INTRODUCTION: Infections caused by Acinetobacter junii are rarely reported. However, some outbreaks of septicemia in neonates and pediatric oncology patients, as well as meningitis, peritonitis, and ocular infection have been described. Since it is highly infrequent to find the molecular characterization of A. junii strains in literature, in this study we described the molecular characterization of A. junii isolates recovered from blood samples of a renal transplant patient. METHODOLOGY: The case was defined as a catheter-related bacteremia caused by A. junii. The patient responded favorably after catheter removal and treatment with ciprofloxacin. RESULTS AND CONCLUSION: The complete molecular characterization of the isolate showed that it harbored a class 1 integron and diverse DNA mobile elements. This explains its genomic plasticity for acquiring antimicrobial resistance determinants and for adapting to a nosocomial niche.Fil: Traglia, German Matias. Get PDF
This study evaluated the effect of two concentrate supplementation strategies on performance, metabolic profile and economic evaluation of suckling lambs and ewes in ryegrass (Lolium multiflorum) pasture. Twenty-seven ewes and 45 lambs were divided into three groups: (1) ryegrass pasture without supplementation - control (CON); (2) CON plus supplemented ewes and lambs at 1% of live weight (SEL), and (3) CON plus creep feeding supplemented lambs at 1% of live weight (CSL). Concentrate use increased (p<0.05) average daily gain (ADG) by 19.95% over CON (21.6 and 18.3% for SEL and CSL, respectively). Concentrate use contributed to minimizing forage quality fluctuation and provided greater ADG stability, mainly when ryegrass nutritional content and digestibility decreased. Blood metabolites profiles did not differ between groups, with exception of phosphorus which was higher for CON than SEL, and calcium which was higher for CSL than CON (p<0.05). Compared to CON, stoking rate values were greater to SEL (p<0.05). Compared to CSL, ewe and total stocking rate were greater (p<0.05) to SEL. Considering the control group as break even feed investment, SEL strategy had a positive economic return, while CSL showed economic losses. Concentrate use increased ADG of lambs and decrease the impact of nutrient quality changes of forage on daily gains, but must be considered that supplemental strategy used could affect negatively economic return

AbstractHuman herpesvirus type 6-(HHV-6) has been associated with morbidity after liver transplantation.ObjectiveThe aim of this study was to determine the HHV-6 seroprevalence among donorrecipient pairs, analyze the incidence of early active infection, its clinical manifestation, interaction with CMV, and the related morbidity in the first year after kidney transplantation.Methods46 donor-recipient pairs had IgG evaluated by ELISA before transplantation: HHV-6-(Pambio – USA) and CMV-(Roche – USA). A frozen whole blood sample collected weekly (from the 1st to the 6th week) was retrospectively tested for HHV-6 viral load (VL) determination by real time quantitative PCR (qPCR, Nanogen – Italy). Patients were preemptively surveyed for CMV by pp65 antigenemia (Ag, APAAP, immunohistochemistry, Biotest – Germany) from the 4th to the 12th week after transplantation. Active infection was defined as qPCR-HHV6+ (viral-load/mL-VL) and Ag+ (+cells/100.000 granulocytes), for HHV-6 and CMV, respectively. DCMV was defined as simultaneous positive antigenemia and suggestive signs/symptoms. Concerning +qPCR-HHV6, associated factors, clinical manifestation, interaction with CMV and morbidity were searched.ResultsPre-transplant HHV-6 seroprevalence was significantly higher among kidney recipients compared to their donors (82.6×54.8%; p=0.005 [3.9 (1.4–10.4)]). Active infection by this virus occurred in 26.1% (12/46), with no association with previous IgG (p=0.412). Median VL was 125 copies/mL (53–11.264), and the median Ag was 21 +cells (2–740). There was no association between HHV-6 and CMV activation after transplantation (p=0.441), neither concerning DCMV (p=0.596). Median highest Ag+ and days of ganciclovir treatment were similar between qPCR-HHV6 + or − (p=0.206 and p=0.124, respectively). qPCR-HHV6+ was associated with higher incidence of bacterial (p=0.009) and fungal (p = 0.001) infections, and higher number (p=0.001) of hospital admission and longer duration of hospitalization over the first 6 and 12 months post-transplantation (p=0.033 and p=0.001).ConclusionLatent HHV-6 infection is more common among recipients than donors before transplantation. Early active infection by this pathogen after transplantation does not increase DCMV incidence or severity during the first 3 months of follow-up. However, early HHV-6 replication is associated with other infections and hospitalizations in the first year

The objective of this work was to evaluate the productivity, morphological and bromatological composition of maize (Zea mays L.) for silage managed under increasing levels of nitrogen fertilization in the cover. The treatments consisted of four levels of cover fertilization: T1 - (112.5 kg ha - 1 of N); T2 - (225.0 kg ha-1 of N); T3 - (337.5 kg ha-1 of N); And T4 - (450.0 kg ha-1 of N). It was verified that the level of fertilization did not affect (P> 0.05) the parameters related to plant height, spike insertion height, fresh and dry biomass production, and grain yield, with mean values of 2, 18 m and 1.16 m, 46,361 kg ha-1, 19,516 kg ha-1 and 9,148 kg ha-1, respectively.

Objective: characterizing the sociodemographic, health and assistential aspects of people with venous ulcers treated at the Family Health Strategy (FHS) in Maceió-Alagoas and analyzing the quality of care provided. Method: a cross-sectional study with a quantitative approach conducted in 36 FHS units with 59 people with venous ulcers through a structured form. Results: people with venous ulcers treated > 1 year (69,5%), female (71,2%) and ≥ 60 years old (67,8%). Most were nonsmoker and nonalcoholic and 100,0% had two or more risk factors and pathological personal antecedents each. Had time of injury > 6 months (64,4%), pain in the ulcer / member (86,4%) and rocker ≤ 30% granulation/epithelialization (78,0%). The quality of care was poor in 57,6% and the aspects that mostly affected were the inadequacy of: professional that was accompanying/performing curative, products in the past 30 days and access to consultation with angiologist. Conclusions: the people with venous ulcers had low socioeconomic status, chronic diseases and unfavorable lesion characteristics contributing to chronicity of the lesions

Cost-utility analysis of opportunistic and systematic diabetic retinopathy screening strategies from the perspective of the Brazilian Public Healthcare System

    Get PDF
Objective: To perform a cost-utility analysis of diabetic retinopathy (DR) screening strategies from the perspective of the Brazilian Public Healthcare System. Methods: A model-based economic evaluation was performed to estimate the incremental costs per quality-adjusted life-year (QALY) gained between three DR screening strategies: (1) the opportunistic ophthalmology referral-based (usual practice), (2) the systematic ophthalmology referral-based, and (3) the systematic teleophthalmology-based. The target population included individuals with type 2 diabetes (T2D) aged 40 years, without retinopathy, followed over a 40-year time horizon. A Markov model was developed with five health states and a 1-year cycle. Model parameters were based on literature and country databases. One-way and probabilistic sensitivity analyses were performed to assess model parameters’ uncertainty. WHO willingness-to-pay (WHO-WTP) thresholds were used as reference (i.e. one and three times the Brazilian per capita Gross Domestic Product of R32747in2018).Results:Comparedtousualpractice,thesystematicteleophthalmologybasedscreeningwasassociatedwithanincrementalcostofR32747 in 2018). Results: Compared to usual practice, the systematic teleophthalmology-based screening was associated with an incremental cost of R21445/QALY gained (9792/QALYgained).Thesystematicophthalmologyreferralbasedscreeningwasmoreexpensive(incrementalcosts=R9792/QALY gained). The systematic ophthalmology referral-based screening was more expensive (incremental costs = R4) and less effective (incremental QALY = −0.012) compared to the systematic teleophthalmology-based screening. The probability of systematic teleophthalmology-based screening being cost-effective compared to usual practice was 0.46 and 0.67 at the minimum and the maximum WHO-WTP thresholds, respectively. Conclusion: Systematic teleophthalmology-based DR screening for the Brazilian population with T2D would be considered very cost effective compared to the opportunistic ophthalmology referral-based screening according to the WHO-WTP threshold. However, there is still a considerable amount of uncertainty around the results
