    De novo erythroleukemia chromosome features include multiple rearrangements, with special involvement of chromosomes 11 and 19

    Erythroid leukemia (ERL or AML-M6) is an uncommon subtype of acute myeloid leukemia, the clinical, morphological, and genetic behavior of which needs further characterization. We analyzed a homogeneous group of 23 de novo AML-M6 patients whose bone marrow cells showed complex karyotypes. We also analyzed eight leukemia cell lines with erythroid phenotype, performing detailed molecular cytogenetic analyses, including spectral karyotyping (SKY) in all samples. The main features are: (1) A majority of patients (56%) had hypodiploidy. Loss of genetic material was the most common genetic change, especially monosomies of chromosome 7 or 18, and deletions of chromosome arm 5q. Taken together, 87% of the cases displayed aberrations involving chromosome 5 or 8. (2) We describe a novel, cryptic, and recurrent translocation, t(11;19)(p11.2;q13.1). Another translocation, t(12;21)(p11.2;q11.2), was found to be recurrent in a patient with ERL and in the K562 cell line. (3) MLL gene rearrangements were detected in 20% of cases (three translocations and three amplifications) and, overall, we defined 52 rearrangements (excluding deletions) with a mean of 2.3 translocations per patient. (4) Of the structural aberrations, 21% involved chromosomes 11 and 19. Most of the rearrangements were unbalanced; only 13 reciprocal translocations were observed. The general picture of chromosomal aberrations in cell lines did not reflect what occurred in patient samples. However, both primary samples and cell lines shared three common breakpoints at 19q13.1, 20q11.2, and 21q11.2. This is the first molecular cytogenetic description of the karyotype abnormalities present in patients with ERL. It should assist in the identification of genes involved in erythroleukemogenesis

    SpadaHC: a database to improve the classification of variants in hereditary cancer genes in the Spanish population

    Accurate classification of genetic variants is crucial for clinical decision-making in hereditary cancer. In Spain, genetic diagnostic laboratories have traditionally approached this task independently due to the lack of a dedicated resource. Here we present SpadaHC, a web-based database for sharing variants in hereditary cancer genes in the Spanish population. SpadaHC is implemented using a three-tier architecture consisting of a relational database, a web tool and a bioinformatics pipeline. Contributing laboratories can share variant classifications and variants from individuals in Variant Calling Format (VCF) format. The platform supports open and restricted access, flexible dataset submissions, automatic pseudo-anonymization, VCF quality control, variant normalization and liftover between genome builds. Users can flexibly explore and search data, receive automatic discrepancy notifications and access SpadaHC population frequencies based on many criteria. In February 2024, SpadaHC included 18 laboratory members, storing 1.17 million variants from 4306 patients and 16 343 laboratory classifications. In the first analysis of the shared data, we identified 84 genetic variants with clinically relevant discrepancies in their classifications and addressed them through a three-phase resolution strategy. This work highlights the importance of data sharing to promote consistency in variant classifications among laboratories, so patients and family members can benefit from more accurate clinical management.Database URL: https://spadahc.ciberisciii.es/ Overview of SpadaHC and its main views.     Effectiveness of an mHealth intervention combining a smartphone app and smart band on body composition in an overweight and obese population: Randomized controlled trial (EVIDENT 3 study)

    Background: Mobile health (mHealth) is currently among the supporting elements that may contribute to an improvement in health markers by helping people adopt healthier lifestyles. mHealth interventions have been widely reported to achieve greater weight loss than other approaches, but their effect on body composition remains unclear. Objective: This study aimed to assess the short-term (3 months) effectiveness of a mobile app and a smart band for losing weight and changing body composition in sedentary Spanish adults who are overweight or obese. Methods: A randomized controlled, multicenter clinical trial was conducted involving the participation of 440 subjects from primary care centers, with 231 subjects in the intervention group (IG; counselling with smartphone app and smart band) and 209 in the control group (CG; counselling only). Both groups were counselled about healthy diet and physical activity. For the 3-month intervention period, the IG was trained to use a smartphone app that involved self-monitoring and tailored feedback, as well as a smart band that recorded daily physical activity (Mi Band 2, Xiaomi). Body composition was measured using the InBody 230 bioimpedance device (InBody Co., Ltd), and physical activity was measured using the International Physical Activity Questionnaire. Results: The mHealth intervention produced a greater loss of body weight (–1.97 kg, 95% CI –2.39 to –1.54) relative to standard counselling at 3 months (–1.13 kg, 95% CI –1.56 to –0.69). Comparing groups, the IG achieved a weight loss of 0.84 kg more than the CG at 3 months. The IG showed a decrease in body fat mass (BFM; –1.84 kg, 95% CI –2.48 to –1.20), percentage of body fat (PBF; –1.22%, 95% CI –1.82% to 0.62%), and BMI (–0.77 kg/m2, 95% CI –0.96 to 0.57). No significant changes were observed in any of these parameters in men; among women, there was a significant decrease in BMI in the IG compared with the CG. When subjects were grouped according to baseline BMI, the overweight group experienced a change in BFM of –1.18 kg (95% CI –2.30 to –0.06) and BMI of –0.47 kg/m2 (95% CI –0.80 to –0.13), whereas the obese group only experienced a change in BMI of –0.53 kg/m2 (95% CI –0.86 to –0.19). When the data were analyzed according to physical activity, the moderate-vigorous physical activity group showed significant changes in BFM of –1.03 kg (95% CI –1.74 to –0.33), PBF of –0.76% (95% CI –1.32% to –0.20%), and BMI of –0.5 kg/m2 (95% CI –0.83 to –0.19). Conclusions: The results from this multicenter, randomized controlled clinical trial study show that compared with standard counselling alone, adding a self-reported app and a smart band obtained beneficial results in terms of weight loss and a reduction in BFM and PBF in female subjects with a BMI less than 30 kg/m2 and a moderate-vigorous physical activity level. Nevertheless, further studies are needed to ensure that this profile benefits more than others from this intervention and to investigate modifications of this intervention to achieve a global effect

    Epidemiological trends of HIV/HCV coinfection in Spain, 2015-2019

    Altres ajuts: Spanish AIDS Research Network; European Funding for Regional Development (FEDER).Objectives: We assessed the prevalence of anti-hepatitis C virus (HCV) antibodies and active HCV infection (HCV-RNA-positive) in people living with HIV (PLWH) in Spain in 2019 and compared the results with those of four similar studies performed during 2015-2018. Methods: The study was performed in 41 centres. Sample size was estimated for an accuracy of 1%. Patients were selected by random sampling with proportional allocation. Results: The reference population comprised 41 973 PLWH, and the sample size was 1325. HCV serostatus was known in 1316 PLWH (99.3%), of whom 376 (28.6%) were HCV antibody (Ab)-positive (78.7% were prior injection drug users); 29 were HCV-RNA-positive (2.2%). Of the 29 HCV-RNA-positive PLWH, infection was chronic in 24, it was acute/recent in one, and it was of unknown duration in four. Cirrhosis was present in 71 (5.4%) PLWH overall, three (10.3%) HCV-RNA-positive patients and 68 (23.4%) of those who cleared HCV after anti-HCV therapy (p = 0.04). The prevalence of anti-HCV antibodies decreased steadily from 37.7% in 2015 to 28.6% in 2019 (p < 0.001); the prevalence of active HCV infection decreased from 22.1% in 2015 to 2.2% in 2019 (p < 0.001). Uptake of anti-HCV treatment increased from 53.9% in 2015 to 95.0% in 2019 (p < 0.001). Conclusions: In Spain, the prevalence of active HCV infection among PLWH at the end of 2019 was 2.2%, i.e. 90.0% lower than in 2015. Increased exposure to DAAs was probably the main reason for this sharp reduction. Despite the high coverage of treatment with direct-acting antiviral agents, HCV-related cirrhosis remains significant in this population

    Estudio espectroscĂłpico y DRX de afloramientos terrestres volcĂĄnicos en la isla de Tenerife como posibles anĂĄlogos de la geologĂ­a marciana

    Several volcanic outcrops from Tenerife Island (Las Cañadas Caldera, historical volcanism and Gu.imar-Malpaís outcrop) has been selected as a potential terrestrial analog for Mars, regarding the Martian mineralogy and its volcanic characteristic. Diverse alteration processes, including weathering and hydrothermal alteration have been detected in these volcanics environments, which could be considered as part of a model for the primitive volcanic activity of Mars. The selected materials have been measured by micro-Raman spectroscopy, XRD and Mössbauer spectroscopy. The results show the primary mineralogy comprises on olivine, pyroxene and feldspar. Also, a wide variety of alterational materials, including oxides, clay minerals and carbonates have been detected. The results have proven to be a starting point to develop research focused to the development of science instrumentation for planetary exploration in volcanic environments. The instrumentation used was primarily through twin prototypes applied in space exploration such as XRD (on board at the NASA-MSL-Curiosity mission), Mössbauer spectroscopy (on board at the NASA-MER mission) and the future Raman instrument on ESA mission -ExoMars. In addition, the detected mineralogy is consistent with results reported on Mars. The Raman spectral analisys methods, characterization and identification, have been applied, where the analysis of the Raman profiles are extremely useful to clarify the geochemical origin of the mineral species. On the other hand, the results obtained by Raman have been confirmed by Mössbauer spectroscopy and X-ray diffraction.<br><br>Se ha llevado a cabo una selección de varios afloramientos volcånicos en la Isla de Tenerife (La Caldera de las Cañadas, vulcanismo histórico y la zona del Malpaís de Gu.imar) como posibles anålogos terrestres de Marte, considerando los procesos volcånicos ocurridos durante algunas de las etapas geológicas del planeta rojo. En la selección de las åreas de estudio se han tenido en cuenta la diversidad de procesos de alteración, que incluyen fenómenos como meteorización y alteración hidrotermal. Estos procesos terrestres podrían servir como modelo de la actividad volcånica primitiva en Marte. Los materiales seleccionados se han analizado mediante espectroscopia micro-Raman, difracción de rayos X (DRX) y espectroscopia Mössbauer. Los resultados revelan que la mineralogía de los afloramientos estå constituida por una matriz de olivinos, piroxenos y feldespatos. Ademås, se ha detectado una gran variedad de especies minerales correspondientes a procesos de alteración como óxidos, arcillas y carbonatos. Los resultados obtenidos han demostrado ser un punto de partida para desarrollar investigaciones en estos entornos volcånicos especialmente enfocados al desarrollo de la ciencia de instrumentación para exploración planetaria. La instrumentación utilizada en la investigación ha sido a través de prototipos gemelos empleados en la exploración espacial, como DRX (actualmente en la misión NASA-MSL-Curiosity), espectroscopia Mössbauer (a bordo de la misión NASA-MER) y el futuro instrumento Raman de la misión ESA-ExoMars. Ademås, la mineralogía detectada coincide con los resultados reportados en Marte. Los métodos de anålisis Raman, tanto de caracterización e identificación, mediante el estudio de patrones espectrales se han aplicado exitosamente, donde el anålisis de los perfiles Raman son de extrema utilidad para aclarar el origen de las especies minerales.
