    O ATUAL CENÁRIO PENITENCIÁRIO BRASILEIRO: DESCASO E ABANDONO DA FUNÇÃO REABILITADORA

    A política criminal brasileira apresenta à população a imagem de que o sistema penitenciário está de acordo com as diretrizes humanitárias e respeita princípios como o da dignidade da pessoa humana. A ideia de recuperação foi, assim, inserida ao ideal de punição e fez com que a finalidade da pena privativa de liberdade possuísse um duplo caráter (retributivo e reformador). Esse conceito criou raízes no imaginário da sociedade contemporânea, entretanto o aumento constante e significativo da população carcerária e a realidade dos estabelecimentos demonstra o oposto. Desse modo, por meio de uma pesquisa qualitativa bibliográfica e documental, usando-se de literatura e de dados estatísticos, pretende-se demonstrar algumas questões da realidade carcerária brasileira e como a reabilitação não ocorre no sistema penitenciário brasileiro, uma vez que o seu conceito não mais se aplica na formação social atual

    The relation of HPV infection and expression of p53 and p16 proteins in esophageal squamous cells carcinoma

    GOAL: To investigate the HPV prevalence and characterize the expression of potential molecular surrogate markers of HPV infection in esophageal squamous cell carcinoma. MATERIALS AND METHODS: The prevalence of HPV in individuals with and without esophageal cancer (EC) was determined by using multiplex PCR; p16 and p53 protein levels were assessed by immunohistochemistry (IHC). RESULTS: High-risk HPV (hr-HPV) was found in the same frequency (13.8%) in esophageal squamous cell carcinoma (ESCC) and in healthy individuals. The p53 expression was positive in 67.5% of tumor tissue, 20.0% of adjacent non-tumoral tissue and 1.8% of normal esophageal tissue. p16 was positive in 11.6% of esophageal cancer cases and 4.7% of adjacent non-tumoral tissue. p16 was undetectable among control group samples. p53 and p16 levels were not significantly associated with the HPV status.     Secagem de toras de clones de Eucalyptus urograndis em estufa solar

    Drying is an important stage in the wood industrialization process, with the purpose of improving the quality of the material. However, it is an expensive process, and there is a need to reduce its costs by using methods that require less investment. Thus, the objectives of this study were to evaluate the efficiency of a solar drying oven for drying Eucalyptus urograndis clone logs installed in the municipality of Macaíba, state of Rio Grande do Norte, in order to check whether the region is suitable for this practice; and ii) to evaluate the influence of different log diameter classes in the drying process. 1.50m logs were sectioned and separated according to four diametric classes. They were then weighed and placed in a solar oven, with daily measurements of air temperature and relative humidity and biweekly weighings during the observation period until they reached the fiber saturation point. The data were submitted to regression analysis, applying the variable selection procedure. At 35 days, all logs had already reached the PSF. However, the diameter measurements did not significantly affect the drying process, which proved to be efficient when compared to conventional and natural methods. Therefore, the solar kiln drying of logs becomes a viable method for the region.A secagem é uma etapa importante no processo da industrialização da madeira, tendo como finalidade melhorar a qualidade do material. Porém, é um processo oneroso havendo a necessidade da redução dos seus custos, por meio de métodos que demandem menos investimento. Desse modo, os objetivos deste estudo foram avaliar a eficiência de estufa solar na secagem de toras de clones de Eucalyptus urograndis instalada no município de Macaíba, estado do Rio Grande do Norte, com o intuito de verificar se a região é adequada para esta prática; e ii) avaliar a influência de diferentes classes de diâmetro das toras no processo de secagem. Foram seccionadas toras de 1,50m e separadas em função de quatro classes de diamétricas. Em seguida foram pesadas e acondicionadas em estufa solar, sendo realizado medições diárias da temperatura e umidade relativa do ar e pesagens quinzenais durante o período de observação até que atingissem o ponto de saturação das fibras. Os dados foram submetidos a análise de regressão, aplicando-se o procedimento de seleção de variáveis. Aos 35 dias, todas as toras já haviam atingido o PSF. Entretanto, as medidas do diâmetro não afetaram significativamente na secagem, que se demonstrou eficiente quando comparada aos métodos convencionais e naturais.     The control and prevention of COVID-19 transmission in children : a protocol for systematic review and meta-analysis

    Background: The pandemic following the rapid spread of the new SARS-CoV-2 virus has hit all continents and caused thousands of deaths worldwide. Evidence has been published on epidemiological and clinical characteristics of population groups considered at risk; however, information for the other population groups, especially for the child population, is needed. In this context, this protocol describes a systematic review that will aim to identify the evidence on control and prevention of COVID-19 transmission among children and adolescents, as well as to describe the epidemiological profile and clinical and immunological characteristics of COVID-19 in this population. Methods: This protocol will be developed in accordance with PRISMA-P. The searches will be conducted in PubMed, Web of Science, ScienceDirect, EMBASE, and Scopus, seeking clinical trials. Observational studies and case reports with Children and adolescents (≤19 years) infected with SARS-CoV-2 will be included whether they report information on the control of prevention and COVID-19 transmission. Two independent researchers will perform the selection of articles, removal of duplication, and screening by Rayyan QCRI application. Cochrane's RoB 2.0, ROBINS-I, and CASP tools will be used to assess the risk of bias. Meta-analysis, subgroup analyses, and/or descriptive analyses will be carried out based on the data conditions included. Results: A high-quality synthesis of the available evidences on the epidemiological profile, the clinical and immunological characteristics involved in children, and adolescents diagnosed with COVID-19, as well as the participation of this population in the transmission dynamics of SARS-CoV-2 will be provided. Conclusion: This systematic review has an important relevance in the current context because it has a great potential to help the development of new control and prevention strategies in the pediatric population

    Biodiversity loss is one of the main challenges of our time,1,2 and attempts to address it require a clear un derstanding of how ecological communities respond to environmental change across time and space.3,4 While the increasing availability of global databases on ecological communities has advanced our knowledge of biodiversity sensitivity to environmental changes,5–7 vast areas of the tropics remain understudied.8–11 In the American tropics, Amazonia stands out as the world’s most diverse rainforest and the primary source of Neotropical biodiversity,12 but it remains among the least known forests in America and is often underrepre sented in biodiversity databases.13–15 To worsen this situation, human-induced modifications16,17 may elim inate pieces of the Amazon’s biodiversity puzzle before we can use them to understand how ecological com munities are responding. To increase generalization and applicability of biodiversity knowledge,18,19 it is thus crucial to reduce biases in ecological research, particularly in regions projected to face the most pronounced environmental changes. We integrate ecological community metadata of 7,694 sampling sites for multiple or ganism groups in a machine learning model framework to map the research probability across the Brazilian Amazonia, while identifying the region’s vulnerability to environmental change. 15%–18% of the most ne glected areas in ecological research are expected to experience severe climate or land use changes by 2050.     Diretriz da Sociedade Brasileira de Cardiologia sobre Diagnóstico e Tratamento de Pacientes com Cardiomiopatia da Doença de Chagas

    This guideline aimed to update the concepts and formulate the standards of conduct and scientific evidence that support them, regarding the diagnosis and treatment of the Cardiomyopathy of Chagas disease, with special emphasis on the rationality base that supported it.  Chagas disease in the 21st century maintains an epidemiological pattern of endemicity in 21 Latin American countries. Researchers and managers from endemic and non-endemic countries point to the need to adopt comprehensive public health policies to effectively control the interhuman transmission of T. cruzi infection, and to obtain an optimized level of care for already infected individuals, focusing on diagnostic and therapeutic opportunistic opportunities.   Pathogenic and pathophysiological mechanisms of the Cardiomyopathy of Chagas disease were revisited after in-depth updating and the notion that necrosis and fibrosis are stimulated by tissue parasitic persistence and adverse immune reaction, as fundamental mechanisms, assisted by autonomic and microvascular disorders, was well established. Some of them have recently formed potential targets of therapies.  The natural history of the acute and chronic phases was reviewed, with enhancement for oral transmission, indeterminate form and chronic syndromes. Recent meta-analyses of observational studies have estimated the risk of evolution from acute and indeterminate forms and mortality after chronic cardiomyopathy. Therapeutic approaches applicable to individuals with Indeterminate form of Chagas disease were specifically addressed. All methods to detect structural and/or functional alterations with various cardiac imaging techniques were also reviewed, with recommendations for use in various clinical scenarios. Mortality risk stratification based on the Rassi score, with recent studies of its application, was complemented by methods that detect myocardial fibrosis.  The current methodology for etiological diagnosis and the consequent implications of trypanonomic treatment deserved a comprehensive and in-depth approach. Also the treatment of patients at risk or with heart failure, arrhythmias and thromboembolic events, based on pharmacological and complementary resources, received special attention. Additional chapters supported the conducts applicable to several special contexts, including t. cruzi/HIV co-infection, risk during surgeries, in pregnant women, in the reactivation of infection after heart transplantation, and others.     Finally, two chapters of great social significance, addressing the structuring of specialized services to care for individuals with the Cardiomyopathy of Chagas disease, and reviewing the concepts of severe heart disease and its medical-labor implications completed this guideline.Esta diretriz teve como objetivo principal atualizar os conceitos e formular as normas de conduta e evidências científicas que as suportam, quanto ao diagnóstico e tratamento da CDC, com especial ênfase na base de racionalidade que a embasou. A DC no século XXI mantém padrão epidemiológico de endemicidade em 21 países da América Latina. Investigadores e gestores de países endêmicos e não endêmicos indigitam a necessidade de se adotarem políticas abrangentes, de saúde pública, para controle eficaz da transmissão inter-humanos da infecção pelo T. cruzi, e obter-se nível otimizado de atendimento aos indivíduos já infectados, com foco em oportunização diagnóstica e terapêutica. Mecanismos patogênicos e fisiopatológicos da CDC foram revisitados após atualização aprofundada e ficou bem consolidada a noção de que necrose e fibrose sejam estimuladas pela persistência parasitária tissular e reação imune adversa, como mecanismos fundamentais, coadjuvados por distúrbios autonômicos e microvasculares. Alguns deles recentemente constituíram alvos potenciais de terapêuticas. A história natural das fases aguda e crônica foi revista, com realce para a transmissão oral, a forma indeterminada e as síndromes crônicas. Metanálises recentes de estudos observacionais estimaram o risco de evolução a partir das formas aguda e indeterminada e de mortalidade após instalação da cardiomiopatia crônica. Condutas terapêuticas aplicáveis aos indivíduos com a FIDC foram abordadas especificamente. Todos os métodos para detectar alterações estruturais e/ou funcionais com variadas técnicas de imageamento cardíaco também foram revisados, com recomendações de uso nos vários cenários clínicos. Estratificação de risco de mortalidade fundamentada no escore de Rassi, com estudos recentes de sua aplicação, foi complementada por métodos que detectam fibrose miocárdica. A metodologia atual para diagnóstico etiológico e as consequentes implicações do tratamento tripanossomicida mereceram enfoque abrangente e aprofundado. Também o tratamento de pacientes em risco ou com insuficiência cardíaca, arritmias e eventos tromboembólicos, baseado em recursos farmacológicos e complementares, recebeu especial atenção. Capítulos suplementares subsidiaram as condutas aplicáveis a diversos contextos especiais, entre eles o da co-infecção por T. cruzi/HIV, risco durante cirurgias, em grávidas, na reativação da infecção após transplante cardíacos, e outros.    Por fim, dois capítulos de grande significado social, abordando a estruturação de serviços especializados para atendimento aos indivíduos com a CDC, e revisando os conceitos de cardiopatia grave e suas implicações médico-trabalhistas completaram esta diretriz.&nbsp
