    Programa de formación para profesionales sanitarios sobre la disreflexia autónoma

    La disreflexia autónoma (DA) es un fenómeno clínico que se presenta en el paciente medular y está causado por un estímulo nocivo por debajo del nivel de la lesión, que produce una respuesta exagerada del sistema nervioso simpático. Los factores desencadenantes pueden ser muy diversos, siendo la principal causa de origen urinario o intestinal. Dentro de toda la sintomatología, la de mayor gravedad y preocupación es la hipertensión arterial. La disreflexia autónoma debe considerarse una emergencia médica, ya que puede derivar a complicaciones de mayor gravedad si no se trata correctamente, comprometiendo la vida. Una pronta intervención mejora su pronóstico.<br /

    Semantic Similarity Measures Applied to an Ontology for Human-Like Interaction

    The focus of this paper is the calculation of similarity between two concepts from an ontology for a Human-Like Interaction system. In order to facilitate this calculation, a similarity function is proposed based on five dimensions (sort, compositional, essential, restrictive and descriptive) constituting the structure of ontological knowledge. The paper includes a proposal for computing a similarity function for each dimension of knowledge. Later on, the similarity values obtained are weighted and aggregated to obtain a global similarity measure. In order to calculate those weights associated to each dimension, four training methods have been proposed. The training methods differ in the element to fit: the user, concepts or pairs of concepts, and a hybrid approach. For evaluating the proposal, the knowledge base was fed from WordNet and extended by using a knowledge editing toolkit (Cognos). The evaluation of the proposal is carried out through the comparison of system responses with those given by human test subjects, both providing a measure of the soundness of the procedure and revealing ways in which the proposal may be improved.The development of this approach and its construction as part of the LaBDA-Interactor Human-Like Interaction System, part of the research projects SemAnts (TSI-020110-2009-419) and THUBAN (TIN2008-02711) and CADOOH (TSI-020302-2011-21), is supported by the Spanish Ministry of Industry, Tourism and Commerce and the Spanish Ministry of Education, respectively.     Optical design for MEGARA: a multi-object spectrograph for the GTC

    MEGARA is a multi-object spectrograph project for the 10.4m Gran Telescopio Canarias with medium to high resolution: R ~ 5600 - 17000. The instrument operates in three modes that cover different sky areas and that can run simultaneously: (1) the compact mode through a large central Integral Field Unit with minimum fiber pitch, covering a field of view on sky of 12 arcsec × 14 arcsec, (2) the sparse mode with fibers covering 1 arcmin × 1arcmin in three pointings and (3) the dispersed mode with a grid of nearly 100 robotics positioners able to place 7-fiber minibundles over a large field of view of 3.5 arcmin × 3.5 arcmin. The spectrograph is composed by a pseudo-slit, where the fibers are placed simulating a long slit; a slit shutter is placed just behind the pseudo-slit, a collimator, a 162mm pupil where the volume phase holographic gratings are placed, and the camera with the detector.     No radiographic sacroiliitis progression was observed in patients with early spondyloarthritis at 6 years: results of the Esperanza multicentric prospective cohort

    Objective: To estimate the 6-year radiographic progression of sacroiliitis in patients with early spondyloarthritis (SpA). Patients and methods: Sacroiliac joint (SIJ) radiographs (baseline and 6 years) of 94 patients with recent-onset SpA from the Esperanza cohort were scored, blindly and in a random order, by nine readers. The modified New York criteria were used to define the presence of sacroiliitis. As the gold standard for radiographic (r) sacroiliitis, the categorical opinion of at least five readers was used. Progression was defined as the shift from non-radiographic (nr) to r-sacroiliitis. Results: In the 94 SIJ radiographs (baseline and 6 years), 78/94 (83%) pairs of radiographs had not changed from baseline to 6 years. Sacroiliitis was present in 20 patients at baseline (21.3%) and in 18 (19.2%) patients at 6 years; 11 patients had sacroiliitis at both the baseline and final visits; 9 patients changed from baseline r-sacroiliitis to nr-sacroiliitis at 6 years, and 7 changed from baseline nr-sacroiliitis to r-sacroiliitis at 6 years. The mean continuous change score (range: -8 to +8) was 2.80 at baseline and 2.55 at 6 years (mean net progression of -0.25). The reliability of the readers was fair (mean inter-reader kappa of 0.375 (0.146-0.652) and mean agreement of 73.7% (58.7-90%)). Conclusion: In the early SpA Esperanza cohort, progression from nr-axSpA to r-axSpA over 6 years was not observed, although the SIJ radiographs scoring has limitations to detect low levels of radiographic progression

    Invasive Fusariosis in Nonneutropenic Patients, Spain, 2000-2015

    Invasive fusariosis (IF) is associated with severe neutropenia in patients with concurrent hematologic conditions. We conducted a retrospective observational study to characterize the epidemiology of IF in 18 Spanish hospitals during 2000-2015. In that time, the frequency of IF in nonneutropenic patients increased from 0.08 cases per 100,000 admissions in 2000-2009 to 0.22 cases per 100,000 admissions in 2010-2015. Nonneutropenic IF patients often had nonhematologic conditions, such as chronic cardiac or lung disease, rheumatoid arthritis, history of solid organ transplantation, or localized fusariosis. The 90-day death rate among nonneutropenic patients (28.6%) and patients with resolved neutropenia (38.1%) was similar. However, the death rate among patients with persistent neutropenia (91.3%) was significantly higher. We used a multivariate Cox regression analysis to characterize risk factors for death: persistent neutropenia was the only risk factor for death, regardless of antifungal therapy

    [ES] A través del grupo de Innovación Docente BIOMETAC, nace esta experiencia de ApS para promover en los estudiantes de los grados en Biología, Biotecnología y Ciencias Ambientales de la ULE, la iniciativa cívica de participación en la sociedad. El objetivo general del proyecto es que el alumnado aplique sus conocimientos y habilidades para sensibilizar y dinamizar a la población de varios municipios rurales de León frente a retos de la Agenda 2030 asociados a la salud y la sostenibilidad ambiental. Se establecieron 3 grupos de trabajo, cada uno integrado por 12 estudiantes de los tres grados, que trabajan en 3 municipios rurales de la provincia de León: Cistierna, Truchas y Villablino, con diversas problemáticas en el ámbito de la salud y medioambiente. El Proyecto se realiza en tres fases: Preparación, Ejecución y Evaluación y Difusión. Durante la primera fase (curso 2021/22), se realizaron reuniones con el alumnado para guiarlos en el diagnóstico de cada municipio. Los datos obtenidos permitieron detectar los problemas que más preocupaban a la población de cada municipio. En una segunda fase (curso 2022/23), los alumnos profundizarán en las distintas problemáticas mediante actuaciones propuestas por ellos mismos y presentarán sus conclusiones en cada municipio. Esta experiencia pretende que los estudiantes pongan los conocimientos y las competencias adquiridas al servicio de comunidades rurales, trabajando directamente sobre varios Objetivos de Desarrollo Sostenible (ODS), mejorando su capacidad de iniciativa, autonomía en la organización del trabajo, y responsabilidad para la resolución de problemas
