    Os níveis de maturidade dos processos de negócio de uma organização permitem a promoção de pesquisas de mensuração qualitativas e, ou, quantitativas, realizando análises de desempenho empresarial, e permitindo a otimização de suas próprias atividades executadas e fornecidas. Fundamentando-se no trabalho de Michael Hammer, o Process Enterprise Maturity Model (PEMM) surge como uma estrutura que auxilia os tomadores de decisão no entendimento, análise e avaliação de seus processos. Baseia-se na análise e avaliação da maturidade dos processos organizacionais em uma empresa que busca a otimização de seus processos. O objetivo geral do trabalho centra-se na identificação do nível de maturidade dos processos de negócio na área de PD (Pesquisa e Desenvolvimento) em uma empresa de consultoria do setor de tecnologia da informação, apresentando-se como uma metodologia aplicada do modelo PEMM, por estudo de caso. Os resultados, focando-se, especialmente, a percepção dos profissionais envolvidos no nível hierárquico organizacional operacional, apontam que a organização estudada encontra-se nos níveis P-1 (relação de maturidade comprocessos de negócio) e E-1 (relação de maturidade com capacidades empresariais) em relação ao modelo PEMM, enfatizando a necessidade de realização de trabalhos voltados para as áreas estudadas

    Endodontic management of type II dens invaginatus with open apex and large periradicular lesion using the XP-endo Finisher : a case report

    Dens invaginatus (DI) represents an endodontic challenge because of its complex root canal morphology. This case report presents the clinical management of a 22-year-old woman with type II DI in right maxillary lateral incisor with a painful swelling. Pulp testing revealed no response with the tooth. Type II DI with open apex and large periradicular lesion was seen on radiograph. The treatment was planned by using cone-beam computed tomography (CBCT) imaging. Canal treatment was completed in two appointments with the aid of a dental operating microscope. In the first appointment, the internal anatomy was modified using an ultrasonic tip, and chemo-mechanical preparation was performed using the XP-endo Finisher instrument and NaOCl; calcium hydroxide intracanal dressing was used for one month. In the second appointment, an apical plug of mineral trioxide aggregate (MTA) Repair HP was performed and the remaining pulp space was then filled with gutta-percha and AH Plus sealer using the continuous wave of condensation technique. At the fourteen-month reevaluation, the patient was asymptomatic, the tooth had remained functional, and radiographic and CBCT assessment showed significant osseous healing of the lesion. Successful non-surgical management of the present type II DI was achieved in the present case. The association of CBCT, dental operating microscope, XP-endo Finisher, NaOCl and MTA Repair HP were important for ensuring a predictable outcome

    Cleaning ability of chlorhexidine gel and sodium hypochlorite associated or not with EDTA as root canal irrigants: a scanning electron microscopy study

    The aim of this study was to evaluate the cleaning efficacy of 2% chlorhexidine gluconate gel (CHX) compared to 2.5% sodium hypochlorite solution (NaOCl) associated or not with 17% EDTA used as irrigants during the biomechanical preparation. Fifty freshly extracted single-rooted human teeth with complete apex formation were randomly divided into five groups: G1 - sterile saline, G2 - 2.5% NaOCl, G3 - 2% CHX, G4 - 2.5% NaOCl + EDTA and G5 - 2% CHX + EDTA. The specimens of G1 were subdivided into two control groups. The teeth were decoronated and the coronal and middle root thirds were prepared with Gates-Glidden burs, and the apical third was reserved to manual instrumentation. All procedures were performed by a single operator. In all groups, 2 mL of irrigant was delivered between each file change. The teeth were sectioned and prepared for analyses under scanning electron microscopy (SEM). SEM micrographs were graded according to a score scale by two examiners. Data were analyzed statistically by Kruskal-Wallis and Dunn tests at 1% significance level. The best results were obtained in the groups in which the irrigant was used followed by the chelating agent.     Push-out bond strength of MTA HP, a new high-plasticity calcium silicate-based cement

    This study was designed to investigate the resistance to dislodgment provided by MTA HP, a new high-plasticity calcium silicate-based cement. Biodentine and White MTA Angelus were used as reference materials for comparison. Three discs 1 ± 0.1 mm thick were obtained from the middle third of the roots of 5 maxillary canines. Three 0.8-mm-wide holes were drilled on the axial surface of each root disc. Standardized irrigation was performed. Then the holes were dried with paper points and filled with one of the three tested cements. The filled dental slices were immersed in a phosphate-buffered saline (PBS) solution (pH 7.2) for 7 days before the push-out assessment. The Kruskal-Wallis test was applied to assess the effect of each endodontic cement on the push-out bond strength. Mann-Whitney with Bonferroni correction was used to isolate the differences. The alpha-type error was set at 0.05. All specimens had measurable push-out values and no premature failure occurred. There were significant differences among the materials (p <0.05). The Biodentine specimens had the highest push-out bond strength values (p < 0.05). MTA HP had significantly higher bond strength than White MTA (p < 0.05).     Analysis of the film thickness of a root canal sealer following three obturation techniques

    O objetivo deste estudo foi realizar uma análise quantitativa da espessura da linha de cimento endodôntico formada por três técnicas de obturação. Trinta incisivos centrais superiores humanos foram selecionados e acessados de modo convencional. Uma lima nº 15 K-Flexofile foi usada para a verificação da patência foraminal e para determinação do comprimento de trabalho, que foi estabelecido a 1 mm aquém do forame apical. Os canais foram preparados até a lima nº 45. Hipoclorito de sódio a 5,25% foi usado durante toda a instrumentação. Os dentes foram divididos em 3 grupos e obturados pelos seguintes critérios: G1 - condensação lateral; G2 - compressão hidraúlica e G3 - onda de condensação. Os dentes foram seccionados e analisados nos terços cervical, médio e apical. A espessura do filme de cimento foi determinada por um processo semi-automático de análise e processamento digital de imagens realizado no software KS 400. O tratamento estatístico foi realizado com teste de Wilcox, que revelou diferenças significantes entre G3 e G1 e entre G3 e G2, sendo p £ 0,05. Não houve diferenças estatisticamente significantes entre G1 e G2. De um modo geral, a compressão hidraúlica (G2) e condensação lateral (G1) revelaram uma maior espessura do filme de cimento. A menor espessura do filme de cimento obtida pela técnica de onda de condensação tende a melhorar a performance clínica dessa técnica em relação às outras testadas.The aim of this study was to obtain a quantitative analysis of the film thickness of a root canal sealer formed after filling by three different techniques. Thirty human maxillary incisors were selected and access cavities were prepared using high-speed diamond stones and water spray. A size #15 K-Flexofile was introduced in the canal of each specimen until it was seen just at the apical foramen. The working length was determined to be 1 mm short of that position and the canals were prepared to an apical size of #45 K-Flexofile. Copious irrigation with 5.25% NaOCl (sodium hypochlorite) was used during and after instrumentation. The samples were divided into three groups and obturated as follows: G1 - lateral condensation, G2 - lateral condensation with an accessory cone, and G3 - continuous wave of condensation. The samples were evaluated in the cervical, middle and apical thirds. The film thickness of the root canal sealer was measured through a microscopic evaluation. Statistical analysis was obtained using the Wilcox test. Statistical analysis showed significant differences between G3 and G1, G3 and G2 (p < 0.05). In general, the lowest film thickness was observed in the continuous wave of condensation (G3). Lateral condensation with an accessory cone (G2) and lateral condensation (G1) demonstrated poorer results in this study, showing a higher film thickness. The small film thickness of the sealer obtained by the continuous wave of condensation technique may increase the clinical performance of this technique

    Penetração intratubular de cimentos endodônticos

    The purpose of this study was to evaluate the capacity of penetration of four endodontic sealers (Endo Fill, Sealapex, AH Plus and Pulp Canal Sealer) into dentinal tubules. Seventy-two extracted human maxillary anterior teeth were utilized in this study. The teeth were cleaned and shaped by means of the balanced-forces technique. The work length was established at 1 mm beyond the apex. Copious irrigation with 10 ml of 5.25% sodium hypochlorite was carried out. The teeth were divided in 8 groups - 4 had the smear layer maintained, and 4 had it removed. The smear layer was removed with a commercial solution of 17% EDTA, and the root canal system was flushed for 3 min. Finally, the roots were irrigated with 3 ml of 5.25% sodium hypochlorite. All teeth were sealed by means of the technique of the condensation wave with a medium nonstandardized cone. After filling, the roots were grooved, longitudinally split and examined under a scanning electron microscope (SEM). The focus of observation was the interface between the dentin and the sealing material. The Rickert sealer (Pulp Canal) presented the maximum penetration depths into the dentinal tubules, and Sealapex, the minimum. The Spearman test was used to determine whether there were significant differences between the groups. The removal of smear layer allowed significant penetration of the sealers (pEste estudo foi desenvolvido para avaliar a capacidade de penetração de diferentes cimentos endodônticos (Endo Fill, Sealapex, AH Plus e Pulp Canal Sealer) nos túbulos dentinários em dentes devidamente modelados e obturados. Foram utilizados 72 incisivos centrais superiores, os quais foram instrumentados no sentido coroa-ápice pela técnica de forças balanceadas. O comprimento de trabalho foi estabelecido a 1 mm do ápice radicular. Durante a limpeza e modelagem, todos os dentes foram irrigados com 10 ml de hipoclorito de sódio a 5,25%. Os dentes foram divididos em 4 grupos, sendo 1 para cada tipo de cimento. Estes grupos foram então subdivididos em função do uso ou não de EDTA a 17% previamente à obturação dos canais radiculares, para a remoção da lama dentinária. Todos os dentes foram obturados pela técnica da onda de condensação com cone médio calibrado. Após obturação, as raízes foram seccionadas no sentido mésio-distal e foi escolhida a secção de melhor qualidade visual. Estas foram então analisadas em microscopia eletrônica de varredura (MEV), sendo o foco de observação sempre a interface dentina/material obturador. Após obtenção das imagens, mensurou-se os prolongamentos dos cimentos para o interior dos túbulos dentinários. O cimento de Rickert (Pulp Canal Sealer) apresentou a maior capacidade de penetração nos túbulos dentinários, sendo os piores resultados apresentados pelo grupo em que se utilizou o Sealapex.     Recurrent kala-azar: report of two cured cases after total splenectomy

    In Latin America, the causative agent of kala-azar is the intracellular protozoan Leishmania infantum. Most cases in South America are reported in Brazil. Worldwide, it mainly affects Bangladesh, Ethiopia, India, South Sudan and Sudan. Despite the high morbidity and lethality of kala-azar, most infections are asymptomatic. However, a small portion of patients evolves with recurrence of kala-azar becoming symptomatic even after all available drug treatments. Kala-azar is not a formal indication for splenectomy in adults. Splenectomy is recommended as a saving measure, when kala-azar is associated with symptomatic hypersplenism and for drug-resistant cases. In the study, we report two cases of kala-azar with splenomegaly that presented several hospitalizations due to the recurrence of the kala-azar, in addition to hospitalizations for normalizing the blood count. After splenectomy, kala-azar cases and the effects of hypersplenism are cured. Thus, splenectomy should be seen as a surgical treatment option with a curative purpose in patients with recurrent kala-azar, in whom the possibilities of drug therapy have been exhausted and even so they progressed with hypersplenism and clinical repercussions

    Anatomia descritiva comparativa da artéria femoral de cachorro-do-mato, raposa-do-campo e lobo-guará

    O objetivo deste estudo foi descrever a anatomia da artéria femoral em canídeos selvagens, como o cachorro do mato (Cerdocyon thous), a raposa do campo (Lycalopex vetulus) e o lobo-guará (Chrysocyon brachyurus). Foram utilizados dois espécimes de cada grupo canídeo. Solução de látex vermelha foi injetada no sistema arterial dos animais, que foram então fixados em solução aquosa de formaldeído a 10% e dissecados seguindo as técnicas rotineiras da anatomia macroscópica. Nos três grupos canídeos, o padrão arterial foi semelhante ao descrito para canídeos domésticos, em que a artéria femoral profunda origina da artéria ilíaca externa, ainda na cavidade abdominal, e envia seu primeiro ramo, a artéria femoral circunflexa lateral. Alguns ramos musculares, uma ou duas artérias femorais caudais e os ramos terminais - a artéria genicular descendente, a artéria safena e a artéria poplítea - são originários da artéria femoral. O padrão de origem desses vasos também mostra semelhanças com as de canídeos domésticos, às vezes formando troncos e ocasionalmente originando individualmente. Assim, pode-se concluir que o padrão anatômico da artéria femoral e seus ramos em canídeos selvagens mostra semelhanças com a dos canídeos domésticos, mas variações inerentes em cada espécie também estão presentes.The goal of this study was to describe the anatomy of the femoral artery in wild canids such as the crab-eating fox (Cerdocyon thous), the hoary fox (Lycalopex vetulus), and the maned wolf (Chrysocyon brachyurus). Two specimens from each canid group were used. Red dyed latex was injected in the arterial system of the animals, which were then fixed in 10% formaldehyde aqueous solution and dissected following the routine techniques in macroscopic anatomy. In the three canid groups, the arterial pattern was similar to that described for domestic canids, in which the deep femoral artery rises from the external iliac artery, still in the abdominal cavity, and sends its first branch, the lateral circumflex femoral artery. A few muscular branches, one or two caudal femoral arteries, and the terminal branches – the descending genicular artery, the saphenous artery, and the popliteal artery – are all originated from the femoral artery. The origin pattern of these vessels also shows similarities with those from domestic canids, sometimes forming trunks and occasionally rising individually. Thus, it can be concluded that the anatomical pattern of the femoral artery and its branches in wild canids shows similarities with that from domestic canids, but inherent variations in each species are also present

    Three-dimensional quantitative porosity characterization of syringe- versus hand-mixed set epoxy resin root canal sealer

All rights reserved.The aim of the present study was to compare the porosity characteristics of AH Plus Jet™ syringe-mix and the conventional hand-mixed AH Plus root canal sealers by three-dimensional quantitative high-resolution micro-computed tomography (micro-CT). Seven test specimens were prepared from each tested sealer by a single operator following the manufacturer’s instructions and poured into pre-lubricated plastic split-ring moulds. Set sealer test specimens were scanned using a micro-CT device and the shadow images were reconstructed into cross-sectional slices. The evaluated parameters were (i) total pore count, (ii) total pore volume and mean pore volume, (iii) total porosity (% of pore volume in relation to total sealer volume) and (iv) mean pore distance to the sealer lateral external surface. In both groups, most pores were localized within the external sealer perimeter (0.05 mm from the external surface). Hand-mixed AH Plus specimens showed statistically significant higher mean total pore count, total pore volume and total porosity (p=0.001) than the syringe-mixed specimens. However, mean pore sizes in AH Plus syringe-mixed specimens were significantly higher (p=0.046) than the AH Plus hand-mixed counterparts. Hand-mixed AH Plus was associated with higher total mean pore count, volume and total porosity compared to syringe mixed AH Plus.Link_to_subscribed_fulltex
