    Improving dementia care: a multi-sensory stimulation approach

    This pilot-study examined the effects of a Multisensory Stimulation (MSS) programme on the interaction of residents with dementia. Six residents with moderate/severe dementia from an old people’s home were recruited. Six staff members received 8 training sessions and assistance to implement MSS in dementia care. Observational data on residents’ behaviour during morning care was collected pre- and post-intervention. Results suggest improvements of caregiver-direct gaze, laugh, verbal communication and task involvement, and a reduction of closed-eyes. The introduction of MMS during care routines may improve residents’ involvement and facilitate communication, potentially improving their well-being.Este estudo-piloto investigou os efeitos de um programa de Estimulação Multi-Sensorial (EMS) na interacção das pessoas idosas com demência institucionalizadas. Participaram no estudo 6 idosos com demência moderada/severa. Seis cuidadores formais participaram em 8 sessões sobre a aplicação da EMS nas rotinas de cuidados. Os dados foram recolhidos através de gravação em vídeo dos cuidados matinais, antes e após o programa. Verificou-se um aumento do olhar dirigido ao cuidador, riso, comunicação verbal e envolvimento na tarefa, e redução dos olhos fechados.     Effect of the local administration of betamethasone on pain, swelling and trismus after impacted lower third molar extraction. A randomized, triple blinded, controlled trial

    Objectives: The aim of this study is to compare the analgesic and anti-inflammatory effects of the local postoperative administration of a single 12-mg dose of betamethasone after the surgical removal of impacted lower third molars. Study design: A split-mouth, triple-blind, randomized, placebo-controlled clinical trial of 25 patients requiring the surgical removal of symmetrical lower third molars was performed. In the experimental side, a 12-mg dose of betamethasone was administered submucosally after the surgical procedure, while in the control side a placebo (sterile saline solution) was injected in the same area. To assess postoperative pain, visual analogue scales and the consumption of rescue analgesic were used. The facial swelling and trismus were evaluated by measuring facial reference distances and maximum mouth opening. Results: There were no significant differences between the two study groups regarding postoperative pain, facial swelling and trismus. Conclusions: The injection of a single dose of betamethasone does not seem to reduce pain, facial swelling and trismus after impacted lower third molar removal when compared to placebo

    Inteligência Artificial em Radiologia: Do Processamento de Imagem ao Diagnóstico

    The objective of this article is to present a view on the potential impact of Artificial Intelligence (AI) on processing medical images, in particular in relation to diagnostic. This topic is currently attracting major attention in both the medical and engineering communities, as demonstrated by the number of recent tutorials [1-3] and review articles [4-6] that address it, with large research hospitals, as well as engineering research centers contributing to the area. Furthermore, several large companies like General Electric (GE), IBM/Merge, Siemens, Philips or Agfa, as well as more specialized companies and startups are integrating AI into their medical imaging products. The evolution of GE in this respect is interesting. GE SmartSignal software was developed for industrial applications to identify impending equipment failures well before they happen. As written in the GE prospectus, with this added lead time, one can transform from reactive maintenance to a more proactive maintenance process, allowing the workforce to focus on fixing problems rather than looking for them. With this background experience from the industrial field, GE developed predictive analytics products for clinical imaging, that embodied the Predictive component of P4 medicine (predictive, personalized, preventive, participatory). Another interesting example is the Illumeo software from Philips that embeds adaptive intelligence, i. e. the capacity to improve its automatic reasoning process from its past experience, to automatically pop out related prior exams for radiology in face of a concrete situation. Actually, with its capacity to tackle massive amounts of data of different sorts (imaging data, patient exam reports, pathology reports, patient monitoring signals, data from implantable electrophysiology devices, and data from many other sources) AI is certainly able to yield a decisive contribution to all the components of P4 medicine. For instance, in the presence of a rare disease, AI methods have the capacity to review huge amounts of prior information when confronted to the patient clinical data

    The perception of injury risk and prevention among football players: A systematic review

    Football is associated with a certain risk of injury, leading to short- and long-term health consequences. However, the perception of football players about injury risk and prevention strategies is poorly documented. The present article reviewed the literature about perceptions, beliefs, attitudes and knowledge toward injury risk and prevention strategies in football players. An electronic search was performed in PubMed, Scopus, Web of Science, and APA PsychINFO until July 2022. Studies were eligible if they included the perceptions, beliefs, attitudes, and knowledge about injury risk and prevention in football players from any competitive level. The risk of bias was assessed in included studies using the Joanna Briggs Institute critical appraisal checklist. A total of 14 studies were included. Most football players agreed that their risk of injury is high and prevention strategies are important, however they do not intend to use some of these strategies. The most frequent perceived injury risk factors were low muscle strength, lack of physical fitness, fatigue, excessive training and type and condition of surfaces. The most frequent perceived injury prevention factors were warm-up, workload monitoring and strength and conditioning training. It is essential to acknowledge perceived injury risk factors, as well as a better understanding of how coaching and medical departments' perceptions match with players' perceptions, and a modification in the perceptions of the several stakeholders at different levels of action. 2022 Cardoso-Marinho, Barbosa, Bolling, Marques, Figueiredo and Brito

    Validação da versão portuguesa do Asthma Quality of Life Questionnaire de Marks (AQLQ-M)

    AbstractQuality of Life is one of the leading evaluations in the diagnosis, therapy and investigation of patients with asthma. The aim of this study was to evaluate the psychometric properties of the Portuguese version of the Asthma Quality of Life Questionnaire (AQLQ-M) designed by Marks in Australia. This questionnaire consists of 20 items describing the impact of asthma on patient’s lifestyle and covers 4 dimensions (breathlessness, mood, social limitation and worrying). After a process of translation, backtranslation, and a pretest, the Portuguese version of the AQLQ-M was administered to 82 adult asthmatics living in 3 different urban areas. The following data were also recorded: age, sex, main symptoms, duration of disease, hospital visits, severity of disease, and medication. All patients performed spirometry. The reliability study (internal consistency with Cronbach s alpha coefficient and inter-item correlation analysis) gave satisfactory results (range of r=0.58 to 0.94; a=0.91 to 0.97). Results for construct validity shown that the breathlessness dimension presented the largest number of significant correlations with the most important clinical variables of asthma. In conclusion, this study has shown that the Portuguese version of the AQLQ-M has good psychometric properties, and is suitable for use on individual patients with asthma in our country.Rev Port Pneumol 2005; XI (4): 351-36

    Poster apresentado ao VIII Congresso Nacional de Geologia, em Braga (9-16 de Julho 2010)A exsurgência cársica do sistema Jurássico Olhos da Fervença localiza-se no concelho de Cantanhede, distrito de Coimbra e ocorre no leito da Ribeira da Corujeira, sob uma forma borbulhante, como se fervesse. A formação jurássica está subjacente a uma cobertura de areias de duna, que impede a sua observação directa, recorrendo-se neste estudo à utilização de prospecção electromagnética no domínio do tempo, com o equipamento TEM-FAST 48. Realizaram-se 24 sondagens electromagnéticas cuja interpretação permitiu identificar estruturas de fracturação associadas à ocorrência da exsurgência.The karstic exsurgence of the Jurassic system Olhos da Fervença is located in Cantanhede region, Coimbra district (centre of Portugal) and occurs sparkling in the Ribeira da Corujeira stream. The Jurassic formation is under dune sands and is not possible to observe it directly. This study with electromagnetic prospection in the time domain uses TEM-FAST 48 equipment.     Afadin downregulation by helicobacter pylori Induces epithelial to mesenchymal transition in gastric cells

    Afadin is a cytoplasmic protein of the adherens junctions, which regulates the formation and stabilization of both the adherens and the tight junctions. Aberrant expression of Afadin has been shown in cancer and its loss has been associated with epithelial-tomesenchymal transition (EMT). EMT is characterized by the change from an epithelial to a mesenchymal phenotype, with modifications on the expression of adhesion molecules and acquisition of a migratory and invasive cell behavior. While it is known that Helicobacter pylori disrupts the tight and the adherens junctions and induces EMT, the effect of the bacteria on Afadin is still unknown. The aim of this study was to disclose the effect of H. pylori on Afadin and its impact in the induction of an EMT phenotype in gastric cells. Using two different cell lines, we observed that H. pylori infection decreased Afadin protein levels, independently of CagA, T4SS, and VacA virulence factors. H. pylori infection of cell lines recapitulated several EMT features, displacing and downregulating multiple proteins from cell–cell junctions, and increasing the expression of ZEB1, Vimentin, Slug, N-cadherin, and Snail. Silencing of Afadin by RNAi promoted delocalization of junctional proteins from the cell–cell contacts, increased paracellular permeability, and decreased transepithelial electrical resistance, all compatible with impaired junctional integrity. Afadin silencing also led to increased expression of the EMT marker Snail, and to the formation of actin stress fibers, together with increased cell motility and invasion. Finally, and in line with our in vitro data, the gastric mucosa of individuals infected with H. pylori showed decrease/loss of Afadin membrane staining at cell–cell contacts significantly more frequently than uninfected individuals.     Gastric microbial community profiling reveals a dysbiotic cancer-associated microbiota

    Objective Gastric carcinoma development is triggered by Helicobacter pylori. Chronic H. pylori infection leads to reduced acid secretion, which may allow the growth of a different gastric bacterial community. This change in the microbiome may increase aggression to the gastric mucosa and contribute to malignancy. Our aim was to evaluate the composition of the gastric microbiota in chronic gastritis and in gastric carcinoma. Design The gastric microbiota was retrospectively investigated in 54 patients with gastric carcinoma and 81 patients with chronic gastritis by 16S rRNA gene profiling, using next-generation sequencing. Differences in microbial composition of the two patient groups were assessed using linear discriminant analysis effect size. Associations between the most relevant taxa and clinical diagnosis were validated by real-time quantitative PCR. Predictive functional profiling of microbial communities was obtained with PICRUSt. Results The gastric carcinoma microbiota was characterised by reduced microbial diversity, by decreased abundance of Helicobacter and by the enrichment of other bacterial genera, mostly represented by intestinal commensals. The combination of these taxa into a microbial dysbiosis index revealed that dysbiosis has excellent capacity to discriminate between gastritis and gastric carcinoma. Analysis of the functional features of the microbiota was compatible with the presence of a nitrosating microbial community in carcinoma. The major observations were confirmed in validation cohorts from different geographic origins. Conclusions Detailed analysis of the gastric microbiota revealed for the first time that patients with gastric carcinoma exhibit a dysbiotic microbial community with genotoxic potential, which is distinct from that of patients with chronic gastritis.This research was supported by a Worldwide Cancer Research grant to CF and JCM (Reference 16-1352).     Depression Among Portuguese Pregnant Women During Covid-19 Lockdown: A Cross Sectional Study

    Introduction: Coronavirus disease 2019 was declared as a pandemic on March 2020. Research on its psychological effects is still lacking. Perinatal depression is a medical complication of pregnancy, especially in situations of stress. In this study, we aimed to investigate the presence of symptoms of depression in pregnant women during the lockdown period in Portugal. Methods: This study consisted in a cross-sectional study among Portuguese pregnant women, who completed an online self-report questionnaire between 25th April and 30th April 2020. An anonymous online questionnaire was developed to assess depression and concerns related to COVID-19. This study was approved by the IRB of Hospital Dona Estefânia and performed in accordance with the ethical standards laid down in the 1964 Declaration of Helsinki. Eligibility criteria included pregnant women, ≥ 18 years and living in Portugal. The primary outcome was to evaluate the presence of depressive symptoms and its association to socio-demographic characteristics and to concerns related to COVID-19. Results: A total of 1698 pregnant women were enrolled. The mean age was 31.9 years. 82.4% felt a negative impact of the pandemic in the surveillance of pregnancy and 43% felt insufficient support. 26.3% showed "possible depression" according to the EPDS. A regression analysis revealed the possibility of depression increased as the concerns about COVID increased and was lower for women with support. The possibility of depression was higher for women with psychiatric medical history. Conclusion: This study demonstrated a significant increase in clinically significant depressive symptoms in pregnant women during the lockdown. It also revealed some of the socio-demographic characteristics of women at risk for depression. If left untreated, depression tends to persist, affecting the woman and also the child. Our findings suggest that COVID-19 represents a serious challenge for this population and reinforce the urgent need for early detection and intervention on mental health issues during pregnancy, especially during the pandemic.info:eu-repo/semantics/publishedVersio
