    Adrenal cortex development and related disorders leading to adrenal insufficiency.

    The adult human adrenal cortex produces steroid hormones that are crucial for life, supporting immune response, glucose homeostasis, salt balance and sexual maturation. It consists of three histologically distinct and functionally specialized zones. The fetal adrenal forms from mesodermal material and produces predominantly adrenal C19 steroids from its fetal zone, which involutes after birth. Transition to the adult cortex occurs immediately after birth for the formation of the zona glomerulosa and fasciculata for aldosterone and cortisol production and continues through infancy until the zona reticularis for adrenal androgen production is formed with adrenarche. The development of this indispensable organ is complex and not fully understood. This article gives an overview of recent knowledge gained of adrenal biology from two perspectives: one, from basic science studying adrenal development, zonation and homeostasis; and two, from adrenal disorders identified in persons manifesting with various isolated or syndromic forms of primary adrenal insufficiency

    The use of aerial- and close-range photogrammetry for the mapping of the Lavini di Marco tracksite (Hettangian, Southern Alps, NE Italy)

    (EXCERPT FROM ABSTRACT) Close-range photogrammetry was executed following the procedure proposed by Mallison & Wings (2014). More than seventy 3D models were obtained and interpreted by means of color-coded and contour line images, which allow to improve the ichno- logical knowledge of the tracksite. The 3D models of the best-preserved tracks were used for the osteological reconstruction of the trackmakers’ autopodia, supposing the arthral position of the phalangeal pads. Three indirect methods were used to correlate tracks and their trackmakers: (i) synapomorphy-based approach; (ii) phenetic correlation; (iii) coincidence correlation (see Carrano & Wilson, 2001) The final map was produced with different level of knowledge due to the distribution of tracks and current state of site preservation. Furthermore, it represents a complete documentation that will be used for future work of enhancement, preservation and valorization of the tracksite. The ichnotaxonomical review of the quadrupedal trackways led us to emend the diagnosis of Lavinipes cheminii Avanzini et al. (2003) and to assign several other sparse tracks and trackways to L. chemini. The skeletal reconstruction of fore and hind limbs points towards Gongxianosaurus sp. as the most suitable trackmaker of L. cheminii. The herein supposed Laurasian affinity of the Lavini di Marco dinosaur assemblage clashes with the previous hypotheses that always link the Southern Alps sector with the Gondwana mainland

    Cascading effects of canopy mortality drive long-term changes in understorey diversity in temperate old-growth forests of Europe

    Questions: We investigated the influence of protracted mortality of a dominant canopy tree (Abies alba) on long-term understorey dynamics. We ask (a) how tree regeneration and understorey species diversity and composition changed over 32 years; and (b) whether the observed changes were mainly driven by mortality of A. alba. Location: Three old-growth forest reserves dominated by A. alba and Fagus sylvatica in the Dinaric Mountains of Slovenia. Method: Tree layer and understorey regeneration and herbs were surveyed in 147 plots across the three forest reserves in 1983 and 2015. Soils were also sampled in 2015. The study period coincides with a protracted period of increased A. alba mortality in the canopy layer associated with anthropogenic emissions. Results: Between 1983 and 2015, the decline in canopy layer A. alba caused a recruitment pulse of F. sylvatica regeneration to the subcanopy tree layer across the three reserves. These changes were accompanied by a significant decline in plot level herb species richness. A model-based analysis of beta-diversity revealed significant community convergence during the study period, mainly caused by the loss of rare species. Ellenberg values indicate that these changes were mainly driven by loss of understorey light, while an increase in soil pH may have played a role also. Conclusions: This observational study suggests that the long-term decline of A. alba resulted in a cascade of processes - widespread F. sylvatica recruitment that impeded penetration of light to the forest floor, and possibly a change in soil conditions due to the decline of coniferous litter. These changes caused a significant loss of herb diversity and homogenization of the understorey community across the three sites. This study sheds light on the potential cascading consequences triggered by episodes of increased tree mortality resulting from global-change-type drivers

    Development and function of the fetal adrenal.

    The adrenal cortex undergoes multiple structural and functional rearrangements to satisfy the systemic needs for steroids during fetal life, postnatal development, and adulthood. A fully functional adrenal cortex relies on the proper subdivision in regions or 'zones' with distinct but interconnected functions, which evolve from the early embryonic stages to adulthood, and rely on a fine-tuned gene network. In particular, the steroidogenic activity of the fetal adrenal is instrumental in maintaining normal fetal development and growth. Here, we review and discuss the most recent advances in our understanding of embryonic and fetal adrenal development, including the known causes for adrenal dys-/agenesis, and the steroidogenic pathways that link the fetal adrenal with the hormone system of the mother through the fetal-placental unit. Finally, we discuss what we think are the major open questions in the field, including, among others, the impact of osteocalcin, thyroid hormone, and other hormone systems on adrenal development and function, and the reliability of rodents as models of adrenal pathophysiology

    Correction of sun glint effect on MIVIS data of the Sicily campaign in July 2000

    To assess the suspended and dissolved matter in water in the visible and near infrared spectral regions it is necessary to estimate with adequate accuracy the water leaving radiance. Consequently radiance measured by a remote sensor has to be corrected from the atmospheric and the sea surface effects consisting in the path radiance and the sun and sky glitter radiance contributions. This paper describes the application of the sun glint correction scheme on to airborne hyperspectral MIVIS measurements acquired on the area of the Straits of Messina during the campaign in July 2000. In the Messina case study data have been corrected for the atmospheric effects and for the sun-glitter contribution evaluated following the method proposed by Cox and Munk (1954, 1956). Comparison between glitter contaminated and glitter free data has been made taking into account the radiance profiles relevant to selected scan lines and the spectra of different pixels belonging to the same scan line and located out and inside the sun glitter area. The results show that spectra after correction have the same profile as the contaminated ones, although, at this stage, free glint data have not yet been used in water constituent retrieval and consequently the reliability of such correction cannot be completely evaluated

    Epidemiology and burden of Rotavirus-associated hospitalizations in Ferrara, Italy

    Objective of this study was to provide data on hospitalizations for rotavirus gastroenteritis (RVGE) in Ferrara, Italy. The study was conducted analyzing the hospital discharge forms of all children admitted to the Pediatric Department of the University of Ferrara, Arcispedale Sant'Anna, from January 2001 through December 2005. The database was searched for all gastrointestinal diseases and in particular RVGE. During the period under study 3277 children, of which 2038 inf.60 months of age, were hospitalized; 247 children inf.5 years old were admitted for acute gastroenteritis and 89 (4.4% of all and 36% of gastroenteritis-related hospitalizations) had rapid screening tests positive for rotavirus. A seasonal pattern was observed for RVGE with an increase in winter and early spring. The average length of hospital stay was 5.7 days. The median cost of each hospitalized case of RVGE ranged between 1417 and 1595 Euros. The present research confirms that rotavirus gastroenteritis represents an important cause of hospitalization in children and is responsible for significant costs for the Public Health Care System. An effective vaccination program could significantly reduce the incidence of hospitalization and the associated costs

    Thyroid hormones modifications among COVID-19 patients undergoing pulmonary rehabilitation

    Introduction: Patients with severe COVID-19 often experience long-lasting disabilities that can improve after pulmonary rehabilitation. Moreover patients with severe COVID-19 display thyroid function alterations due to a non-thyroidal illness syndrome (NTIS). The aim of our study was to evaluate thyroid function parameters among patients hospitalized for COVID-19 who were eligible or not to respiratory rehabilitation and their modifications during follow-up. Materials and methods: Post-COVID-19 patients referred to a Respiratory Rehabilitation Unit were evaluated. Outpatients, not candidate for rehabilitation, were enrolled as Control group. Patients who had completed a 4-week-rehabilitation program were enrolled as Rehabilitation Group. All patients were evaluated at T0 (4 weeks after the discharge home in Control Group and after completion of rehabilitation in Rehabilitation Group) and at T1 (3 months after T0). Results: The final study group included 39 patients (20 in the Rehabilitation group and 19 in the Control group). Patients in the Rehabilitation Group had more frequently received invasive or non-invasive ventilation, had a longer length-of-stay in referring hospitals, had a higher number of comorbidities and displayed a worse performance at 6-minute-walking-test (6MWT) and Short-Physical-Performance-Battery-test (SPPB). FT3 values were lower at T0 in the Rehabilitation Group, while TSH and FT4 values were similar in the two groups. While no significant modifications in thyroid-function-parameters were observed in the Control Group, a significant increase in FT3 value was observed in the Rehabilitation Group at T1. Participants of both groups had improved the results of 6MWT at T1, while SPPB values improved only in the Rehabilitation Group. Conclusions: COVID-19 patients after pulmonary rehabilitation experience an increase in FT3 values during follow-up, paralleled with an amelioration of functional capabilities

    Effectiveness of a Pulmonary Rehabilitation Program on Persistent Asthma Stratified for Severity

    BACKGROUND: Asthma is defined by airway inflammation associated with various respiratory symptoms, and pharmacologic treatment is based on inhaled corticosteroids and bronchodilators. Physical activity, educational training, nutritional support, and psychological counseling are considered part of non-pharmacologic treatment; however, studies so far have investigated the effect of single non-pharmacologic treatment. There are few studies that demonstrate the effect of comprehensive pulmonary rehabilitation, but no clear data are available regarding factors that can predict who could benefit the most. Our study aimed to assess the effect of a comprehensive 3-week pulmonary rehabilitation program on exercise tolerance and to identify baseline subject characteristics that may predict a better response to treatment. METHODS: This was a retrospective study. A team planned a pulmonary rehabilitation program: educational support; endurance training; and optional components, such as respiratory exercises and airway clearance techniques. The following data were collected before and after pulmonary rehabilitation: subject characteristics, smoking history, asthma severity, respiratory function and 6-min walk test (6MWT). RESULTS: We collected data on 515 subjects (202 males 39.2%), age, mean \ub1 SD 63.9 \ub1 10.4 y), with 413 (80.2%) having moderate-to-severe disease; and 455 (88.4%) with stable respiratory symptoms 455 (88.35%). At baseline, the percentage of predicted 6MWT in all subjects categorized by the Global Initiative for Asthma (GINA) steps was in the normal range, except for the subjects at step 5, for which it was significantly lower (P = .01). All subjects showed a significant improvement in exercise tolerance and oxygen saturation, together with a decrease in baseline dyspnea, muscle fatigue, and heart rate after pulmonary rehabilitation. Improvement of 6MWT was statistically significant, irrespective of the GINA categorization. The variables related to the improvement in 6MWT were age (P < .001), smoking habit (P = .034), and baseline 6MWT (P < .001). CONCLUSIONS: Subjects with asthma at any GINA step seemed to benefit from a pulmonary rehabilitation program; analysis of our data highlighted that pulmonary rehabilitation was more beneficial in younger subjects with a smoking history and worse baseline exercise tolerance
