    Fatores de risco socioambientais para desnutrição por defeito

    Se realizó un estudio observacional analítico, tipo caso-control, no pareado, para identificar los factores socio-ambientales de riesgo de malnutrición por defecto en niños menores de 5 años pertenecientes a los consultorios # 5 y 6, del Policlínico “13 de Marzo”, Bayamo, Granma, durante Marzo de 2018 a Marzo de 2019. Universo: 148 niños de esos consultorios, con edades comprendidas entre 3 meses y 4 años, 11 meses y 29 días. Por muestreo aleatorio se seleccionaron 31 casos (desnutridos) y 93 controles (eutróficos), en proporción 1:3. Previo consentimiento, y a partir de revisión documental y entrevistas a madres, informantes claves de la comunidad y equipos de salud, se estudiaron variables maternas y familiares. Se calculó el odds ratio, con sus intervalos de confianza; se determinó chi cuadrado y la fracción atribuible en expuestos. Se trabajó con una confiabilidad de 95%. Resultados: Los hábitos dietéticos inadecuados, la deficiente cultura sanitaria, el bajo per cápita, la disfunción familiar, la desvinculación laboral, el inadecuado desempeño en el cuidado de los hijos, la familia no nuclear y la baja escolaridad, estuvieron significativamente asociados a la desnutrición infantil. Los cinco primeros fueron los factores que más aportaron a la aparición de desnutrición energo-nutrimental en la población investigada. Conclusiones: Los factores socio-ambientales de riesgo de malnutrición por defecto en los niños estudiados coincidieron, mayoritariamente, con la hipótesis enunciada y la bibliografía revisada. An analytical observational, case-control type study, not paired, was carried out to identify the socio-environmental risk factors for malnutrition due to defect in children under 5 years old, belonging to offices # 5 and 6, of the "13 de Marzo" Polyclinic, Bayamo, Granma, from March 2018 to March 2019. Universe: 148 children from these clinics, aged between 3 months and 4 years, 11 months and 29 days. 31 cases (malnourished) and 93 controls (eutrophic) were selected by random sampling, in a 1: 3 ratio. With prior consent, and based on a documentary review and interviews with mothers, key informants from the community and health teams, maternal and family variables were studied. Odds ratio was calculated, with its confidence intervals; chi square and the attributable fraction in exposed were determined. A 95% reliability was used. Results: Inadequate dietary habits, poor health culture, low per capita, family dysfunction, job separation, inadequate performance in childcare, the non-nuclear family and low schooling were significantly associated with malnutrition. childish. The first five were the factors that contributed most to the appearance of energy-nutritional malnutrition in the investigated population.     Effect of immunocastration on behaviour and blood parameters (cortisol and testosterone) of Holstein bulls

    To evaluate the effect that immunocastration has on behaviour, testosterone and cortisol levels of feedlot Holstein bulls, 720 intact animals aged between 7 and 8 months, weighing 232±1.19 kg were randomly assigned to two treatments: immunocastration using the Bopriva vaccine and a placebo (360 animals per treatment). The bulls were slaughtered at day 239 of treatment. Animals were vaccinated on days 1, 21, 101, and 181, and on those same days testosterone levels were measured; while cortisol, glucose and creatin kinase measurements were done on day 181 and during exanguination at slaughter. Sexual, aggressive and social behaviours were evaluated and it was found that intact bulls showed a higher average of head butts, mounting, threats, flehmen sign and sniffing (P<0.05), no differences were found for vocalisations, lowering of the head and grooming (P>0.05). Testosterone levels in intact bulls remained at 0.47ng/mL throughout the study, however, by day 181 differences (P<0.05) were observed in immunised bulls, with values of 0.22ng/mL. At slaughter, testosterone levels were 0.21 ± 0.06 ng/mL in immunocastrated bulls and 0.54 ± 0.06 ng/mL in the placebo group. The use of immunocastration with Bopriva has shown to be effective to reduce testosterone, sexual and aggressive behaviours on Holstein bulls

    Comorbidities in patients with Primary Sjögren's Syndrome and Systemic Lupus Erythematosus: A comparative registries-based study

    OBJECTIVE: To compare the prevalence of the main comorbidities in 2 large cohorts of patients with primary Sjögren's syndrome (SS) and systemic lupus erythematosus (SLE), with a focus on cardiovascular (CV) diseases. METHODS: This was a cross-sectional multicenter study where the prevalence of more relevant comorbidities in 2 cohorts was compared. Patients under followup from SJOGRENSER (Spanish Rheumatology Society Registry of Primary SS) and RELESSER (Spanish Rheumatology Society Registry of SLE), and who fulfilled the 2002 American-European Consensus Group and 1997 American College of Rheumatology classification criteria, respectively, were included. A binomial logistic regression analysis was carried out to explore potential differences, making general adjustments for age, sex, and disease duration and specific adjustments for each variable, including CV risk factors and treatments, when appropriate. RESULTS: A total of 437 primary SS patients (95% female) and 2,926 SLE patients (89% female) were included. The mean age was 58.6 years (interquartile range [IQR] 50.0-69.9 years) for primary SS patients and 45.1 years (IQR 36.4-56.3 years) for SLE patients (P?<?0.001), and disease duration was 10.4 years (IQR 6.0-16.7 years) and 13.0 years (IQR 7.45-19.76 years), respectively (P?<?0.001). Smoking, dyslipidemia, and arterial hypertension were associated less frequently with primary SS (odds ratio [OR] 0.36 [95% confidence interval (95% CI) 0.28-0.48], 0.74 [95% CI 0.58-0.94], and 0.50 [95% CI 0.38-0.66], respectively) as were life-threatening CV events (i.e., stroke or myocardial infarction; OR 0.57 [95% CI 0.35-0.92]). Conversely, lymphoma was associated more frequently with primary SS (OR 4.41 [95% CI 1.35-14.43]). The prevalence of severe infection was lower in primary SS than in SLE (10.1% versus 16.9%; OR 0.54 [95% CI 0.39-0.76]; P?<?0.001). CONCLUSION: Primary SS patients have a consistently less serious CV comorbidity burden and a lower prevalence of severe infection than those with SLE. In contrast, their risk of lymphoma is greater

    Factors associated with primary multidrug-resistant tuberculosis in patients from Callao, Peru

    Introducción: En el Perú, Lima y Callao constituyen regiones con riesgo alto de transmisión activa de tuberculosis. La tuberculosis multidrogorresistente se presenta en el 5,54% de los casos nuevos de tuberculosis pulmonar (TB MDR primaria) en Lima y Callao. Sus factores de riesgo no han sido estudiados en pacientes del Callao. Objetivos: Determinar los factores de riesgo para TB MDR primaria en pacientes atendidos en centros de salud de Callao, durante los años 2009- 2010. Diseño: Estudio caso control. Lugar: Centros de Salud de Callao. Participantes: Participaron 29 pacientes con TB MDR primaria y 37 con tuberculosis sensible, según los registros del Programa de Control de Tuberculosis y las historias clínicas ubicadas en los centros de salud. Intervenciones: Toda la información fue obtenida de fuentes secundarias. En el análisis bivariado se calculó el OR de cada covariable. Para el cálculo de los OR multivariados se utilizó modelos de regresión logística. Principales medidas de resultados: Factores de riesgo para TB MDR. Resultados: En el análisis bivariado, el haber tenido contacto con pacientes TB MDR o XDR fue un factor de riesgo significativo (OR: 5,56; IC95%: 1,05 a 29,27); lo mismo se demostró en el análisis multivariado (OR: 14,56; IC95%: 1,52 a 139,54). Además, en el análisis multivariado, la variable edad ≥ 40 años mostró ser un factor protector significativo (OR: 0,94; IC95%: 0,90 a 0,99). Conclusiones: El tener contacto con pacientes TB MDR/XDR fue factor de riesgo y una edad menor a 40 años un factor protector para contraer TB MDR primaria en los pacientes del Callao estudiados.Introduction: Lima and Callao are high risk areas for tuberculosis active transmission in Peru. Multidrug-resistant tuberculosis represents 5.54% of the new cases of pulmonary tuberculosis (primary MDR-TB) in Lima and Callao. Involved risk factors have not been studied in patients from Callao. Objectives: To determine risk factors for primary MDR-TB in patients attending health care facilities in Callao during 2009-2010. Design: Case-control study. Setting: Health care facilities from Callao. Participants: Twenty nine primary MDR-TB patients and 37 drug-sensitive TB patients were studied through assessment of the Tuberculosis Control Program Registries and healthcare facilities’ clinical records. Interventions: Information was obtained from secondary sources. Odds-ratios were calculated by bivariate analysis. Logistic regression models were used to calculate multivariate odd-ratios. Main outcome measures: Risk factors for primary MDR-TB. Results: Prior contact with a MDR-TB or XDR-TB patient was a significant risk factor (OR: 5.56; 95%CI: 1.05-29.27) in bivariate analysis; the same was found in multivariate analysis (OR: 14.56; 95%CI: 1.52–139.54). Being 40 or more years old was a significant protective factor in multivariate analysis (OR: 0.94; 95% CI: 0.90–0.99). Conclusions: Previous contact with a MDR-TB or XDR-TB patient was a risk factor, and being 40 or more years old was a protective factor for primary MDR-TB in Callao patients studied

    A Molecular Characterization of the Allelic Expression of the BRCA1 Founder Δ9–12 Pathogenic Variant and Its Potential Clinical Relevance in Hereditary Cancer:International Journal of Molecular Sciences

    Hereditary breast and ovarian cancer (HBOC) syndrome is a genetic condition that increases the risk of breast cancer by 80% and that of ovarian cancer by 40%. The most common pathogenic variants (PVs) causing HBOC occur in the BRCA1 gene, with more than 3850 reported mutations in the gene sequence. The prevalence of specific PVs in BRCA1 has increased across populations due to the effect of founder mutations. Therefore, when a founder mutation is identified, it becomes key to improving cancer risk characterization and effective screening protocols. The only founder mutation described in the Mexican population is the deletion of exons 9 to 12 of BRCA1 (BRCA1Δ9–12), and its description focuses on the gene sequence, but no transcription profiles have been generated for individuals who carry this gene. In this study, we describe the transcription profiles of cancer patients and healthy individuals who were heterozygous for PV BRCA1Δ9–12 by analyzing the differential expression of both alleles compared with the homozygous BRCA1 control group using RT–qPCR, and we describe the isoforms produced by the BRCA1 wild-type and BRCA1Δ9–12 alleles using nanopore long-sequencing. Using the Kruskal–Wallis test, our results showed a similar transcript expression of the wild-type allele between the healthy heterozygous group and the homozygous BRCA1 control group. An association between the recurrence and increased expression of both alleles in HBOC patients was also observed. An analysis of the sequences indicated four wild-type isoforms with diagnostic potential for discerning individuals who carry the PV BRCA1Δ9–12 and identifying which of them has developed cancer

    Evolving trends in the management of acute appendicitis during COVID-19 waves. The ACIE appy II study

    Background: In 2020, ACIE Appy study showed that COVID-19 pandemic heavily affected the management of patients with acute appendicitis (AA) worldwide, with an increased rate of non-operative management (NOM) strategies and a trend toward open surgery due to concern of virus transmission by laparoscopy and controversial recommendations on this issue. The aim of this study was to survey again the same group of surgeons to assess if any difference in management attitudes of AA had occurred in the later stages of the outbreak. Methods: From August 15 to September 30, 2021, an online questionnaire was sent to all 709 participants of the ACIE Appy study. The questionnaire included questions on personal protective equipment (PPE), local policies and screening for SARS-CoV-2 infection, NOM, surgical approach and disease presentations in 2021. The results were compared with the results from the previous study. Results: A total of 476 answers were collected (response rate 67.1%). Screening policies were significatively improved with most patients screened regardless of symptoms (89.5% vs. 37.4%) with PCR and antigenic test as the preferred test (74.1% vs. 26.3%). More patients tested positive before surgery and commercial systems were the preferred ones to filter smoke plumes during laparoscopy. Laparoscopic appendicectomy was the first option in the treatment of AA, with a declined use of NOM. Conclusion: Management of AA has improved in the last waves of pandemic. Increased evidence regarding SARS-COV-2 infection along with a timely healthcare systems response has been translated into tailored attitudes and a better care for patients with AA worldwide

El Congreso ofreció un espacio para la presentación, difusión e intercambio de importantes investigaciones nacionales e internacionales ante la comunidad universitaria que se dio cita en el encuentro. El uso de herramientas tecnológicas para la gestión de los trabajos de investigación como la plataforma Open Conference Systems y la web de presentación del Congreso http://citis.blog.ups.edu.ec/, hicieron de CITIS 2017 un verdadero referente entre los congresos que se desarrollaron en el país. La preocupación de nuestra Universidad, de presentar espacios que ayuden a generar nuevos y mejores cambios en la dimensión humana y social de nuestro entorno, hace que se persiga en cada edición del evento la presentación de trabajos con calidad creciente en cuanto a su producción científica.
