The determinants of use of maternal and child health services in Metro Cebu, the Philippines
The determinants of the use of family planning, prenatal care, childhood immunizations and oral rehydration salts (ORS) were studied with survey data of 8000 women in Metro Cebu, the Philippines. Polytomous logistic regression methods were used. The level of maternal education was the most consistent and important determinant of use of these four health services in both urban and rural areas. For example, the estimated odds of using modern contraception increased by six and eleven per cent for each additional year of schooling in urban and rural strata respectively. Economic status and access to service variables had less consistent patterns: women’s work status, for example, was associated only with contraceptive use
Adrenal Cushing Syndrome Diagnosed during Pregnancy: Successful Medical Management with Metyrapone
Adrenal Cushing syndrome during pregnancy is rare, and there is limited information on the effect and safety of metyrapone treatment both for mother and fetus. We present a 24-year-old woman diagnosed with adrenal Cushing syndrome at the end of the second trimester. We elected treatment with metyrapone titrated to 250 mg 3 times daily, resulting in good clinical response and maternal serum and saliva cortisol levels in the upper half of the normal pregnancy range. A healthy male infant was born at 35 weeks\u27 gestation, with no clinical signs of adrenal insufficiency, this despite a low cortisol of 5 nmol/L on the first day of life. We measured metyrapone in maternal and umbilical cord blood samples, demonstrating fetal venous metyrapone levels similar to maternal venous concentration, and a fetal arterial cord concentration at about 60% of the fetal venous cord concentration. This case demonstrates that salivary cortisol levels may be used to monitor the effect of metyrapone on adrenal Cushing syndrome during pregnancy. We show, for the first time in humans, that metyrapone does cross the placenta and may suppress fetal cortisol production without necessarily causing clinical signs of adrenal insufficiency

Financial Data Transparency, International Institutions, and Sovereign Borrowing Costs
Recent events in international finance illustrate the close connection between the viability of a country's major private financial institutions and the sustainability of its sovereign debt. We explore the precise nature of this connection and the ways in which it shapes investors’ expectations of sovereign creditworthiness. We consider how investors use the overall level of information available about the private financial sector—and the potential risks it poses to government finances—when making decisions about investing in sovereign debt. We expect that governments providing more information about the private financial sector will have lower, and less volatile, borrowing costs. In order to test this argument, we create a new Financial Data Transparency (FDT) Index measuring governments’ willingness to release credible financial system data. Using the FDT and a sample of high-income OECD countries, we find that such transparency reduces sovereign borrowing costs. The effects are conditional on the level of public indebtedness. Transparent countries with low debt enjoy lower and less volatile borrowing costs
Biased competition through variations in amplitude of γ-oscillations
Experiments in visual cortex have shown that the firing rate of a neuron in response to the simultaneous presentation of a preferred and non-preferred stimulus within the receptive field is intermediate between that for the two stimuli alone (stimulus competition). Attention directed to one of the stimuli drives the response towards the response induced by the attended stimulus alone (selective attention). This study shows that a simple feedforward model with fixed synaptic conductance values can reproduce these two phenomena using synchronization in the gamma-frequency range to increase the effective synaptic gain for the responses to the attended stimulus. The performance of the model is robust to changes in the parameter values. The model predicts that the phase locking between presynaptic input and output spikes increases with attention
The Timing of the Cognitive Cycle
We propose that human cognition consists of cascading cycles of recurring brain
events. Each cognitive cycle senses the current situation, interprets it with
reference to ongoing goals, and then selects an internal or external action in
response. While most aspects of the cognitive cycle are unconscious, each cycle
also yields a momentary “ignition” of conscious broadcasting.
Neuroscientists have independently proposed ideas similar to the cognitive
cycle, the fundamental hypothesis of the LIDA model of cognition. High-level
cognition, such as deliberation, planning, etc., is typically enabled by
multiple cognitive cycles. In this paper we describe a timing model LIDA's
cognitive cycle. Based on empirical and simulation data we propose that an
initial phase of perception (stimulus recognition) occurs 80–100 ms from
stimulus onset under optimal conditions. It is followed by a conscious episode
(broadcast) 200–280 ms after stimulus onset, and an action selection phase
60–110 ms from the start of the conscious phase. One cognitive cycle would
therefore take 260–390 ms. The LIDA timing model is consistent with brain
evidence indicating a fundamental role for a theta-gamma wave, spreading forward
from sensory cortices to rostral corticothalamic regions. This posteriofrontal
theta-gamma wave may be experienced as a conscious perceptual event starting at
200–280 ms post stimulus. The action selection component of the cycle is
proposed to involve frontal, striatal and cerebellar regions. Thus the cycle is
inherently recurrent, as the anatomy of the thalamocortical system suggests. The
LIDA model fits a large body of cognitive and neuroscientific evidence. Finally,
we describe two LIDA-based software agents: the LIDA Reaction Time agent that
simulates human performance in a simple reaction time task, and the LIDA Allport
agent which models phenomenal simultaneity within timeframes comparable to human
subjects. While there are many models of reaction time performance, these
results fall naturally out of a biologically and computationally plausible
cognitive architecture
