    The challenge of maintaining successful aging at 87 years old: the Octabaix study two-year follow-up

    Preserving physical and cognitive function is crucial to successful aging. The objective of this study is to determine how many 87-year-old community-dwelling subjects continued to age successfully, according to a definition using a quantitative approach, and to assess the predictive value of certain factors evaluated 2 years previously. A total of 162 participants were assessed. Sociodemographic variables, the Barthel Index (BI), the Spanish version of the Mini-Mental State Examination (MEC), the Mini Nutritional Assessment (MNA), the Charlson Index, the Gait Rating Scale, social risk, quality of life, prevalent chronic diseases, and chronic drug prescription were collected. All subjects with scores over 90 points on the BI and above 23 points on the MEC were compared with the rest of the participants. A multiple regression analysis was performed. With the selected criteria, 90 (61.6%) community-dwelling subjects continued to age successfully. The multiple logistic regression analysis showed that the following were significantly associated with continued successful aging: A higher level of studies (p<0.02, odds ratio [OR] 3.223, 95% confidence interval [CI] 1.158-8.975), better MEC scores (p<0.01, OR 1.204, 95% CI 1.046-1.386) and Tinetti gait scale scores (p<0.01, OR 1.433, 95% CI 1.013-2.027), and fewer chronic drug prescriptions (p<0.001, OR 0.768, 95% CI 0.655-0.899). In conclusion, more than half of the individuals continued to age successfully. In subjects aged 87 years, the factors associated with continued successful aging were longer schooling, better cognition scores, lower risk of falls, and fewer chronic drug prescriptions at baseline evaluation

    El schwannoma es tumor de que se localiza en cabeza y cuello en la mitad de los casos. La localización más frecuente es el VIII par craneal generando hipoacusia y vértigo. El tratamiento consiste en la exéresis quirúrgica. A continuación exponemos una localización rara de este tumor, el seno maxilar. La clínica consiste en cefalea e insuficiencia respiratoria nasal. No existe consenso sobre la vía de abordaje para la cirugía.     Biofabrication of a Tubular Model of Human Urothelial Mucosa Using Human Wharton Jelly Mesenchymal Stromal Cells

    Several models of bioartificial human urothelial mucosa (UM) have been described recently. In this study, we generated novel tubularized UM substitutes using alternative sources of cells. Nanostructured fibrin–agarose biomaterials containing fibroblasts isolated from the human ureter were used as stroma substitutes. Then, human Wharton jelly mesenchymal stromal cells (HWJSC) were used to generate an epithelial-like layer on top. Three differentiation media were used for 7 and 14 days. Results showed that the biofabrication methods used here succeeded in generating a tubular structure consisting of a stromal substitute with a stratified epithelial-like layer on top, especially using a medium containing epithelial growth and differentiation factors (EM), although differentiation was not complete. At the functional level, UM substitutes were able to synthesize collagen fibers, proteoglycans and glycosaminoglycans, although the levels of control UM were not reached ex vivo. Epithelial differentiation was partially achieved, especially with EM after 14 days of development, with expression of keratins 7, 8, and 13 and pancytokeratin, desmoplakin, tightjunction protein-1, and uroplakin 2, although at lower levels than controls.     Mortality and risk of tuberculosis among people living with HIV in whom TB was initially ruled out

    Tuberculosis (TB) misdiagnosis remains a public health concern, especially among people living with HIV (PLHIV), given the high mortality associated with missed TB diagnoses. The main objective of this study was to describe the\xC2\xA0all-cause mortality, TB incidence rates and their\xC2\xA0associated risk factors in a cohort of PLHIV with presumptive TB in whom TB was initially ruled out. We retrospectively followed a cohort of PLHIV with presumptive TB over a 2\xC2\xA0year-period in a rural district in Southern Mozambique. During the study period 382 PLHIV were followed-up. Mortality rate was 6.8/100 person-years (PYs) (95% CI 5.2-9.2) and TB incidence rate was 5.4/100 PYs (95% CI 3.9-7.5). Thirty-six percent of deaths and 43% of TB incident cases occurred in the first 12\xC2\xA0months of the follow up. Mortality and TB incidence rates in the 2-year period after TB was initially ruled out was very high. The\xC2\xA0TB diagnostic work-up and linkage to HIV\xC2\xA0care should be strengthened to decrease TB burden and all-cause mortality among PLHIV with presumptive TB
