    Equine Rabies in the Southern Region of Piauí State

    Background: Rabies is an infectious disease that is important in the "One Health" worldwide with high lethality rate. The etiological agent is a neurotropic virus, genus Lyssavirus, transmitted mainly through the saliva of infected animals. For equines, the bite of hematophagous bats is the main source of infection. Piauí is an important state for equestrian sports and the increase in the number of horses with neurological clinical signs without diagnosis has increased in recent years. In this context, the aim of this study is to report to the scientific community a confirmed case of equine rabies in the Santa Luz county, Southernmost state of Piauí, Brazil.Case: A 3-year-old female non-defined breed horse, was admitted to the Hospital Veterinário da Universidade Federal do Piauí (UFPI/CPCE). The equine had difficulty walking 2 days ago, in the panoramic inspection was restless and disoriented in the paddock. Rectal temperature of 38.2oC, heart rate of 60 bpm, respiratory rate of 40 mpm, congested mucosa and dyspnea were verified. With the progression of the neurological signals, it positioned itself in a lateral decubitus with pedaling movements, hyperesthesia, dysphagia and paralysis of the hindlimbs. The clinical suspicion was rabies and the Agência de Defesa Agropecuária do Piauí (ADAPI) was communicated to euthanize the animal and collect samples for diagnosis in accordance with official standards of the Ministério da Agricultura, Pecuária e Abastecimento (MAPA). At necropsy, there was slight brain hyperemia, with no other significant organ changes. Fragments of the cerebellum, cortex, hippocampus and spinal cord were collected and sent at a temperature of 4oC to perform the Direct Immunofluorescence (DIF) assay. Samples for histopathology were not collected because they do not include assay for confirmatory diagnosis of rabies. The DIF technique with antigen-labeled antibodies were performed in the imprint lamina of these fragments. The fragments were treated according to specific protocol. The results were negative for DIF in the collected equine fragments. For complementary exam, the samples were homogenized, clarified and inoculated intracranial in BALB/C mice, being observed for up to 30 days. The samples were positive after the bioassay.Discussion: Piauí is a state with great equestrian activity that expose the animals to the risks of transmission of infectious diseases. Among these diseases, rabies is important for affecting horses, but also humans (veterinarians and owners). In the present report, the equine showed clinical signs of furious rabies for a short period, rapidly evolving to paralytic form. This clinical aspect must be carefully evaluated by the veterinarian, in order to avoid false clinical suspicions such as tetanus or other non-infectious diseases. The official diagnosis of the rabies is DIF technique, with high sensitive (80-100%). According to the results of the DIF technique, it was possible to confirm the clinical suspicion of rabies in mice previously inoculated with emulsion of fragments of the equine central nervous system (CNS). This fact demonstrates that in negative results for CNS samples from the horse, the bioassay increases the sensitivity of the test and avoids false negative diagnoses. Thus, it was possible to prove that rabies is affecting the equines in the southern region of Piauí state and alerts the breeders and the community to intensify surveillance and control of the hematophagous bats. For the authors, this is the first scientific report of rabies in the region studied

    Use of threaded rigid cannula and flexible endoscope for single access video laparoscopy in standing horses

    Com o objetivo de promover, por meio de acesso único e com o uso de endoscópio flexível, ampla exploração da cavidade peritoneal de equinos em estação, foi concebida uma cânula laparoscópica para dar sustentação ao endoscópio e possibilitar o acesso sob visualização. O procedimento foi realizado a partir da fossa paralombar. Após pequena incisão cutânea, o endoscópio foi inserido na cânula e os músculos e o peritônio foram divulsionados mediante rotação da cânula. Logo depois da perfuração do peritônio, foi realizada a exploração da cavidade e a identificação das estruturas. Em seguida à exploração do lado ipsilateral ao acesso, realizou-se a transposição do conjunto cânula/endoscópio ventralmente à porção caudal do cólon descendente, seguida de exploração do lado contralateral. Concluída a técnica, foi executado, para fins de comparação, o mesmo procedimento por meio da fossa paralombar contralateral. Foi possível a transposição do conjunto cânula/endoscópio para o lado contralateral ao acesso em todos os procedimentos. Também foi possível a identificação da maioria das estruturas abdominais tanto pelo acesso esquerdo quanto pelo direito. A abordagem por acesso único mostrou-se viável para a exploração ampla da cavidade peritoneal, demonstrando ser uma alternativa à técnica laparoscópica convencional.A laparoscopic cannula was designed to support a single access approach with a flexible endoscope for the wide exploration of the peritoneal cavity of standing horses. It provides support to the endoscope and allows access to the peritoneal cavity with a visual aid. This procedure was performed through the paralumbar fossa. After a small cutaneous incision, the endoscope was inserted into the cannula, and the muscles and peritoneum were divulsed through the rotation of the cannula. After the peritoneal perforation, cavity exploration and identification of structures were performed. After the exploration of the ipsilateral side of the access, the cannula/endoscope was transposed ventrally to the caudal portion of the descending colon; this was followed by the exploration of the contralateral side. Once this process was completed, the same procedure was performed through the contralateral paralumbar fossa for comparison. It was possible to transpose the cannula/endoscope set to the contralateral access side in all procedures. Further, it was possible to identify most of the abdominal structures in both the left and right access. This single access approach proved to be feasible for the extensive exploration of the peritoneal cavity, thereby indicating it can be an alternative to the conventional laparoscopic technique

    Evidence about playing in the hospital from the perspective of the child's family: integrative review / Evidências acerca do brincar no hospital na perspectiva do familiar da criança: revisão integrativa

    Objective: to identify studies in the national and international literature about playing in the hospital from the perspective of the child's family. Method: Integrative review carried out from March to May 2022, in the information sources: Scielo; LILACS and BDENF via VHL; MEDLINE via PUBMED; Scopus and Google Scholar as an additional strategy. Original articles in English, Portuguese and Spanish were included; no temporal clipping; and that had family members and/or companions of hospitalized children as participants. Results: Families recognize playing as a promoter in reducing children's anxiety in the face of invasive procedures, as well as helping to cope with hospitalization and improving their mood and behavior. They also highlighted the hospital toy library as an important space that promotes distraction and continuity of development. Conclusion: from the perspective of family members about playing, the nursing team should encourage them to be a co-participant in this activity during the child's hospitalization.Objetivo: identificar estudios en la literatura nacional e internacional sobre el juego en el hospital desde la perspectiva de la familia del niño. Método: Revisión integradora realizada de marzo a mayo de 2022, en las fuentes de información: Scielo; LILACS y BDENF vía BVS; MEDLINE vía PUBMED; Scopus y Google Scholar como estrategia adicional. Se incluyeron artículos originales en inglés, portugués y español; sin recorte temporal; y que tuvo como participantes a familiares y/o acompañantes de niños hospitalizados. Resultados: Las familias reconocen jugar como un promotor en la reducción de la ansiedad de los niños frente a los procedimientos invasivos, además de ayudar a sobrellevar la hospitalización y mejorar su estado de ánimo y comportamiento. También destacaron la ludoteca del hospital como un espacio importante que promueve la distracción y la continuidad del desarrollo.     Hemoglobin Uptake by Paracoccidioides spp. Is Receptor-Mediated

    Iron is essential for the proliferation of fungal pathogens during infection. The availability of iron is limited due to its association with host proteins. Fungal pathogens have evolved different mechanisms to acquire iron from host; however, little is known regarding how Paracoccidioides species incorporate and metabolize this ion. In this work, host iron sources that are used by Paracoccidioides spp. were investigated. Robust fungal growth in the presence of the iron-containing molecules hemin and hemoglobin was observed. Paracoccidioides spp. present hemolytic activity and have the ability to internalize a protoporphyrin ring. Using real-time PCR and nanoUPLC-MSE proteomic approaches, fungal growth in the presence of hemoglobin was shown to result in the positive regulation of transcripts that encode putative hemoglobin receptors, in addition to the induction of proteins that are required for amino acid metabolism and vacuolar protein degradation. In fact, one hemoglobin receptor ortholog, Rbt5, was identified as a surface GPI-anchored protein that recognized hemin, protoporphyrin and hemoglobin in vitro. Antisense RNA technology and Agrobacterium tumefaciensmediated transformation were used to generate mitotically stable Pbrbt5 mutants. The knockdown strain had a lower survival inside macrophages and in mouse spleen when compared with the parental strain, which suggested that Rbt5 could act as a virulence factor. In summary, our data indicate that Paracoccidioides spp. can use hemoglobin as an iron source most likely through receptor-mediated pathways that might be relevant for pathogenic mechanisms

    Echocardiographic study of the coronary sinus in the indeterminate form of Chagas disease

    INTRODUCTION: We investigated the occurrence of coronary sinus abnormalities in the indeterminate form of Chagas disease (CD). METHODS: Differences between the maximum and minimum diameters of the coronary sinus (∆%) on echocardiography were evaluated in individuals with the indeterminate form of CD (n=14) and those without (n=16) CD. The association of the difference with abnormalities detected by echocardiography and myocardial scintigraphy was assessed. RESULTS: The mean Δ% values did not differ significantly between the groups. There was no correlation of the measurements with echocardiographic and myocardial scintigraphy findings. CONCLUSIONS: The coronary sinus evaluation revealed no differences between the groups

    Retrospective study of clinical cases in ruminants at the UFRGS veterinary teaching hospital

    Background: The pillars of animal production are sanity, genetics and nutrition. Sanitary control of the herd is important to reduce production costs and maintain health. The Veterinary Clinics Hospital (HCV) of the Federal University of Rio Grande do Sul (UFRGS), located in Porto Alegre - RS, is the place of greatest casuistry of the state, with 20.000 annual attendances, between small and large animals. In view of this scenario, the present study aimed to determine the frequency and characterize the ruminants attended at HCV, in order to help in the future, in the construction of control and prevention strategies of diseases found. Materials, Methods & Results: The documents of ruminant care between January 2007 and May 2018 were searched in the archives of the Hospital of Veterinary Clinics of UFRGS. Data on species, race, sex, age and diagnosis were collected. Diagnoses were classified as conclusive and inconclusive and the cases with conclusive diagnosis were classified according to etiology: infectious and parasitic diseases, metabolic and nutritional diseases, reproductive and obstetric diseases, toxic diseases, traumatic diseases. The prevalence of diseases and characteristics of ruminants attended (species, sex, category) was calculated. During the study period, between January 2007 and May 2018, 341 ruminants were attended, with emphasis on sheep (42%), goats (39%) and cattle (18%). In addition, a camel with foreign body obstruction, a sambar deer with fracture of the first thoracic vertebra and a buffalo with ruminal impaction were attended. The care profile was mapped, with predominance of females (57%) and adults (59%). Most of the animals did not present a defined breed, but among the breeds stands out Texel, of cutting aptitude, in sheeps and Saanen and Holstein, of dairy aptitude, in goats and cattle, respectively. In the retrospective study, the diseases with infectious and parasitic etiology presented the highest prevalence with 27.5% of the attendances, followed by reproductive (17.5%), traumatic (13.5%), metabolic (10%), others (10%) and toxic (2.5%). Sixty-six animals had inconclusive diagnosis (19.5%). Discussion: The highest prevalences were infectious and parasitic diseases, with emphasis on myiasis and verminosis. So, it is perceived that management corrections are sufficient to reduce the number of occurrences of diseases and prophylactic measures such as vaccination protocol, vermifugation and adequate nutritional management are allied in this walk. Among the reproductive diseases, dystocia (42.85%), which is one of the main causes of mortality in the peripartum period, has been highlighted. Dog attack was the major cause of traumas in ruminants and urolithiasis was highlighted in metabolic diseases. In toxic diseases, copper intoxication was the most important. Sheep are extremely sensitive to this intoxication, as they tend to accumulate copper in the organism. The retrospective study made it possible to visualize the panorama of HCV UFRGS visits to ruminants in the last years, mapping the profile and determining the casuistry of the diseases. Studies of hospital veterinary casuistry are rare, mainly involving ruminants. At the end, it is concluded that studies referring to casuistry are important to know the predominant diseases in a specific area and its risk factors considering differential diagnosis and future prevention programs

    Avaliação da qualidade de vida dos pacientes DM1 no ambulatório do centro de especialidades médicas de uma instituição/ Assessment of quality of life of DM1 patients in the ambulatory of the medical specialties center of an institution

    Introdução: O diabetes mellitus tipo 1 (DM1) é doença autoimune, poligênica, decorrente de destruição das células ? pancreáticas, ocasionando deficiência completa na produção de insulina. É uma doença crônica que interfere em vários aspectos da vida diária da pessoa, podendo afetar a sua qualidade de vida (QV). A partir disso, foram criados diversos instrumentos para avaliar a QV da pessoa diabética. O Diabetes Quality of Life Measure (DQOL) é um instrumento conhecido mundialmente e validado no Brasil. Por meio dele, são avaliados 4 domínios: satisfação, impacto, preocupações social/vocacional e preocupações relacionadas à diabetes. Os escores são calculados pela média dos itens individuais e quanto mais próximos de 1, melhor a qualidade de vida. Objetivo: Avaliar a qualidade de vida do paciente portador de DM1 em acompanhamento no Ambulatório de DM1 do Centro de Especialidades Médicas do CESUPA – PA. Método: Estudo transversal, analítico e descritivo. Foi avaliada a qualidade de vida de 20 pacientes com mais 15 anos de idade, através da aplicação da versão resumida do Diabetes Quality of Life Measure (DQOL-Brasil-8). Resultados: Os pacientes tinham entre 17 e 65 anos, alcançando média de 29.8 anos. Dos 20 pacientes estudados, 55% eram do sexo feminino. Os domínios avaliados pelo questionário alcançaram média geral de 3,2, indicando uma qualidade de vida regular dos componentes da amostra. Os domínios que obtiveram maiores pontuações, indicando menor qualidade de vida foram as preocupações social/vocacional (3,1) e as relacionadas com a diabetes (4,0). As perguntas analisadas pelo DQOL-Brasil-8, alcançaram médias a partir de 2,7, na questão da satisfação com a vida sexual e interferência da diabetes no exercício físico. Os valores mais altos obtidos foram, a preocupação de sofrer complicações em decorrência do diabetes (4,1), seguido da preocupação se virá a desmaiar (3,9) e incômodo em ter diabetes (3.5). Os homens alcançaram médias superiores às mulheres em todos os domínios, assim como no score geral, mostrando uma menor qualidade de vida neste gênero (2,8 mulheres e 3,6 homens). Conclusão: A QV nos pacientes com DM1 pesquisados foi avaliada como regular. Os domínios considerados insatisfatórios foram: preocupações relacionadas à diabetes e preocupações social/vocacional. Tais resultados servem como base para uma melhor compreensão das condições que atuam na QV do paciente diabético. Mensurar a QV nesses pacientes e compreender quais são os fatores que interferem, é uma forma de ajudar os profissionais a traçar estratégias que os motivem para o autocuidado

    EFEITOS DA PREMATURIDADE NO DESENVOLVIMENTO INFANTIL

    Introduction: Prematurity, defined as birth before 37 completed weeks of gestation, is a condition that affects millions of babies around the world. Prematurity can result in a variety of medical complications and developmental delays that can have lasting, lifelong impacts and can occur for a variety of reasons, from complications during pregnancy to the mother's underlying medical conditions, genetic and environmental factors, becoming one of the main causes of neonatal morbidity and mortality in many countries, representing a significant challenge for health systems and affected families. Objectives: Objectively describe the impacts caused by prematurity on child development and the biggest challenges faced as a result of this factor. Methodology: Research carried out between April and May 2024, based on an integrative review of scientific literature in the SciELo and Virtual Health Library (VHL) databases, using the descriptors: “Prematurity” and “Child development”. Results and discussion: Prematurity has several impacts on child development, from delays in neurological evolution, physical growth to respiratory, immunological and nutritional health. Children born prematurely are at greater risk of developing cognitive and motor delays, attention deficits, hyperactivity and learning difficulties throughout their lives, facing challenges related to physical health and well-being. Conclusion: Premature babies face a series of challenges from the moment of birth, where they often require intensive care to ensure their survival and promote adequate growth and development. Furthermore, throughout childhood, these children often require specialized medical and therapeutic support to monitor and intervene in possible health complications, such as chronic respiratory, visual and hearing problems. Therefore, it is crucial to ensure a sensitive and welcoming care environment, in addition to offering emotional support to parents, to promote the best possible development for these children. Introdução: A prematuridade, definida como o nascimento antes das 37 semanas completas de gestação, é uma condição que afeta milhões de bebês em todo o mundo. A prematuridade pode resultar em uma variedade de complicações médicas e atrasos no desenvolvimento, que podem ter impactos duradouros ao longo da vida, podendo ocorrer por uma variedade de razões, desde complicações durante a gravidez até condições médicas subjacentes da mãe, fatores genéticos e ambientais, se tornando uma das principais causas de morbidade e mortalidade neonatais em muitos países, representando um desafio significativo para os sistemas de saúde e para as famílias afetadas. Objetivos: Descrever de forma objetiva quais os impactos causados pela prematuridade no desenvolvimento infantil e os maiores desafios enfrentados por consequência desse fator. Metodologia: Pesquisa realizada entre abril e maio do ano de 2024, com base em uma revisão integrativa da literatura científica nas bases de dados SciELo e Biblioteca Virtual em Saúde (BVS), utilizando os descritores: "Prematuridade", "Manejo da prematuridade" e "Desafios da prematuridade". Resultados e discussão: A prematuridade acarreta diversos impactos no desenvolvimento infantil, desde atrasos na evolução neurológica, no crescimento físico até a saúde respiratória, imunológica e nutricional. Crianças nascidas prematuramente apresentam maior risco de desenvolver atraso cognitivo e motor, déficits de atenção, hiperatividade e dificuldades de aprendizagem ao longo da vida, enfrentando desafios relacionados à saúde física e ao bem-estar. Conclusão: Os prematuros enfrentam uma série de desafios desde o momento do nascimento, onde muitas vezes necessitam de cuidados intensivos para garantir sua sobrevivência e promover um crescimento e desenvolvimento adequado. Além disso, ao longo da infância, essas crianças frequentemente necessitam de acompanhamento médico e terapêutico especializado para monitoração e intervenção em possíveis complicações de saúde, como problemas respiratórios crônicos, visuais e auditivos. Assim, é crucial garantir um ambiente de cuidados sensível e acolhedor, além de oferecer apoio emocional aos pais, para promover o melhor desenvolvimento possível para essas crianças.&nbsp
