Probabilistic Analysis of Cost-Effectiveness Models: Choosing between Treatment Strategies for Gastroesophageal Reflux Disease
A review of health utilities across conditions common in paediatric and adult populations
<p>Abstract</p> <p>Background</p> <p>Cost-utility analyses are commonly used in economic evaluations of interventions or conditions that have an impact on health-related quality of life. However, evaluating utilities in children presents several challenges since young children may not have the cognitive ability to complete measurement tasks and thus utility values must be estimated by proxy assessors. Another solution is to use utilities derived from an adult population. To better inform the future conduct of cost-utility analyses in paediatric populations, we reviewed the published literature reporting utilities among children and adults across selected conditions common to paediatric and adult populations.</p> <p>Methods</p> <p>An electronic search of Ovid MEDLINE, EMBASE, and the Cochrane Library up to November 2008 was conducted to identify studies presenting utility values derived from the Health Utilities Index (HUI) or EuroQoL-5Dimensions (EQ-5D) questionnaires or using time trade off (TTO) or standard gamble (SG) techniques in children and/or adult populations from randomized controlled trials, comparative or non-comparative observational studies, or cross-sectional studies. The search was targeted to four chronic diseases/conditions common to both children and adults and known to have a negative impact on health-related quality of life (HRQoL).</p> <p>Results</p> <p>After screening 951 citations identified from the literature search, 77 unique studies included in our review evaluated utilities in patients with asthma (n = 25), cancer (n = 23), diabetes mellitus (n = 11), skin diseases (n = 19) or chronic diseases (n = 2), with some studies evaluating multiple conditions. Utility values were estimated using HUI (n = 33), EQ-5D (n = 26), TTO (n = 12), and SG (n = 14), with some studies applying more than one technique to estimate utility values. 21% of studies evaluated utilities in children, of those the majority being in the area of oncology. No utility values for children were reported in skin diseases. Although few studies provided comparative information on utility values between children and adults, results seem to indicate that utilities may be similar in adolescents and young adults with asthma and acne. Differences in results were observed depending on methods and proxies.</p> <p>Conclusions</p> <p>This review highlights the need to conduct future research regarding measurement of utilities in children.</p

Sliding mode control of spatial mechanical systems decoupling translation and rotation
This paper looks at the robust trajectory control of spatial mechanical systems using sliding mode techniques. Two distinctions of the proposed method from reported methods are: (1) The measure of attitudinal error used is intrinsically defined, Euclidean-geometric, and intuitive. From Euler's theorem it follows that given a desired and actual attitude of a rigid body there exists an axis and angle of rotation relating the two attitudes. This defines a relative rotation vector, which is used as an intrinsically defined, intuitive measure of error. Reported methods use algebraic differences of entities such as generalized coordinates representing attitude. While functionally correlated to attitudinal error, these measures are not intrinsically defined. (2) A novel, dynamically nonlinear sliding function is used that results in a simple control law. The parameters of this function are dynamically and geometrically intuitive. Simulation results are given for a spacecraft tracking a complex desired trajectory
W-Algebra Symmetries of Generalised Drinfel'd-Sokolov Hierarchies
Using the zero-curvature formulation, it is shown that W-algebra
transformations are symmetries of corresponding generalised Drinfel'd-Sokolov
hierarchies. This result is illustrated with the examples of the KdV and
Boussinesque hierarchies, and the hierarchy associated to the

Efficiency in a forced contribution threshold public good game
We contrast and compare three ways of predicting efficiency in a forced contribution threshold public good game. The three alternatives are based on ordinal potential, quantal response and impulse balance theory. We report an experiment designed to test the respective predictions and find that impulse balance gives the best predictions. A simple expression detailing when enforced contributions result in high or low efficiency is provided
Pair Interaction Potentials of Colloids by Extrapolation of Confocal Microscopy Measurements of Collective Structure
A method for measuring the pair interaction potential between colloidal
particles by extrapolation measurement of collective structure to infinite
dilution is presented and explored using simulation and experiment. The method
is particularly well suited to systems in which the colloid is fluorescent and
refractive index matched with the solvent. The method involves characterizing
the potential of mean force between colloidal particles in suspension by
measurement of the radial distribution function using 3D direct visualization.
The potentials of mean force are extrapolated to infinite dilution to yield an
estimate of the pair interaction potential, . We use Monte Carlo (MC)
simulation to test and establish our methodology as well as to explore the
effects of polydispersity on the accuracy. We use poly-12-hydroxystearic
acid-stabilized poly(methyl methacrylate) (PHSA-PMMA) particles dispersed in
the solvent dioctyl phthalate (DOP) to test the method and assess its accuracy
for three different repulsive systems for which the range has been manipulated

Benevolent characteristics promote cooperative behaviour among humans
Cooperation is fundamental to the evolution of human society. We regularly
observe cooperative behaviour in everyday life and in controlled experiments
with anonymous people, even though standard economic models predict that they
should deviate from the collective interest and act so as to maximise their own
individual payoff. However, there is typically heterogeneity across subjects:
some may cooperate, while others may not. Since individual factors promoting
cooperation could be used by institutions to indirectly prime cooperation, this
heterogeneity raises the important question of who these cooperators are. We
have conducted a series of experiments to study whether benevolence, defined as
a unilateral act of paying a cost to increase the welfare of someone else
beyond one's own, is related to cooperation in a subsequent one-shot anonymous
Prisoner's dilemma. Contrary to the predictions of the widely used inequity
aversion models, we find that benevolence does exist and a large majority of
people behave this way. We also find benevolence to be correlated with
cooperative behaviour. Finally, we show a causal link between benevolence and
cooperation: priming people to think positively about benevolent behaviour
makes them significantly more cooperative than priming them to think
malevolently. Thus benevolent people exist and cooperate more

Optimistic irrationality and overbidding in private value auctions
Bidding one’s value in a second-price, private-value auction is a weakly dominant solution (Vickrey in J Finance 16(1):8–37, 1961), but repeated experimental studies find more overbidding than underbidding. We propose a model of optimistically irrational bidders who understand that there are possible gains and losses associated with higher bids but who may overestimate the additional probability of winning and/or underestimate the potential losses when bidding above value. These bidders may fail to discover the dominant strategy—despite the fact that the dominant strategy only requires rationality from bidders—but respond in a common sense way to out-of-equilibrium outcomes. By varying the monetary consequences of losing money in experimental auctions we observe more overbidding when the cost to losing money is low, and less overbidding when the cost is high. Our findings lend themselves to models in which less than fully rational bidders respond systematically to out-of-equilibrium incentives, and we find that our model better fits the effects of our manipulations than most of the existing models we consider
