    Avaliação das propriedades físico-químicas e biológicas de um cimento endodôntico a base de silicato de cálcio ¿ MTA Fillapex

Entretanto, ainda existe uma escassez de trabalhos na literatura avaliando as suas características físico-químicas e biológicas. Dessa forma, os objetivos deste estudo foram: 1) avaliar propriedades físico-químicas (radiopacidade, escoamento e pH) e biológicas (citotoxicidade) do cimento endodôntico MTA Fillapex e comparar com o cimento AH Plus, 2) realizar um acompanhamento em longo prazo do efeito citotóxico em fibroblastos 3T3 de diferentes cimentos endodônticos contemporâneos, e 3) avaliarem a partir de um ensaio multiparamétrico em longo prazo os efeitos citotóxicos do MTA Fillapex e do AH Plus em uma cultura primária de osteoblastos humanos. Os resultados do estudo mostraram que embora o AH Plus tenha apresentado radiopacidade estatisticamente maior que o MTA Fillapex (P0.05). No entanto, uma semana após sua manipulação o AH Plus se tornou não citotóxico em todos os parâmetros avaliados. Por outro lado, o MTA Fillapex permaneceu citotóxico durante todo o período experimental, mostrando diferenças estatisticamente significantes quando comparados com o AH Plus (P0.05). After one week AH Plus become noncytotoxic, on all three parameters evaluated. Conversely, MTA Fillapex remained cytotoxic over the entire experimental period, showing significantly differences when compared to AH Plus (P<0.05). Within the employed methodology and according to the results can be concluded that although MTA Fillapex showed suitable physicochemical properties for use in endodontic therapy, it appeared highly cytotoxic in the different tested conditionsDoutoradoEndodontiaDoutor em Clínica Odontológic

    Long-term cytotoxic effects of contemporary root canal sealers

    Objectives The aim of the present study was to investigate the effects of root canal sealers on the cytotoxicity of 3T3 fibroblasts during a period of 5 weeks. Material and Methods Fibroblasts (3T3, 1×105 cells per well) were incubated with elutes of fresh specimens from eight root canal sealers (AH Plus, Epiphany, Endomethasone N, EndoREZ, MTA Fillapex, Pulp Canal Sealer EWT, RoekoSeal and Sealapex) and with elutes of the same specimens for 5 succeeding weeks after immersing in simulated body fluid. The cytotoxicity of all root canal sealers was determined using the MTT assay. Data were analyzed using ANOVA and Tukey's test. Results RoekoSeal was the only sealer that did not show any cytotoxic effects (p&lt;0.05). All the other tested sealers exhibited severe toxicity initially (week 0). MTA Fillapex remained moderately cytotoxic after the end of experimental period. Toxicity of the other tested sealers decreased gradually over time. The evaluated root canal sealers presented varying degrees of cytotoxicity, mainly in fresh mode. Conclusions RoekoSeal had no cytotoxic effect both freshly mixed and in the other tested time points.     Endodontic management of type II dens invaginatus with open apex and large periradicular lesion using the XP-endo Finisher : a case report

    Dens invaginatus (DI) represents an endodontic challenge because of its complex root canal morphology. This case report presents the clinical management of a 22-year-old woman with type II DI in right maxillary lateral incisor with a painful swelling. Pulp testing revealed no response with the tooth. Type II DI with open apex and large periradicular lesion was seen on radiograph. The treatment was planned by using cone-beam computed tomography (CBCT) imaging. Canal treatment was completed in two appointments with the aid of a dental operating microscope. In the first appointment, the internal anatomy was modified using an ultrasonic tip, and chemo-mechanical preparation was performed using the XP-endo Finisher instrument and NaOCl; calcium hydroxide intracanal dressing was used for one month. In the second appointment, an apical plug of mineral trioxide aggregate (MTA) Repair HP was performed and the remaining pulp space was then filled with gutta-percha and AH Plus sealer using the continuous wave of condensation technique. At the fourteen-month reevaluation, the patient was asymptomatic, the tooth had remained functional, and radiographic and CBCT assessment showed significant osseous healing of the lesion. Successful non-surgical management of the present type II DI was achieved in the present case. The association of CBCT, dental operating microscope, XP-endo Finisher, NaOCl and MTA Repair HP were important for ensuring a predictable outcome

    Sealing ability promoted by three different endodontic sealers

    Introduction: The aim of this study was to evaluate the apical and coronal sealing of endodontic fillings promoted by the associations of Sealer 26/Gutta-percha, Endofill/Gutta-percha and Resilon/ Epiphany. Materials and Methods: It was used 38 human canines, 10 teeth for each test group and 4 teeth for each control group. After teeth conventional endodontic treatment, they were immersed in saline solution for thirty days, sealed and stored in India ink for seven days. Then were submitted to a clearing technique and evaluated for infiltration by stereoscopic microscope. The data were statistically analyzed by ANOVA and Bonferroni tests. Results: The results showed no significant differences (p> 0.05) between the three groups of filling materials used. Conclusion: Considering the limitations of this in vitro study, it seems that no material showed superiority related to the ability of apical and coronary sealing

    Surgical alternative for treatment of vertical root fracture: A case report

    Vertical root fracture (VRF) has been a great challenge in dentistry; most fractures often result in tooth extraction. Inflammation of tissues around the fractured root is the main reason for tooth extraction. Based on the strategic importance of some fractured teeth, treatment may be necessary and often complicated. However, performing a proper repair or even splinting the fractured segments may result in tooth preservation. Accordingly, in this case we report a new method for fractured tooth preservation. The surgical exposition of the fracture tooth was carried out through the radicular portion of the element via ultrasonic preparation, filling with composed resin and a synthetic hydroxyapatite graft. All these were performed around the tooth which received five sections of low-power laser. The patient was followed for two years with no signs or symptoms of inflammation and gingival recession. In conclusion, the used treatment protocol could be considered as a promising approach for VRF treatment, especially in cases where there is advanced or moderate bone loss in the surrounding sites of the fractured tooth

    Analysis of the physicochemical properties, cytotoxicity and volumetric changes of AH Plus, MTA Fillapex and TotalFill BC Sealer

    To evaluate the physicochemical properties and cytotoxicity of AH Plus, MTA Fillapex and TotalFill BC Sealer. Volumetric changes were also evaluating using micro-computed tomography (micro-CT). Radiopacity and flow were evaluated in accordance with the ISO 6876, while setting time was evaluated in accordance with the ASTM- C266-08 specifications. The release of Ca2+ ions and pH were measured with spectrophotometer and pH meter, respectively, after different time intervals (1h, 3h, 24h, 72h, 168h, and 360h). Cytotoxicity was evaluated by MTT reduction assay to check 3T3 cells viability at 24, 48 and 72 hours. Volumetric change was evaluated by micro-CT, by using 30 acrylic teeth, filled with gutta-percha cones and the tested root canal sealer. The samples were evaluated after 168h, 360h and 720h of immersion in distilled water. Data were statistically analyzed by one-way ANOVA and Tukey test or by Kruskal-Wallis and Dunn tests (P<0.05). MTA Fillapex and TotalFill BC Sealer showed lower radiopacity than AH Plus (P<0.05). The MTA Fillapex showed the highest flow, while AH Plus showed the lowest flow (P<0.05). The initial and final setting time of AH Plus were lower than MTA Fillapex and TotalFill BC Sealer (P<0.05). In general, TotalFill BC Sealer presented higher Ca2+ ion release and pH than the other tested sealers. TotalFill BC Sealer also showed overall lower cytotoxicity when compared to the other sealers. Volumetric change of AH Plus and TotalFill BC Sealer was lower than MTA Fillapex (P<0.05). AH Plus, MTA Fillapex and TotalFill BC Sealer showed slight differences in the physicochemical properties and cytotoxicity, but all suitable for an endodontic sealer. However, AH Plus and TotalFill BC Sealer showed low volumetric changes when compared to MTA Fillapex

    Characterization of the file-specific heat-treated ProTaper Ultimate rotary system

International Endodontic Journal published by John Wiley & Sons Ltd on behalf of British Endodontic Society.Aim: To compare design, metallurgy and mechanical performance of the ProTaper (PT) Ultimate system with instruments of similar dimensions from the ProGlider, PT Gold and PT Universal systems. Methodology: New PT Ultimate instruments (n = 248) were compared with instruments of similar dimensions from ProGlider (n = 31), PT Gold (n = 155) and PT Universal (n = 155) systems regarding their number of spirals, helical angle, blade symmetry, tip geometry, surface finishing, nickel/titanium ratio, phase transformation temperatures and mechanical performance. One-way anova and nonparametric Mood's median tests were used for statistical comparison (α = 5%). Results: All instruments had symmetrical blades without radial lands or flat sides, similar surface finishing and an almost equiatomic nickel/titanium ratio, whilst the number of spirals, helical angles and the tip geometry were different. PT Ultimate instruments showed 3 distinct heat treatments that matched with the colour of their metal wire. Slider and ProGlider instruments had similar R-phase start (Rs) and R-phase finish (Rf) temperatures. SX, F1, F2, F3 and Shaper instruments showed equivalent heat treatments (Rs ~45.6°C and Rf ~28.3°C) that were similar to their PT Gold counterparts (Rs ~47.9°C and Rf ~28.2°C), but completely distinct to the PT Universal ones (Rs ~16.2°C and Rf ~−18.2°C). Amongst the PT Ultimate instruments, the lowest maximum torques were observed in the SX (0.44 N cm), Slider (0.45 N cm) and Shaper (0.60 N cm) instruments, whilst the highest was noted in the FXL (4.90 N cm). PT Ultimate Slider and ProGlider had similar torsional (~0.40 N cm) and bending loads (~145.0 gf) (p = 1.000), whilst the other PT Ultimate instruments showed statistically significantly lower maximum torque, higher angle of rotation and lower bending load (higher flexibility) than their counterparts of the PT Universal and PT Gold systems. Conclusions: The PT Ultimate system comprises instruments with 3 distinct heat treatments that showed similar phase transformation temperatures to their heat-treated analogues. PT Ultimate instruments presented lower torsional strength and superior flexibility than their counterparts, whilst maximum torque, angle of rotation and bending loads progressively increased with their sizes.publishersversionpublishe

    Antimicrobial and cytotoxic effects of phosphoric acid solution compared to other root canal irrigants

    Phosphoric acid has been suggested as an irrigant due to its effectiveness in removing the smear layer. The purpose of this study was to compare the antimicrobial and cytotoxic effects of a 37% phosphoric acid solution to other irrigants commonly used in endodontics. The substances 37% phosphoric acid, 17% EDTA, 10% citric acid, 2% chlorhexidine (solution and gel), and 5.25% NaOCl were evaluated. The antimicrobial activity was tested against Candida albicans, Staphylococcus aureus, Enterococcus faecalis, Escherichia coli, Actinomyces meyeri, Parvimonas micra, Porphyromonas gingivalis, and Prevotella nigrescensaccording to the agar diffusion method. The cytotoxicity of the irrigants was determined by using the MTT assay. Phosphoric acid presented higher antimicrobial activity compared to the other tested irrigants. With regard to the cell viability, this solution showed results similar to those with 5.25% NaOCl and 2% chlorhexidine (gel and solution), whereas 17% EDTA and 10% citric acid showed higher cell viability compared to other irrigants.
