    Ultra Low-Power Analog Median Filters

    The design and implementation of three analog median filter topologies, whose transistors operate in the deep weak-inversion region, is described. The first topology is a differential pairs array, in which drain currents are driven into two nodes in a differential fashion, while the second topology is based on a wide range OTA, which is used to maximize the dynamic range. Finally, the third topology uses three range-extended OTAs. The proposed weak-inversion filters were designed and fabricated in ON Semiconductor 0.5 micrometer technology through MOSIS. Experimental results of three-input fabricated prototypes for all three topologies are show, where power consumptions of 90nW in the first case, and 270nW in the other two cases can be noticed. A dual power supply +/-1.5 Volts were used

    ResumenIntroducciónLa arteriolopatía urémica calcificante, también conocida como calcifilaxis, es una entidad caracterizada por la presencia de áreas de necrosis isquémica junto con extensas calcificaciones de la capa media de las arteriolas de la piel; la etiopatogenia es compleja, las formas de presentación y evolución son diversas. La calcifilaxis del pene es una entidad muy rara. El dolor y la ulceración del glande es una de sus manifestaciones clínicas iniciales, el diagnóstico en estas etapas es difícil de establecer y el manejo debe ser integral. El tratamiento médico seguido del manejo quirúrgico se establecerá de acuerdo a la etapa y evolución de la enfermedad.La reconstrucción aislada del glande se recomienda tras una amputación traumática o quirúrgica por alguna enfermedad benigna o maligna. El objetivo es conseguir una apariencia estética razonable, permitir una micción fisiológica y tratar de conservar la mayor cantidad de tejido eréctil. En pacientes con calcifilaxis peniana, el manejo quirúrgico convencional es radical y consiste en falectomía parcial o total según sea el caso. El manejo conservador ha sido reportado mediante la desbridación de las lesiones ulcerosas en pene, tratamiento local y vigilancia.Caso clínicoSe presenta el caso de un paciente masculino de 59 años de edad con insuficiencia renal crónica en tratamiento con hemodiálisis, hospitalizado por cuadro de 5 días de evolución con úlcera dolorosa en glande, refractaria a manejo analgésico. Se realizó biopsia escisional de la lesión con estudio histopatológico confirmando calcifilaxis. Se plantea a paciente falectomía parcial la cual rechaza por lo que se ofrece reconstrucción en un tiempo como opción terapéutica.ConclusiónUtilizar injertos o colgajos de piel para realizar la reconstrucción del pene en pacientes con calcifilaxis no representa una opción viable. Por su gran aporte vascular, la uretra representa una buena opción de reconstrucción en pacientes con calcifilaxis en etapa inicial. La selección y evaluación de los pacientes debe ser muy cautelosa para que los resultados estéticos y funcionales sean aceptables.AbstractIntroductionCalcific uremic arteriolopathy, also known as calciphylaxis, is an entity characterized by areas of ischemic necrosis with extensive calcifications of the middle layer of the skin arterioles. Its etiopathogenesis is complex and its forms of presentation and progression are diverse. Calciphylaxis of the penis is an extremely rare entity. Pain and ulceration of the glans penis are initial clinical manifestations and diagnosis at these stages is difficult. Management should be comprehensive. Medical treatment, followed by surgical management, is established according to disease stage and progression.Isolated reconstruction of the glans penis is recommended when there is traumatic amputation or a surgical one due to benign or malignant disease. The goal is to obtain a reasonably esthetic appearance, to have physiologic micturition, and to spare the greatest amount of erectile tissue possible. Conventional surgical management in patients with penile calciphylaxis is radical and consists of partial or total phallectomy, depending on the case. Conservative management through ulcerous penile lesion debridement, local treatment, and surveillance has been reported.Case reportA 59-year-old man with chronic renal insufficiency in hemodialysis treatment was hospitalized for symptoms of 5-day progression of a painful ulcer on the glans penis that was refractory to analgesics. An excisional biopsy of the lesion was taken and the histopathologic study confirmed calciphylaxis. Partial phallectomy was suggested to the patient, but he rejected it, and so single stage reconstruction was offered as a therapeutic option.ConclusionThe use of skin grafts or flaps for penile reconstruction in patients with calciphylaxis is not a viable option. Because of its large blood supply, the urethra is a good reconstruction option in patients with initial-stage calciphylaxis. Patient selection and evaluation must be carried out with great care in order to have acceptable functional and esthetic results

    Use of firewood in an indigenous community from the Sierra Norte de Puebla: a gender perspective on the access to natural resources

    Objective: To analyze some elements considered key to understanding the rights of use, access and ownership of natural resources according to the social roles assigned to each sex in the community of Reyeshogpan, Puebla. Design/methodology/approach: The methodology used is that of participant observation complemented by the application of 24 semi-structured interviews with households in the community. Results: The results indicate that the surface areas range between 0.25 ± 3.0 hectares, where 40.79% of those consulted own surfaces of less than 0.5 hectares, an insufficient surface area to cover their annual energy needs (10,752 m3 / household), which is why they resort to renting, borrowing land, and buying firewood.  In terms of access to land, 82.14% is in the hands of men and 17.86% in the hands of women. This percentage of female holders does not imply that they can exercise their rights as owners, as men, thus confirming that access to land influences the availability of firewood and its use for energy. Limitations on study/implications: The analysis of the use and quantity of fuelwood used by households used information provided in the interviews, which may underestimate or overestimate quantities. Findings/conclusions: For the community of Reyeshogpan, firewood collection is gender-based, based on the physical differences between men and women, finding that there are specific tasks for men to collect firewood that are related to their physical strength and tasks that women carry out in a complementary way and without any economic value whatsoever.     Transcriptome Metabolic Characterization of Tuber borchii SP1—A New Spanish Strain for In Vitro Studies of the Bianchetto Truffle

    Truffles are ascomycete hypogeous fungi belonging to the Tuberaceae family of the Pezizales order that grow in ectomycorrhizal symbiosis with tree roots, and they are known for their peculiar aromas and flavors. The axenic culture of truffle mycelium is problematic because it is not possible in many cases, and the growth rate is meager when it is possible. This limitation has prompted searching and characterizing new strains that can be handled in laboratory conditions for basic and applied studies. In this work, a new strain of Tuber borchii (strain SP1) was isolated and cultured, and its transcriptome was analyzed under different in vitro culture conditions. The results showed that the highest growth of T. borchii SP1 was obtained using maltose-enriched cultures made with soft-agar and in static submerged cultures made at 22 °C. We analyzed the transcriptome of this strain cultured in different media to establish a framework for future comparative studies, paying particular attention to the central metabolic pathways, principal secondary metabolite gene clusters, and the genes involved in producing volatile aromatic compounds (VOCs). The results showed a transcription signal for around 80% of the annotated genes. In contrast, most of the transcription effort was concentrated on a limited number of genes (20% of genes account for 80% of the transcription), and the transcription profile of the central metabolism genes was similar in the different conditions analyzed. The gene expression profile suggests that T. borchii uses fermentative rather than respiratory metabolism in these cultures, even in aerobic conditions. Finally, there was a reduced expression of genes belonging to secondary metabolite clusters, whereas there was a significative transcription of those involved in producing volatile aromatic compounds

    Interspecies DNA acquisition by a naturally competent Acinetobacter baumannii strain

    The human pathogen Acinetobacter baumannii possesses high genetic plasticity and frequently acquires antimicrobial resistance genes. Here we investigated the role of natural transformation in these processes. Genomic DNA from different sources, including from carbapenem-resistant Klebsiella pneumoniae strains, was mixed with A. baumannii A118 cells. Selected transformants were analysed by whole-genome sequencing. In addition, bioinformatics analyses and in silico gene flow prediction were also performed to support the experimental results. Transformant strains included some that became resistant to carbapenems or changed their antimicrobial susceptibility profile. Foreign DNA acquisition was confirmed by whole-genome analysis. The acquired DNA most frequently identified corresponded to mobile genetic elements, antimicrobial resistance genes and operons involved in metabolism. Bioinformatics analyses and in silico gene flow prediction showed continued exchange of genetic material between A. baumannii and K. pneumoniae when they share the same habitat. Natural transformation plays an important role in the plasticity of A. baumannii and concomitantly in the emergence of multidrug-resistant strains.Fil: Traglia, German Matias.     Quantitative ultrasound texture analysis of fetal lungs to predict neonatal respiratory morbidity

    Objective To develop and evaluate the performance of a novel method for predicting neonatal respiratory morbidity based on quantitative analysis of the fetal lung by ultrasound. Methods More than 13¿000 non-clinical images and 900 fetal lung images were used to develop a computerized method based on texture analysis and machine learning algorithms, trained to predict neonatal respiratory morbidity risk on fetal lung ultrasound images. The method, termed ‘quantitative ultrasound fetal lung maturity analysis’ (quantusFLM™), was then validated blindly in 144 neonates, delivered at 28¿+¿0 to 39¿+¿0¿weeks' gestation. Lung ultrasound images in DICOM format were obtained within 48¿h of delivery and the ability of the software to predict neonatal respiratory morbidity, defined as either respiratory distress syndrome or transient tachypnea of the newborn, was determined. Results Mean (SD) gestational age at delivery was 36¿+¿1 (3¿+¿3) weeks. Among the 144 neonates, there were 29 (20.1%) cases of neonatal respiratory morbidity. Quantitative texture analysis predicted neonatal respiratory morbidity with a sensitivity, specificity, positive predictive value and negative predictive value of 86.2%, 87.0%, 62.5% and 96.2%, respectively. Conclusions Quantitative ultrasound fetal lung maturity analysis predicted neonatal respiratory morbidity with an accuracy comparable to that of current tests using amniotic fluid.Peer ReviewedPostprint (published version

    Spine Instability Neoplastic Score: agreement across different medical and surgical specialties

    [eng] Background Context Spinal instability is an acknowledged complication of spinal metastases; in spite of recent suggested criteria, it is not clearly defined in the literature. Purpose This study aimed to assess intra and interobserver agreement when using the Spine Instability Neoplastic Score (SINS) by all physicians involved in its management. Study Design Independent multicenter reliability study for the recently created SINS, undertaken with a panel of medical oncologists, neurosurgeons, radiologists, orthopedic surgeons, and radiation oncologists, was carried out. Patient Sample Ninety patients with biopsy-proven spinal metastases and magnetic resonance imaging, reviewed at the multidisciplinary tumor board of our institution, were included. Outcome Measures Intraclass correlation coefficient (ICC) was used for SINS score agreement. Fleiss kappa statistic was used to assess agreement on the location of the most affected vertebral level; agreement on the SINS category ("stable," "potentially stable," or "unstable"); and overall agreement with the classification established by tumor board. Methods Clinical data and imaging were provided to 83 specialists in 44 hospitals across 14 Spanish regions. No assessment criteria were pre-established. Each clinician assessed the SINS score twice, with a minimum 6-week interval. Clinicians were blinded to assessments made by other specialists and to their own previous assessment. Subgroup analyses were performed by clinicians' specialty, experience (≤7, 8-13, ≥14 years), and hospital category (four levels according to size and complexity). This study was supported by Kovacs Foundation. Results Intra and interobserver agreement on the location of the most affected levels was "almost perfect" (κ>0.94). Intra-observer agreement on the SINS score was "excellent" (ICC=0.77), whereas interobserver agreement was "moderate" (ICC=0.55). Intra-observer agreement in SINS category was "substantial" (k=0.61), whereas interobserver agreement was "moderate" (k=0.42). Overall agreement with the tumor board classification was "substantial" (κ=0.61). Results were similar across specialties, years of experience, and hospital category. Conclusions Agreement on the assessment of metastatic spine instability is moderate. The SINS can help improve communication among clinicians in oncology care
