    Development of PLS–path model for understanding the role of precursors on ground level ozone concentration in Gulfport, Mississippi, USA

    AbstractGround-level ozone (GLO) is produced by a complex chain of atmospheric chemical reactions that depend on precursor emissions from natural and anthropogenic sources. GLO concentration in a particular location is also governed by local weather and climatic factors. In this work an attempt was made to explore a Partial Least Squares Path Modeling (PLS–PM) approach to quantify the interrelationship between local conditions (weather parameters and primary air pollution) and GLO concentrations. PLS path modeling algorithm was introduced and applied to GLO concentration analyses at Gulfport, Mississippi, USA. In the present analysis, three latent variables were selected: PRC (photochemical reaction catalyst), MP (meteorological factor), and OPP (other primary air pollutants). The three latent variables included 14 indicators for the analysis; with PRC having two (extraterrestrial radiation on horizontal surface, and extraterrestrial radiation normal to the sun), MP having nine (temperature, dew point, relative humidity, pressure, visibility, maximum wind speed, average wind speed, precipitation, and wind direction) and OPP having three (NO2, PM2.5, and SO2) parameters. The resulting model revealed that PRC had significant direct impact on GLO concentration but very small overall effect. This is because PRC had significant indirect negative impact on GLO via MP. Thus, when both direct and indirect effects were taken into account, PRC emerged as having the weakest effect on GLO. The third variable (OPP) also had a positive impact on GLO concentration

    Qualitative observation of reversible phase change in astrochemical ethanethiol ices using infrared spectroscopy

    Here we report the first evidence for a reversible phase change in an ethanethiol ice prepared under astrochemical conditions. InfraRed (IR) spectroscopy was used to monitor the morphology of the ice using the Ssingle bondH stretching vibration, a characteristic vibration of thiol molecules. The deposited sample was able to switch between amorphous and crystalline phases repeatedly under temperature cycles between 10 K and 130 K with subsequent loss of molecules in every phase change. Such an effect is dependent upon the original thickness of the ice. Further work on quantitative analysis is to be carried out in due course whereas here we are reporting the first results obtained

    Infrared attenuation due to phase change from amorphous to crystalline observed in astrochemical propargyl ether ices

    Astrochemical ices are known to undergo morphological changes, from amorphous to crystalline, upon warming the ice from lower (10 K) to higher temperatures. Phase changes are mostly identified by the observation of significant changes in the InfraRed (IR) spectrum, where the IR bands that are broad in the amorphous phase are narrower and split when the ice turns crystalline. To-date all the molecules that are studied under astrochemical conditions are observed to follow such a behaviour without significant attenuation in the IR wavelength. However, in this paper we report a new observation when propargyl ether (C3H3OC3H3C_3H_3OC_3H_3) is warmed from the amorphous phase, at 10 K, through the phase transition temperature of 170 K, the crystalline ice being found to strongly attenuate IR photons at the mid-IR wavelengths

    Oral bisphosphonate compliance and persistence: a matter of choice?

    Compliance to oral bisphosphonates is suboptimal, with negative consequences of increased healthcare utilization and less effective fracture risk reduction. Extending dose interval increased adherence only moderately. We used literature derived from multiple chronic conditions to examine the problem of noncompliance with osteoporosis medication. We reviewed the literature on adherence to osteoporosis medication as well as that across multiple chronic conditions to understand what is known about the cause of the poor adherence. Poor compliance to oral medications is due mostly, not to forgetfulness, but to deliberate choice. Gender differences and style of healthcare management also play a role. Preliminary data suggest psychobehavioral interventions may help to improve motivation. We need to understand better reasons for poor compliance before effective interventions can be developed. Forgetfulness is only a small part of poor compliance. Patient preferences must be considered in medication decision making

    CYP17 genetic polymorphism, breast cancer, and breast cancer risk factors: Australian Breast Cancer Family Study

    INTRODUCTION: Because CYP17 can influence the degree of exposure of breast tissues to oestrogen, the interaction between polymorphisms in this gene and hormonal risk factors is of particular interest. We attempted to replicate the findings of studies assessing such interactions with the -34T→C polymorphism. METHODS: Risk factor and CYP17 genotyping data were derived from a large Australian population-based case-control-family study of 1,284 breast cancer cases and 679 controls. Crude and adjusted odds ratio (OR) estimates and 95% confidence intervals (CIs) were calculated by unconditional logistic regression analyses. RESULTS: We found no associations between the CYP17 genotype and breast cancer overall. Premenopausal controls with A(2)/A(2 )genotype had a later age at menarche (P < 0.01). The only associations near statistical significance were that postmenopausal women with A(1)/A(1 )(wild-type) genotype had an increased risk of breast cancer if they had ever used hormone replacement therapy (OR 2.40, 95% CI 1.0 to 5.7; P = 0.05) and if they had menopause after age 47 years (OR 2.59, 95% CI 1.0 to 7.0; P = 0.06). We found no associations in common with any other studies, and no evidence for interactions. CONCLUSION: We observed no evidence of effect modification of reproductive risk factors by CYP17 genotype, although the experiment did not have sufficient statistical power to detect small main effects and modest effects in subgroups. Associations found only in subgroup analyses based on relatively small numbers require cautious interpretation without confirmation by other studies. This emphasizes the need for replication in multiple and large population-based studies to provide convincing evidence for gene–environment interactions

    Oncoplastic Breast Consortium consensus conference on nipple-sparing mastectomy.

    Purpose Indications for nipple-sparing mastectomy (NSM) have broadened to include the risk reducing setting and locally advanced tumors, which resulted in a dramatic increase in the use of NSM. The Oncoplastic Breast Consortium consensus conference on NSM and immediate reconstruction was held to address a variety of questions in clinical practice and research based on published evidence and expert panel opinion. Methods The panel consisted of 44 breast surgeons from 14 countries across four continents with a background in gynecology, general or reconstructive surgery and a practice dedicated to breast cancer, as well as a patient advocate. Panelists presented evidence summaries relating to each topic for debate during the in-person consensus conference. The iterative process in question development, voting, and wording of the recommendations followed the modified Delphi methodology. Results Consensus recommendations were reached in 35, majority recommendations in 24, and no recommendations in the remaining 12 questions. The panel acknowledged the need for standardization of various aspects of NSM and immediate reconstruction. It endorsed several oncological contraindications to the preservation of the skin and nipple. Furthermore, it recommended inclusion of patients in prospective registries and routine assessment of patient-reported outcomes. Considerable heterogeneity in breast reconstruction practice became obvious during the conference. Conclusions In case of conflicting or missing evidence to guide treatment, the consensus conference revealed substantial disagreement in expert panel opinion, which, among others, supports the need for a randomized trial to evaluate the safest and most efficacious reconstruction techniques
