    La presente investigación lleva por título “ACTITUD DE LOS DOCENTES FRENTE A ESTUDIANTES INCLUSIVÓS DE UNA I.E. PÚBLICA DE CAYALTÍ, LAMBAYEQUE 2020”, está basada en determinar cuál es el nivel de las actitudes de los maestros en Educación Secundaria con los educandos inclusivos en la entidad educativa San Martin de Porras, Cayaltí. Para ello se utilizaron instrumentos para recopilar datos como cuestionario, con el propósito fina de adquirir un panorama claro y extenso de la actualidad y la situación educativa de la inclusión en la institución educativa en mención con la cual se realizó con gran esfuerzo y dedicación. Se logró obtener los datos principales de acuerdo a cada dimensión de la variable estudiada, se tomó un 100% de manera global, se tuvo una población censal que se conformó por 24 docentes que estuvieron aptos y dispuestos para su aplicación de los cuestionarios. En base a los resultados e inferencias conseguidas se dedujo que la gran mayoría de los docentes no expresan una adecuada actitud con los estudiantes inclusivos de la institución educativa San Martin de Porras, Cayaltí.Tesi

    Estrategias didácticas y educación inclusiva en estudiantes del nivel primaria en una Institución Educativa de Lambayeque

    La presente investigación considera como objetivo determinar la relación existente entre estrategias didácticas y educación inclusiva en estudiantes del nivel primaria en una institución educativa de Lambayeque; donde se utilizó el diseño descriptivo correlacional, con enfoque cuantitativo para una población muestral de dos docentes y 57 estudiantes, donde se aplicó el no probabilístico como muestreo, a juicio de los investigadores. El instrumento utilizado en el recojo de datos, es el cuestionario para cada variable, el cual tuvo la apreciación aprobada a juicios de expertos, el programa Excel 2016 y del software SPSS número 23, arrojando la fiabilidad del alfa de Crombach en 0,910 de la variable estrategias didácticas y 0.899 en la variable educación inclusiva. Esta indagación concluye, que se determina que si existe relación significativa dado que p < 0.01, de acuerdo a la prueba no paramétrica de la Rho de Spearman con un coeficiente de correlación positiva perfecta de 1,000, entre las estrategias didácticas y la educación inclusiva de los estudiantes del nivel primaria de una institución educativa Lambayeque, aceptándose la hipótesis alterna H1 y el rechazo de la hipótesis nula H0.Tesi

    Unleashing the Diagnostic, Prognostic and Therapeutic Potential of the Neuronostatin/GPR107 System in Prostate Cancer

    Certain components of the somatostatin-system play relevant roles in Prostate Cancer (PCa), whose most aggressive phenotype (Castration-Resistant-PCa (CRPC)) remains lethal nowadays. However, neuronostatin and the G protein-coupled receptor 107 (GPR107), two novel members of the somatostatin-system, have not been explored yet in PCa. Consequently, we investigated the pathophysiological role of NST/GPR107-system in PCa. GPR107 expression was analyzed in well-characterized PCa patient′s cohorts, and functional/mechanistic assays were performed in response to GPR107-silencing and NST-treatment in PCa cells (androgen-dependent (AD: LNCaP) and androgen-independent (AI: 22Rv1/PC-3), which are cell models of hormone-sensitive and CRPC, respectively), and normal prostate cells (RWPE-1 cell-line). GPR107 was overexpressed in PCa and associated with key clinical parameters (e.g., advance stage of PCa, presence of vascular invasion and metastasis). Furthermore, GPR107-silencing inhibited proliferation/migration rates in AI-PCa-cells and altered key genes and oncogenic signaling-pathways involved in PCa aggressiveness (i.e., KI67/CDKN2D/MMP9/PRPF40A, SST5TMD4/AR-v7/In1-ghrelin/EZH2 splicing-variants and AKT-signaling). Interestingly, NST treatment inhibited proliferation/migration only in AI-PCa cells and evoked an identical molecular response than GPR107-silencing. Finally, NST decreased GPR107 expression exclusively in AI-PCa-cells, suggesting that part of the specific antitumor effects of NST could be mediated through a GPR107-downregulation. Altogether, NST/GPR107-system could represent a valuable diagnostic and prognostic tool and a promising novel therapeutic target for PCa and CRPC

    Weber approach for retromolar adenoid cyst carcinoma: case report and literature review

    Adenoid cyst carcinoma (ACC) is an uncommon facial tumor and challenging to treat; surgical treatment is challenging due to its localization. A 49-year-old Latin female patient with a one-year history of soft tissue-depending mass of slow growth in the right maxillary region. Laboratory tests reported normal. Contrast-enhanced computed tomography showed a maxillary tumor. Patient underwent to surgical resection by Weber-Fergusson approach. The patient received postoperative adjuvant radiotherapy. ACC is a challenging entity to treat, surgical resection, and radiotherapy is required. The combination of surgery and radiotherapy represents the definitive treatment for ACC. Chemotherapy remains controversial and is reserved for palliation in selected cases.

    Intestinal obstruction by deep enteric endometriosis: case report and literature review

    Deep endometriosis (DE) is an uncommon cause of bowel obstruction; preoperative diagnosis is a challenging task due to its rarity and pathological confirmation. Surgery is the appropriate treatment and complications are common. A 26-year-old Latin female was admitted to emergency department with 72 hours history of abdominal pain associated with inability to pass stool or gas, vomiting and nausea. Abdominal distention and pain without acute abdomen signs. Laboratory tests reported normal. Abdominal contrast-enhanced computed tomography showed distal small bowel obstruction. Patient underwent exploratory laparotomy with segmental resection bearing ileal strictures and Brook´s ileostomy was performed. Postoperative course of patient was uneventful and after pathology report treatment with dienogest was established. DE remains challenging entity to treat, medical treatment can reduce symptoms, but surgical resection is required. Bowel resection is reserved for mayor stenosis lesions. Anastomotic leakage is frequent. Surgery represents the definitive treatment for bowel obstruction by DE. Resection improves pain and intestinal symptoms. Recurrence, stenosis, and anastomotic leakage rates vary across the studies. Surgical and medical treatment should be considered

    Clinical Utility of Ghrelin-O-Acyltransferase (GOAT) Enzyme as a Diagnostic Tool and Potential Therapeutic Target in Prostate Cancer

    Recent data suggested that plasma Ghrelin O-Acyl Transferase enzyme (GOAT) levels could represent a new diagnostic biomarker for prostate cancer (PCa). In this study, we aimed to explore the diagnostic and prognostic/aggressiveness capacity of GOAT in urine, as well as to interrogate its putative pathophysiological role in PCa. We analysed urine/plasma levels of GOAT in a cohort of 993 patients. In vitro (i.e., cell-proliferation) and in vivo (tumor-growth in a xenograft-model) approaches were performed in response to the modulation of GOAT expression/activity in PCa cells. Our results demonstrate that plasma and urine GOAT levels were significantly elevated in PCa patients compared to controls. Remarkably, GOAT significantly outperformed PSA in the diagnosis of PCa and significant PCa in patients with PSA levels ranging from 3 to 10 ng/mL (the so-called PSA grey-zone). Additionally, urine GOAT levels were associated to clinical (e.g., Gleason-score, PSA levels) and molecular (e.g., CDK2/CDK6/CDKN2A expression) aggressiveness parameters. Indeed, GOAT overexpression increased, while its silencing/blockade decreased cell-proliferation in PCa cells. Moreover, xenograft tumors derived from GOAT-overexpressing PCa (DU145) cells were significantly higher than those derived from the mock-overexpressing cells. Altogether, our results demonstrate that GOAT could be used as a diagnostic and aggressiveness marker in urine and a therapeutic target in PCa

    Oncogenic Role of Secreted Engrailed Homeobox 2 (EN2) in Prostate Cancer

    Engrailed variant-2 (EN2) has been suggested as a potential diagnostic biomarker; however, its presence and functional role in prostate cancer (PCa) cells is still controversial or unknown. Here, we analyzed 1) the expression/secretion profile of EN2 in five independent samples cohorts from PCa patients and controls (prostate tissues and/or urine) to determine its utility as a PCa biomarker; and 2) the functional role of EN2 in normal (RWPE1) and tumor (LNCaP/22Rv1/PC3) prostate cells to explore its potential value as therapeutic target. EN2 was overexpressed in our two cohorts of PCa tissues compared to control and in tumor cell lines compared with normal-like prostate cells. This profile was corroborated in silico in three independent data sets [The Cancer Genome Atlas(TCGA)/Memorial Sloan Kettering Cancer Center (MSKCC)/Grasso]. Consistently, urine EN2 levels were elevated and enabled discrimination between PCa and control patients. EN2 treatment increased cell proliferation in LNCaP/22Rv1/PC3 cells, migration in RWPE1/PC3 cells, and PSA secretion in LNCaP cells. These effects were associated, at least in the androgen-sensitive LNCaP cells, with increased AKT and androgen-receptor phosphorylation levels and with modulation of key cancer-associated genes. Consistently, EN2 treatment also regulated androgen-receptor activity (full-length and splicing variants) in androgen-sensitive 22Rv1 cells. Altogether, this study demonstrates the potential utility of EN2 as a non-invasive diagnostic biomarker for PCa and provides novel and valuable information to further investigate its putative utility to develop new therapeutic tools in PCa

    Prospective study of diagnostic accuracy in the detection of high-grade prostate cancer in biopsy-naïve patients with clinical suspicion of prostate cancer who underwent the Select MDx test

    [Objectives] This study aimed to externally validate the diagnostic accuracy of the Select MDx test for Significant prostate cancer (Sig PCa) (ISUP > 1), in a contemporaneous, prospective, multicenter cohort with a prostate-specific antigen (PSA) between 3 and 10 ng/ml and a non-suspicious digital rectal examination.[Methods and Participants] For all enrolled patients, the Select Mdx test, the risk calculator ERSPC3 + DRE, and a prostatic magnetic resonance imaging (MRI) were carried out. Subsequently, a systematic 12-core trans-rectal biopsy and a targeted biopsy, in the case of a prostate imaging–reporting and data system (PIRADS) > 2 lesion (max three lesions), were performed. To assess the accuracy of the Select MDx test in the detection of clinically Sig PCa, the test sensitivity was evaluated. Secondary objectives were specificity, negative predictive value (NPV), positive predictive value (PPV), and area under the curve (AUC). A direct comparison with the ERSPC + DRE risk calculator and MRI were also performed. We also studied the predictive ability to diagnose Sig PCa from the combination of the Select MDx test with MRI using clinical decision-curve analysis.[Results] There were 163 patients enrolled after meeting the inclusion criteria and study protocol. The Select MDx test showed a sensitivity of 76.9% (95% CI, 63.2–87.5), 49.6% specificity (95% CI, 39.9–59.2), 82.09% (95% CI, 70.8–90.4) NPV, and 41.67% (95% CI, 31.7–52.2) PPV for the diagnosis of Sig PCa. COR analysis was also performed, which showed an AUC of 0.63 (95% CI, 0.56–0.71). There were no differences in the accuracy of Select MDx, ERSPC + DRE, or MRI. The combination of Select MDX + MRI showed the highest impact in the decision-curve analysis, with an NPV of 93%.[Conclusion] Our study showed a worse performance for the SelectMdx test than previously reported, within a cohort of patients with a PSA 3–10 ng/ml and a normal DRE, with results similar to those from ERSPC + DRE RC and MRI, but with an improvement in the usual PSA pathway.     Anandamide and 2-arachidonoylglycerol baseline plasma concentrations and their clinical correlate in gambling disorder

    Introduction Different components of the endocannabinoid (eCB) system such as their most well-known endogenous ligands, anandamide (AEA) and 2-arachidonoylglycerol (2-AG), have been implicated in brain reward pathways. While shared neurobiological substrates have been described among addiction-related disorders, information regarding the role of this system in behavioral addictions such as gambling disorder (GD) is scarce.AimsFasting plasma concentrations of AEA and 2-AG were analyzed in individuals with GD at baseline, compared with healthy control subjects (HC). Through structural equation modeling, we evaluated associations between endocannabinoids and GD severity, exploring the potentially mediating role of clinical and neuropsychological variables.MethodsThe sample included 166 adult outpatients with GD (95.8% male, mean age 39 years old) and 41 HC. Peripheral blood samples were collected after overnight fasting to assess AEA and 2-AG concentrations (ng/ml). Clinical (i.e., general psychopathology, emotion regulation, impulsivity, personality) and neuropsychological variables were evaluated through a semi-structured clinical interview and psychometric assessments.ResultsPlasma AEA concentrations were higher in patients with GD compared with HC (p = .002), without differences in 2-AG. AEA and 2-AG concentrations were related to GD severity, with novelty-seeking mediating relationships.ConclusionsThis study points to differences in fasting plasma concentrations of endocannabinoids between individuals with GD and HC. In the clinical group, the pathway defined by the association between the concentrations of endocannabinoids and novelty-seeking predicted GD severity. Although exploratory, these results could contribute to the identification of potential endophenotypic features that help optimize personalized approaches to prevent and treat GD

    Fish Oil Enriched Intravenous Lipid Emulsions Reduce Triglyceride Levels in Non-Critically Ill Patients with TPN and Type 2 Diabetes. A Post-Hoc Analysis of the INSUPAR Study

    There are no studies that have specifically assessed the role of intravenous lipid emulsions (ILE) enriched with fish oil in people with diabetes receiving total parenteral nutrition (TPN). The objective of this study was to assess the metabolic control (glycemic and lipid) and in-hospital complications that occurred in non-critically ill inpatients with TPN and type 2 diabetes with regard to the use of fish oil emulsions compared with other ILEs. We performed a post-hoc analysis of the Insulin in Parenteral Nutrition (INSUPAR) trial that included patients who started with TPN for any cause and that would predictably continue with TPN for at least five days. The study included 161 patients who started with TPN for any cause. There were 80 patients (49.7%) on fish oil enriched ILEs and 81 patients (50.3%) on other ILEs. We found significant decreases in triglyceride levels in the fish oil group compared to the other patients. We did not find any differences in glucose metabolic control: mean capillary glucose, glycemic variability, and insulin dose, except in the number of mild hypoglycemic events that was significantly higher in the fish oil group. We did not observe any differences in other metabolic, liver or infectious complications, in-hospital length of stay or mortality
