The MAORY ICS software architecture
The Multi Conjugate Adaptive Optics RelaY (MAORY) for ESO's Extremely Large Telescope (ELT) is an adaptive optics module offering multi-conjugate (MCAO) and single-conjugate (SCAO) compensation modes. In MCAO, it relies on the use of up to six Laser Guide Stars (LGS) and three Natural Guide Stars (NGS) for atmospheric turbulence sensing and multiple mirrors for correction, providing high Strehl and high sky coverage. In SCAO mode, a single natural source is used as reference, providing better correction but in a smaller field. MAORY will be installed at the Nasmyth focus of the ELT. It will feed the MICADO first-light diffraction limited imager and a future second instrument. MAORY is being built by a Consortium composed by INAF in Italy and IPAG in France and is currently approaching end of phase B. In this paper we describe the preliminary design of the MAORY Instrument Control System Software (ICS SW). We start with an overview of the MAORY module and then describe the general architecture of the MAORY control network and software. We then describe the main software components, with particular emphasis to those managing the NGS and LGS wavefront sensors functions and the AO off-load and secondary loops, and the main interfaces to subsystems and external systems. We then conclude with a description of the software engineering practices adopted for the development of MAORY ICS SW
Effects of Anacetrapib in Patients with Atherosclerotic Vascular Disease
BACKGROUND:
Patients with atherosclerotic vascular disease remain at high risk for cardiovascular events despite effective statin-based treatment of low-density lipoprotein (LDL) cholesterol levels. The inhibition of cholesteryl ester transfer protein (CETP) by anacetrapib reduces LDL cholesterol levels and increases high-density lipoprotein (HDL) cholesterol levels. However, trials of other CETP inhibitors have shown neutral or adverse effects on cardiovascular outcomes.
METHODS:
We conducted a randomized, double-blind, placebo-controlled trial involving 30,449 adults with atherosclerotic vascular disease who were receiving intensive atorvastatin therapy and who had a mean LDL cholesterol level of 61 mg per deciliter (1.58 mmol per liter), a mean non-HDL cholesterol level of 92 mg per deciliter (2.38 mmol per liter), and a mean HDL cholesterol level of 40 mg per deciliter (1.03 mmol per liter). The patients were assigned to receive either 100 mg of anacetrapib once daily (15,225 patients) or matching placebo (15,224 patients). The primary outcome was the first major coronary event, a composite of coronary death, myocardial infarction, or coronary revascularization.
RESULTS:
During the median follow-up period of 4.1 years, the primary outcome occurred in significantly fewer patients in the anacetrapib group than in the placebo group (1640 of 15,225 patients [10.8%] vs. 1803 of 15,224 patients [11.8%]; rate ratio, 0.91; 95% confidence interval, 0.85 to 0.97; P=0.004). The relative difference in risk was similar across multiple prespecified subgroups. At the trial midpoint, the mean level of HDL cholesterol was higher by 43 mg per deciliter (1.12 mmol per liter) in the anacetrapib group than in the placebo group (a relative difference of 104%), and the mean level of non-HDL cholesterol was lower by 17 mg per deciliter (0.44 mmol per liter), a relative difference of -18%. There were no significant between-group differences in the risk of death, cancer, or other serious adverse events.
CONCLUSIONS:
Among patients with atherosclerotic vascular disease who were receiving intensive statin therapy, the use of anacetrapib resulted in a lower incidence of major coronary events than the use of placebo.
We intend to do a comparative study of the invention of flag day in Mexico and Argentina. In both cases, there were processes that occurred between 1934 and 1940 or so. The demand that there be a day dedicated to the worship of national flags arose from the fear of some groups that "communism" would replace with their red flags those of the homelands. Behind this apparent similarity are notorious differences: these are different political contexts, in the Argentine case it is a government with ideological traits related to anti-communism and the initiative arises from sectors of conservative elites. In Mexico the government of Lázaro Cardenas was a radical democracy in the social and ideological and the initiative arises from conservative groups of Catholic popular sectorsNos proponemos hacer un estudio comparativo de la invención del día de la bandera en México y Argentina.
construcciones deliberadas, arbitrarias y necesariamente contradictorias y conflictivas.
Virosome-mediated delivery of tumor antigen to plasmacytoid dendritic cells.
International audienceCytotoxic T lymphocytes (CTL) are crucial in viral clearance and tumor growth control. Thus the induction of CTL activity is an important aim in vaccine development. We investigate an innovative delivery system for peptide transfer to the MHC class I processing pathway of APC with the aim to trigger CTL in the context of an antitumoral response. The strategy relies on a novel antigen delivery system termed "chimeric immunopotentiating reconstituted influenza virosomes" (CIRIV) targeting plasmacytoid dendritic cells (PDC). By using virosomes containing encapsulated Melan-A peptide and a PDC line developed in our laboratory, we evaluated the response of Melan-A specific T cells. Virosomes have the capacity to bind PDC and are endocyted within vesicles in the cytosol. This endocytosis is inhibited by neuraminidase, suggesting that it is mediated by sialic acid present on cell surface. Furthermore, PDC loaded with Melan-A virosomes can induce a Melan-A specific T cell activation. Interestingly, they activate T cells with a better efficiency than PDC loaded with a free peptide and when PDC where previously activated by a TLR ligand. These results indicate that virosomes could be a suitable delivery system for tumor peptide in immunotherapy of cancer
Procalcitonin (PCT)-guided antibiotic stewardship: an international experts consensus on optimized clinical use
Procalcitonin (PCT)-guided antibiotic stewardship (ABS) has been shown to reduce antibiotics (ABxs), with lower side-effects and an improvement in clinical outcomes. The aim of this experts workshop was to derive a PCT algorithm ABS for easier implementation into clinical routine across different clinical settings. Clinical evidence and practical experience with PCT-guided ABS was analyzed and discussed, with a focus on optimal PCT use in the clinical context and increased adherence to PCT protocols. Using a Delphi process, the experts group reached consensus on different PCT algorithms based on clinical severity of the patient and probability of bacterial infection. The group agreed that there is strong evidence that PCT-guided ABS supports individual decisions on initiation and duration of ABx treatment in patients with acute respiratory infections and sepsis from any source, thereby reducing overall ABx exposure and associated side effects, and improving clinical outcomes. To simplify practical application, the expert group refined the established PCT algorithms by incorporating severity of illness and probability of bacterial infection and reducing the fixed cut-offs to only one for mild to moderate and one for severe disease (0.25 μg/L and 0.5 μg/L, respectively). Further, guidance on interpretation of PCT results to initiate, withhold or discontinue ABx treatment was included. A combination of clinical patient assessment with PCT levels in well-defined ABS algorithms, in context with continuous education and regular feedback to all ABS stakeholders, has the potential to improve the diagnostic and therapeutic management of patients suspected of bacterial infection, thereby improving ABS effectiveness.</p
