    Malignant fibrous histiocytoma (MFH) usually presents in the extremities or retroperitoneum. Cases involving the kidney are rare and portend a poor prognosis. Although radical nephrectomy is the most beneficial curative choice for this neoplasm, patients are often treated with adjuvant chemotherapy due to high risk of local recurrence and distant metastases. We describe a case of a 68-year-old woman affected by MFH, treated with both nephrectomy and radiotherapy without systemic therapy showing an unexpected twenty-four-month postsurgery survival outcome

    A measure of interrater absolute agreement for ordinal categorical data

    A measure of interrater absolute agreement for ordinal scales is proposed capitalizing on the dispersion index for ordinal variables proposed by Giuseppe Leti. The procedure allows to overcome the limits affecting traditional measures of interrater agreement in different fields of application. An unbiased estimator of the proposed measure is introduced and its sampling properties are investigated. In order to construct confidence intervals for interrater absolute agreement both asymptotic results and bootstrapping methods are used and their performance is evaluated. Simulated data are employed to demonstrate the accuracy and practical utility of the new procedure for assessing agreement. Finally, an application to a real case is provided

    Involvement of the sigma factor sigma H in the regulation of a small heat shock protein gene in Lactobacillus plantarum WCFS1

    The relative expression of the heat shock protein hsp18.55 gene was monitored in Lactobacillus plantarum ΔctsR and ΔftsH mutant strains. Transcription of the hsp18.55 gene was drastically repressed in the ΔctsR mutant of L. plantarum; conversely, significant transcriptional induction of the hsp18.55 gene was noted in the L. plantarum ΔftsH strain. Overall, these results suggest a possible regulation of the hsp18.55 gene by the alternative sigma H factor in L. plantarum. We also noted a similarity with the small heat shock gene (shs) locus of Lactobacillus brevis, which might indicate a possible evolutionary relationship

    A comparison between preference judgments of curvature and sharpness in architectural façades

    Can curvature drive preference, perceived familiarity, complexity, stability and approachability for architectural façades? In this study, we generated four versions of the same reference building, varying only the amount of curvature introduced in the façade. Participants’ judgments were measured using three experimental methodologies. Multidimensional scaling on forced choices showed that the curved façade was the most preferred. Multidimensional unfolding on ranking task showed that the majority of participants expressed higher preferences for the curved façade compared to the sharp-angled and rectilinear ones. Ratings on different psychological variables provided supporting evidence for curvature significantly influencing liking and approachability judgments. Results from image analyses –using a dynamical model of the visual cortex and a model that characterizes discomfort in terms of adherence to the statistics of natural images – matched behavioural data. We discuss the implications of the findings on our understanding of human preferences, which are intrinsically dynamic and influenced by context and experience.PostprintPeer reviewe

    Filling Gaps in Trawl Surveys at Sea through Spatiotemporal and Environmental Modelling

    International scientific fishery survey programmes systematically collect samples of target stocks’ biomass and abundance and use them as the basis to estimate stock status in the framework of stock assessment models. The research surveys can also inform decision makers about Essential Fish Habitat conservation and help define harvest control rules based on direct observation of biomass at the sea. However, missed survey locations over the survey years are common in long-term programme data. Currently, modelling approaches to filling gaps in spatiotemporal survey data range from quickly applicable solutions to complex modelling. Most models require setting prior statistical assumptions on spatial distributions, assuming short-term temporal dependency between the data, and scarcely considering the environmental aspects that might have influenced stock presence in the missed locations. This paper proposes a statistical and machine learning based model to fill spatiotemporal gaps in survey data and produce robust estimates for stock assessment experts, decision makers, and regional fisheries management organizations. We apply our model to the SoleMon survey data in North-Central Adriatic Sea (Mediterranean Sea) for 4 stocks: Sepia officinalis, Solea solea, Squilla mantis, and Pecten jacobaeus. We reconstruct the biomass-index (i.e., biomass over the swept area) of 10 locations missed in 2020 (out of the 67 planned) because of several factors, including COVID-19 pandemic related restrictions. We evaluate model performance on 2019 data with respect to an alternative index that assumes biomass proportion consistency over time. Our model’s novelty is that it combines three complementary components. A spatial component estimates stock biomass-index in the missed locations in one year, given the surveyed location’s biomass-index distribution in the same year. A temporal component forecasts, for each missed survey location, biomass-index given the data history of that haul. An environmental component estimates a biomass-index weighting factor based on the environmental suitability of the haul area to species presence. Combining these components allows understanding the interplay between environmental-change drivers, stock presence, and fisheries. Our model formulation is general enough to be applied to other survey data with lower spatial homogeneity and more temporal gaps than the SoleMon dataset

    Early loss of blood-brain barrier integrity precedes NOX2 elevation in the prefrontal cortex of an animal model of psychosis

    The social isolation rearing of young adult rats is a model of psychosocial stress and provides a nonpharmacological tool to study alterations reminiscent of symptoms seen in psychosis. We have previously demonstrated that social isolation in rats leads to increased oxidative stress and to cerebral NOX2 elevations. Here, we investigated early alterations in mRNA expression leading to increased NOX2 in the brain. Rats were exposed to a short period of social isolation (1 week) and real-time polymerase chain reaction (PCR) for mRNA expression of genes involved in blood-brain barrier (BBB) formation and integrity (ORLs, Vof 21 and Vof 16, Leng8, Vnr1, and Trank 1 genes) was performed. Real-time PCR experiments, immunohistochemistry, and Western blotting analysis showed an increased expression of these genes and related proteins in isolated rats with respect to control animals. The expression of specific markers of BBB integrity, such as matrix metalloproteinase 2 (MMP2), matrix metalloproteinase 9 (MMP9), occludin 1, and plasmalemmal vesicle associated protein-1 (PV-1), was also significantly altered after 1 week of social isolation. BBB permeability, evaluated by quantification of Evans blue dye extravasation, as well as interstitial fluid, was significantly increased in rats isolated for 1 week with respect to controls. Isolation-induced BBB disruption was also accompanied by a significant increase of Interleukin 6 (IL-6) expression. Conversely, no differences in NOX2 levels were detected at this time point. Our study demonstrates that BBB disruption precedes NOX2 elevations in the brain. These results provide new insights in the interplay of mechanisms linking psychosocial stress to early oxidative stress in the brain, disruption of the BBB, and the development of mental disorders

    Perfusion-CT monitoring of cryo-ablated renal cells tumors

    <p>Abstract</p> <p>Background</p> <p>No single and thoroughly validated imaging method in monitoring of cryoablated renal cell carcinoma (RCC) is available. The purpose of our study was to determine the feasibility of dynamic contrast-enhanced perfusion CT (pCT) in evaluating the hemodynamic response of RCC.</p> <p>Methods</p> <p>15 patients (14 male, 1 female; age range, 43-81 years; mean age, 62 years) with cryoablated RCC via a transperitoneal approach, underwent to pCT 6-8 months after cryo-therapy. pCT was performed for 65 seconds after intravenous injection of contrast medium (80 mL, 370 mg iodine per millilitre, 4 mL/sec). Perfusion parameters (Time/Density curve; Blood flow, BF; Blood Volume, BV; Mean Transit Time, MTT; Permeability-Surface Area Product, PS) were sampled in the cryoablated tumor area and in ipsilateral renal cortex using deconvolution-based method. A tumor was considered to be not responsive to treatment by CT evidence of pathological contrast enhancement in the cryoablated area or renal mass persistence compared with the preoperative CT control. Written informed consent was obtained from all participants before the study.</p> <p>Results</p> <p>After cryotherapy, successfully ablated tumor (n = 13) showed decrease in BV (5,39 +/- 1,28 mL/100 g), BF (69,92 +/- 20,12 mL/100 g/min) and PS (16,66 +/- 5,67 mL/100 g/min) value and increased value of MTT (25,35 +/- 4,3 sec) compared with those of normal renal cortex (BV: 117,86 +/- 31,87 mL/100 g/min; BF: 392,39 +/- 117,32 mL/100 g/min; MTT: 18,02 +/- 3,6 sec; PS: 81,68 +/- 22,75 mL/100 g/min). In one patient, assessment of perfusion parameters was not feasible for breathing artifacts. One tumor showed poor response to treatment by the evidence of nodular contrast enhancement in the region encompassing the original lesion. Two typical enhancement patterns were obtained comparing the Time-Density curves of responsive and not responsive ablated tumors.</p> <p>Conclusion</p> <p>Perfusion CT seems to be a feasible and promising technique in monitoring the effects of cryoablation therapy.</p

    Surgical and Bioengineering Integration in the Anatomy Course of Medicine and Surgery High Technology: Knowledge and Perception of Anatomy

    The Locomotor System Anatomy (LSA) course, placed in the first semester of the first year of the new Master’s degree in Medicine and Surgery High Technology (MSHT) at the Sapienza University of Rome, was integrated with surgical and bioengineering content. This study investigated the educational value and the students’ perceptions of the effectiveness of these two types of integration, comparing surgical integration (SI) with engineering integration (EI). Anatomy knowledge and students’ opinions attending the LSA course in MSHT degree (n = 30) were compared with those of students (n = 32) attending another medical and surgery course not comprising EI. Data show that students in the MSHT course like in-depth SI much more than in-depth EI. However, those who like in-depth SI also like in-depth EI. Significant differences were in anatomy knowledge between the two groups in the three sections of the test. There was no significant correlation between the three test scores and the levels of liking, while there was a significant correlation between students liking SI and those liking EI. A statistically significant correlation was also found in students who correctly responded to questions on the head and trunk, with students responding correctly to questions on the upper limbs. This study will be important in optimizing the deepening of SI and EI in the LSA course.     Genetically driven predisposition leads to an unusually genomic unstable renal cell carcinoma

    renal cell carcinoma originates from the lining of the proximal convoluted renal tubule and represents the most common type of kidney cancer. risk factors and comorbidities might be associated to renal cell carcinoma, while a small fraction of 2-3% emerges from patients with predisposing cancer syndromes, typically associated to hereditary mutations in VHL, folliculin, fumarate hydratase or MET genes. here, we report a case of renal cell carcinoma in patient with concurrent germline mutations in BRCA1 and RAD51 genes. this case displays an unusual high mutational burden and chromosomal aberrations compared to the typical profile of renal cell carcinoma. mutational analysis on whole genome sequencing revealed an enrichment of the MMR2 mutational signature, which is indicative of impaired DNA repair capacity. overall, the tumor displayed a profile of unusual high genomic instability which suggests a possible origin from germline predisposing mutations in the DNA repair genes BRCA1 and RAD51. while BRCA1 and RAD51 germline mutations are well-characterised in breast and ovarian cancer, their role in renal cell carcinoma is still largely unexplored. the genomic instability detected in this case of renal cell carcinoma, along with the presence of unusual mutations, might offer support to clinicians for the development of patient-tailored therapies
