    Molecular Characterization Of Intestinal Protozoa In Two Poor Communities In The State Of Sao Paulo, Brazil

The burden of disease is particularly high among children living in developing areas of the world, where transmission is favored by lower hygienic standards and scarce availability of safe water. However, asymptomatic infection and polyparasitism are also commonly observed in poor settings. Here, we investigated the prevalence of intestinal protozoa in two small fishing villages, Porto Said (PS) and Santa Maria da Serra (SM), situated along the river Tiete in the State of Sao Paolo, Brazil. The villages lack basic public infrastructure and services, such as roads, public water supply, electricity and public health services. Methods: Multiple fecal samples were collected from 88 individuals in PS and from 38 individuals in SM, who were asymptomatic at the time of sampling and had no recent history of diarrheal disease. To gain insights into potential transmission routes, 49 dog fecal samples (38 from PS and 11 from SM) and 28 river water samples were also collected. All samples were tested by microscopy and PCR was used to genotype Giardia duodenalis, Blastocystis sp., Dientamoeba fragilis and Cryptosporidium spp. Results: By molecular methods, the most common human parasite was Blastocystis sp. (prevalence, 45% in PS and 71% in SM), followed by D. fragilis (13.6% in PS, and 18.4% in SM) and G. duodenalis (18.2% in PS and 7.9% in SM); Cryptosporidium spp. were not detected. Sequence analysis revealed large genetic variation among Blastocystis samples, with subtypes (STs) 1 and 3 being predominant, and with the notable absence of ST4. Among G. duodenalis samples, assemblages A and B were detected in humans, whereas assemblages A, C and D were found in dogs. Finally, all D. fragilis samples from humans were genotype 1. A single dog was found infected with Cryptosporidium canis. River water samples were negative for the investigated parasites.     Peginterferon plus ribavirin and sustained virological response rate in HCV-related advanced fibrosis: a real life study

    Background: Tolerance and response to antiviral HCV treatment is poor in advanced fibrosis. The aim of this study was to assess SVR rate and its predictive factors in HCV advanced fibrosis patients treated in real life with full dose PEG-IFN plus RBV and to evaluate the adverse events related to treatment. Methods: A multicentric, retrospective study was conducted at six university hospitals. METAVIR F3 and F4 HCV monoinfected patients who were treated with PEG-IFN and RBV had their data analyzed. A stepwise logistic regression analysis was performed to identify the variables independently related to SVR. Adverse events were recorded during treatment. Results: 308 patients were included, 75% genotype 1 and 23% genotype 3. METAVIR F3 was present in 39% and F4 in 61% of patients. The median Child Pugh score for F4 patients was 5 (5–9). The global SVR rate was 34%, 11% were relapsers and 55% were nonresponders. SVR rates were similar between patients treated with PEG-IFN alfa 2a or alfa 2b (p = 0.24). SVR rates according to Child–Pugh score were 26% (Child A) and 18% (Child B). The independent factors related to SVR in F4 patients were genotype 3, RVR and fewer Child Pugh score points. Treatment interruption occurred in 31% patients and death occurred in 1.9%, all with liver cirrhosis. Conclusion: Treatment of HCV in patients with advanced fibrosis should not be postponed.     Lessons from the terra preta de índios of the amazon region for the utilisation of charcoal for soil amendment

    peer-reviewedThe potential of charcoal and of partially combusted organic waste to mimic the soil organic matter of the Terras Pretas de Índios (Amazonian Dark Earths) from the Amazon Region is discussed. These materials serve as soil conditioners and as sequesterers of carbon in recalcitrant and in reactive forms. Studies carried out by Brazilian and by international groups have contributed to the emergence of an awareness of the compositions and of the uses of these materials. In this contribution we report on chemical studies that are leading to the development of a scientific and technological awareness, and of innovations that will have value in finding novel uses in applications to soil of chars from organic wastes such as those from the biofuel industry, and from metallurgical and various coal plant residues

    Agricultura orgânica: características básicas do seu produtor.

    A agricultura orgânica é ainda pouco expressiva no Brasil. Sua relevância é, no entanto, crescente, seguindo tendência observada em outros países, em especial na Europa. Este estudo analisa as características básicas do produtor de produtos orgânicos, contribuindo para preencher uma lacuna existente na literatura de economia rural. Utilizando informações primárias sobre agricultores orgânicos da região próxima à cidade de Curitiba, Paraná, identificamos as suas características pessoais e econômicas, por meio de estatística descritiva, análise de correlação e regressão linear múltipla. Os produtores foram divididos em dois grupos: o primeiro com aqueles que ainda estão em conversão para a agricultura orgânica e o segundo grupo composto por produtores orgânicos que já obtiveram a certificação de seus produtos. Os resultados obtidos indicam que alta participação de capital próprio no financiamento da produção e elevado nível de escolaridade são duas das características mais marcantes dos produtores rurais orgânicos paranaenses. Outras características que influenciam o resultado líquido da atividade são a disponibilidade de mão de obra e a integração de atividades agrícolas, pecuárias e florestais

    Melanoma do intestino delgado: primário ou metastático? um caso clínico

    Malignant melanoma often originated from melanocytes can occur wherever these cells are present. Most common locations are the skin, retina, anus and under the nail. Although rare, primary gastrointestinal tract melanoma has been reported, mostly from the oesophagus and anorectal region. In the small bowel represents 5% of primary neoplasms. We report a case of a 70 year old man, who presented with abdominal pain, weigh loss and anaemia. Based on the findings of CT, endoscopy and entheroscopy a preoperative diagnosis of a small bowel tumour was established. Was submitted to a multiple segmental enterectomy, the histopathological examination revealed a malignant melanoma. A thorough postoperative investigation did not reveal a primary lesion in the skin, oculus or any other location. Brain and pulmonary metastasis where diagnosed afterwards and he died three months after surgery. In a patient without any obvious primary tumour, with a gastrointestinal melanoma and without extraintestinal lesions we should rule out primary gastrointestinal melanoma. Keywords: Malignant melanoma, small bowel, gastrointestinal tract. O melanoma tem geralmente a sua origem em células da linha melanocítica, podendo ocorrer em todos os locais onde estas células estão presentes. Localiza-se mais frequentemente na pele, retina, ânus, leito ungueal e plexos coroideus. Embora muito raro, foi documentado melanoma primitivo do tracto gastrointestinal, sobretudo no esófago e região ano-rectal. No intestino delgado representa cerca de 5% das neoplasias primárias. Expõe-se o caso de um homem de 70 anos, que se apresentou com dor abdominal, emagrecimento e anemia. Com base na TAC, endoscopia e enteroscopia por cápsula estabeleceu-se o diagnóstico clínico pré-operatório de neoformação do intestino delgado. Foi submetido a enterectomia segmentar múltipla, o exame anatomo-patológico da peça excisada revelou tratar-se de melanoma. Perante este diagnóstico, o doente foi exaustivamente estudado, no sentido de excluir lesões prévias, ou não, na pele ou em outras localizações que justificassem a hipótese de metastização de melanoma no tracto gastrointestinal mas nada foi encontrado. Posteriormente foram-lhe diagnosticadas duas lesões expansivas cerebrais e múltiplas lesões pulmonares, vindo a falecer três meses após a cirurgia. Num doente sem tumor primário óbvio e com melanoma gastrointestinal, sem evidência de lesões extra-intestinais, coloca-se a hipótese diagnóstica de melanoma primário do tracto gastrointestinal.     Avaliação eletrocardiográfica em cães submetidos à pneumonectomia direita

    O propósito das ressecções pulmonares em cães e gatos, quer sejam por lobectomia ou pneumonectomia, é a cura ou paliação de processos broncopulmonares sempre que os meios conservadores de tratamento clínico sejam considerados ineficientes. Tendo em vista as significativas alterações resultantes da pneumonectomia, novos estudos experimentais devem ser feitos para avaliar as vantagens dessa intervenção cirúrgica e determinar a maneira como aplicá-la com segurança. O presente estudo tem como objetivo avaliar as alterações eletrocardiográficas em dez cães adultos de ambos os sexos, sem raça definida, com 10-30 kg, submetidos à pneumonectomia direita. Foram avaliados diariamente os parâmetros clínicos de cada cão e as alterações em todas as derivações do eletrocardiograma. Todos os cães apresentaram um bom desenlace pós-operatório. Apenas um cão apresentou alteração de relevância clínica, um caso de complexos ventriculares prematuros, possivelmente decorrente da parada cardiorrespiratória, que foi revertido com sucesso. Houve diminuição da amplitude dos complexos QRS nos primeiros 14 dias, retornado ao normal após 60 dias de pós-operatórioThe purpose for using lobectomy or pneumonectomy in dogs and cats, is to cure or palliate of bronchopulmonary diseases whenever conservative clinical treatment proves ineffective. Considering the marked changes caused by pneumonectomy, new experimental studies have to be done to assess the advantages of this surgical intervention allow its performance without any risk. The aim of the current study was to evaluate the electrocardiographic alterations in ten adult mongrel dogs of both sexes with between 10 and 30 Kg that were submitted to right pneumonectomy. The clinical parameters of the dogs were evaluated in a daily basis and the alterations in every electrocardiogram derivation were recorded. All dogs presented a good post-operative outcome. In most cases there were no electrocardiographic alterations; when these alterations were observed they were of no clinical significance and included premature ventricular complexes in one dog, most likely resulting from a cardiorespiratory arrest that was reverted to successfully, and a decreased width in the QRS complex amplitude on the first 0-14 days post surgery which returned to normal after sixty days post surger

    A análise dos eletrogramas endocavitários (EGM) é de grande importância no seguimento de portadores de marcapasso. Estudos realizados com modelos antigos revelaram taxas elevadas de falsos diagnósticos. O objetivo do presente estudo foi analisar a incidência de verdadeiros e falsos positivos nas detecçoes de arritmias atriais e ventriculares com marcapassos atuais, bem como realizar uma análise comparativa entre diversos sistemas e fabricantes, observando a existência de correlaçao entre os eventos registrados e o impacto na conduta clínica. Foram analisados 228 registros endocavitários de 111 pacientes, tendo sido observada uma taxa global de acertos de 65,4%, estatisticamente maior para diagnósticos de arritmias atriais que ventriculares.
