The stent graft barnacle - edge hyperplasia following femoral artery implant in a young female
A 37-year-old female underwent an emergent implant of a covered self-expandable stent graft (8mm x 10cm Viabahn, W. L. Gore) due to acute bleeding from a femoral access following closure of a patent foramen ovale. Over the following months, intermittent claudication developed, progressively more incapacitant. Work-up revealed near-occlusion of the stent graft due to edge stenosis. Elective explant of the stent graft was performed, revealing extensive proximal and distal edge miointimal hyperplasia (marked with *). Endarterectomy was performed and the arterial defect was corrected with a bovine pericardium patch
High coral bycatch in bottom-set Gillnet Coastal fisheries reveals rich coral habitats in Southern Portugal
Bottom-contact fisheries are unquestionably one of the main threats to the ecological integrity and functioning of deep-sea and circalittoral ecosystems, notably cold-water corals (CWC) and coral gardens. Lessons from the destructive impact of bottom trawling highlight the urgent need to understand how fisheries affect these vulnerable marine ecosystems. At the same time, the impact of other fishing gear and small-scale fisheries remains sparsely known despite anecdotal evidence suggesting their impact may be significant. This study aims to provide baseline information on coral bycatch by bottom-set gillnets used by artisanal fisheries in Sagres (Algarve, southwestern Portugal), thereby contributing to understand the impact of the activity but also the diversity and abundance of corals in this region. Coral bycatch frequency and species composition were quantified over two fishing seasons (summer-autumn and spring) for 42 days. The relationship with fishing effort was characterized according to metiers (n = 6). The results showed that 85% of the gillnet deployments caught corals. The maximum number of coral specimens per net was observed in a deployment targeting Lophius budegassa (n = 144). In total, 4,326 coral fragments and colonies of 22 different species were captured (fishing depth range of 57-510 m, mean 139 +/- 8 m). The most affected species were Eunicella verrucosa (32%), Paramuricea grayi (29%), Dendrophyllia cornigera (12%), and Dendrophyllia ramea (6%). The variables found to significantly influence the amount of corals caught were the target species, net length, depth, and mesh size. The 22 species of corals caught as bycatch belong to Orders Alcyonacea (80%), Scleractinia (18%), Zoantharia (1%), and Antipatharia (1%), corresponding to around 13% of the coral species known for the Portuguese mainland coast. These results show that the impact of artisanal fisheries on circalittoral coral gardens and CWC is potentially greater than previously appreciated, which underscores the need for new conservation measures and alternative fishing practices. Measures such as closure of fishing areas, frequent monitoring onboard of fishing vessels, or the development of encounter protocols in national waters are a good course of action. Hydrography-driven variability of optically active constituents of water in the South Brazilian Bight : Biogeochemical implications
The South Brazilian Bight (SBB) is a hydrographically dynamic environment with strong seasonality that sustains a diverse planktonic community involved in diverse biogeochemical processes. The inherent optical properties (IOPs; e.g., absorption and scattering coefficients) of optically actives constituents of water (OACs; phytoplankton, non-algal particlesâNAP, and colored dissolved organic matterâCDOM) have been widely employed to retrieve information on biogeochemical parameters in the water. In this study conducted in the SBB, a cross-shelf transect was performed for biogeochemistry and hydrographic sampling during a summer expedition. Our research aimed to determine the distribution and amount of the OACs based on their spectral signature, in relation to the distribution of water masses in the region. That allows us to get insights into the biogeochemical processes within each water mass and in the boundaries between them. We observed a strong intrusion of South Atlantic Central Water (SACW) over the shelf, mainly driven by the wind action. With that, phytoplankton development was fueled by the input of nutrients, and increased chlorophyll-a (Chl-a) concentrations were observed within the shallowest stations. Colored dissolved organic matter did not follow the distribution of dissolved organic carbon (DOC). Both CDOM and DOC presented high values at the low salinity Coastal Water (CW), as an indication of the continental influence over the shelf. However, CDOM was inversely correlated with salinity and lowest values were observed within Tropical Water (TW), whereas DOC values within TW were as high as within CW, indicating an autochthonous DOM source. Additionally, a deep Chl-a maximum (DCM) was noticed in the boundary between the TW and SACW. Along with the DCM, we observed the production of fresh, non-colored DOM attributed to the microbial community. Finally, our results suggest that CDOM is photodegraded at the surface of CW. This is mainly due to the Ekman transport effect over the region that traps CW at the surface, making it longer exposed to solar radiation
Aneurysm Sac Dynamics and its Prognostic Significance Following Fenestrated and Branched Endovascular Aortic Aneurysm Repair
Objective: This study aimed to assess aneurysm sac dynamics and its prognostic significance following fenestrated and branched endovascular aneurysm repair (F/BEVAR). Methods: Patients undergoing F/BEVAR for degenerative complex aortic aneurysm from 2008 to 2020 at two large vascular centres with two imaging examinations (30 day and one year) were included. Patients were categorised as regression and non-regression, determined by the proportional volume change (> 5%) at one year compared with 30 days. All cause mortality and freedom from graft related events were assessed using KaplanâMeier methods. Factors associated with non-regression at one year and aneurysm sac volume over time were examined for FEVAR and BEVAR independently using multivariable logistic regression and linear mixed effects modelling. Results: One hundred and sixty-five patients were included: 122 FEVAR, of whom 34% did not regress at one year imaging (20% stable, 14% expansion); and 43 BEVAR, of whom 53% failed to regress (26% stable, 28% expansion). Following F/BEVAR, after risk adjusted analysis, non-regression was associated with higher risk of all cause mortality within five years (hazard ratio [HR] 2.56, 95% confidence interval [CI] 1.09 â 5.37; p = .032) and higher risk of graft related events within five years (HR 2.44, 95% CI 1.10 â 5.26; p = .029). Following multivariable logistic regression, previous aortic repair (odds ratio [OR] 2.56, 95% CI 1.11 â 5.96; p = .029) and larger baseline aneurysm diameter (OR/mm 1.04, 95% CI 1.00 â 1.09; p = .037) were associated with non-regression at one year, whereas smoking history was inversely associated with non-regression (OR 0.21, 95% CI 0.04 â 0.96; p = .045). Overall following FEVAR, aneurysm sac volume decreased significantly up to two years (baseline vs. two year, 267 [95% CI 250 â 285] cm 3 vs. 223 [95% CI 197 â 248] cm 3), remaining unchanged thereafter. Overall following BEVAR, aneurysm sac volume remained stable over time. Conclusion: Like infrarenal EVAR, non-regression at one year imaging is associated with higher five year all cause mortality and graft related events risks after F/BEVAR. Following FEVAR for juxtarenal aortic aneurysm, aneurysm sacs generally displayed regression (66% at one year), whereas after BEVAR for thoraco-abdominal aortic aneurysm, aneurysm sacs displayed a concerning proportion of growth at one year (28%), potentially suggesting a persistent risk of rupture and consequently requiring intensified surveillance following BEVAR. Future studies will have to elucidate how to improve sac regression following complex EVAR, and whether the high expansion risk after BEVAR is due to advanced disease extent.</p
