late 1970s, social scientists have turned considerable attention to investigating martial arts and combat sports (MACS). In particular, this broad range of fighting disciplines has been shown to offer numerous avenues for scholarly enquiry into social change and personal transformation via processes of embodiment. Adopting a thematic structure, we assess the empirical literature in this area via four interconnecting categories pertaining to MACS and embodiment: (1) body cultures; (2) body pedagogies; (3) the embodiment of gender; and (4) bodily harm. Following this review, we identify several gaps in the existing literature, suggesting potential new topics and strategies for research connecting to the social world of physical culture more generally

    Effects of antiplatelet therapy on stroke risk by brain imaging features of intracerebral haemorrhage and cerebral small vessel diseases: subgroup analyses of the RESTART randomised, open-label trial

    Background Findings from the RESTART trial suggest that starting antiplatelet therapy might reduce the risk of recurrent symptomatic intracerebral haemorrhage compared with avoiding antiplatelet therapy. Brain imaging features of intracerebral haemorrhage and cerebral small vessel diseases (such as cerebral microbleeds) are associated with greater risks of recurrent intracerebral haemorrhage.
