    Rejeição e efeitos da ciclosporina nos transplantes cutâneos alógenos microcirúrgicos em ratos

    Os efeitos da rejeição e da imunossupressão com ciclosporina A em retalhos epigástricos utilizados como transplantes cutâneos alógenos microcirúrgicos, foram estudados em 58 ratos, distribuídos em três grupos: Wistar-Furth isogênicos doadores e receptores; e Brown-Norway doadores e Wistar-Furth receptores, imunossuprimidos com cicloporina A(10mg/kg/dia). Biópsias dos retalhos e da pele normal contralateral utilizadas como contrôle foram colhidas no terceiro, sétimo, 15º e 30º dias pós transplante. As biópsias foram preparadas em historesina, e coradas em azul de toluidina, para avaliação do infiltrado inflamatório local, permitindo estudo quantitativo dos linfócitos infiltrantes nos retalhos. O modelo revelou-se eficiente, obtendo-se sobrevivência dos transplantes por 30 dias, de 83,3% entre animais isogênicos, e de 60% nos alogênicos sob ação da ciclosporina A. Em ratos alogênicos não imunossuprimidos houve 100% de rejeição dos retalhos, até o nono dia pós-operatório. Nos transplantes desses animais, verificou-se aumento significativo do número de linfócitos infiltrantes no terceiro dia, quando comparados aos ratos isogênicos. A linfocitose observada antecedeu os achados macroscópicos da rejeição, verificados apenas no quinto ou sexto dias.The rejection of allotransplantation of epigastric microsurgical flaps and the effect of immunosuppression have been studied in 58 rats. Three sets of experiments were planned: (1) Wistar Furth isogenic donors and receptors (control set); (2) Brown Norway donors and Wistar Furth receptors (rejection set); and (3) Brown Norway donors and Wistar Furth immunosuppressed receptors (cyclosporin A set). Cyclosporin A (10 mg/kg/d) treated rats had a transplantation survival rate of up to 30 days: 83.3% among isogenic animals and 60% among allogeneic. There was 100% rejection by the 9th day after the transplantation in allogeneic non-immunosuppressed rats. Biopsies embedded with historesin were taken from the flap and normal contralateral skin (used as control) on the 3rd, 7th, 15th, and 30th days after the surgery. A quantitative study of infiltrating lymphocytes in the flaps, with and without cyclosporin A, was done by evaluating the local inflammatory infiltrate. A significant increase in the number of lymphocytes among the rejection and immunosuppressed groups was seen, as compared to the isogenic set. Local lymphocytosis in allogeneic non-immunosuppressed transplantations reached its highest level on the 3rd day after surgery, before gross findings of rejection, which could only be seen by naked eye on the 5th or 6th day. Therefore, we conclude that cyclosporin A is effective in preserving allogenic transplantation in rats. Biopsies of transplanted areas may contribute to earlier diagnosis of the need for immunosuppressive therapy

    Sudden spleen rupture in a Plasmodium vivax-infected patient undergoing malaria treatment

    BACKGROUND: Splenomegaly is one of the most common features of malaria. However, spontaneous splenic rupture, although unusual, represents a severe complication often leading to death. It is mostly seen in acute infection and primary attack, and it is most commonly associated with Plasmodium vivax. Here, a case of spontaneous splenic rupture diagnosed with a portable ultrasound apparatus shortly after starting treatment and with recurrent parasitaemia after splenectomy, is reported. CASE DESCRIPTION: In November 2015, a 45-year-old Brazilian man presented to the hospital in Manaus with fever, headache and myalgia. He was diagnosed with P. vivax malaria and, after a normal G6PD test, he started treatment with chloroquine and primaquine and was discharged. Two days later, he went back to the hospital with abdominal pain, dyspnea, dry cough, pallor, oliguria and fever. Using a portable ultrasound, he was diagnosed of rupture of the spleen, which was removed by emergency surgery. After this episode, he suffered two more malaria episodes with high parasitaemia at approximately 2-month intervals. DNA from different portions of the spleen was extracted and a qualitative PCR was performed to detect P. vivax. CONCLUSIONS: The splenic rupture suffered by this patient occurred 2 days after starting the treatment. Having a portable ultrasound apparatus may have saved the patient's life, as it revealed a haemorrhage needing an urgent surgery. Parasites were detected by PCR in the extracted spleen. This patient suffered two more vivax malaria diagnosed episodes in spite of receiving and completing treatment with chloroquine and primaquine for each clinical attack. Splenic rupture during acute malaria is uncommon, but it is likely underdiagnosed and underreported, because the lack of means and equipment hinders diagnostic confirmation, especially in endemic areas

    Efetividade da estimulação diafragmática elétrica transcutânea na força muscular respiratória, volumes e capacidades pulmonares: revisão sistemática

    Introduction: Transcutaneous electrical diaphragmatic stimulation (TEDS) is to stimulate the diaphragm, through rhythmic electrical stimulation short promoting a negative pleural pressure that can influence both the ventilation and in venous return and cardiac output, representing another important tool for respiratory therapy. Objective: To analyze the outcomes of the application of TEDS on respiratory muscle strength and lung volumes and capacities. Methods: Through a systematic review of the literature, clinical trials published between 2003 and 2013 were analyzed The search involved the LILACS, SciELO, MEDLINE and PEDro database, using the keywords diaphragm, respiratory muscles and physical therapy crossed with the descriptor electrical stimulation. Results: Six studies totaling 105 subjects, aged between 20 and 75 years and showed that the use of TEDS caused a significant increase (p <0,05) in maximal inspiratory pressure (MIP) and maximal expiratory pressure were included maximum (MEP), tidal volume (VT) and the volumes of inspiratory and expiratory reserve. Conclusion: The TEDS promotes an increase in respiratory muscle strength and in some lung volumes in healthy subjects and / or patients with chronic diseases or post-operative recovery.Introdução: A estimulação diafragmática elétrica transcutânea (EDET) consiste em estimular o diafragma, por meio de estímulos elétricos rítmicos de curta duração promovendo uma pressão intrapleural negativa que pode influenciar tanto na ventilação pulmonar quanto no retorno venoso e no débito cardí- aco, representando mais uma importante ferramenta para a fisioterapia respiratória. Objetivo: Analisar os desfechos da aplicação da EDET na força muscular respiratória e nos volumes e capacidades pulmonares. Métodos: Por meio de uma revisão sistemática da literatura, foram analisados ensaios clínicos publicados entre 2003 e 2013. A busca envolveu as bases de dados LILACS, SciELO, MedLine e PEDro, usando os descritores “diaphragm”, “respiratory muscles” e “physical therapy” em cruzamento com o descritor “electrical stimulation”. Resultados: Foram incluídos 6 estudos, que somaram 105 indivíduos, com idade variando entre 20 e 75 anos, e que demonstraram que a utilização da EDET causou incremento significativo (p <0,05) da pressão inspiratória máxima (PImáx) e da pressão expiratória máxima (PEmáx), do volume corrente (VC) e dos volumes de reserva inspiratório e expiratório. Conclusão: A EDET promove aumento na força muscular respiratória e em alguns volumes pulmonares, em indivíduos saudáveis e/ ou pacientes com doenças crônicas ou em pós-operatório de cirurgias

    Researching AI Legibility Through Design

    Everyday interactions with computers are increasingly likely to involve elements of Artificial Intelligence (AI). Encompassing a broad spectrum of technologies and applications, AI poses many challenges for HCI and design. One such challenge is the need to make AI’s role in a given system legible to the user in a meaningful way. In this paper we employ a Research through Design (RtD) approach to explore how this might be achieved. Building on contemporary concerns and a thorough exploration of related research, our RtD process reflects on designing imagery intended to help increase AI legibility for users. The paper makes three contributions. First, we thoroughly explore prior research in order to critically unpack the AI legibility problem space. Second, we respond with design proposals whose aim is to enhance the legibility, to users, of systems using AI. Third, we explore the role of design-led enquiry as a tool for critically exploring the intersection between HCI and AI research

    O contexto deste anuário é a consolidação dos estudos de casos na Faculdade de Direito da Universidade Federal Fluminense. Desde 2007, existem projetos de monitoria, intitulados “Observatórios de Jurisprudência”, como, por exemplo, o “Observatório de Jurisprudência do STF”, coordenado pelo professor Cláudio Pereira de Souza Neto. Em 2009, projetos assim se multiplicaram, chegando a contar com o “Observatório de Legislação, Doutrina e Jurisprudência do Mercosul, coordenado pelo professor Eduardo Val, tendo sido, inclusive, promovido um ciclo de palestras sobre a “Jurisprudencialização do Direito” pela Revista de Direitos dos Monitores da UFF no mesmo ano.     We describe a case series of 17 patients hospitalized in Manaus (western Brazilian Amazon) with PCR-confirmed Plasmodium vivax infection who were treated with chloroquine and primaquine. The major complications were jaundice and severe anemia. No in vivo chloroquine resistance was detected. These data help characterize the clinical profile of severe P. vivax malaria in Latin America
