    Free functional gracilis muscle transfer for reconstruction of massive ectopic calcification following a lower extremity fracture: a case report and overview of the literature

    We present a patient with massive posttraumatic ectopic calcification in the lower extremity. The patient complained of an increasing tender soft tissue mass with signs of acute inflammation and foot dorsiflexion weakness following a tibial fracture for 34years. A large radiopaque mass was resected and reconstructed using a free functional gracilis muscle transfer, resulting in recovery of stable soft tissue and foot dorsiflexion. The resected material showed highly fibrosed soft tissue with extensive dystrophic calcifications. There were no sign of recurrence at 18-month follow-u

    Correction of Forearm Malunion Guided by the Preoperative Complaint

    Diaphyseal malunion of the forearm may cause loss of pronation and supination, a painful distal radioulnar joint, and aesthetic problems. Seventeen patients (10 males, seven females; mean age, 20.6±9.3years) were operated on because of symptomatic malunion after a pediatric forearm fracture. Six patients had predominant loss of pronation (Group 1), four had predominant loss of supination (Group 2), and seven had a painful distal radioulnar joint (Group 3). An osteotomy of the radius was performed in seven patients and of both forearm bones in 10. All patients were available for clinical and radiologic assessments at a minimum followup of 6months (mean±standard deviation, 3.7±2.3years; range, 0.5-9.9years). Release of the contracted interosseous membrane frequently was necessary for patients in Groups 1 and 2 to allow for correction and did not result in weakness, instability of the distal radioulnar joint, or synostosis. The overall improvement in range of motion after osteotomies for patients with a supination deficit was much better than in those with a pronation deficit. All patients in Group 3 gained a pain-free and stable distal radioulnar joint and their range of motion was unchanged.     Collaborative Virtual Environments for Ergonomics: Embedding the Design Engineer Role in the Loop

    International audienceThe aim of this paper is to define the role and duties of a design engineer involved in a collaborative ergonomic design session supported by a 3D collaborative virtual environment. For example, such a session can be used to adapt the manual task an operator must achieve in the context of an industrial assembly line. We first present the interest of such collaborative sessions. Then we present a related work explaining the need of proper 3DCVE and metaphors to obtain efficient collaborative ergonomic design sessions. Then, after a short definition of the role of the engineer in such sessions, we propose a use case highlighting the type of metaphor such engineers need to have to be efficient in such a framework.     Performance characteristics of a new structured packing

    A new structured packing using carbon fibres, called SepcarbÂź 4D, is presented. This packing has several attractive properties, such as high voidage (Δ=94%) and high effective area (a=420 m2 m−3). These properties are advantageous for packing used as a gas–liquid contactor for separation units. To determine the internal characteristics of this packing, we performed several experiments using a 150-mm-internal-diameter column. Firstly, hydrodynamics experiments were conducted using an air–water counter current flow to determine the pressure drop (for both dry and wet packing) and flooding point. Secondly, the mass transfer efficiency was determined in terms of HETP (height equivalent to theoretical plate) by total reflux experiments with an n-heptane/cyclohexane mixture at atmospheric pressure.     Simulation-Based Fitting of Protein-Protein Interaction Potentials to SAXS Experiments

    AbstractWe present a new method for computing interaction potentials of solvated proteins directly from small-angle x-ray scattering data. An ensemble of proteins is modeled by Monte Carlo or molecular dynamics simulation. The global x-ray scattering of the whole model ensemble is then computed at each snapshot of the simulation, and averaged to obtain the x-ray scattering intensity. Finally, the interaction potential parameters are adjusted by an optimization algorithm, and the procedure is iterated until the best agreement between simulation and experiment is obtained. This new approach obviates the need for approximations that must be made in simplified analytical models. We apply the method to lambda repressor fragment 6-85 and fyn-SH3. With the increased availability of fast computer clusters, Monte Carlo and molecular dynamics analysis using residue-level or even atomistic potentials may soon become feasible

    Physeal Distraction for Joint Preservation in Malignant Metaphyseal Bone Tumors in Children

    Background: Physeal distraction facilitates metaphyseal bone tumor resection in children and preserves the adjacent joint. The technique was first described by Cañadell. Tumor resection procedures allowing limb-sparing reconstruction have been used increasingly in recent years without compromising oncologic principles. Questions/purposes: We report our results with Cañadell's technique by assessing tumor control, functional outcome, and complications. Methods: Six consecutive children with primary malignant metaphyseal bone tumors underwent physeal distraction as a part of tumor resection. Tumor location was the distal femur in four patients, the proximal humerus in one patient, and the proximal tibia in one patient. The functional outcome was evaluated after a minimum of 18months (median, 62months; range, 18-136months) using the Musculoskeletal Tumor Society (MSTS) score and the Toronto Extremity Salvage Score (TESS). Results: At latest followup, five patients were alive and disease-free and one had died from metastatic disease. All tumor resections resulted in local control; there were no local recurrencies. The mean MSTS score was 79% (range, 53%-97%) and corresponding mean TESS was 83% (range, 71%-92%). In one case, postoperative infection required amputation of the proximal lower leg. All physeal distractions were successful except for one patient in whom distraction resulted in rupturing into the tumor. This situation was salvaged by transepiphyseal resection. Conclusions: We consider Cañadell's technique a useful tool in the armamentarium to treat children with malignant tumors that are in close proximity to an open physis.     Recurrent aggressive chondrosarcoma of the middle phalanx of the index finger: excision and reconstruction with an osteocartilaginous allograft

    Introduction: Chondrosarcomas are malignant tumours and need to be treated aggressively including ablative surgery. Bovée et al. and Mankin have recently drawn attention to a less aggressive behaviour of chondrosarcomas of the phalanges compared with those of other localizations including the metacarpals. Materials and methods: An 12year follow-up of a patient with a chondrosarcoma of the middle phalanx of the index finger is presented. The lesion was curetted initially, and a repeat curettage was performed 4years later. Finally, 8years later the phalanx was excised and reconstructed with an osteocartilaginous allograft. Histologically, the lesion changed from a chondrosarcoma grade I to grade II. Result: The patient continues to be free of recurrence and metastases 4years after the final resection. Conclusion: The potential for systemic disease of chondrosarcomas of the phalanges is probably much lower than in chondrosarcomas of other localizations, and therefore digit-sparing techniques may be considered rather than ablative procedure
