Kardiyak resenkronizasyon tedavisine yanıtın öngörülmesinde sistemik bağışıklık-inflamatuvar indeksinin rolü
Aim: Cardiac resynchronization therapy (CRT) is a reliable treatment
modality in patients with systolic dysfunction. However, not every patient
appears to benefit from CRT. The systemic immune inflammation index
(SII) is closely linked to the poor prognosis of various cardiovascular
disorders. However, there is no study investigating whether SII has
predictive value in determining response to CRT in dilated cardiomyopathy
patients. Therefore, we intend to investigate the association between SII
and response to CRT.
Methods: A total of 220 patients (mean age 61.2±10.8 years; 120 men)
implanted with CRT were involved in this study. Echocardiographic and
laboratory measurements were evaluated prior to CRT. Response to CRT
was determined as a≥ 15% decrease in left ventricular end-systolic volume
at one-year follow-up.
Results: Patients grouped as CRT responders and non-responders. Of
these, 143 (64.6%) were considered to be CRT responders, while the
remaining 77 (33.4%) were non-responders. Female sex (OR: 3.823, CI:
1.568-9.324 p=0.003), QRS duration (OR: 1.224, CI: 1.158-1.335
p<0.001), and SII (OR: 0.996 CI: 0.995-0.997 p<0.001) were shown to be
independent predictors of CRT response in multivariate analysis. A cut-off
value of SII >825 estimated no response to CRT with 80% sensitivity and
75% specificity.
Conclusions: SII was associated with unresponsiveness to CRT. Therefore,
it may be used to determine optimal patient selection for CRT implantation
in routine clinical practice.Amaç: Kardiyak resenkronizasyon tedavisi (KRT), sistolik disfonksiyonu
olan hastalarda güvenilir bir tedavi yöntemidir. Ancak, KRT'nin faydası belli
hasta grupları ile sınırlıdır. Sistemik immün inflamatuvar indeks (SII), çeşitli
kardiyovasküler bozuklukların kötü prognozu ile ilişkilidir. Bununla birlikte,
dilate kardiyomiyopati hastalarında SII'nin KRT'ye yanıtı belirlemede
prediktif değeri olup olmadığını araştıran bir çalışma bulunmamaktadır. Bu
nedenle, bu çalışmada SII ile KRT'ye yanıt arasındaki ilişkiyi araştırmak
amaçlandı.
Yöntemler: Bu çalışmaya KRT implante edilen toplam 220 hasta (ortalama
yaş 61,2±10,8 yıl; 120 erkek) dahil edildi. KRT öncesi ekokardiyografi ve
laboratuvar ölçümleri değerlendirildi. KRT'ye yanıt, bir yıllık takipte sol
ventrikül sistol sonu hacminde ≥ %15 azalma olarak belirlendi.
Bulgular: Hastalar, KRT'ye yanıt verenler ve yanıt vermeyenler olarak
gruplandırıldı. Bunlardan 143'ü (%64,6) KRT'ye yanıt veren olarak kabul
edilirken, kalan 77'si (%33,4) yanıt vermeyendi. Kadın cinsiyet (OR: 3.823,
CI: 1.568-9.324 p=0.003), QRS süresi (OR: 1.224, CI: 1.158-1.335
p<0.001) ve SII (OR: 0.996 CI: 0.995-0.997 p<0.001) çok değişkenli
analizde KRT yanıtının bağımsız öngörücüleri olarak bulundu. SII >825'lik
bir sınır değeri, %80 duyarlılık ve %75 özgüllük ile KRT'ye yanıt olmadığını
öngördürmüştür.
Sonuç: Bu çalışmada SII’nin KRT'ye yanıtsızlığı öngördüğü gösterilmiştir.
Bu nedenle SII rutin klinik uygulamada KRT implantasyonu için optimal
hasta seçimini belirlemede kullanılabilir
The frequency of Duchenne muscular dystrophy/Becker muscular dystrophy and Pompe disease in children with isolated transaminase elevation: results from the observational VICTORIA study
IntroductionElevated transaminases and/or creatine phosphokinase can indicate underlying muscle disease. Therefore, this study aims to determine the frequency of Duchenne muscular dystrophy/Becker muscular dystrophy (DMD/BMD) in male children and Pompe disease (PD) in male and female children with isolated hypertransaminasemia.MethodsThis multi-center, prospective study enrolled patients aged 3–216 months with serum alanine transaminase (ALT) and/or aspartate transaminase (AST) levels >2× the upper limit of normal (ULN) for ≥3 months. Patients with a known history of liver or muscle disease or physical examination findings suggestive of liver disease were excluded. Patients were screened for creatinine phosphokinase (CPK) levels, and molecular genetic tests for DMD/BMD in male patients and enzyme analysis for PD in male and female patients with elevated CPK levels were performed. Genetic analyses confirmed PD. Demographic, clinical, and laboratory characteristics of the patients were analyzed.ResultsOverall, 589 patients [66.8% male, mean age of 63.4 months (standard deviation: 60.5)] were included. In total, 251 patients (188 male and 63 female) had CPK levels above the ULN. Of the patients assessed, 47% (85/182) of male patients were diagnosed with DMD/BMD and 1% (3/228) of male and female patients were diagnosed with PD. The median ALT, AST, and CPK levels were statistically significantly higher, and the questioned neurological symptoms and previously unnoticed examination findings were more common in DMD/BMD patients than those without DMD/BMD or PD (p < 0.001).DiscussionQuestioning neurological symptoms, conducting a complete physical examination, and testing for CPK levels in patients with isolated hypertransaminasemia will prevent costly and time-consuming investigations for liver diseases and will lead to the diagnosis of occult neuromuscular diseases.

A Hygieia-Hypnos Statue from Anazarbos
Anazarbos (Anavarza) was a city in ›Plain Cilicia‹, located in the modern-day village of Dilekkaya in the District of Kozan at the Province of Adana. Together with the two ›Large Herculaneum Women‹ type statues (A1 and A2), this Hygieia-Hypnos statue recovered during the archaeological excavations at Anazarbos stands out as one of the first large scale statues from this city. More importantly, the Hygieia-Hypnos statue was recovered during the first systematic and scientific archaeological excavations that took place in ›Plain Cilicia‹, which defines them as statues with a definite find spot. The Hygieia-Hypnos statue is covered in this article iconographically as well as stylistically. Based on stylistic characteristics, the statue is dated to the late second century A.D. Locally known also as Salus, Hygieia gained popularity in Rome around this time. The goddess Hygieia became the personification of medicine and gradually acquired a political meaning as she became frequently depicted to represent first the health of the Roman Empire (Salus Publica) and then of the emperor (Salus Augusti). The Anazarbos statue of Hygieia accompanied by Hypnos was found during excavations taking place in front of the Anazarbos monumental gateway which is thought to have been built after Lucius Verus’ victorious campaigns against the Parthians in A.D. 162 and 166. It would not be surprising to find the statue of a Roman emperor who carries significance for Anazarbos like Lucius Verus in one of the arch niches in future excavation work. Therefore, it could be concluded that the Hygieia statue was erected for Salus Augusti – that is the health of the emperor, the empire and Anazarbos itself
Effect of platelet rich fibrin on edema and pain following third molar surgery: a split mouth control study
Abstract Background To evaluate the efficacy of platelet-rich fibrine (PRF) on postoperative edema and pain after impacted mandibular third molar surgery. Methods The prospective study was comprised 30 patients who presented for the removal of bilateral impacted mandibular third molar teeth. After extraction, the sockets were filled with PRF or without PRF in the study and control groups, respectively. Postoperative edema was measured with a flexible tape measure by calculating the distance between several facial landmarks on postoperative days two and seven. Postoperative pain was evaluated with a line-type visual analogue scale (VAS) and a verbal scale (VRS). SPSS version 20.0 was used for data analysis. Results Both groups recorded significant improvement compared to the baseline levels in almost all of the outcome variables. There was no statistically significant difference between the study and control groups (p > 0.05). Conclusions Using or not using PRF to reduce postoperative pain and edema in third molar surgery was equally successful. Understandıng Of Chıld Labour In Turkey: An Emprıcal Analysıs
Özet: Bu çalışma Türkiye’nin kentsel bölgesinde çocuk işgücünün belirleyicilerini araştırmaktadır. Çalışmada, Türkiye’de çocuk işgücüne katılım kararı için bir olasılık tahmin etmek amacıyla ikili cevap değişkenleri temelinde, konu ile ilgili çeşitli karakteristikler verilmiş, önceki çalışmaların bulguların özetlenmiş ve bu çerçevede Türkiye'de çocuk işçiliğinin belirleyicilerinin ortaya konulması hedeflenmiştir. Bu amaçla, Türkiye İstatistik Kurumu tarafından toplanan 2006 yılına ait veri seti kullanılmıştır ve Probit Regresyon adı verilen bir regresyon yöntemi uygulanmıştır. Bulgularımıza göre çocuk işgücüne katılım kararı bireylerin karakteristiklerinin yanı sıra hane halkı üyelerinin mali kısıtlamaları tarafından belirlenmektedir. Özellikle, çocuk işgücü katılım kararı babanın geliri ile ilişkilidir. Bu nedenle bulgularımız, Türkiye’de çocuk işgücüne katılmama kararının bir lüks mal niteliğinde olduğunu göstermektedir.This paper investigates determinants of child labour in urban Turkey, on the basis of binary response variables in order to estimate a probability for child labour participation decision, given some sort of characteristics and reviews previous finds in this field with a newer data set. For this purpose, we use a newer data, collected by official statistic institution of Turkey and implement a regression procedure, namely, Probit Regression. According to our finds, there does exist significant relations between child labour participation decision and individuals’ characteristics, as well as financial constraints of household members in Turkey. In particular, child labour participation decision is significantly associated with father’s income. Therefore, we find implies about luxury good property of counter decision of child labour participation in Turkey
İstanbul, Şehzade ve Edirne, Selimiye Külliyelerinde İşlevsel Sistemler ve Kalite Analizi
<p>Tarihi yapılarda işlevsel sistemler olarak adlandırılan ısıtma, soğutma, havalandırma, aydınlatma, akustik, temiz ve atık su sistemleri ile çatı akaçlama sistemleri, yapıların uzun süre ayakta kalmasında en önemli etkenler arasında olmuştur. Yapıların su ihtiyacını karşılamak, atık ya da yağmur/kar sularının tahliye edilmesini sağlamak, mekânların ve kullanıcılarının ısıl konfor, doğal havalandırma, serinleme ve aydınlatma koşullarına yardımcı olmak üzere tasarlanan bu sistemler, yapıyla birlikte çoğunlukla bir vakıf eseri olarak inşa edilmişlerdir. Bu araştırma kapsamında Mimar Sinan’ın çıraklık eseri olarak atfedilen İstanbul, Şehzade Külliyesi ile ustalık eseri olarak anılan Edirne, Selimiye Külliyesi sahip oldukları işlevsel sistemleri açısından incelenmiş, bu sistemler, türleri, mimari özellikleri, konumları bakımından ilk kez tanıtılarak işlevsel kalite analizi bağlamında değerlendirilmiştir. Böylece Mimar Sinan’ın mesleğinin ilk dönemi ve son döneminde inşa ettiği iki külliye yapısında uyguladığı işlevsel sistemlerde karşılaştırmalı bir analiz yapılmıştır. Mimar Sinan’ın inşa ettiği külliye yapıları plan, mimari ve süsleme özellikleri bakımından oldukça fazla çalışılan bir konu olmasına rağmen, bu yapıların pasif yaşam koşullarının bir parçası olan işlevsel sistemleri açısından çok kısıtlı düzeyde incelenmiş olması, konunun özgünlük değerini artırmaktadır.</p>
