    The global contribution of soil mosses to ecosystem services

    Soil mosses are among the most widely distributed organisms on land. Experiments and observations suggest that they contribute to terrestrial soil biodiversity and function, yet their ecological contribution to soil has never been assessed globally under natural conditions. Here we conducted the most comprehensive global standardized field study to quantify how soil mosses influence 8 ecosystem services associated with 24 soil biodiversity and functional attributes across wide environmental gradients from all continents. We found that soil mosses are associated with greater carbon sequestration, pool sizes for key nutrients and organic matter decomposition rates but a lower proportion of soil-borne plant pathogens than unvegetated soils. Mosses are especially important for supporting multiple ecosystem services where vascular-plant cover is low. Globally, soil mosses potentially support 6.43 Gt more carbon in the soil layer than do bare soils. The amount of soil carbon associated with mosses is up to six times the annual global carbon emissions from any altered land use globally. The largest positive contribution of mosses to soils occurs under a high cover of mat and turf mosses, in less-productive ecosystems and on sandy and salty soils. Our results highlight the contribution of mosses to soil life and functions and the need to conserve these important organisms to support healthy soils.The study work associated with this paper was funded by a Large Research Grant from the British Ecological Society (no.     Association Between Preexisting Versus Newly Identified Atrial Fibrillation and Outcomes of Patients With Acute Pulmonary Embolism

    Background Atrial fibrillation (AF) may exist before or occur early in the course of pulmonary embolism (PE). We determined the PE outcomes based on the presence and timing of AF. Methods and Results Using the data from a multicenter PE registry, we identified 3 groups: (1) those with preexisting AF, (2) patients with new AF within 2 days from acute PE (incident AF), and (3) patients without AF. We assessed the 90-day and 1-year risk of mortality and stroke in patients with AF, compared with those without AF (reference group). Among 16 497 patients with PE, 792 had preexisting AF. These patients had increased odds of 90-day all-cause (odds ratio [OR], 2.81; 95% CI, 2.33-3.38) and PE-related mortality (OR, 2.38; 95% CI, 1.37-4.14) and increased 1-year hazard for ischemic stroke (hazard ratio, 5.48; 95% CI, 3.10-9.69) compared with those without AF. After multivariable adjustment, preexisting AF was associated with significantly increased odds of all-cause mortality (OR, 1.91; 95% CI, 1.57-2.32) but not PE-related mortality (OR, 1.50; 95% CI, 0.85-2.66). Among 16 497 patients with PE, 445 developed new incident AF within 2 days of acute PE. Incident AF was associated with increased odds of 90-day all-cause (OR, 2.28; 95% CI, 1.75-2.97) and PE-related (OR, 3.64; 95% CI, 2.01-6.59) mortality but not stroke. Findings were similar in multivariable analyses. Conclusions In patients with acute symptomatic PE, both preexisting AF and incident AF predict adverse clinical outcomes. La Odontología Estética es una rama de la odontología que soluciona problemas relacionados con la salud bucal y la armonía dentofacial. La expresión de la cara es lo primero que ve la persona con quien se entabla conversación, por lo que cobra importancia tanto a nivel social como individual, pues una sonrisa con alteraciones estéticas afecta de manera vital a nuestro atractivo y autoestima. Innumerables son las distorsiones que pueden provocar una alteración estética de la sonrisa o una desarmonía facial. La ausencia o perdida de piezas dentarias en el sector anterior, por distintas patologías, además de interferir en la armonía estética afecta la vida personal, la seguridad y autoestima de la persona, sobre todo en casos de gran destrucción, lo que incluso puede desencadenar problemas sociales. El odontólogo, por lo tanto, debe abordar la rehabilitación de piezas dentarias perdidas, y sus consecuencias desde el punto de vista funcional y estético. Actualmente los pacientes, buscan soluciones armónicas, para restaurar la función y estética perdida, mejorando su calidad de vida. Para ello la rehabilitación debe reproducir, copiar, y crear tratamientos lo más natural e imperceptible posible. Por ello, son muchos los métodos con que debe contar la odontología para diseñar una sonrisa estética y funcional adaptada a la cara del paciente y capaz de proporcionar ese aspecto natural y saludable

    Influence of different treatments of the ceramic surface and thermal cycling on the bond strength of brackets to ceramic

    Abstract Objective To evaluate in vitro the effect of different treatments of the ceramic surface and thermal cycling on the shear bond strength (SBS) of metallic brackets bonded to feldspathic ceramic. Material and method Ceramic cylinders were divided into four groups (n=4) according to the treatment of ceramic surface: G1-Clearfil Ceramic Primer silane and Transbond XT (CCPT); G2-etched with 10% hydrofluoric acid (HFA) for 60 s, CCP and Transbond XT (ACCPT); G3-etched with 10% HFA for 60 s, Ambar Adhesive and Transbond XT (AAAT); and, G4 - etched with 10% HFA for 60 s, RelyX Ceramic Primer silane -RCP, adhesive primer Transbond and Transbond XT (ACPPT). Brackets were bonded to the cylinders with Transbond XT and light-activated for 40 s with LED Radii Plus. All specimens were stored in deionized water at 37 °C for 24 h, and two cylinders from each group were subject to 7,000 thermal cycles in a thermal cycler (5 °C/55 °C). After storage and thermal cycling, the SBS test was performed at a crosshead speed of 1 mm/min. Data were subjected to two-way ANOVA and Tukey’s post hoc test (α=0.05). Result The SBS of ACCPT was significantly higher than the other groups (p<0.05). The specimens submitted to thermal cycling showed significantly lower SBS than those without thermal cycling (p<0.05), regardless the ceramic surface treatment. The ARI showed predominance of score 0 for all groups. Conclusion Acid etching, CCP silane and Transbond XT method obtained the best results for bracket bonding. Thermal cycling reduced SBS for all groups. Score 0 was predominant for ARI in all groups

    Challenging measles case definition: three measles outbreaks in three Health Regions of Portugal, February to April 2018

The mean age of cases was 30 years, 79% worked in a healthcare setting and 87% were vaccinated. Genotype B3 was identified in 84 cases from the three outbreaks. Primary cases in each outbreak were imported.     Deep-sequencing reveals broad subtype-specific HCV resistance mutations associated with treatment failure.

    A percentage of hepatitis C virus (HCV)-infected patients fail direct acting antiviral (DAA)-based treatment regimens, often because of drug resistance-associated substitutions (RAS). The aim of this study was to characterize the resistance profile of a large cohort of patients failing DAA-based treatments, and investigate the relationship between HCV subtype and failure, as an aid to optimizing management of these patients. A new, standardized HCV-RAS testing protocol based on deep sequencing was designed and applied to 220 previously subtyped samples from patients failing DAA treatment, collected in 39 Spanish hospitals. The majority had received DAA-based interferon (IFN) α-free regimens; 79% had failed sofosbuvir-containing therapy. Genomic regions encoding the nonstructural protein (NS) 3, NS5A, and NS5B (DAA target regions) were analyzed using subtype-specific primers. Viral subtype distribution was as follows: genotype (G) 1, 62.7%; G3a, 21.4%; G4d, 12.3%; G2, 1.8%; and mixed infections 1.8%. Overall, 88.6% of patients carried at least 1 RAS, and 19% carried RAS at frequencies below 20% in the mutant spectrum. There were no differences in RAS selection between treatments with and without ribavirin. Regardless of the treatment received, each HCV subtype showed specific types of RAS. Of note, no RAS were detected in the target proteins of 18.6% of patients failing treatment, and 30.4% of patients had RAS in proteins that were not targets of the inhibitors they received. HCV patients failing DAA therapy showed a high diversity of RAS. Ribavirin use did not influence the type or number of RAS at failure. The subtype-specific pattern of RAS emergence underscores the importance of accurate HCV subtyping. The frequency of "extra-target" RAS suggests the need for RAS screening in all three DAA target regions
