Valoración de la viabilidad miocárdica mediante SPET miocárdico de perfusión con 99mTecnecio-metoxi-isobutil-isonitrilo

Consultable des del TDXTítol obtingut de la portada digitalitzadaEl objetivo de este estudio ha sido valorar la eficacia del SPET miocárdico de perfusión con 99mTc-MIBI en la predicción de la mejoría de la contractilidad post-revascularización. Pacientes y metodología: Se estudiaron 82 pacientes (59+/-10 años, 12 mujeres) mediante SPET en reposo con 99mTc-MIBI y 38 pacientes mediante SPET en reposo/reinyección con 201-Talio (57+/-9 años, 2 mujeres), que presentaban al menos un segmento con severa alteración de la contractilidad y que fueron sometidos a revascularización coronaria mediante cirugía (64 del grupo estudiado con 99mTc-MIBI y 25 del grupo del 201-Talio) o angioplastia (18 y 13 respectivamente). Se practicó una ventriculografía isotópica en equilibrio en tres proyecciones antes y a los 3-6 meses de la revascularización . El ventrículo izquierdo se dividió en 9 segmentos en los que se cuantificó la captación de 99mTc-MIBI o 201-Talio y la contractilidad regional. A 40 de los 82 pacientes estudiados con 99mTc-MIBI se les practicó además un SPETde esfuerzo pre-revascularización. Resultados: El valor medio de fracción de eyección no se modificó tras la revascularización en ninguno de lo dos grupos de pacientes (41+/-14% vs 42+/-16% en el grupo del 99mTc-MIBI y 39+/-14% vs 40+/-15 % en el grupo del 201-Talio). No hubieron diferencias en el porcentaje de pacientes estudiados con 99mTc-MIBI y con 201-Talio que mejoraron (24% vs 26%), empeoraron (18% vs 19%) o no modificaron (58% vs 55%) la fracción de eyección tras la revascularización. El porcentaje de segmentos disfuncionantes que mejoró fue del 47% en el grupo del 99mTc-MIBI y del 54 % en el 201-Talio. Se evidenció un incremento significativo de la sensibilidad (52%, 72% y 91%, p 30% is the viability criteria for 99mTc-MIBI SPET with better results to predict myocardial viability. No differences were observed in the results of 99mTc-MIBI and 201-Thallium SPET in the prediction of functional recovery after revascularization. Imperfect factor mobility, unemployment, and the short-period incidence of a capital income tax

A crucial feature of the short-run perspective in many policy-relevant issues is the existence of unemployment due to wage rigidities. At the same time, imperfections in the degree of factor mobility between sectors or regions determine the nature and flexibility of the responses of the economy to exogenous shocks. It is no wonder then that the incidence of taxation upon employment and income distribution is a central preoccupation of the fiscal authorities. El modelo de acreditación del sistema sanitario público en Andalucía

The implementation of the quality model proposed by the Andalusian regional government to manage its health system encounters various resistances that hinder the adoption of the model as a management tool by the stakeholders in the organisations that constitute the system. In this paper, we address this problem based on an analysis of discourse whereby, relying on the concept of interpretative repertoire put forward by Potter and Wetherell (1987). Next, we present the results of this analysis in terms of the characteristics of the objects that make up the system. These organizational objects explain the complex reality of health system showing the existence of implicit partial consensus (public nature of the health system; economic incentives do not useful for increasing motivation or the negative effect of the traditional politic manners into the public management) that it could give the option for a dialogic management model putting together all the interests towards the same strategic way.La implantación del modelo de calidad propuesto por la Administración andaluza para gestionar su sistema sanitario se encuentra con múltiples resistencias que dificultan su asunción como herramienta de gestión. En este artículo, abordamos esta problemática partiendo de un análisis del discurso donde, apoyándonos en el concepto de repertorio interpretativo propuesto por Potter y Wetherell (1987). A continuación, presentamos los resultados de dicho análisis en cuanto a las características de los objetos más relevantes que hemos identificado. Estos objetos-organizacionales nos explican la naturaleza poliédrica de la realidad del sistema sanitario destacando, no obstante, la existencia de ciertos consensos parciales (aceptación generalizada del sistema sanitario como servicio público, ineficacia de los incentivos económicos como motivadores, o lo negativo de la intromisión de la política tradicional en la gestión pública) que podrían hacer posible la adopción de un modelo dialógico de gestión que incluya los distintos intereses presentes en el sistema hacia una misma dirección estratégica

El sustantivo como clase de palabra en español

El objetivo de este trabajo es caracterizar el sustantivo español, como clase independiente de palabra respecto de otras, especialmente del adjetivo. Los criterios adoptados son funcionales (el núcleo del sintagma nominal) y morfológicos (morfología “flexiva”). Se hace hincapié en el hecho de que el género se puede atribuir a la sustantiva sólo, si es, o no se muestra en sus “morfemas”. El artículo y los adjetivos no tienen género, sino que sólo presentan “morfemas flexivos” que concuerdan en número y género con el sustantivo. Los criterios semánticos son fundamentales en el establecimiento de las diversas clases de sustantivos (comunes y propios, “continuos” y “discontinuos”, etc.) Criterios de combinación sintagmática mostrarán si las clasificaciones semánticas son correctas o relevantes.The aim of this paper is to characterize the substantive in Spanish, as an independent class of word different from others, especially the adjective. The criteria adopted are functional (the nucleus of the noun phrase) and morphological (“flexive” morphology). Emphasis is placed on the fact that gender can be attributed to the substantive alone, whether or not this is shown in its “morphemes”. The article and adjectives have no gender at all; they only present “flexive morphemes” to agree in number and gender with the substantive. Semantic criteria are fundamental in establishing the diverse classes of substantives (common and proper, “continuous” and “discontinuous”, etc.). Sobre el modo verbal en español

En la primera sección de este trabajo se analizan las dificultades creadas por el estudio del modo verbal español. Más adelante, se realiza un breve estudio de las ideas lingüísticas que van alrededor del modo verbal desde los griegos hasta el momento presente. Se sintetizan diferentes actitudes y contra-ejemplos con las teorías mejor fundamentadas. También insistimos en la necesidad de utilizar criterios morfológico, sintáctico, semántico, textual y pragmático de una manera adecuada. Al final, y con carácter provisional, se propone una caracterización global del modo en los verbos en español, al pasar de la distinción entre la posibilidad de la alternancia modal y la falta de alternancia.In the first section of this paper, are analysed the difficulties created by the study of verb mood in Spanish. Later on a brief survey of the linguistic ideas going around the verb mood from the Greeks to the present moment is carried out. Different attitudes are synthesized and counter-examples are offered for the best founded theories. We also insist on the need of using the morphologic, syntactic, semantic, textual and pragmatic criteria in a suitable way. In the end, and provisionally, we propose a global characterization of the Spanish verb mood, going from the distinction between the possibility of modal alternation and the lack of alternation.notPeerReviewe

En torno al concepto de oración

En este artículo los criterios semánticos, morfológicos, sintácticos, funcionales y prodosémicos son considerados desde un nuevo punto de vista. Estos criterios han sido empleados para definir la oración como una unidad en el lenguaje. Llegamos a la conclusión que los criterios de la independencia sintáctica junto con la función predicativa básica son elementos que caracterizan la oración mejor que ningún otro.In this paper, the semantic, morphological, syntactic, functional and prosodemic criteria are considered from a new point of view. These criteria have been employed to define the sentence as a unit in the language. We come to the conclusion that the criterion of syntactic independence, together with the basic predicate function, are the elements that characterize the sentence better than any other.notPeerReviewe
