    Politics of immigration - quotas of entrance and hidden economy

    In spite of the fact that in recent years there has been a growing interest for the hidden economy, little attention has been lent to its relation with the illegal immigration as a consequence of the establishment of quotas. The objective of this work is to analyze the effect of a change in the quotas of legal immigrants in the size of the legal and illegal migration, the underground economy, and in the incomes perceived by the workers. The main conclusions are that, as opposed to the short time, in the long-term equilibrium, an increment of the quota does not have effect on the size of the migration, but in the composition between legal and illegal. On the other hand diminishes the work destined to the management of the underground economy by the reduction of illegal immigrants in the stationary state, however the effect is positive in the short run by the “called effect” of illegal immigrants that carries an increment in the quota.

    Poverty in Europe by gender: the role of education and labour status

    The objective of this paper is to analyse the determinants of poverty among workers in Europe by gender, using data for 25 countries obtained from the European UnionStatistics on Income and Living Conditions (EU-SILC) database, for the period 2010 to2013.The raw data show that the risk of poverty is similar for working men and women. Our study reveals that this homogeneity is only superficial; women should be in a relatively better position, since they have higher rates of tertiary education, but this advantage disappears when they access the market, because most part-time jobs are occupied by women. Asymmetries by gender are also present among occupations, but not between permanent and temporary workers

    Preventive health and active ageing: the elderly are not a burden

    Governments concerned with public finance are introducing reforms to push their citizens to remain in the labor market beyond the normal retirement age. We build a stylized theoretical framework in which we show that the labor supply among older workers and preventive health investment go hand in hand. In particular, those workers with the highest levels of productivity are those who remain longer in the labor market and, at the same time, who invest more in preventive health. We also find that a certain level of productivity in the health sector emerges as a prerequisite for active ageing. Furthermore, an increase in such productivity leads to growth in the effective labor supply, leading to an increase in demand for health care, while liberating additional resources for the non-health sectors

    The use of caspase inhibitors in pulsed-field gel electrophoresis may improve the estimation of radiation-induced DNA repair and apoptosis

    Background: Radiation-induced DNA double-strand break (DSB) repair can be tested by using pulsed-field gel electrophoresis (PFGE) in agarose-encapsulated cells. However, previous studies have reported that this assay is impaired by the spontaneous DNA breakage in this medium. We investigated the mechanisms of this fragmentation with the principal aim of eliminating it in order to improve the estimation of radiation-induced DNA repair. Methods: Samples from cancer cell cultures or xenografted tumours were encapsulated in agarose plugs. The cell plugs were then irradiated, incubated to allow them to repair, and evaluated by PFGE, caspase-3, and histone H2AX activation (gamma H2AX). In addition, apoptosis inhibition was evaluated through chemical caspase inhibitors. Results: We confirmed that spontaneous DNA fragmentation was associated with the process of encapsulation, regardless of whether cells were irradiated or not. This DNA fragmentation was also correlated to apoptosis activation in a fraction of the cells encapsulated in agarose, while non-apoptotic cell fraction could rejoin DNA fragments as was measured by gamma H2AX decrease and PFGE data. We were able to eliminate interference of apoptosis by applying specific caspase inhibitors, and improve the estimation of DNA repair, and apoptosis itself. Conclusions: The estimation of radiation-induced DNA repair by PFGE may be improved by the use of apoptosis inhibitors. The ability to simultaneously determine DNA repair and apoptosis, which are involved in cell fate, provides new insights for using the PFGE methodology as functional assay

    High-resolution scan of the Pyrenean crustal structure combining magnetotelluric and gravity data

    The Pyrenees have undergone complex geodynamic evolution starting with experiencing significant tectonic events during the Variscan Orogeny, followed by the intrusion of large granitic complexes during the late Variscan stage, then the collision between the Iberian and European plates during the Alpine Orogeny, and finally, Mesozoic extension. Despite extensive studies and the application of various geophysical methods (two-dimensional seismic reflection data, gravity, and long period magnetotellurics) to investigate the Pyrenean structure, there are still fundamental questions regarding its basement and cover architecture. Specifically, the geometry at depth of significant bodies such as the Late Variscan intrusive granites and Triassic evaporitic accumulations, remains unclear. To better understand these issues, we have conducted joint magnetotelluric and gravity surveys along a 60-kilometer-long transect, spanning the boundary between the Axial and South Pyrenean Zones. Our final geological interpretation shows that the La Maladeta batholith consists of two distinct granitic bodies related to different intrusive pulses. In addition, we identify important Triassic evaporitic accumulations at depth. This work shows the high potential of integrating two geophysical models for understanding the geological evolution of structurally complex areas. The magnetotelluric and gravity data are complementary, with each dataset providing a different resolution for investigating the basement and cover architecture of the Pyrenees. These resolutions depend on the varied petrophysical properties of the rocks involved, including water content and deformation grade

    Screening of CACNA1A and ATP1A2 genes in hemiplegic migraine: clinical, genetic and functional studies

    Hemiplegic migraine (HM) is a rare and severe subtype of autosomal dominant migraine, characterized by a complex aura including some degree of motor weakness. Mutations in four genes (CACNA1A, ATP1A2, SCN1A and PRRT2) have been detected in familial and in sporadic cases. This genetically and clinically heterogeneous disorder is often accompanied by permanent ataxia, epileptic seizures, mental retardation, and chronic progressive cerebellar atrophy. Here we report a mutation screening in the CACNA1A and ATP1A2 genes in 18 patients with HM. Furthermore, intragenic copy number variant (CNV) analysis was performed in CACNA1A using quantitative approaches. We identified four previously described missense CACNA1A mutations (p.Ser218Leu, p.Thr501Met, p.Arg583Gln, and p.Thr666Met) and two missense changes in the ATP1A2 gene, the previously described p.Ala606Thr and the novel variant p.Glu825Lys. No structural variants were found. This genetic screening allowed the identification of more than 30% of the disease alleles, all present in a heterozygous state. Functional consequences of the CACNA1A-p.Thr501Met mutation, previously described only in association with episodic ataxia, and ATP1A2-p.Glu825Lys, were investigated by means of electrophysiological studies, cell viability assays or Western blot analysis.     Work like a Doc: a comparison of regulations on residents' working hours in 14 high-income countries

    Background: Medical residents work long, continuous hours. Working in conditions of extreme fatigue has adverse effects on the quality and safety of care, and on residents' quality of life. Many countries have attempted to regulate residents’ work hours. Objectives: We aimed to review residents’ work hours regulations in different countries with an emphasis on night shifts. Methods: Standardized qualitative data on residents’ working hours were collected with the assistance of experts from 14 high-income countries through a questionnaire. An international comparative analysis was performed. Results: All countries reviewed limit the weekly working hours; North-American countries limit to 60–80 h, European countries limit to 48 h. In most countries, residents work 24 or 26 consecutive hours, but the number of long overnight shifts varies, ranging from two to ten. Many European countries face difficulties in complying with the weekly hour limit and allow opt-out contracts to exceed it. Conclusions: In the countries analyzed, residents still work long hours. Attempts to limit the shift length or the weekly working hours resulted in modest improvements in residents’ quality of life with mixed effects on quality of care and residents’ education

    Incidence of co-infections and superinfections in hospitalised patients with COVID-19: a retrospective cohort study

    Objectives: To describe the burden, epidemiology and outcomes of co-infections and superinfections occurring in hospitalized patients with coronavirus disease 2019 (COVID-19). Methods: We performed an observational cohort study of all consecutive patients admitted for ≥48 hours to the Hospital Clinic of Barcelona for COVID-19 (28 February to 22 April 2020) who were discharged or dead. We describe demographic, epidemiologic, laboratory and microbiologic results, as well as outcome data retrieved from electronic health records. Results: Of a total of 989 consecutive patients with COVID-19, 72 (7.2%) had 88 other microbiologically confirmed infections: 74 were bacterial, seven fungal and seven viral. Community-acquired co-infection at COVID-19 diagnosis was uncommon (31/989, 3.1%) and mainly caused by Streptococcus pneumoniae and Staphylococcus aureus. A total of 51 hospital-acquired bacterial superinfections, mostly caused by Pseudomonas aeruginosa and Escherichia coli, were diagnosed in 43 patients (4.7%), with a mean (SD) time from hospital admission to superinfection diagnosis of 10.6 (6.6) days. Overall mortality was 9.8% (97/989). Patients with community-acquired co-infections and hospital-acquired superinfections had worse outcomes. Conclusions: Co-infection at COVID-19 diagnosis is uncommon. Few patients developed superinfections during hospitalization. These findings are different compared to those of other viral pandemics. As it relates to hospitalized patients with COVID-19, such findings could prove essential in defining the role of empiric antimicrobial therapy or stewardship strategies

    Bacterial co-infection at hospital admission in patients with COVID-19

    Objectives: We described the current incidence and risk factors of bacterial co-infection in hospitalized patients with COVID-19. Methods: Observational cohort study was performed at the Hospital Clinic of Barcelona (February 2020-February 2021). All patients with COVID-19 who were admitted for >48 hours with microbiological sample collection and procalcitonin (PCT) determination within the first 48 hours were included. Results: A total of 1125 consecutive adults met inclusion criteria. Co-infections were microbiologically documented in 102 (9.1%) patients. Most frequent microorganisms were Streptococcus pneumoniae (79%), Staphylococcus aureus (6.8%), and Haemophilus influenzae (6.8%). Test positivity was 1% (8/803) for blood cultures, 10.1% (79/780) for pneumococcal urinary antigen test, and 11.4% (15/132) for sputum culture. Patients with PCT higher than 0.2, 0.5, 1, and 2 ng/mL had significantly more co-infections than those with lower levels (p=0.017, p=0.031, p94%
