    Peripartum Cardiomyopathy. Post Partum Cardiorespiratory Arrest Complicated by Placental Accretism

    Peripartum cardiomiopathy is a rare and life-threatening cardiac disease that affects young women previously healthy, during the peripartum period. It is a form of dilated cardiomyopathy with left-sided systolic dysfunction, which may lead to symptoms and signs of congestive heart failure. The exclusion diagnosis is based essentially on clinical presentation and initial symptoms may mimic physiologic alterations of pregnancy. The authors present a case of a 34 week multiple gestation with a growth restriction of one of the fetus and with a suspicion of a mild pre-eclampsia, motive by which we decided labour induction. During placental expulsion, in which we noticed difficulty in finding placental cleavage, the patient presented an assistoly, recovering after cardiorespiratory reanimation. However, the profuse bleeding after labour leaded to a life saving hysterectomy. Histological examination revealed placenta accreta. The echocardiography performed post-operatively diagnosed a dilated cardiomyopathy

    Nonlinear interaction between two heralded single photons

    Harnessing nonlinearities strong enough to allow two single photons to interact with one another is not only a fascinating challenge but is central to numerous advanced applications in quantum information science. Currently, all known approaches are extremely challenging although a few have led to experimental realisations with attenuated classical laser light. This has included cross-phase modulation with weak classical light in atomic ensembles and optical fibres, converting incident laser light into a non-classical stream of photon or Rydberg blockades as well as all-optical switches with attenuated classical light in various atomic systems. Here we report the observation of a nonlinear parametric interaction between two true single photons. Single photons are initially generated by heralding one photon from each of two independent spontaneous parametric downconversion sources. The two heralded single photons are subsequently combined in a nonlinear waveguide where they are converted into a single photon with a higher energy. Our approach highlights the potential for quantum nonlinear optics with integrated devices, and as the photons are at telecom wavelengths, it is well adapted to applications in quantum communication.Comment: 4 pages, 4 figure

    Dark matter from cosmic defects on galactic scales?

    We discuss the possible dynamical role of extended cosmic defects on galactic scales, specifically focusing on the possibility that they may provide the dark matter suggested by the classical problem of galactic rotation curves. We emphasize that the more standard defects (such as Goto-Nambu strings) are unsuitable for this task, but show that more general models (such as transonic wiggly strings) could in principle have a better chance.     Self-reported variables as determinants of upper limb musculoskeletal symptoms in assembly line workers

    Background: Assembly lines work is frequently associated to work-related upper limb musculoskeletal disorders. The related disability and absenteeism make it important to implement efficient health surveillance systems. The main objective of this study was to identify self-reported variables that can determine work-related upper limb musculoskeletal symptoms—discomfort/pain–during a 6-month follow-up. Methods: This was a prospective study with a 6-month follow-up period, performed in an assembly line. Upper limb musculoskeletal discomfort/pain was assessed through the presence of self-reported symptoms. Uni- and multivariate logistic regression analyses were used to evaluate which self-reported variables were associated to upper limb symptoms after 6 months at the present and to upper limbs symptoms in the past month. Results: Of the 200 workers at baseline, 145 replied to the survey after 6 months. For both outcomes, “having upper limb symptoms during the previous 6 months” and “education” were possible predictors. Conclusion: Our results suggest that having previous upper limb symptoms was related to its maintenance after 6 months, sustaining it as a specific determinant. It can be a hypothesis that this population had mainly workers with chronic symptoms, although our results give only limited support to self-reported indicators as determinants for upper limb symptoms. Nevertheless, the development of an efficient health surveillance system for high demanding jobs should implicate self-reported indicators, but also clinical and work conditions assessment should be accounted on the future.publishersversionpublishe

    The occurrence of de novo psychogenic seizures after epilepsy surgery is rare, and is estimated in 1.8% to 3.6%. Seizures after epilepsy surgery should be carefully evaluated, and de novo psychogenic seizures should be considered especially when there is a change in the ictal semiology. We report a patient with de novo psychogenic seizures after anterior temporal lobe removal for refractory temporal lobe epilepsy. Once psychogenic seizures were diagnosed and psychiatric treatment was started, seizures stopped.58535-

    Path analysis from COVID‐19 perceptions to psychological health: The roles of critical distance and mastery

    Objective: The Coronavirus disease 2019 (COVID-19) pandemic was previously associated with psychopathological symptoms. However, the psychological mechanisms underlying these associations are largely unexplored. Previous studies suggested associations between metacognitive abilities (e.g., mastery) and symptomatology, which may have impacts on COVID-19 perceptions. This study aims to explore, using path analysis, the mediational role of Critical Distance (differentiation and decentration abilities) and Mastery on the relationships between COVID-19 perceptions and psychological well-being and distress. Methods: In a cross-sectional design, 227 participants (M = 34.21, SD = 10.9) filled self-report questionnaires. Results: Metacognitive abilities were negatively correlated with psychopathological symptoms. Both Critical Distance and Mastery mediated the path from COVID-19 perceived severity and anxiety to psychological distress and well-being. Critical Distance seems to augment Mastery which tends to increase psychological well-being and limited psychological distress. Conclusions: Metacognition seems to play a mediational role on the relationship between COVID-19 perceptions and mental health. Clinical psychologists and psychotherapists may enhance psychological interventions regarding COVID-19 psychopathological symptomatology by working on metacognitive Critical Distance and Mastery abilities.info:eu-repo/semantics/acceptedVersio

    An examination of the influence of emotional solidarity on value cocreation with international muslim travelers

    Given the rising Muslim travel market and the scarcity of research on Muslim inbound market, it is important to elucidate how international Muslim travelers are perceived through the lens of residents in non-Islamic countries. This study provided an insight into how value cocreation is created through emotional solidarity and attitudes in the context of the Muslim travel market. Results of the structural model for American residents were comparatively similar to those of their Korean counterparts, which is indicative of both countries’ acceptance and tolerance of international Muslim travelers. In particular, residents’ openness to experience and the perceived benefit of international Muslim travelers for development of the tourist destination were significant predictors of emotional solidarity. Residents’ emotional solidarity subsequently explained their attitude toward international Muslim travelers. Attitude toward international Muslim travelers would lead to a positive attitude toward Muslim tourism, which in turn, triggered value cocreation behavior with international Muslim travelers.info:eu-repo/semantics/acceptedVersio

    Estudos sobre a diversidade genética de espécies com finalidade de melhoramento genético representam importante preocupação dos melhoristas. Diversas técnicas moleculares têm permitido revelar uma diversidade genética presente no genoma que até então era desconhecida pelos cientistas. Muitas espécies de interesse agronômico têm sido alvo de estudos que abordam a diversidade genética em nível molecular. De uma maneira geral, os resultados obtidos a partir do uso dos marcadores têm sido bastante satisfatórios, principalmente para aquelas espécies que apresentam ampla base genética e insuficiente número de descritores botânicos para diferenciação de genótipos, como é o caso da mandioca. A base genética formada pelos cultivares de C. arabica, principal espécie cultivada, é considerada estreita. Em relação ao Brasil, principal produtor mundial desta cultura, a própria história explica em parte a ausência de variação genética nos materiais atualmente em cultivo. Historicamente, as primeiras plantações de café formaram-se há mais de dois séculos a partir de poucas plantas, constituindo material muito uniforme e de pouca variabilidade genética. De fato, estudos sobre a diversidade genética de cafeeiros em cultivo realizados no IAC utilizando descritores botânicos e agronômicos têm revelado baixo nível de variação genética entre as diferentes linhagens que compõem um cultivar. Entre diferentes cultivares esta variação é mais notória, mão não tão acentuada como se pensava, mesmo quando diferentes espécies participam da genealogia destes cultivares. Desta maneira, este trabalho teve como objetivo principal a avaliação das técnicas de RAPD e AFLP para a identificação e caracterização de diversas linhagens comerciais de C. arabica selecionadas pelo IAC. Os resultados obtidos com estes marcadores confirmam os dados obtidos anteriormente de que a variabilidade genética entre as linhagens é pequena. Além disso, os dados sugerem que a técnica de RAPD não é eficiente para a identificação e determinação da distância genética entre as linhagens avaliadas, apesar de representar uma ferramenta adequada para avaliação da variabilidade no cafeeiro. No entanto, embora o nível de variação observado através dos marcadores moleculares seja baixo, sabe-se que estas linhagens apresentam comportamento agronômico bastante diferenciado. Isso nos leva a pensar que poucos genes diferenciam os principais materiais genéticos de cafeeiro de importância agronômica e que o uso de marcadores moleculares para estudos de diversidade genética e localização de genes de interesse agronômico deve ser repensada

    Ruin probabilities and capital requirement for open automobile portfolios with a bonus-malus system based on claim counts

    For a large motor insurance portfolio, on an open environment, we study the impact of experience rating in finite and continuous time ruin probabilities. We consider a model for calculating ruin probabilities applicable to large portfolios with a Markovian Bonus‐Malus System (BMS), based on claim counts, for an automobile portfolio using the classical risk framework model. New challenges are brought when an open portfolio scenario is introduced. When compared with a classical BMS approach ruin probabilities may change significantly. By using a BMS of a Portuguese insurer, we illustrate and discuss the impact of the proposed formulation on the initial surplus required to target a given ruin probability. Under an open portfolio setup, we show that we may have a significant impact on capital requirements when compared with the classical BMS, by having a significant reduction on the initial surplus needed to maintain a fixed level of the ruin probabilityinfo:eu-repo/semantics/publishedVersio
