    Erst- und Rezidivoperationen von Patienten mit renalem sekundÀrem und tertiÀrem Hyperparathyreoidismus

    Chronische Nierenerkrankungen stellen die hĂ€ufigste Ursache fĂŒr die Entwicklung eines sekundĂ€ren und tertiĂ€ren Hyperparathyreoidismus dar. Charakteristisch fĂŒr diese beiden Formen des renalen Hyperparathyreoidismus (HPT) sind erhöhte Parathormon-, Phosphat- sowie verĂ€nderte Kalziumspiegel, die mit einer Vielzahl an klinischen Symptomen einhergehen können und mit einer erhöhten MortalitĂ€t assoziiert sind. Die Therapiemöglichkeiten beinhalten medikamentöse und operative Maßnahmen. Bedenken hinsichtlich der Parathyreoidektomie (PTx) bei renalem HPT betreffen vor allem das perioperative Management dieses multimorbiden Patientenkollektivs sowie intra- und postoperative Komplikationen. Ziel der aktuellen Studie war es daher, die PTx bei renalem HPT an einem Zentrum fĂŒr endokrine Chirurgie in MĂŒnchen hinsichtlich EffektivitĂ€t und Sicherheit zu evaluieren. In die Studie eingeschlossen wurden in retrospektivem Vorgehen alle Patienten, die zwischen Januar 2016 und Dezember 2019 im Krankenhaus Martha-Maria wegen eines renalen HPT operiert wurden. Als primĂ€rer Endpunkt und Maß fĂŒr die Sicherheit der PTx wurde die Komplikationsrate definiert. Als sekundĂ€rer Endpunkt und Maß fĂŒr die EffektivitĂ€t des Eingriffs wurde die intra- und postoperative Reduktion des intakten Parathormons (PTH) in festgelegte Zielbereiche definiert. Insgesamt wurden 152 Patienten in die Studie eingeschlossen und 158 Erst- und Rezidiveingriffe erfasst. Eine frĂŒhpostoperative Recurrensparese entwickelten 2,5 % der Patienten. Eine schwere HypokalzĂ€mie im postoperativen Verlauf (Serum-Gesamtkalzium < 1,8 mmol/l) trat bei 41,1 % der Patienten auf und war assoziiert mit einem lĂ€ngeren Krankenhausaufenthalt. Zu unspezifischen postoperativen Komplikationen kam es bei 10,8 % der Patienten, wobei kardio-pulmonale Ereignisse mit 3,8 % die hĂ€ufigste Ursache darstellten. 94,3 % der Patienten konnten im Median nach 4 Tagen wie geplant aus dem Krankenhaus entlassen werden, bei 5,7 % kam es wegen unspezifischer Komplikationen zu einer Verlegung auf eine nicht-chirurgische Normal- oder Intensivstation. Die perioperative MortalitĂ€t lag bei 0 %. Im Zuge der PTx zeigte sich ein deutlicher und stabiler Abfall des PTH, im Median von 1168 auf 13 pg/ml. Dies entspricht einer Reduktion von 99 % und kann insbesondere auch deswegen als Erfolg gewertet werden, weil knapp 70 % der PTH-Werte postoperativ im definierten Erfolgsbereich zwischen 2,5 und 65 pg/ml und 92,4 % im erweiterten Erfolgsbereich < 300 pg/ml zu liegen kamen. Zusammenfassend kann die PTx bei Patienten mit renalem HPT als effektives Verfahren bewertet werden, das kurzfristig mit relevanten, jedoch kontrollierbaren Risiken einhergeht. Randomisierte und kontrollierte Studien sind notwendig, um die vorhandenen Therapieoptionen sowie deren kurzfristige Risiken und langfristigen Nutzen zu vergleichen und Behandlern so zukĂŒnftig die Indikationsstellung zur PTx zu erleichtern

    Topological band inversion in HgTe(001): surface and bulk signatures from photoemission

    HgTe is a versatile topological material and has enabled the realization of a variety of topological states, including two- and three-dimensional (3D) topological insulators and topological semimetals. Nevertheless, a quantitative understanding of its electronic structure remains challenging, in particular due to coupling of the Te 5p-derived valence electrons to Hg 5d core states at shallow binding energy. We present a joint experimental and theoretical study of the electronic structure in strained HgTe(001) films in the 3D topological-insulator regime, based on angle-resolved photoelectron spectroscopy and density functional theory. The results establish detailed agreement in terms of (i) electronic band dispersions and orbital symmetries, (ii) surface and bulk contributions to the electronic structure, and (iii) the importance of Hg 5d states in the valence-band formation. Supported by theory, our experiments directly image the paradigmatic band inversion in HgTe, underlying its non-trivial band topology

    The HLA ligandome of oropharyngeal squamous cell carcinomas reveals shared tumour-exclusive peptides for semi-personalised vaccination

    Background The immune peptidome of OPSCC has not previously been studied. Cancer-antigen specific vaccination may improve clinical outcome and efficacy of immune checkpoint inhibitors such as PD1/PD-L1 antibodies. Methods Mapping of the OPSCC HLA ligandome was performed by mass spectrometry (MS) based analysis of naturally presented HLA ligands isolated from tumour tissue samples (n = 40) using immunoaffinity purification. The cohort included 22 HPV-positive (primarily HPV-16) and 18 HPV-negative samples. A benign reference dataset comprised of the HLA ligandomes of benign haematological and tissue datasets was used to identify tumour-associated antigens. Results MS analysis led to the identification of naturally HLA-presented peptides in OPSCC tumour tissue. In total, 22,769 peptides from 9485 source proteins were detected on HLA class I. For HLA class II, 15,203 peptides from 4634 source proteins were discovered. By comparative profiling against the benign HLA ligandomic datasets, 29 OPSCC-associated HLA class I ligands covering 11 different HLA allotypes and nine HLA class II ligands were selected to create a peptide warehouse. Conclusion Tumour-associated peptides are HLA-presented on the cell surfaces of OPSCCs. The established warehouse of OPSCC-associated peptides can be used for downstream immunogenicity testing and peptide-based immunotherapy in (semi)personalised strategies

    Dynamic magnetic resonance defecography in 10 asymptomatic volunteers

    AIM: Evaluation of the wide range of normal findings in asymptomatic women undergoing dynamic magnetic resonance (MR) defecography. METHODS: MR defecography of 10 healthy female volunteers (median age: 31 years) without previous pregnancies or history of surgery were evaluated. The rectum was filled with 180 mL gadolinium ultrasound gel mixture. MR defecography was performed in the supine position. The pelvic floor was visualized with a dynamic T2-weighted sagittal plane where all relevant pelvic floor organs were acquired during defecation. The volunteers were instructed to relax and then to perform straining maneuvers to empty the rectum. The pubococcygeal line (PCGL) was used as the line of reference. The movement of pelvic floor organs was measured as the vertical distance to this reference line. Data were recorded in the resting position as well as during the defecation process with maximal straining. Examinations were performed and evaluated by two experienced abdominal radiologists without knowledge of patient history. RESULTS: Average position of the anorectal junction was located at -5.3 mm at rest and -29.9 mm during straining. The anorectal angle widened significantly from 93° at rest to 109° during defecation. A rectocele was diagnosed in eight out of 10 volunteers showing an average diameter of 25.9 mm. The bladder base was located at a position of +23 mm at rest and descended to -8.1 mm during defecation in relation to the PCGL. The bladder base moved below the PCGL in six out of 10 volunteers, which was formally defined as a cystocele. The uterocervical junction was located at an average level of +43.1 mm at rest and at +7.9 mm during straining. The uterocervical junction of three volunteers fell below the PCGL; described formally as uterocervical prolapse. CONCLUSION: Based on the range of standard values in asymptomatic volunteers, MR defecography values for pathological changes have to be re-evaluated

    Registration in a quality register : a method to improve end-of-lifecare—a cross-sectional study

    OBJECTIVES: Structured methods to assess and support improvement in the quality of end-of-life care are lacking and need to be developed. This need is particularly high outside the specialised palliative care. This study examines whether participation in a national quality register increased the quality of end-of-life care. DESIGN: This study is a cross-sectional longitudinal register study. SETTING: The Swedish Register of Palliative Care (SRPC) collects data about end-of-life care for deaths in all types of healthcare units all over Sweden. Data from all 503 healthcare units that had reported patients continuously to the register during a 3-year period were analysed. PRIMARY AND SECONDARY OUTCOME MEASURES: Data on provided care during the last weeks of life were compared year-by-year with logistic regression. PARTICIPANTS: The study included a total 30 283 patients. The gender distribution was 54% women and 46% men. A total of 60% of patients in the study had a cancer diagnosis. RESULTS: Provided end-of-life care improved in a number of ways. The prevalence of six examined symptoms decreased. The prescription of 'as needed' medications for pain, nausea, anxiety and death rattle increased. A higher proportion of patients died in their place of preference. The patient's next of kin was more often offered a follow-up appointment after the patient's death. No changes were seen with respect to providing information to the patient or next of kin.     Unmet Palliative Care Needs Among Patients With End-Stage Kidney Disease : A National Registry Study About the Last Week of Life

    Context: End-stage kidney disease (ESKD) is characterized by high physical and psychological burden, and therefore, more knowledge about the palliative care provided close to death is needed. Objectives: To describe symptom prevalence, relief, and management during the last week of life, as well as end-of-life communication, in patients with ESKD. Methods: This study was based on data from the Swedish Register of Palliative Care. Patients aged 18 or older who died from a chronic kidney disease, with or without dialysis treatment (International Classification of Diseases, Tenth Revision, Sweden; N18.5 or N18.9), during 2011 and 2012 were selected. Results: About 472 patients were included. Of six predefined symptoms, pain was the most prevalent (69%), followed by respiratory secretion (46%), anxiety (41%), confusion (30%), shortness of breath (22%), and nausea (17%). Of patients with pain and/or anxiety, 32% and 44%, respectively, were only partly relieved or not relieved at all. Of patients with the other symptoms, a majority (55%-84%) were partly relieved or not relieved at all. End-of-life discussions were reported in 41% of patients and 71% of families. A minority died in specialized palliative care: 8% in hospice/inpatient palliative care and 5% in palliative home care. Of all patients, 19% died alone. Bereavement support was offered to 38% of families. Conclusion: Even if death is expected, most patients dying with ESKD had unmet palliative care needs regarding symptom management, advance care planning, and bereavement support

    Intracranial neuromuscular choristoma: report of a case with literature review

    Neuromuscular choristoma (NMC), also called neuromuscular hamartoma or nerve rhabdomyoma, is a rare lesion of the spinal and cranial nerves composed of skeletal muscle intimately associated with nerve fibers. Its origin has not been precisely clarified and a malformative event, resulting from aberrant differentiation or a true neoplastic growth, have been proposed by authors. We hereby present a cerebellopontine angle NMC enlarging the eighth cranial nerve in a 3-year-old child, that histologically appeared composed of a large amount of striated muscle mixed with nerve fibers. We also provide a review of the intracranial NMC cases reported in the literature and an analysis of proposed hypotheses to explain the presence of muscle cells in nerve trunks
