    VALIDITA’ DELL’ANTROSTOMIA MEDIA A SCOPO PROFILATTICO NELLA PREVENZIONE DELLE SINUSITI MASCELLARI CONSEGUENTI A INSERZIONE DI IMPIANTI ZIGOMATICI

    Scopo dello studio Lo scopo dello studio è stato di valutare l’efficacia dell’antrostomia endoscopica del meato medio nella prevenzione delle sinusiti mascellari conseguenti al posizionamento di impianti zigomatici. Materiali e metodi È stato effettuato uno studio prospettico longitudinale su pazienti arruolati in una singola Unità Operativa di Chirurgia Maxillo-Facciale. I pazienti sono stati sottoposti a posizionamento di 4 impianti zigomatici con tecnica intrasinusale e contestuale antrostomia endoscopica del meato medio. Il criterio di inclusione contemplati è stato l’essere candidabili a posizionamento di impianti zigomatici per atroDia estrema del mascellare superiore. I criteri di esclusione sono stati l’anamnesi positiva per pregressi interventi chirurgici o patologie a carico del distretto rinosinusale, il tabagismo e l’evidenza radiologica di opacizzazione sinusale od inspessimento mucoso, valutata per mezzo di scansione TC a raggio conico (CBCT). L’evidenza radiologica alla CBCT di opacizzazione sinusale o di inspessimento mucoso è stata inoltre valutata 12 mesi dopo l’intervento chirurgico mediante il Lund-Mackay Staging Sysyem (LMS). La sintomatologia dei pazienti è stata valutata sia preoperatoriamente sia 12 mesi dopo l’intervento mediante il Sino-Nasal-Outcome Test-20 (SNOT- 20). È stata inoltre osservata l’incidenza di lacerazione intraoperatoria della membrana di Schneider. Risultati L’evidenza radiologica postoperatoria di opacizzazione o inspessimento mucoso sinusale è risultata considerevolmente inferiore rispetto ai controlli storici effettuati nella stessa Unità Operativa e a quanto disponibile in letteratura. Il punteggio SNOT-20 si è ridotto nella maggioranza dei pazienti. Si è verificata lacerazione intraoperatoria della membrana di Schneider in 12 seni (8 pazienti). Conclusioni Il restringimento del complesso ostiomeatale verosimilmente gioca un ruolo importante nell’insorgenza di sinusite mascellare in seguito al posizionamento di impianti zigomatici. L’antrostomia del meato medio sembra essere una misura efficace nella prevenzione di questa categoria di complicanze

    The effect of subspinal Le Fort I osteotomy and alar cinch suture on nasal widening

    The aim of this retrospective study was to evaluate the relationships between upper jaw movements and nasal soft-tissue changes in patients who have undergone subspinal Le Fort I osteotomy combined with alar cinch suture.Single and multivariate linear regression analyses were used to examine the relationships between greatest inter-alar width (GAW) and maxillary advancement, maxillary impaction, and rotational movements. The database of our referral hospital was searched for patients who had undergone upper jaw surgery with a subspinal LFI osteotomy to correct dentoskeletal deformities between April 2012 and June 2016.Thirty-eight of the patients (15 men and 23 women) who were identified were eligible for inclusion. The average change in inter-alar width (Delta GAW) was +1.7 +/- 1.2 mm. GAW increased by 0.3 mm (p < 0.0001) for each millimetre of maxillary advancement, and increased by 0.5 mm (p < 0.0001) for each millimetre of maxillary impaction. GAW increased by 0.2 mm for each degree of counterclockwise rotation of the occlusal plane (p < 0.0001).An analysis of our data compared with the current literature confirmed that subspinal Le Fort I combined with alar cinch suture reduced alar base widening.     The goal of modern dentistry and plastic surgery is to restore the patient to normal function, health and aesthetics, regardless of the disease or injury to the stomatognathic and cutaneous system respectively. In recent years tissue engineering and regenerative medicine have yielded many novel tissue replacements and implementation strategies. Scientific advances in biomaterials, stem cell isolation, growth and differentiation factors and biomimetic environments have created unique opportunities to fabricate tissues in the laboratory. Repairing of bone and skin is likely to become of clinical interest when three dimensional tissue reconstructive procedures and the appropriate supporting biomimetic materials are correctly assembled. In the present review, we provide an overview of the most promising biopolymers that may find clinical application in dento-maxillo-facial and plastic surgery

    Peripheral ameloblastoma of the upper gingiva: Report of a case and literature review

    According to the 2005 histological classification of odontogenic neoplasms by the World Health Organization, ameloblastoma is a benign, locally invasive epithelial odontogenic tumor of putative enamel organ origin. There are four distinct subgroups in which this neoplasm can be gathered: the solid/multicystic type, the unicystic type, the desmoplastic and the peripheral type. Peripheral ameloblastoma is believed to be the rarest subgroup, making up for 2 to 10% of all ameloblastomas. From its first description by Kuru in 1911 to date, less than 200 cases of PA have been described in literature. PAs commonly affect the mandible, in the maxilla the most common location is the soft palatal tissue of the tuberosity area. The present report discusses a rare case of PA aroused in the gingiva of upper jaw in a 64-year-old woman. The treatment of the lesion and its immunohistochemical phenotype are described. A review of the literature is also performed, focusing on the epidemiological and pathological aspects of the lesions and their implications on the therapy

    Morphology of the toe flexor muscles in older people with toe deformities

    Objective: Despite suggestions that atrophied, or weak toe flexor muscles are associated with the formation of toe deformities, there has been little evidence to support this theory. This study aimed to determine whether the size of the toe flexor muscles differed in older people with and without toe deformities. Methods: Forty-four older adults (&gt;60 years) were recruited for the study. Each participant had their feet assessed for the presence of hallux valgus or lesser toe deformities. Intrinsic and extrinsic toe flexor muscles were imaged with an ultrasound system using a standardised protocol. Assessor blinded muscle thickness and cross-sectional area was measured using Image J software. Results: Participants with lesser toe deformities (n=20) were found to have significantly smaller quadratus plantae (p=0.003), flexor digitorum brevis (p=0.013), abductor halluces (p=0.004) and flexor halluces brevis (p=0.005) muscles than the participants without any toe deformities (n=19). Female participants with hallux valgus (n=10) were found to have significantly smaller abductor hallucis (p=0.048) and flexor halluces brevis (p=0.013) muscles than the female participants without any toe deformities (n=10; p&lt;0.05). Conclusion: This is the first study to use ultrasound to investigate the size of the toe flexor muscles in older people with hallux valgus and lesser toe deformities compared to otherwise healthy older adults. The size of the abductor hallucis and flexor hallucis brevis muscles were decreased in participants with hallux valgus whereas the quadratus plantae, flexor digitorum brevis, abductor hallucis and flexor halluces brevis muscles were smaller in those participants with lesser toe deformities

    The stability of money demand in the long-run: Italy 1861–2011

    Money demand stability is a crucial issue for monetary policy efficacy, and it is particularly endangered when substantial changes occur in the monetary system. By implementing the ARDL technique, this study intends to estimate the impact of money demand determinants in Italy over a long period (1861–2011) and to investigate the stability of the estimated relations. We show that instability cannot be excluded when a standard money demand function is estimated, irrespectively of the use of M1 or M2. Then, we argue that the reason for possible instability resides in the omission of relevant variables, as we show that a fully stable demand for narrow money (M1) can be obtained from an augmented money demand function involving real exchange rate and its volatility as additional explanatory variables. These results also allow us to argue that narrower monetary aggregates should be employed in order to obtain a stable estimated relation

    Lo scopo del presente elaborato di tesi è la creazione di uno strumento utile ai fini dell’informatica forense per la rilevazione di Trojan ad Accesso Remoto dal traffico di rete da essi generato. Il RAT è un malware che consente a un utente non autorizzato il controllo amministrativo da remoto del dispositivo su cui è installato. L’utilizzo dei RAT è illegale ad eccezion fatta per i casi ammessi dal diritto, la cui trattazione sarà oggetto del capitolo Secondo. L’attività di rilevazione è quindi da inquadrare in quelli che sono gli utilizzi illeciti di tali strumenti. Sotto il profilo giuridico sarà analizzato l’impiego dei RAT nelle fonti giuridiche riguardanti le intercettazioni prendendo in esame i diritti fondamentali di libertà di espressione, l’inviolabilità delle comunicazioni discernendo l’attività di RAT da strumento per la cd. perquisizione on-line, e quindi come strumento non tipico per l’ottenimento di prove, a strumento per l’intercettazione tra presenti in seno alla cosiddetta ”riforma Orlando” del c.p.p. Passando invece al punto di vista funzionale, il progetto è orientato sull’intercettazione, indicizzazione e analisi del traffico dati riguardante un telefono con RAT installato per rilevare patterns nella rete utili per l’identificazione, senza alterare il telefono. La cattura dei pacchetti avverrà tramite l’adozione di un wiretapper, che dirotterà i pacchetti provenienti ed indirizzati al telefono in esame. Contestualmente alla cattura dei dati effettuata con Wireshark da riga di comando7, i pacchetti saranno caricati su Elasticsearch, un motore di ricerca full text, e analizzati dall’interfaccia web Kibana, opportunamente configurata per mostrare attività anomale e pattern inusuali nel traffico di rete. L’intento del presente elaborato risiede quindi nell’esaminare gli aspetti giuridici dei RAT, realizzando un dispositivo che permetta l’autotutela nei casi non previsti dagli istituti giuridici presi in esame
