LâEROP : 10 ans pour le rĂ©tablissement des oiseaux de proie au QuĂ©bec
LâĂquipe de rĂ©tablissement des oiseaux de proie du QuĂ©bec (EROP) a Ă©tĂ© fondĂ©e en 2004, Ă la suite de la fusion des Ă©quipes de rĂ©tablissement du faucon pĂšlerin (Falco peregrinus), du pygargue Ă tĂȘte blanche (Haliaeetus leucocephalus) et de lâaigle royal (Aquila chrysaetos). Ă ces espĂšces dâintĂ©rĂȘt pour lâEROP sâest ajoutĂ© rĂ©cemment le hibou des marais (Asio flammeus). Ă lâaide des plans de rĂ©tablissement de chacune des espĂšces, lâEROP veille Ă la mise en oeuvre de mesures de conservation (p. ex. acquisition de connaissances, sensibilisation, protection) visant Ă redresser la situation des populations dâoiseaux de proie en situation prĂ©caire au QuĂ©bec. Cet article prĂ©sente le mandat, le mode de fonctionnement et les principales rĂ©alisations de lâEROP au cours de la derniĂšre dĂ©cennie.
The Québec raptor recovery team (EROP) was established in 2004 following the merger of the peregrine falcon (Falcoperegrinus), bald eagle (Haliaeetus leucocephalus) and golden eagle (Aquilachrysaetos) recovery teams. Recently, the short-eared owl (Asioflammeus) has also been added to the list of species covered by EROP. The team aims to foster conservation measures (e.g., data collection, public awareness and protection) to improve the population status of endangered birds of prey, and achieves this through the publication of species recovery plans. This article outlines the mission, and the working and main achievements of EROP over the past decade
Rapid response to the M_w 4.9 earthquake of November 11, 2019 in Le Teil, Lower RhĂŽne Valley, France
On November 11, 2019, a Mw 4.9 earthquake hit the region close to Montelimar (lower RhĂŽne Valley, France), on the eastern margin of the Massif Central close to the external part of the Alps. Occuring in a moderate seismicity area, this earthquake is remarkable for its very shallow focal depth (between 1 and 3 km), its magnitude, and the moderate to large damages it produced in several villages. InSAR interferograms indicated a shallow rupture about 4 km long reaching the surface and the reactivation of the ancient NE-SW La Rouviere normal fault in reverse faulting in agreement with the present-day E-W compressional tectonics. The peculiarity of this earthquake together with a poor coverage of the epicentral region by permanent seismological and geodetic stations triggered the mobilisation of the French post-seismic unit and the broad French scientific community from various institutions, with the deployment of geophysical instruments (seismological and geodesic stations), geological field surveys, and field evaluation of the intensity of the earthquake. Within 7 days after the mainshock, 47 seismological stations were deployed in the epicentral area to improve the Le Teil aftershocks locations relative to the French permanent seismological network (RESIF), monitor the temporal and spatial evolution of microearthquakes close to the fault plane and temporal evolution of the seismic response of 3 damaged historical buildings, and to study suspected site effects and their influence in the distribution of seismic damage. This seismological dataset, completed by data owned by different institutions, was integrated in a homogeneous archive and distributed through FDSN web services by the RESIF data center. This dataset, together with observations of surface rupture evidences, geologic, geodetic and satellite data, will help to unravel the causes and rupture mechanism of this earthquake, and contribute to account in seismic hazard assessment for earthquakes along the major regional CĂ©venne fault system in a context of present-day compressional tectonics
Healthcare resource utilization, healthcare costs and dose escalation in psoriasis patients initiated on ustekinumab versus adalimumab: a retrospective claim study
Background: Adalimumab and ustekinumab are effective psoriasis treatments. This study compares healthcare resource utilization (HRU), costs and dose escalation and describes starting dose trends in ustekinumab versus adalimumab psoriasis patients. Methods: Adult psoriasis patients initiating adalimumab/ustekinumab on/after 25 September 2009 were selected from a US claims database and classified into biologic-naĂŻve and biologic-experienced samples. Results: A total of 602 ustekinumab and 3470 adalimumab biologic-naĂŻve and 1193 ustekinumab and 1467 adalimumab biologic-experienced patients were included. In both samples, ustekinumab patients had significantly more days with medical services (biologic naĂŻve: IRR =1.14; biologic experienced: IRR =1.08) and higher average total costs by more than $14,000 annually. Ustekinumab users were 2.6 and 1.9 times more likely to have a dose escalation (increase â„45âmg in ustekinumab;ââ„40âmg in adalimumab) in biologic-naive and biologic-experienced patients, respectively. Between S2/2009 and S1/2012, the proportion of patient initiating on high dose (ustekinumab:â>45âmg/28 days; adalimumab: >160âmg/28 days) increased substantially for ustekinumab patients (biologic naĂŻve:â+18.6 percentage points [PP]; biologic experienced:â+29.9 PP) but remain stable for adalimumab patients (biologic naĂŻve: â0.3 PP; biologic experienced: +2.3 PP). Conclusion: Ustekinumab patients had more HRU, higher total costs and were more likely to have a dose escalation. The proportion of patients initiating ustekinumab high dose increased substantially between 2009 and 2012

The Use of Cavitron Ultrasonic Surgical Aspirator for High-Risk Neuroblastoma with Image-Defined Risk Factors in Children
Aim of the study: The cavitron ultrasonic surgical aspirator (CUSA) has gained popularity in adult surgical oncology, but its application in children is limited to liver surgery and neurosurgical procedures. The complete resection of neuroblastoma with image-defined risk factors (IDRFs) is still considered one of the most difficult procedures to achieve in pediatric surgical oncology, with a high morbidity rate and potential risk of intraoperative mortality. The aim of our study is to describe the application of ultrasonic dissection in neuroblastoma with IDRFs. Methods: A retrospective study was performed, analyzing patients operated on from 2000 to 2018. Patient characteristics, resection completeness, and postoperative surgical and oncology outcomes were analyzed. Main results: Twenty-six patients with high-risk neuroblastoma and IDRFs were operated on in the study period with a CUSA. A complete macroscopic resection was performed in 50% of patients, while the other half was operated on with minimal residual (Conclusions: The application of the CUSA in neuroblastoma with IDRFs can be considered an effective and safe alternative technique to achieve a radical resection
Treatment patterns, healthcare resource utilization, and costs following first-line antidepressant treatment in major depressive disorder: a retrospective US claims database analysis
Abstract Background Although the symptoms of major depressive disorder (MDD) are often manageable with pharmacotherapy, response to first-line antidepressant treatment is often less than optimal. This study describes long-term treatment patterns in MDD patients in the United States and quantifies the economic burden associated with different treatment patterns following first-line antidepressant therapy. Methods MDD patients starting first-line antidepressant monotherapy and having continuous enrollment â„12Â months before and â„24Â months following the index date (i.e., the first documented prescription fill) were selected from the Truven Health Analytics MarketScan (2003â2014) database. Based on the type of first treatment change following initiation, six treatment cohorts were defined a priori (âpersistenceâ; âdiscontinuationâ; âswitchâ; âdose escalationâ; âaugmentationâ; and âcombinationâ). Treatment patterns through the fourth line of therapy within each cohort, healthcare resource utilization (HCRU), and cost analyses were restricted to patients with adequate treatment duration (defined as â„42Â days) in each line (analysis sub-sample, NÂ =Â 21,088). HCRU and costs were described at the cohort and pattern levels. Treatment cohorts representing <5% of the analysis sub-sample were decided a priori not to be analyzed due to limited sample size. Results 39,557 patients were included. Mean age was 42.1Â years, 61.1% of patients were female, and mean follow-up was 4.1Â years. Among the analysis sub-sample, the discontinuation (49.1%), dose escalation (37.4%), and switch (6.6%) cohorts were the most common of all treatment cohorts. First-line antidepressant discontinuation without subsequent MDD pharmacotherapy (22.9%) and cycling between discontinuation and resumption (11.2%) were the two most common treatment patterns. Median time to discontinuation was 23Â weeks. The switch cohort exhibited the highest HCRU (18.9Â days with medical visits per-patient-per-year) and greatest healthcare costs ($11,107 per-patient-per-year) following the index date. Treatment patterns representing a cycling on and off treatment in the switch cohort were associated with the greatest healthcare costs overall. Conclusion A high proportion of patients discontinue first-line antidepressant shortly after initiation. Patterns representing a cycling on and off treatment in the switch cohort were associated with the highest healthcare costs. These findings underscore challenges in effectively treating patients with MDD and a need for personalized patient management
