Questions éthiques et cliniques posées par l'adoption de l'initiative sur l'internement à vie des délinquants particulièrement dangereux par le peuple suisse
Une initiative populaire se concluant par un vote national a conduit le peuple suisse à demander une modification de sa constitution pour permettre d'interner définitivement certains délinquants particulièrement dangereux. Une fois la décision prise, aucune véritable réévaluation de la situation ne pourra être effectuée. Cette décision se basera principalement sur l'expertise psychiatrique qui devra se prononcer sur le caractère inamendable ou inaccessible à tout traitement de tels sujets. Cet article présente l'historique de la démarche populaire qui a conduit à ce vote et la manière dont l'Etat suisse a rendu compte de cette votation en tentant de l'inscrire dans son code pénal. Pourtant le texte voté se présentait d'une manière telle qu'il rendait impossible son inscription autant dans un discours juridique que dans un discours psychiatrique. Il confronte maintenant les praticiens à des questions éthiques majeures. Doit-on accepter de s'inscrire dans un tel processus pénal? Peut-on valider par une expertise un pronostic qui engage la vie entière d'une personne lorsque l'on connaît le caractère incertain et donc discutable des approches prédictives? Comment tout un Etat de droit a-t-il pu laisser se développer un tel leurre quant à la place de la psychiatrie dans la cité? [Auteurs]]]>

<![CDATA[The European Conference on Infections in Leukemia (ECIL) provides recommendations for diagnostic strategies and prophylactic, pre-emptive or targeted therapy strategies for various types of infection in patients with hematologic malignancies or hematopoietic stem cell transplantation recipients. Meetings are held every two years since 2005 and evidence-based recommendations are elaborated after evaluation of the literature and discussion among specialists of nearly all European countries. In this manuscript, the ECIL group presents the 2015-update of the recommendations for the targeted treatment of invasive candidiasis, aspergillosis and mucormycosis. Current data now allow a very strong recommendation in favor of echinocandins for first-line therapy of candidemia irrespective of the underlying predisposing factors. Anidulafungin has been given the same grading as the other echinocandins for hemato-oncological patients. The beneficial role of catheter removal in candidemia is strengthened. Aspergillus guidelines now recommend the use of either voriconazole or isavuconazole for first-line treatment of invasive aspergillosis, while first-line combination antifungal therapy is not routinely recommended. As only few new data were published since the last ECIL guidelines, no major changes were made to mucormycosis recommendations
Interoperability between Heterogeneous Federation Architectures: Illustration with SAML and WS-Federation
International audienceDigital identity management intra and inter information systems, and, service oriented architectures, are the roots of identity federation. This kind of security architectures aims at enabling information system interoperability. Existing architectures, however, do not consider interoperability of heterogeneous federation architectures, which rely on different federation protocols.In this paper, we try to initiate an in-depth reflection on this issue, through the comparison of two main federation architecture specifications: SAML and WS-Federation. We firstly propose an overall outline of identity federation. We furthermore address the issue of interoperability for federation architectures using a different federation protocol. Afterwards, we compare SAML and WS-Federation. Eventually, we define the ways of convergence, and therefore, of interoperability
Minimum Degree up to Local Complementation: Bounds, Parameterized Complexity, and Exact Algorithms
The local minimum degree of a graph is the minimum degree that can be reached
by means of local complementation. For any n, there exist graphs of order n
which have a local minimum degree at least 0.189n, or at least 0.110n when
restricted to bipartite graphs. Regarding the upper bound, we show that for any
graph of order n, its local minimum degree is at most 3n/8+o(n) and n/4+o(n)
for bipartite graphs, improving the known n/2 upper bound. We also prove that
the local minimum degree is smaller than half of the vertex cover number (up to
a logarithmic term). The local minimum degree problem is NP-Complete and hard
to approximate. We show that this problem, even when restricted to bipartite
graphs, is in W[2] and FPT-equivalent to the EvenSet problem, which
W[1]-hardness is a long standing open question. Finally, we show that the local
minimum degree is computed by a O*(1.938^n)-algorithm, and a
O*(1.466^n)-algorithm for the bipartite graphs
The determinants of individual health care expenditures in prison: evidence from Switzerland.
Prison health systems are subject to increasing pressures given the specific health needs of a growing and aging prison population. Identifying the drivers of medical spending among incarcerated individuals is therefore key for health care governance in prisons. This study assesses the determinants of individual health care expenditures within the prisons of the canton of Vaud, a large region of Switzerland.
We use a unique dataset linking demographic and prison stay characteristics as well as objective measures of morbidity to detailed medical invoice data. We adopt a multivariate regression approach to model total, somatic and psychiatric outpatient health care expenditures.
We find that chronic infectious, musculoskeletal and skin diseases are strong predictors of total and somatic costs. Schizophrenia, neurotic and personality disorders as well as the abuse of illicit drugs and pharmaceuticals drive total and psychiatric costs. Furthermore, cumulating psychiatric and somatic comorbidities has an incremental effect on costs.
By identifying the characteristics associated with health care expenditures in prison, this study constitutes a key step towards a more efficient use of medical resources in prison
Characterizing extremal digraphs for identifying codes and extremal cases of Bondy's theorem on induced subsets
An identifying code of a (di)graph is a dominating subset of the
vertices of such that all distinct vertices of have distinct
(in)neighbourhoods within . In this paper, we classify all finite digraphs
which only admit their whole vertex set in any identifying code. We also
classify all such infinite oriented graphs. Furthermore, by relating this
concept to a well known theorem of A. Bondy on set systems we classify the
extremal cases for this theorem
Exploring differences in healthcare utilization of prisoners in the Canton of Vaud, Switzerland.
Prison healthcare is an important public health concern given the increasing healthcare needs of a growing and aging prison population, which accumulates vulnerability factors and suffers from higher disease prevalence than the general population. This study identifies the key factors associated with outpatient general practitioner (GP), nursing or psychiatric healthcare utilization (HCU) within prisons. Cross-sectional data systematically collected by the prison medical staff were obtained for a sample of 1664 adult prisoners of the Canton of Vaud, Switzerland, for the year 2011. They contain detailed information on demographics (predisposing factors), diagnosed chronic somatic and psychiatric disorders (needs factors), as well as prison stay characteristics (contextual factors). For GP, nurse and psychiatric care, two-part regressions are used to model separately the probability and the volume of HCU. Predisposing factors are generally not associated with the probability to use healthcare services after controlling for needs factors. However, female inmates use higher volumes of care, and the volume of GP consultations increases with age. Chronic somatic and psychiatric conditions are the most important predictors of the probability of HCU, but associations with volumes differ in their magnitude and significance across disease groups. Infectious, musculoskeletal, nervous and circulatory diseases actively mobilize GP and nursing staff. Schizophrenia, illicit drug and pharmaceuticals abuse are strongly positively associated with psychiatric and nurse HCU. The occupancy rate displays positive associations among contextual factors. Prison healthcare systems face increasingly complex organizational, budgetary and ethical challenges. This study provides relevant insights into the HCU patterns of a marginalized and understudied population
Spin relaxation in low-dimensional systems
We review some of the newest findings on the spin dynamics of carriers and
excitons in GaAs/GaAlAs quantum wells. In intrinsic wells, where the optical
properties are dominated by excitonic effects, we show that exciton-exciton
interaction produces a breaking of the spin degeneracy in two-dimensional
semiconductors. In doped wells, the two spin components of an optically created
two-dimensional electron gas are well described by Fermi-Dirac distributions
with a common temperature but different chemical potentials. The rate of the
spin depolarization of the electron gas is found to be independent of the mean

Thermally driven spin injection from a ferromagnet into a non-magnetic metal
Creating, manipulating and detecting spin polarized carriers are the key
elements of spin based electronics. Most practical devices use a perpendicular
geometry in which the spin currents, describing the transport of spin angular
momentum, are accompanied by charge currents. In recent years, new sources of
pure spin currents, i.e., without charge currents, have been demonstrated and
applied. In this paper, we demonstrate a conceptually new source of pure spin
current driven by the flow of heat across a ferromagnetic/non-magnetic metal
(FM/NM) interface. This spin current is generated because the Seebeck
coefficient, which describes the generation of a voltage as a result of a
temperature gradient, is spin dependent in a ferromagnet. For a detailed study
of this new source of spins, it is measured in a non-local lateral geometry. We
developed a 3D model that describes the heat, charge and spin transport in this
geometry which allows us to quantify this process. We obtain a spin Seebeck
coefficient for Permalloy of -3.8 microvolt/Kelvin demonstrating that thermally
driven spin injection is a feasible alternative for electrical spin injection
in, for example, spin transfer torque experiments
Energy spectra of fractional quantum Hall systems in the presence of a valence hole
The energy spectrum of a two-dimensional electron gas (2DEG) in the
fractional quantum Hall regime interacting with an optically injected valence
band hole is studied as a function of the filling factor and the
separation between the electron and hole layers. The response of the 2DEG
to the hole changes abruptly at of the order of the magnetic length
. At , the hole binds electrons to form neutral () or
charged () excitons, and the photoluminescence (PL) spectrum probes the
lifetimes and binding energies of these states rather than the original
correlations of the 2DEG. The ``dressed exciton'' picture (in which the
interaction between an exciton and the 2DEG was proposed to merely enhance the
exciton mass) is questioned. Instead, the low energy states are explained in
terms of Laughlin correlations between the constituent fermions (electrons and
's) and the formation of two-component incompressible fluid states in the
electron--hole plasma. At , the hole binds up to two Laughlin
quasielectrons (QE) of the 2DEG to form fractionally charged excitons
QE. The previously found ``anyon exciton'' QE is shown to be
unstable at any value of . The critical dependence of the stability of
different QE complexes on the presence of QE's in the 2DEG leads to the
